The objective of this study was to determine associated factors to the consumption of fruit and vegetables (FV) in the adult population of each socioeconomic status (SES) in Bucaramanga, Colombia. We conducted a cross-sectional in a sample of 317 people and we measured socio-demographic, economic and anthropometric variables and dietary factors related to the purchase and consumption of FV. We made a survey, a 24-Hour dietary recall and we did anthropometric measurements. Portion sizes were estimated with a standardized set of utensils and food-photography atlas. Frequencies, measures of central tendency and dispersion were established and Logistic Regression models were used for the bi and multivariate analysis. Average consumption of FV was 162.6 g/day (188.5 g in high SES and 145.7 g in low SES, p = 0.056). On average 2 servings per day of FV (2.3 servings in high SES and 1.5 in low SES, p = 0.055). The prevalence of adequate intake of FV (more than 5 servings / day) was 8.6% (8.9% in high SES and 8.4% in low SES, p = 0.896). The associated factor to consumption of FV was BMI > or =25 (OR: 0.31, CI (95%): 0.11-0.89, p = 0.030), adjusted for occupation, income, SES, and number of family members. FV consumption is low; excess weight is the only variable associated to no-consumption of five or more servings; causes: cost to the consumption of fruit and flavor in the case of vegetables.

BACKGROUND: In recent years, the role of vitamin D (VD) as a protective factor in cardiovascular disease has been recognized. Thus, there is a need to study the effect of vitamin D supplementation in the control of different cardiovascular risk factors and metabolic syndrome, especially in young populations where few studies have been conducted. METHODS: Pilot study of a randomized, parallel two-arm, triple-blind clinical controlled trial in 150 adolescents and young adults in the city of Bucaramanga-Colombia. The intervention group will receive 1000 IU of VD and the control group 200 IU of VD daily for 15 weeks. The main outcomes are: serum calcifediol levels (25(OH) D), body mass index and lipid profile; secondary outcomes are complementary to the previous ones (skin folds, waist-hip ratio). Other variables will be analyzed such as assessment of dietary intake, physical activity, sun exposure, cigarette and tobacco consumption and compliance with VD supplementation. DISCUSSION: This study is innovative since there is little evidence from clinical trials in adolescents and young adults; similar studies are not known in our context. The results of this study may facilitate the recommendation of oral vitamin D supplementation in the population of interest. In addition, it is a low-cost and easy-to-apply intervention that could contribute to the formulation and implementation of health policies. TRIAL REGISTRATION: NCT04377386.

BACKGROUND: Metabolic syndrome (MS) is one of the conditions that increase the risk of developing cardiovascular diseases (CVD) and type-2 diabetes in the early future if it appears during childhood or adolescence. The purpose of the study to compare the MS prevalence of MS estimated in a representative sample of school-age population in Bucaramanga, Colombia, and the MS prevalence estimated in a subsample from the same population in the adolescent stage. METHODS: An analytical cross-sectional survey (in the adolescent stage) (n=494) was carried out, nested in a population-based cohort assembled when children were of school age (n=1,282). Selection employed a bi-stage randomized sampling per neighborhoods and houses across the city. Sociodemographic and anthropometric variables, as well as cardiometabolic factors were analyzed in accordance with their distribution, and statistical significance tests were applied according to each case. MS was determined using the Adult Treatment Panel III (ATP III) and International Diabetes Federation (IDF) criteria. RESULTS: Estimated MS prevalence in school age according to the ATP III criteria was 9.5% (95% CI: 8.0-11.3%) and according to the IDF criteria it was 8.0% (95% CI: 6.6-9.7%). At the time of follow up the prevalence of MS was 13.2% and 14.8% according to the ATP III and IDF criteria, respectively. CONCLUSIONS: MS prevalence of MS increased in 4% from the school age (9.5%) to the adolescence (13.1%).

Introducción: Algunos estudios han encontrado relación entre exceso de peso y baja fuerza de prensión relativa. En países de ingresos socioeconómicos medianos y bajos hay pocas evidencias que evalúen la fuerza prensil y su relación con la composición corporal en población pediátrica. Objetivo: Evaluar la correlación entre la fuerza prensil y la composición corporal de escolares de Bucaramanga, Colombia. Materiales y métodos: Se realizó un estudio piloto de corte transversal, analítico, correlacional. Participaron niños en edad escolar de Bucaramanga, Colombia. Las principales variables dependientes fueron masa muscular, agua corporal total, proteínas (kg) y el porcentaje de grasa corporal. La variable independiente correspondió a la fuerza prensil. Se usó el coeficiente de correlación de Spearman para explorar la relación entre variables. Resultados: El promedio de la fuerza prensil en el total de la muestra fue de 13,8 ± 3,2 Newton. Se encontraron correlaciones positivas y estadísticamente significativas entre la fuerza prensil y la masa musculoesquelética (r = 0,73), agua corporal total (r = 0,73) y proteínas (r = 0,74). Discusión: Nuestros resultados son consistentes con evidencias previas que identifican a la fuerza prensil como un indicador de la composición corporal, específicamente en las variables de masa musculoesquelética y proteínas. Conclusión: Los niños en el tercil más alto de fuerza prensil presentan los terciles más altos de masa musculoesquelética, agua corporal total y proteínas.


Introduction: Some studies have found a relationship between excess weight and low relative grip strength. In countries with medium and low socioeconomic income, there is little evidence evaluating grip strength and its relationship with body composition in the pediatric population. Objective: To evaluate the correlation between grip strength and body composition of schoolchildren from Bucaramanga, Colombia. Materials and Methods: A cross-sectional, analytical, correlational pilot study was conducted. Schoolchildren from Bucaramanga, Colombia participated. The main dependent variables were muscle mass, total body water, protein (kg), and percentage of body fat. The independent variable corresponded to the prehensile force. Spearman's Correlation Coefficient was used to explore the relationship between variables. Results: The average prehensile force in the total sample was 13.8±3.2 Newton. Positive and statistically significant correlations were found between grip strength and musculoskeletal mass (r=0.73), total body water (r=0.73), and protein (r=0.74). Discussion: Our results are consistent with previous evidence that identifies grip strength as an indicator of body composition, specifically in the variables of musculoskeletal mass and protein. Conclusion: Children in the highest tertile of prehensile strength present the highest tertiles of skeletal muscle mass, total body water, and protein.

INTRODUCTION: exclusive breastfeeding (EB) is a protective factor against the development of insulin resistance (IR) as mainstay of the metabolic syndrome. OBJECTIVE: the objective of the study was to evaluate the relationship between EB during the first six months of life and the development of IR through the Homeostasis Model Assessment (HOMA-IR) and Quantitative Insulin Sensitivity Check Index (QUICKI) indexes. MATERIAL AND METHODS: a population-based cohort study included 494 adolescent participants, from whom socio-demographic, anthropometric, and cardiometabolic variables were assessed using the HOMA-IR and QUICKI indexes to determine the degree of association between EB and the development IR. The results were expressed in medians. RESULTS: the prevalence of EB was 42%. The results suggest a protective effect of EB on IR, in such a way that for each extra month of EB, the HOMA-IR index decreases in 0.32 (p = 0.005) and the index QUICKI increases in 0.70 (p = 0.008). Regarding waist circumference, body mass index and waist size index, this relationship was not significant. CONCLUSION: the bivariate and multivariate analyses indicate an epidemiologically significant relationship that demonstrates a protective effect of EB on the HOMA-IR and QUICKI indexes.


INTRODUCCIÓN: la lactancia materna exclusiva (LME) es un factor protector contra el desarrollo de resistencia a la insulina (RI) como causa del síndrome metabólico. OBJETIVO: el objetivo del estudio fue evaluar la relación entre la LME los  primeros seis meses de vida y el desarrollo de RI, mediante los índicesHomeostasis Model Assessment (HOMA-IR) y Quantitative Insulin Sensitivity Check Index (QUICKI). MATERIAL Y MÉTODOS: estudio de cohorte de tipo poblacional; se incluyeron 494 participantes adolescentes, de los cuales se evaluaron variables sociodemográficas, antropométricas y factores cardiometabólicos mediante los índices HOMA-IR y QUICKI para determinar el grado de asociación entre la LME y el desarrollo de RI. Los resultados fueron expresados en medianas. RESULTADOS: la prevalencia de LME fue del 42%. Los resultados sugieren un efecto protector de la LME sobre la resistencia a la insulina, de tal manera que por cada mes más de duración de LME, disminuye en 0,32 (p = 0,005) el índice HOMA-IR y aumenta en 0,70 (p = 0,008) el índice QUICKI. Para la circunferencia de cintura, el índice de masa corporal (IMC) y el índice-cintura talla (ICT) esta relación no fue significativa. CONCLUSIÓN: los análisis bivariados y multivariados, indican una relación epidemiológicamente significativa que demuestra un efecto protector de la Lactancia Materna Exclusiva sobre el índice HOMA-IR y el QUICKI.

Introduction: Human vitamin D levels have been increasingly related to a wide range of clinical outcomes. There is a large amount of reports on its associations, especially with obstetric complications, including preeclampsia and gestational diabetes. These results are scarcely consistent and there is still a lack of quality intervention studies to confirm the role of vitamin D in those outcomes. Objective: To review the available scientific evidence on the role of maternal vitamin D in the development of preeclampsia. Materials and methods: The methodology used followed the recommendations of the Cochrane guide for the preparation of systematic reviews, and for metaanalysis, the Guide of the Metaanalysis of Observational Studies in Epidemiology group (MOOSE). The search included both observational studies and controlled clinical trials. Results: Low vitamin D levels, measured by the 25-hydroxyvitamin D test, are common in pregnancy. The results of this systematic review and metaanalysis suggest an inverse ratio between vitamin D levels and the development of preeclampsia. There was heterogeneity among the studies with regard to the design, population, geographic location, definitions of exposure, and the outcome. We excluded randomized controlled trials from this meta-analysis. Conclusion: The inverse association we found suggests that the higher the levels of vitamin D the lesser the probability of developing preeclampsia, in spite of the heterogeneity of the global measurement in this type of analysis.


Introducción: Cada vez son más los hallazgos sobre la relación entre las concentraciones de vitamina D en el ser humano y diversas condiciones clínicas. Hay una gran cantidad de estudios que informan sobre dicha asociación, especialmente con complicaciones obstétricas, incluidas la preeclampsia y la diabetes mellitus de la gestación, entre otras, pero sus resultados todavía no son definitivos, por lo que se requieren estudios de intervención de calidad que confirmen la relación de la vitamina D con dichos resultados. Objetivo: Revisar la información plasmada en estudios en torno al papel de la vitamina D materna y el desarrollo de la preeclampsia. Materiales y métodos: La metodología usada siguió las recomendaciones de la guía Cochrane para la elaboración de revisiones sistemáticas y de la guía del grupo Meta-analysis of Observational Studies in Epidemiology (MOOSE) para los metaanálisis. La búsqueda incluyó estudios observacionales y ensayos clínicos controlados. Resultados: Los niveles bajos de vitamina D, medida con el examen de 25-hidroxivitamina D, son comunes en el embarazo. Los resultados de esta revisión sistemática y del metaanálisis sugieren una asociación inversa entre los niveles de vitamina D y el desarrollo de preeclampsia. Hubo heterogeneidad en los estudios en cuanto a su diseño, población y ubicación geográfica, así como a las definiciones de exposición y resultado. Los ensayos clínicos controlados aleatorizados se excluyeron del metaanálisis. Conclusión: Se encontró una asociación inversa que sugiere que, a mayores concentraciones de vitamina D, menor es la probabilidad de desarrollar preclampsia, a pesar de la heterogeneidad de la medida global en este tipo de análisis.

Resumen Introducción: La obesidad infantil es un problema global de salud pública. Una de las causas subyacentes de este evento complejo y multicausal se relaciona con la alta ingesta de energía. El uso inadecuado de suplementos dietarios podría superar los requerimientos de energía y nutrientes y generar una ganancia excesiva de peso corporal en los niños. Objetivo: Describir el tipo, cantidad y frecuencia de consumo de suplementos dietarios ingeridos por niños y niñas durante la edad preescolar, y evaluar la correlación entre este consumo y la composición corporal de los mismos en la edad escolar. Metodología: Estudio de corte transversal analítico. Estudio piloto realizado en niños en edad escolar de Bucaramanga, Colombia. Variables dependientes: porcentaje de grasa corporal y masa músculo esquelética. Principal variable independiente: consumo de suplementos dietarios. La correlación entre variables fue evaluada usando el coeficiente de correlación de Spearman (rho). Resultados: La prevalencia del consumo de suplementos dietarios en algún momento, antes de los cinco años, fue de 51,11% (IC 95%, 0,35 a 0,66). Las formas de presentación usadas con mayor frecuencia fueron en polvo, granulado y líquido. Los tipos de suplementos utilizados correspondieron a fórmulas poliméricas (40,91%), seguidos por aceite de hígado de bacalao (36,36%). A mayor consumo de suplementos dietarios, se observó mayor porcentaje de grasa corporal (rho = 0,346) y menores valores de masa muscular esquelética (rho = -0,286). Sin embargo, estas correlaciones no fueron estadísticamente significativas (p = 0,114 y p = 0,286, respectivamente). Conclusión: Este estudio encontró una correlación débil y positiva entre el consumo de suplementos dietarios en la primera infancia y el porcentaje de grasa corporal y una correlación débil y negativa con la masa músculo esquelética; sin embargo, no hubo significancia estadística. Es necesario continuar investigando acerca de potenciales efectos no deseados del consumo inadecuado de suplementos durante la primera infancia.


Abstract Introduction: Childhood obesity is a global public health problem. One of the underlying causes of this complex and multicausal event is related to high energy intake. Inappropriate use of dietary supplements could exceed energy and nutrient requirements resulting in excessive body weight gain in children. Objective: To describe the type, quantity and frequency of consumption of dietary supplements ingested by boys and girls during preschool age and to evaluate the correlation between this consumption and their body composition at school age. Methods: Analytical cross-sectional study. Pilot study, carried out in school children from Bucaramanga, Colombia. Dependent variables: body fat percentage and skeletal muscle mass. Main independent variable: consumption of dietary supplements. The correlation between variables was evaluated using Spearman's correlation coefficient (rho). Results: The prevalence of consumption of dietary supplements at some point before the age of five was 51.11% (95% CI, 0.35 to 0.66). The most frequently used forms of presentation were powder, granules and liquid. The types of supplements used corresponded to polymeric formulas (40.91%), followed by cod liver oil (36.36%). A higher consumption of dietary supplements showed a higher percentage of body fat (rho=0.346) and lower values of skeletal muscle mass (rho= -0.286). However, these correlations were not statistically significant (p=0.114 and p=0.286, respectively). Conclusion: This study found a weak positive correlation between dietary supplement use in early childhood and body fat percentage and a weak negative correlation with skeletal muscle mass; however, there was no statistical significance. Further research is needed on the potential undesirable effects of inappropriate supplement use in early childhood.

BACKGROUND: Currently, the waist-to-height ratio (WHtR) has emerged as a practical and useful indicator for identifying cardiometabolic risk in child and adolescent population. OBJECTIVE: To evaluate the association between the WHtR and cardiometabolic risk factors (CMRFs) in a cohort of children in Bucaramanga, Colombia. METHODS: Analytical cross-sectional study. Child population between 6-10 years old from Bucaramanga (n = 1,282). Dependent variables: hypertension, pre-hypertension, overweight/obesity, impaired glucose, insulin resistance and dyslipidemia. Main independent variable: waist-toheight ratio. The association between dependent and independent variables was evaluated using binomial regression models. RESULTS: 9.77% of the children presented a WHtR of risk (95% CI: 8.19% to 11.52%). After adjustment for age, sex, socioeconomic status and physical activity, the WHtR of risk was significantly associated with lower HDL levels and higher levels of triglycerides, insulin resistance, hypertension and pre-hypertension (PR = 2.46 95% CI :1.70 to 3.55, p = 0.000, PR = 2.24 95% CI: 1.50 to 3.35, p = 0.000, PR = 9.39 95% CI: 5.40 to 16.30, p = 0.000, PR = 8.03 95% CI: 2.01 to 32.07, p = 0.003 y PR = 2.67 95% CI: 1.60 to 4.47, p = 0.000, respectively). CONCLUSION: Children with WHtR of risk were more likely to develop high levels of blood pressure and triglycerides, insulin resistance and lower levels of HDL.

BACKGROUND: Birth weight could be a medium-term risk factor in the development of overweight and obesity and a long-term risk factor of cardio-metabolic diseases. OBJECTIVE: To evaluate the association between birth weight and cardio-metabolic risk factors in children from Bucaramanga, Colombia. METHODS: This cross-sectional study included 1,282 children between five and eleven years of age in Bucaramanga. Dependent variables: arterial hypertension, arterial prehypertension, overweight/obesity, glucose intolerance, insulin resistance, dyslipidemia. The main independent variable: birth weight (grams). The association between the independent and dependent variables was evaluated through logistic and multiple linear regression. Data analysis was performed using Stata v. 12.0 software. RESULTS: The overweight prevalence (overweight or obesity) was 22.93% (95% CI 20.64%-25.33%), while the low birth weight prevalence was 3.70% (95% CI 2.65%-5.00%). The most prevalent cardiovascular risk factor was overweight, followed by high LDL-cholesterol levels (17.24%, 95% CI 15.20-19.41%). Following an adjustment by age, sex, socioeconomic level and physical activity and obstetric maternal history, overweight, BMI Z-scores for age and systolic blood pressure had a statistically significant association with birth weight (OR: 1.44; 95% CI 1.10-1.89; p = 0.006, ß= 0.23, 95% CI: 0.10-0.36, p = 0.001 and ß= 1.18, CI 95%: 0.24-2.12, p = 0.013, respectively). CONCLUSIONS: Children with higher birth weight showed more likely to develop overweight or obesity during childhood.

INTRODUCTION: Physical inactivity, smoking, harmful use of alcohol and an unhealthy diet are considered by the World Health Organization (WHO) as the four main behavioral risk factors that increase the risk of suffering non-communicable diseases. OBJECTIVE: To describe the prevalence of the behavioral risk factors in college students according to the scientific database analyzed. MATERIALS AND METHODS: A systematic review was carried out with 585 articles analyzed through electronic search in academic databases. After the analysis, 93 studies that fulfill the inclusion criteria were selected. RESULTS: The final mean of the prevalence found in the articles analyzed was of 52,57% ± 22,53% for physical inactivity, 22,61% ± 12,91% for smoking, 26,2 (RI 24,5) for harmful use of alcohol and 81,3% ± 16,3% for low intake of fruits and vegetables. CONCLUSIONS: The results show the existence of an unhealthy lifestyle in Latin-American college students.
