RESUMO

The method of forced diuresis by furosedium after adenomectomy is an effective method which decreases the postoperative complications of such surgery (haemorrhage, infection, etc.) and reduces the time of hospitalisation.

RESUMO

After reviewing the history and technique used for transureteral cutaneous ureterostomy, the authors describe 5 cases treated by this method during the 3-year period, 1975--1977. This prodecure has many advantages when compared to the transintestinal cutaneous ureterostomy as described by Bricker, uteterosigmoidoscopy, and "double-barreleed" cutaneous ureterostomy. The authors conclude that transureteral cutaneous ureterostomy should be performed in cases of ureteropyelocalicial distention with renal failure from operable or inoperable bladder cancer, in women with inoperable or recurrent tumors of the genital organs and vesicovaginal, vesicourethrovaginal or vesicorectovaginal fistulae, and in patients with a neurological bladder. They propose that this surgical technique be known as "Y-shaped cutaneous ureterostomy" instead of transureteral cutaneous ureterostomy because of its similarity to the method of Roux for Y-shaped digestive tract anastomoses, and because this new terminology is simpler and describes the operative procedure in a clearer manner.

RESUMO

Repair operations were performed in 10 men with recent ruptures of the posterior urethra, between 1972 and 1980. One-stage segmental urethrectomy and end-to-end urethrorrhaphy was performed immediately (5 cases) or deferred to a late date (5 cases), with (2 cases) or without (8 cases) protective cystostomy. The origin of the lesions in these cases is discussed. The results obtained in these 10 patients are compared with those observed in 6 other cases treated urgently by cystostomy, with or without perineal drainage, followed by urethral reconstruction after a period of 2 to 6 months. Healing occurred within a mean period of 27 days in the first group, and 185 days in the second. Urinary fistulae developed in 20 p. cent of the patients in the first group, and 17 p. cent of those in the second group. Urinary continence was good in 100 p. cent of the first and 67 p. cent of the second group, a sterile urine being obtained in 100 p. cent and 83 p. cent of cases respectively. Sexual disorders were reported in 10 p. cent of the first and 67 p. cent of the second group. The authors conclude that urgent immediate or deferred reconstruction should be conducted in recent ruptures of the posterior urethra in men. Micturition urethrography after IVU. and retrograde urethrography, are essential investigations for detecting the lesions. Retrograde urethrography should be conducted with a small quantity (10 ml) of contrast medium, as soon as possible after plastic surgery. The authors employ segmental urethrectomy followed by end-to-end urethrorrhaphy, which has given better results than Solovov-Badenoch's urethroplasty.

RESUMO

The authors present their personal experience in connection with two cases to priapism, one of which was solved by unilateral saphenocavernous anastomosis in a leukemic patient, and the second one by bilateral caverno-spongious anastomosis in a patient with idiopathic priapism. Both interventions had a good outcome. The emergency character is stressed, of this type of intervention, that should b performed bilaterally and followed by anticoagulant treatment.

Malignant tumors were responsible for 19 cases of urogenital fistula, including 10 ureterovaginal and 9 vesicovaginal fistulae, treated between 1974 and 1982. Details of the cases are reviewed and the therapeutic attitude to adopt towards urogenital fistulae of malignant tumor origin discussed. Recommended steps are: repeat plastic surgery as soon as possible after the postoperative appearance of the fistula, conservation of the kidney for as long as possible, and small intestine replacement of the ureter in selected cases. External as compared with internal urinary shunts are preferred in cases of advanced lesions with renal failure, tumor recurrence, or cobalt therapy. Cutaneous ureterostomy in Y with a single median or right lateral skin opening appears to be an excellent bypass procedure enabling a left colostomy to be performed, either primarily or secondary in case of rectal invasion.

RESUMO

Between 1985 and 1990, superficial epithelial tumors of gallbladder were found in 98 patients. Peculiar aspects such as age, sex, symptoms, clinical and laboratory diagnosis, surgical treatment, additional interventions, postoperative treatment, anatomo-pathological examination, pTNM classification and tumoral grading, postoperative recurrences, occurring after the primary and adjuvant treatment, were analysed. After some general comments, the authors point out that in their experience the lowest rate of postoperative recurrences was obtained by a multimodal treatment, that is surgery and vesical instillations with cytostatics or B.C.G. vaccine, according to EORCT scheme.

RESUMO

157 cases of malignant infiltrative tumors of urinary bladder admitted and treated in a 5-year interval (1983-1988) are presented. Considerations on age, sex, urological history, associated urological disorders, symptoms and clinical examination, diagnostic methods, surgical procedures and their association with other non-surgical procedures, postoperative evolution, pathological examination of the resection sample, tumoral stage, recurrences are made. Five cases treated by various methods are presented in detail. The clinical and laboratory diagnosis, therapeutical methods and the staged therapeutical indications are also commented upon. In the malignant infiltrative tumors of urinary bladder the authors recommend the partial cystectomy which, when correctly indicated and performed, gave similar results as total cystectomy. It is also underlined that in case of an urographically silent kidney the attitude should be conservative and the treatment of the malignant infiltrative tumors of urinary bladder should be multimodal.

RESUMO

The authors present their experience of 122 partial cystectomy cases made on a number of 288 vesical tumors in our clinic, during 5 years (1986-1991) from which 23 are superficial tumors and 99 are infiltrative ones. The treatment of vesical tumors can be done in many ways; this partial cystectomy must be completed with an other therapeutic method: irradiation on chemotherapy in conformity with the anatomo-pathological form, the steady and tumoral grading.

Thirty-five cases of upper urinary tract tumors treated by the authors in an interval of 10 years (1979-1988) are presented. After a detailed analysis of the cases some general considerations on the anatomopathologic forms of upper urinary tract tumors, stage classification, symptoms and clinical and laboratory diagnosis, therapeutical indications are made.

RESUMO

One hundred and thirteen new cases of urogenital tuberculosis, admitted and treated in the interval 1980-1986 (an average number of 16 cases yearly) are reported. The analysis of these patients includes age, sex, bacillary history, symptoms, clinical forms, diagnostic methods, therapeutical management and late results. Seven cases are detailed. It is concluded that the treatment with the actual tuberculostatic drugs is efficient provided the urogenital tuberculosis is early detected. In case this disease is late detected the treatment is associated, drug and surgical.
