The aims of the present study were to estimate the frequency of change in self-rated health (SRH) among community-dwelling older adults, between two measures taken at a 9-year interval; and determine factors associated with a decline and an improvement in SRH, in relation to aspects of physical/emotional health and subjective wellbeing. Data were derived from a community-based study on frailty among Brazilian elderly. Associations were investigated using Pearson's chi-square test and relative risk ratios were estimated using multinomial logistic regression analysis. 39.3% of participants did not change their SRH at both assessment times, 21.7% rated it as worse and 39.0% rated it as better. The relative risk ratio of an improvement in SRH for individuals with disability in basic activities of daily living (ADLs) was lower than for individuals with independence in basic ADLs (IRR=0.22; IC95%: 0.08-0.63). Understanding the complex interactions between self-rated health and the dimensions that influence the improvement of health perception may shed light on key determinants of the wellbeing among older adults.

OBJECTIVE: To characterize the profile of transvestites and transgender women participating in a survey regarding tuberculosis and HIV/Aids in the city of São Paulo. METHOD: This cross-sectional study employed the Knowledge, Attitudes and Practices (KAP) questionnaire adapted for data collection. Sociodemographic aspects as well as aspects related to history of imprisonment, process of gender transition, use of condoms, previous treatment of sexually transmitted diseases and tuberculosis and knowledge regarding HIV/Aids were addressed. RESULTS: The study included 58 transvestites and 66 transgender women. There were differences between the two groups regarding schooling (P = 0.008), prostitution (P < 0.001), imprisonment (P < 0.001), silicone injections (P = 0.005), and hormone therapy without medical guidance (P = 0.004). All trans women (100%) and 80% of transvestites mentioned treatment of syphilis; 25.9% of transvestites reported tuberculosis/HIV co-infection. The investigation regarding knowledge of HIV/AIDS revealed inconsistent use of condoms and mistaken beliefs related to modes of transmission. CONCLUSION: The present results show that transvestites and trans women were vulnerable to tuberculosis and HIV/AIDS. The differences observed between the two groups suggest the need for different approaches to transvestites and transgender women in future studies.

BACKGROUND: Smoking and unhealthy diet are important risk factors for cardiovascular and metabolic diseases, contributing to public health crises. OBJECTIVE: To evaluate the consumption of natural/minimally processed and ultra-processed foods by Brazilian adults (18-59 years old) according to smoking status. DESIGN AND SETTING: Cross-sectional study of a representative population sample from 26 state capitals and the Federal District (Brazil-2018). METHODS: Data were obtained from Vigitel-Surveillance System for Risk and Protection Factors for Chronic Diseases by Telephone Survey. Participants were categorized as smokers, ex-smokers, and never smokers. Multinomial logistic regression was used for analyses. RESULTS: Of the 30,800 adults evaluated, 9.4% (95%CI: 8.7-10.2) were smokers and 16.5% (95%CI: 15.8-17.3) were ex-smokers. Smokers were less likely to consume fruit and natural juice, and more likely to consume soda or artificial juice (≥ 5 days/week) than ex-smokers and never smokers. Regarding the daily frequency of consumption, smokers were observed to be less likely to eat fruit more than 1 time/day and more likely to drink ≥ 3 cups/cans of soda/day. Compared to never smokers, smokers had a 42% higher chance of consuming ≥ 3 glasses of natural juice/day. On the day before the interview, fruit, milk, tubers, squash, and okra consumption were lower among smokers than non-smokers. Smokers were more likely to report consuming soft drinks, fruit juice, sauces, ready-made dishes, margarine, and sausages. CONCLUSION: Smokers had lower fruit consumption, and higher consumption of natural juices and ultra-processed foods. We highlight the need for strategies that encourage healthy eating and smoking cessation.

OBJECTIVE: To evaluate mortality trends before and after the anti-influenza vaccination campaigns among the elderly in Brazil. METHODS: This was an ecological time-series study of mortality from respiratory diseases among the elderly living in the state of São Paulo, Brazil, in 1980 - 2009. Mortality rates were calculated using death data from the Ministry of Health's Mortality Information System and population data from the Brazilian Institute of Geography and Statistics. Polynomial regression models were used to evaluate mortality trends by sex and age group (60 - 69 years; 70 - 79, and 80 years or older) before and after the beginning of influenza vaccination campaigns. RESULTS: An increase in the respiratory mortality rates was observed in 1980 - 1998, mainly among males. The rate and velocity of the increase was higher among the older age groups. In the years following the vaccine campaigns (1999 - 2009), respiratory mortality rate trends by sex and age stabilized in São Paulo state. CONCLUSIONS: A greater reduction in mortality rates would be observed if vaccinal coverage against influenza were greater and more homogenous. New strategies to increase the uptake of vaccine among the elderly are needed to reach higher coverage levels, especially in municipalities with older and larger populations.

The scope of this article was to evaluate the influence of multimorbidity and associated effects on the activities in the day-to-day lives of community-dwelling elderly individuals. It involved a cohort study with data from the FIBRA Study, the baseline (2008-2009) and follow-up (2016-2017). The basic activities in daily living (ADL) were evaluated using Katz's index, and the chronic diseases were classified as: (1) multimorbidity and multimorbidity patterns; (2) cardiopulmonary; (3) vascular-metabolic; and (4) mental-musculoskeletal. The chi-square test and Poisson regression data were used for analysis. A total of 861 older adults with no functional dependency at baseline were analyzed. Elderly individuals with multimorbidity (RR = 1.58; 95%CI: 1.19-2.10) and classified according to cardiopulmonary (RR = 2.43; 95%CI: 1.77-3.33), vascular-metabolic (RR = 1.50; 95%CI: 1.19-1.89) and mental-musculoskeletal (RR = 1.30; 95%CI: 1.03-1.65) had a higher risk of presenting functional decline in ADL in the follow-up compared to those who didn't have the same disease patterns. Multimorbidity and its patterns increased the risk of functional disability in older adults over the nine-year period.


O objetivo foi avaliar a influência da multimorbidade e seus padrões nas atividades básicas de vida diária da pessoa idosa residente na comunidade. Trata-se de estudo de coorte com dados provenientes do Estudo FIBRA, linha de base (2008-2009) e seguimento (2016-2017). As atividades básicas de vida diária (ABVD) foram avaliadas pelo questionário de Katz e as doenças crônicas foram classificadas como (1) multimorbidade e padrões de multimorbidade: (2) cardiopulmonar; (3) vascular-metabólico; e (4) mental-musculoesquelético. Para a análise de dados, utilizou-se o teste qui-quadrado e a regressão de Poisson. Foram analisados 861 indivíduos sem limitação para ABVD na linha de base. As pessoas idosas com multimorbidade (RR = 1,58; IC95%: 1,19-2,10) e classificados nos padrões cardiopulmonar (RR = 2,43; IC95%: 1,77-3,33), vascular-metabólico (RR = 1,50; IC95%: 1,19-1,89) e mental-musculoesquelético (RR = 1,30; IC95%: 1,03-1,65) tiveram maior risco de apresentar declínio funcional nas ABVD no seguimento em comparação aos que não tinham os mesmos padrões de doenças. A multimorbidade e seus padrões aumentaram o risco de incapacidade na pessoa idosa ao longo de nove anos.

Information about the prevalence of specific chronic diseases and the ownership of a health plan can help size and monitor care demands. This study aimed to estimate and compare the prevalence of chronic diseases among the elderly, according to the possession of a health plan in 2013 and 2019. This is a population-based cross-sectional study with data from elderly people (age ≥ 60 years) from the Brazilian National Health Survey (2013: n = 11,177; 2019: n = 22,728). The prevalence of self-reported chronic diseases and adjusted prevalence ratios were estimated, according to health plan ownership (medical and/or dental) and by year. In the period, increased prevalence was observed for hypertension (PR = 1.11; 95%CI: 1.06-1.16), diabetes (PR = 1.12; 95%CI: 1.01-1.24), heart disease (PR = 1.21; 95%CI: 1.05-1.39), stroke (PR = 1.27; 95%CI: 1.04-1.54), back pain (PR = 1.14; 95%CI: 1.05-1.23), hypercholesterolemia (PR = 1.09; 95%CI: 1.01-1.18), and depression (PR = 1.23; 95%CI: 1.05-1.43) among those without a health plan. In 2019, arthritis/rheumatism (PR = 1.21; 95%CI: 1,03-1,43), hypercholesterolemia (PR = 1.13; 95%CI: 1.01-1.26), and depression (PR = 1.26; 95%CI: 1.03-1.53) increased among elderly patients with a health plan. The findings showed differences in the prevalence of chronic diseases according to health plan ownership and an increase for some diseases in the period. Health promotion policies with an emphasis on reducing modifiable risk factors need to be maintained and intensified. Particularly for the elderly population, the importance of expanding actions focused on case tracking and early diagnosis, prevention and control of complications that favor equity in care is highlighted.


Informações sobre prevalências de doenças crônicas específicas e posse de plano de saúde podem contribuir para o dimensionamento e monitoramento de demandas assistenciais. O objetivo do estudo foi estimar e comparar as prevalências de doenças crônicas em pessoas idosas, conforme posse de plano de saúde em 2013 e 2019. Trata-se de um estudo transversal de base populacional com dados de pessoas idosas (idade ≥ 60 anos) da Pesquisa Nacional de Saúde (2013: n = 11.177; 2019: n = 22.728). Estimaram-se as prevalências das doenças crônicas autorreferidas e razões de prevalência ajustadas, segundo posse de plano de saúde (médico e/ou odontológico) e ano. No período, houve elevação das prevalências de hipertensão (RP = 1,11; IC95%: 1,06-1,16), diabetes (RP = 1,12; IC95%: 1,01-1,24), doença do coração (RP = 1,21; IC95%: 1,05-1,39), AVC (RP = 1,27; IC95%: 1,04-1,54), problema na coluna (RP = 1,14; IC95%: 1,05-1,23), hipercolesterolemia (RP = 1,09; IC95%: 1,01-1,18) e depressão (RP = 1,23; IC95%: 1,05-1,43) naqueles sem plano de saúde. Em 2019, artrite/reumatismo (RP = 1,21; IC95%: 1,03-1,43), hipercolesterolemia (RP = 1,13; IC95%: 1,01-1,26) e depressão (RP = 1,26; IC95%: 1,03-1,53) aumentaram nas pessoas idosas com plano. Os achados mostraram diferenças nas prevalências das doenças crônicas segundo posse de plano de saúde e aumento para algumas doenças no período. As políticas de promoção de saúde com ênfase na redução dos fatores de risco modificáveis precisam ser mantidas e intensificadas. Particularmente na população idosa, ressalta-se a importância da ampliação de ações voltadas para o rastreamento de casos e diagnóstico precoce, prevenção e controle de complicações que favoreçam a equidade no cuidado.


La información sobre las prevalencias de determinadas enfermedades crónicas y la posesión del plan de salud pueden contribuir a dimensionar y monitorear las demandas asistenciales. El objetivo del estudio fue estimar y comparar las prevalencias de enfermedades crónicas en las personas mayores, conforme la posesión del plan de salud en 2013 y 2019. Se trata de un estudio transversal de base poblacional con datos de personas mayores (edad ≥ 60 años) de la Encuesta Nacional de Salud brasileña (2013: n = 11.177; 2019: n = 22.728). Se estimaron las prevalencias de las enfermedades crónicas autoinformadas y razones de prevalencia ajustadas, según posesión de plan de salud (médico y/o odontológico) y año. En el periodo, se produjo un aumento de la prevalencia de la hipertensión (RP = 1,11; IC95%: 1,06-1,16), diabetes (RP = 1,12; IC95%: 1,01-1,24), enfermedad del corazón (RP = 1,21; IC95%: 1,05-1,39), AVC (RP = 1,27; IC95%: 1,04-1,54), problema en la columna (RP = 1,14; IC95%: 1,05-1,23), hipercolesterolemia (RP = 1,09; IC95%: 1,01-1,18) y depresión (RP = 1,23; IC95%: 1,05-1,43) en aquellos sin seguro de salud. En el 2019, artritis/reumatismo (RP = 1,21; IC95%: 1,03-1,43), hipercolesterolemia (RP = 1,13; IC95%: 1,01-1,26) y la depresión (RP = 1,26; IC95%: 1,03-1,53) aumentaron en las personas mayores con un plan. Los resultados mostraron diferencias en las prevalencias de las enfermedades crónicas según la posesión de plan de salud y un aumento para algunas enfermedades en el período. Es necesario mantener e intensificar las políticas de promoción de la salud con énfasis en la reducción de los factores de riesgo modificables. Particularmente en la población adulta mayor, se resalta la importancia de aumentar el seguimiento de casos y de diagnóstico precoz, prevención y control de complicaciones que favorezcan la equidad en el cuidado.

BACKGROUND: A healthy diet is a protection factor against type 2 diabetes and plays an important role in the treatment of the disease, as well as associated comorbidities. OBJECTIVE: Characterize the eating habits of older adults (≥ 65 years) with and without diabetes residing in capital cities and the Federal District of Brazil. METHODS: A cross-sectional study was conducted using data from the Surveillance of Risk and Protection Factors for Chronic Diseases Through a Telephone Survey (Vigitel, 2016). The prevalence of diabetes mellitus was estimated according to sociodemographic variables, physical inactivity level, self-rated health status and body mass index. Dietary habits were assessed based on the frequency (weekly and daily) of consumption of healthy and unhealthy foods and the replacement of food by snacks. Differences were determined using Pearson's chi-square test (Rao-Scott), with the significance level set at 5%. RESULTS: A total of 13,649 older adults were interviewed. The prevalence of self-reported diabetes was 27.2% (95% CI: 25.5; 29.0). Compared to non-diabetics, diabetic individuals had a higher consumption of raw vegetables (32.1% vs. 26.5%/3-4 days/week) and lower consumption of chicken (3.8% vs. 6.4%/hardly ever/never), fruit juice (24.0% vs. 29.6%) and sweets (6.8% vs. 16.2%) ≥ 5 days/week. Compared to non-diabetics, diabetic individuals consumed more skim milk (51.5% vs. 44.6%) and diet soda (60.0% vs. 17.3%) ≥ 5 days/week, raw vegetables (9.1% vs. 2.5%/at dinner) and sweets (37.7% vs. 20.5%/twice/day) 3-4 days/week. CONCLUSION: The observed differences emphasize the need for healthy eating interventions for all older adults, as well as specific counseling for those with diabetes.


FUNDAMENTOS: A alimentação saudável é um fator de proteção contra o diabetes tipo 2 e desempenha importante papel no tratamento do diabetes e das comorbidades associadas. OBJETIVO: Caracterizar o hábito alimentar de idosos diabéticos e não diabéticos com 65 anos ou mais, residentes nas capitais brasileiras e no Distrito Federal. MÉTODOS: Estudo transversal com dados da pesquisa Vigilância de Fatores de Risco e Proteção para as Doenças Crônicas por Inquérito Telefônico (Vigitel, 2016). Foram estimadas as prevalências de diabetes melito segundo variáveis sociodemográficas, inatividade física, autoavaliação da saúde e índice de massa corporal (IMC). O hábito alimentar foi avaliado pela frequência (semanal e diária) de consumo de alimentos saudáveis e não saudáveis, e pela substituição da comida por lanches. As diferenças foram verificadas por meio do teste Qui-quadrado de Pearson (Rao-Scott) com nível de significância de 5%. RESULTADOS: Foram entrevistados 13.649 idosos, e a prevalência de diabetes autorreferido foi de 27,2% (IC95%:25,5; 29,0). Nos pacientes diabéticos, observou-se maior consumo de hortaliças cruas (32,1% vs. 26,5%/3-4 dias/semana) e menor de frango (3,8% vs. 6,4%/quase nunca/nunca), suco (24,0% vs. 29,6%) e doces (6,8% vs. 16,2%) ≥5 dias/semana. Os percentuais de idosos com consumo de leite desnatado (51,5% vs. 44,6%) e refrigerante dietético (60,0% vs. 17,3%) ≥5 dias/semana, hortaliças cruas (9,1% vs. 2,5%/no jantar) e doces (37,7% vs. 20,5%/2 vezes/dia) 3-4 dias/semana foram maiores nos diabéticos, comparados aos não diabéticos. CONCLUSÃO: As diferenças observadas sinalizam a necessidade de promover intervenções para alimentação saudável entre todos os idosos, bem como orientações específicas para os diabéticos.

This article aims to estimate the prevalence of chronic diseases/conditions in octogenarians according to sex, age groups and private health insurance, and its relationship with difficulty in performing usual activities. Cross-sectional population-based study with elderly data (n = 6,098) from the National Health Survey (PNS) 2019. Prevalences and 95% confidence intervals were estimated. The prevalences were: hypertension 61.7%, chronic back problem 30.0%, hypercholesterolemia 22.0%, diabetes 20.3%; arthritis/rheumatism 19.4%, heart disease 19.3%, depression 9.4%, cancer 8.9%, cerebrovascular accident (CA) 7.5%, asthma 4.9%, lung disease (LD) 4.2% and kidney failure (KF) 3.0%. Hypertension, chronic back problem, hypercholesterolemia, major arthritis/rheumatism and depression in women, and cancer in men. Major heart disease, hypercholesterolemia, arthritis/rheumatism, KF, cancer and depression in those with private health insurance. Restriction of usual activities 14.8%, more frequent in cardiac patients, with chronic back problem, arthritis/rheumatism, KF, depression, CA, cancer and LD. There were higher prevalences in women and in those who have health insurance. Difficulties in usual activities related to diseases demand the expansion of care for the older adults.


O objetivo deste artigo é estimar a prevalência de doenças/condições crônicas em octogenários segundo sexo, faixas etárias e plano de saúde, e sua relação com a dificuldade para realização de atividades habituais. Estudo transversal de base populacional com dados de idosos (n = 6.098) da Pesquisa Nacional de Saúde 2019. Estimaram-se as prevalências e intervalos de confiança de 95%. As prevalências foram: hipertensão 61,7%, problema de coluna 30,0%, hipercolesterolemia 22,0%, diabetes 20,3%, artrite/reumatismo 19,4%, cardiopatias 19,3%, depressão 9,4%, câncer 8,9%, AVC 7,5%, asma 4,9%, doença pulmonar (DP) 4,2% e insuficiência renal (IR) 3,0%. Hipertensão, problema de coluna, hipercolesterolemia, artrite/reumatismo e depressão maiores nas mulheres, e câncer nos homens. Cardiopatias, hipercolesterolemia, artrite/reumatismo, IR, câncer e depressão maiores naqueles com plano de saúde. Restrição de atividades habituais, 14,8% mais frequente nos cardiopatas, com problema de coluna, artrite/reumatismo, IR, depressão, AVC, câncer e DP. Observaram-se maiores prevalências nas mulheres e nos que possuem plano de saúde. Dificuldades para atividades habituais relacionadas às doenças demandam a ampliação do cuidado aos mais idosos.

This study aimed to estimate the prevalence of non-vaccination and the reasons for nonadherence to the influenza vaccine among older Brazilians according to sociodemographic characteristics. A cross-sectional study was conducted with data from older people (≥ 60 years of age; n = 23,815) who participated in the 2013 National Health Survey. Frequencies of non-vaccination and the main reasons for nonadherence were calculated with respective 95% confidence intervals. The prevalence of non-vaccination was 26.9% (approximately 7,106,730 older people). The reason rarely gets the flu was the most cited among the men (28.2%), the 60-to-69-year-old age group (29.6%), individuals with higher education (41.9%), and those with health insurance (32.3%). Fear of a reaction was the most cited reason in the northeastern region (25.4%), among women (29.3%), longer-lived individuals (≥70 years; 28.7%), and those who did not know how to read/write (26.7%). A total of 12.1% reported not believing in the vaccine's protection, and 5.5% did not know that it was necessary to take vaccine. The proportions of the main reasons for non-vaccination varied by sociodemographic characteristics. This study's findings highlight the need to increase older people's knowledge regarding influenza and influenza vaccines. Healthcare providers should be encouraged to counsel older people-especially those in subgroups with lower adherence, such as residents in the Northeast region, those aged 60-69 years, those who do not know how to read/write, those without a spouse/companion, and those without health insurance-regarding the different aspects of the vaccine and formally indicate it for groups at risk.

OBJECTIVE: To estimate the prevalence of use of oral medications for the treatment of diabetes, as well as the distribution of sources for obtaining according to sociodemographic variables, in the Brazilian states' capitals and in the Federal District, and their evolution from 2012 to 2018. METHODS: Cross-sectional and population-based study with individuals aged ≥ 20 years who reported a medical diagnosis of diabetes, interviewed through Vigitel from 2012 to 2018. We estimated the prevalence of use and the distribution of sources for obtaining according to sociodemographic variables (95%CI). We checked differences among proportions using the Pearson's χ2 test (Rao-Scott), with a significance level of 5%. RESULTS: There was an increase in the prevalence of use of oral medications for the treatment of diabetes from 77.4 to 85.2% between 2012 and 2018, and a decrease in obtaining in the Health Unit Pharmacies of the Unified Health System (SUS), while there was an increase in obtaining in Popular Pharmacies. CONCLUSIONS: In Brazil, SUS remained the main source for obtaining oral antidiabetic drugs, financing more than 70% of them in the country, considering the Health Unit Pharmacies and Popular Pharmacies, thereby showing the importance of public Pharmaceutical Policies in guaranteeing the access to medications by the Brazilian population, as well as in reducing inequities in the country. Nevertheless, the migration of obtaining by users from SUS Health Units to Popular Pharmacies suggests the weakening the responsibility of Primary Health Care in the provision oral antidiabetic drugs, thereby undermining the bond and the longitudinal care.


OBJETIVO: Estimar as prevalências de uso de medicamento oral para tratamento de diabetes, bem como a distribuição das fontes de obtenção segundo variáveis sociodemográficas, nas capitais dos estados brasileiros e no Distrito Federal e sua evolução no período de 2012 a 2018. MÉTODOS: Estudo transversal de base populacional com indivíduos de 20 anos ou mais que referiram diagnóstico médico de diabetes, entrevistados pelo Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico (Vigitel) de 2012 a 2018. Estimaram-se a prevalência de uso e a distribuição das fontes de obtenção segundo variáveis sociodemográficas (IC95%). Verificaram-se as diferenças entre as proporções pelo teste χ2 de Pearson (Rao-Scott), com nível de significância de 5%. RESULTADOS: Houve aumento na prevalência de uso de medicamento oral para tratamento de diabetes de 77,4 para 85,2%, entre 2012 e 2018, e diminuição da obtenção nas farmácias de unidade de saúde do Sistema Único de Saúde (SUS) com aumento da obtenção nas farmácias populares. CONCLUSÕES: O SUS manteve-se como a principal fonte de obtenção de antidiabéticos orais no Brasil, financiando mais de 70% dos medicamentos orais para tratamento de diabetes no país, considerando as farmácias de unidades de saúde e as farmácias populares, mostrando, assim, a importância das políticas farmacêuticas públicas na garantia do acesso a medicamentos pela população brasileira e na diminuição das iniquidades no país. Contudo a migração da obtenção pelos usuários nas unidades de saúde do SUS para as farmácias populares sugere enfraquecimento da responsabilidade da atenção primária à saúde na oferta de medicamentos antidiabéticos orais, fragilizando o vínculo e o cuidado longitudinal.

This study aimed to assess characteristics of healthcare networks in four large Brazilian cities (Campinas, Fortaleza, Porto Alegre, and São Paulo), in the provision of mental healthcare. The following outcomes were used: (i) place of identification of the mental health problem; (ii) mental healthcare in primary care; (iii) pharmaceutical care in mental health; and (iv) social rehabilitation. This is a mixed-methods study with a concurrent and sequential approach, conducted with 10 administrators and 1,642 users of Centers for Psychosocial Care (CAPS, in Portuguese) in the four cities. The study showed the persistence of high-complexity services such as hospitals as the site for initial identification of mental health problems in Campinas (40% of users) and Fortaleza (37%); low proportion of mental health treatment in primary care (Fortaleza, 23%); differences between cities in psychotropic medication prescription in primary care (Porto Alegre, 68%; São Paulo, 64%; Campinas, 39%; Fortaleza, 31%) and in shortages of prescribed medication (higher in Fortaleza, 58%; lower in Campinas, 28%); and overall frailty in enabling return to work (lower in São Paulo, 17%; higher in Campinas, 39%), with better overall results regarding religion and leisure activities (higher in São Paulo, 53% and 56%, respectively). The study contributes to the discussion of the Brazilian scenario of mental healthcare, with evidence of persistent inequalities in the national context, pointing to gaps in some mental healthcare network configurations with the potential for better performance and longitudinal follow-up.


O objetivo deste estudo foi avaliar as características das redes de saúde de quatro grandes municípios brasileiros (Campinas, Fortaleza, Porto Alegre e São Paulo) no que diz respeito à prestação de cuidados em saúde mental. Foram usados como desfechos: (i) local de identificação do problema de saúde mental; (ii) atendimento em saúde mental na atenção básica; (iii) assistência farmacêutica em saúde mental; e (iv) reinserção social. Trata-se de um estudo analítico de métodos mistos, de abordagem concomitante e sequencial, conduzido com 10 gestores e 1.642 usuários de Centros de Atenção Psicossocial (CAPS) dos municípios citados. Observou-se a persistência de serviços de alta complexidade, tais como os hospitais, como locais de identificação inicial dos problemas de saúde mental em Campinas (40% dos usuários) e Fortaleza (37%); baixa proporção de tratamento de saúde mental na atenção básica (Fortaleza, 23%); diferenças entre os municípios no que diz respeito à prescrição de medicamentos psicotrópicos na atenção básica (Porto Alegre, 68%; São Paulo, 64%; Campinas, 39%; Fortaleza, 31%), bem como na falta dos medicamentos prescritos (maior em Fortaleza, 58%; menor em Campinas, 28%); e fragilidade em geral na retomada de atividades laborais (menor em São Paulo, 17%; maior em Campinas, 39%), havendo melhores resultados em geral em relação a atividades religiosas e de lazer (maiores em São Paulo, 53% e 56%, respectivamente). É um estudo que contribui para a discussão do panorama brasileiro da assistência à saúde mental, com evidências da persistência de desigualdades no contexto nacional, e aponta lacunas em algumas configurações das redes de saúde mental com potenciais para melhor desempenho e seguimento longitudinal.

This article aims to estimate the risk of death according to sociodemographic characteristics, chronic diseases, frailty, functional capacity, and social participation in older people as well as determine the median time of death in relation to health status and social participation. A retrospective longitudinal study was conducted with older people (≥65 years) in 2008-09 and 2016-17 in the city of Campinas and the subdistrict of Ermelino Matarazzo in the city of São Paulo. Face-to-face interviews were conducted at community centers and the participants' homes. The cumulative incidence of death was estimated and associations with the predictor variables were analyzed using Poisson multiple regression. The Kaplan-Meier method and the log-rank test were also used. Among the 741 individuals located at follow-up, 192 had deceased. The incidence of death was higher among those who reported having heart disease and those who were dependent on others regarding the performance of instrumental activities of daily living. The incidence of death was lower among women, individuals in the highest income stratum, and those who performed three or more activities related to social inclusion. No differences in median survival times were found. Predictors of mortality can contribute to broadening knowledge on the singularities of the aging process.


O artigo tem por objetivos estimar o risco de óbito segundo características sociodemográficas, doenças crônicas, fragilidade, capacidade funcional e participação social em idosos e verificar, para as variáveis de estado de saúde e participação social, o tempo mediano de ocorrência do óbito. Estudo longitudinal retrospectivo com idosos (≥65 anos), realizado em 2008-09 e 2016-17 em Campinas-SP e Ermelino Matarazzo-SP. Realizaram-se entrevistas face a face em centros de convivência e nos domicílios. Estimou-se a incidência acumulada de óbito e associações com as variáveis preditoras foram analisadas pela regressão múltipla de Poisson. O método de Kaplan-Meier e o teste de Log-rank foram utilizados. Dos 741 idosos localizados no seguimento, 192 faleceram. Observou-se maior incidência de óbito nos mais idosos, nos que relataram doença do coração e nos dependentes para realização de atividades instrumentais da vida diária. Verificou-se menor incidência de óbito nas mulheres, no estrato com maior renda e nos que realizavam três ou mais atividades relacionadas à inserção social. Não se observaram diferenças nos tempos medianos de sobrevida. Os preditores de mortalidade podem contribuir para ampliar o conhecimento sobre as singularidades do processo de envelhecimento.

OBJECTIVE: To report the main results of studies on prejudice, stereotyping, and age-based discrimination (ageism) in the context of the COVID-19 pandemic. METHODS: This is an integrative review of the literature on ageism in the context of the COVID-19 pandemic, conducted between May and June 2020, with data collected from the following databases: Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Web of Science (Thompson Reuters), Scopus (Elsevier Science), Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) and Scientific Eletronic Library Online (SciELO). RESULTS: Twenty-one publications addressing ageism during the pandemics, its origins, consequences, and ethical and political implications were analyzed. All publications were theoretical with a critical/reflexive approach, being 90,5% opinion articles (n = 19) and 9,5% research (n = 2). The main findings indicate criticisms regarding resources allocation and intensive care based exclusively on age. The results also highlight the impacts of social isolation, the use of technologies and social media, and intergenerational relationships within the COVID-19 scenario. CONCLUSION: According to most publications, although ageism has always been present, it became more evident during the COVID-19 pandemic as a form of discrimination against older adults. "Ageist" discourses may exert a negative influence in older adults' lives, causing severe social and psychological impacts.

OBJECTIVE: To estimate the prevalence of multimorbidity in long-lived Brazilian individuals (age ≥80 years) and to associated it with the use of health services. METHODS: Cross-sectional population-based study with data from the 2019 National Survey of Health (n=6,098). Frequencies of use of services were estimated for older people with multimorbidity and according to sex, health insurance ownership, and self-rated health. The prevalence rates, crude and adjusted prevalence ratios, and the respective 95% confidence intervals were calculated. RESULTS: The average age of the older adults was 85 years and about 62% were women; the prevalence of multimorbidity was 57.1%, higher in women, in those who have health insurance, and who reside in the southern region of the country (p<0.05). In the oldest old with multimorbidity, the use of services in the last 15 days reached 64.6%, and more than 70% were hospitalized in the last year or did not carry out activities in the previous two weeks for health reasons. Differences were observed for the indicators of service use in relation to sex, health insurance ownership, and self-rated health, according to multimorbidity. CONCLUSION: Indicators for the use of health services were higher in older individuals who have two or more chronic diseases, regardless of sociodemographic conditions and self-rated health, showing the impact of multimorbidity per se in determining the use of services among the oldest old.

The aim of this study was to estimate the prevalence of non-vaccination against influenza among Brazilian older adults with systemic arterial hypertension and determine the main reasons for non-adherence. A cross-sectional study was conducted using data from older adults (≥ 60 years of age) with hypertension who participated in the 2013 National Health Survey and reported not having been vaccinated against flu over the previous 12 months (n = 1,295). The analyses were performed using the Stata 14.0 software. The data were weighted because of the sampling design. An estimated 3,026,080 older adults with hypertension had not received a flu vaccine over the 12 months prior to the survey (22.6%). No significant associations were found with sex, age group or schooling. The prevalence of unvaccinated older adults was lower in the southern and southeastern regions of Brazil than in the northern and northeastern regions, even after adjusting for age. The prevalence was higher among individuals without private health insurance. The main reasons for non-vaccination were fear of a reaction, rarely having the flu and not believing in the protection of the vaccine. The present findings underscore the need for healthcare professionals to explain to the population the benefits of the vaccine for preventing severe influenza (protective effect and possible reactions) and for secondary prevention of cardiovascular events. Increasing the prevalence of vaccination in older adults with hypertension and other cardiovascular diseases is of fundamental importance within the realm of public health as a strategy for reducing occurrences of complications and deaths associated with influenza.

Objective to identify cut-off points for waist circumference (WC) and waist-to-height ratio (WHtR) with better sensitivity, specificity and accuracy to distinguish overweight elderly people by sex, using three body mass index criteria as anthropometric references. Methods this was a cross-sectional study that analyzed weight, height and WC measurements of 3,477 elderly people (2008-2009); receiver operating characteristic (ROC) curves were used to define the cut-off points. Results WC cut-off points were 86.5cm, 88.7cm and 91.5cm in women, and 93.0cm, 96.0cm and 97.5cm in men, according to World Health Organization/WHO, Nutrition Screening Initiative/NSI and Pan American Health Organization/PAHO criteria, respectively; WHtR cut-off points corresponded to 0.57, 0.58, 0.59 in the female sex, and 0.56, 0.58 and 0.59 in the male sex; highest absolute overall accuracy values: PAHO (men) and WHO (women). Conclusion WC and WHtR cut-off points were similar, regardless of the BMI criterion adopted.

The aim of the present study was to estimate the prevalence of psychotropic drug utilization among adults and seniors and determine associated factors, therapeutic classes and sources of acquisition. Data from the Brazilian National Survey on Access, Use and Promotion of the Rational Use of Medicines (2013-2014) were analyzed. The prevalence of use of at least one psychotropic drug in the overall sample (adults and seniors) was 8.7%. In the adjusted analyses, positive associations were found between the use of psychotropic drugs and the female sex, poorer self-rated health and chronic diseases (p < 0.05). The most frequently used therapeutic classes were antidepressants (55.3%) by the adults and anxiolytics (59.3%) by the seniors. Approximately 23.0% of psychotropic drugs were obtained exclusively from pharmacies of the public healthcare system and 77.0% were acquired from other sources. The findings reveal a low proportion of attaining psychotropic drugs through the Brazilian public healthcare system as well as the need for public policies that encourage the rational use of prescriptions and treatments to promote a better quality of life and ensure the population's right to health.


O objetivo do presente estudo foi verificar a prevalência do uso de psicotrópicos nos adultos e idosos e os fatores associados, classes terapêuticas de medicamentos e fontes de obtenção. Foram analisados dados da Pesquisa Nacional sobre o Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM/2013-2014). A prevalência do uso de ao menos um psicotrópico para o conjunto de adultos brasileiros foi de 8,7%. Nas análises ajustadas para adultos e idosos, observaram-se associações positivas entre uso de psicotrópicos e sexo feminino, pior autoavaliação de saúde e presença de doenças crônicas (p < 0,05). As classes terapêuticas mais utilizadas foram os antidepressivos (55,3%) por adultos e os ansiolíticos (59,3%) por idosos. Cerca de 23,0% dos psicotrópicos foram obtidos exclusivamente nas farmácias do SUS e os outros 77,0% por outras fontes. Os resultados mostraram baixa proporção de obtenção dos psicotrópicos no SUS e a necessidade de políticas que incentivem a prescrição e tratamentos com mais racionalidade, promovendo melhor qualidade de vida e garantia do direito à saúde a população.

OBJECTIVE: The prevalence of hypertension in Brazil and worldwide has been increasing in recent decades, and drug therapy is one of the strategies used to control this condition. The objective of this study was to estimate the prevalence of use and identify the sources for obtaining antihypertensive drugs in Brazil, according to sociodemographic variables, comparing three periods: 2011, 2014 and 2017. METHODS: Data from individuals aged ≥20 years who reported a medical diagnosis of hypertension, interviewed by Vigitel in 2011, 2014 and 2017 were used. Frequency and prevalence of drug use in addition to the sources for obtaining medication were estimated by sociodemographic variables, with 95% confidence intervals. The differences between proportions were verified by Pearson's chi-square test (Rao-Scott), with a significance level of 5%. RESULTS: The prevalence of antihypertensive drug use remained stable (80%). Regarding the sources for obtaining these medicines, there was variation in the period, indicating a decrease in usage through the Brazilian Unified Health System (SUS) (44.2% in 2011; 30.5% in 2017). This decrease was accompanied with increase in PFPB (16.1% in 2011; 29.9% in 2017). The prevalence of other sources for obtaining medicine (private pharmacies/drugstores) showed stability in the period. CONCLUSIONS: The prevalence of medication use remained high and there was a change in the pattern of use according to sources, demonstrating migration between SUS pharmacies to the PFPB, and suggesting a reduction in the availability of medicines from public pharmacies universally, and for free.

In view of the epidemiological relevance of periodontal disease and chronic noncommunicable diseases, the study aimed to evaluate the relationship between them through subclinical indicators of systemic risk in a population group with healthy habits, including alcohol and tobacco abstinence. A complete periodontal examination of six sites per tooth was performed in a sample of 420 participants from the Advento study (Sao Paulo), submitted to anthropometric and laboratory evaluation. Periodontitis was defined and classified based on the Community Periodontal Index score 3 (periodontal pocket = 4-5 mm) and score 4 (periodontal pocket ≥ 6 mm). The prevalence of mild/moderate and severe periodontitis was 20% and 8.2%, respectively. Both categories of periodontal disease had significantly higher levels of triglycerides, C-reactive protein, calcium score, and calcium percentile, whereas blood glucose after tolerance test was significantly higher among people with severe periodontitis and HDL-c levels were lower (p < 0.05). Young adults with severe periodontitis had significantly higher prevalence of obesity, pre-diabetes, hypertension, and metabolic syndrome. Besides these conditions, the older adults with severe periodontitis had significantly higher prevalence of dyslipidemia and subclinical atherosclerosis. The group with periodontitis had also a higher coronary heart disease risk based on the PROCAM score (p < 0.05). The results indicated associations of periodontitis with several systemic indicators for chronic noncommunicable diseases, and highlighted the need for multiprofessional measures in the whole care of patients.
