RESUMO

Hepatocellular carcinoma (HCC) is the most prevalent type of tumor among primary liver tumors and is the second highest cause of cancer-related deaths worldwide. Current therapies are controversial, and more research is needed to identify effective treatments. A new synthetic compound, potassium 5-cyano-4-methyl-6-oxo-1,6-dihydropyridine-2-olate (CPBMF65), is a potent inhibitor of the human uridine phosphorylase-1 (hUP1) enzyme, which controls the cell concentration of uridine (Urd). Urd is a natural pyrimidine nucleoside involved in cellular processes, such as RNA synthesis. In addition, it is considered a promising biochemical modulator, as it may reduce the toxicity caused by chemotherapeutics without impairing its anti-tumor activity. Thus, the objective of this study is to evaluate the effects of CPBMF65 on the proliferation of the human hepatocellular carcinoma cell line (HepG2). Cell proliferation, cytotoxicity, apoptosis, senescence, autophagy, intracellular Urd levels, cell cycle arrest, and drug resistance were analyzed. Results demonstrate that, after incubation with CPBMF65, HepG2 cell proliferation decreased, mainly through cell cycle arrest and senescence, increasing the levels of intracellular Urd and maintaining cell proliferation reduced during chronic treatment. In conclusion, results show, for the first time, the ability of a hUP1 inhibitor (CPBMF65) to reduce HepG2 cell proliferation through cell cycle arrest and senescence.

OBJECTIVES: The aim of this work was to review the scientific literature on the properties, indications and pitfalls related to nystatin and chlorhexidine in oral medicine and also to compare these to other topical antifungal agents, considering the elderly population. BACKGROUND: Nystatin is a polyene antifungal widely used as a topical formulation to treat candidiasis, whereas chlorhexidine is a wide-spectrum antimicrobial, especially used against bacteria, but also effective in treating some fungal infections including those caused by Candida spp. These compounds have been prescribed for immunocompromised patients, hospitalized or not, some of them undergoing head and neck radiation therapy and/or chemotherapy, including elderly patients. MATERIALS AND METHODS: Dental and medical literature concerning the use of nystatin and chlorhexidine in oral medicine were selected and reviewed. RESULTS: Nystatin and chlorhexidine are gold-standard antimicrobial mouthrinses respectively for Candida spp. and bacteria. Although recognized as effective in cotrolling oral infections, both nystatin and chlorhexidine are just complementary to systemic therapy in cases of systemic infections already established. The prescriber should also take into account that some commercial nystatin and chlorhexidine formulations contain compounds such as sugar and ethanol, which can be associated with side effects. Meanwhile, alternative formulations in which these compounds are absent are available and should be considered. CONCLUSIONS: Further studies investigating new drugs and interactions of drug combinations are necessary to improve the therapeutic management of oral infections.

Baccharis anomala DC. (BA) is a plant species found in the tropical regions of South America and is widely used for its hepatoprotective effects, as well as for the treatment of gastrointestinal diseases. Studies have recently reported its antioxidant and anti-inflammatory potential. BA extract can reverse the activated phenotype of hepatic stellate cells (HSC), which plays a central role in extracellular matrix (ECM) deposition in the development of liver fibrosis. Thus, this study aimed to evaluate the effects of the treatment with BA extract on liver fibrosis in a CCl4-induced liver fibrosis model in BALB/c mice. Methanolic extract was obtained from BA leaves, a gas chromatography/mass spectrometry (GC/MS) to detect the compounds present was performed, and then administered by intraperitoneal injection in Balb/C mice at a concentration of 50 and 100 mg/kg together with the administration of CCl4 for inducing liver fibrosis. After 10 weeks, blood analysis, histopathology, oxidative stress, as well as protein and gene expression in the hepatic tissue were performed. Treatment with BA extract was able to reduce profibrotic markers by reducing the expression of α-SMA and Col-1 proteins, as well as reducing the formation of free radicals and lipid peroxidation. (BA extract showed anti-inflammatory effects in the liver by suppressing NF-kB activation and reducing gene expression of signaling targets (IL-6 and iNOS). The data obtained showed that BA extract has antifibrotic and anti-inflammatory effects.

ETHNOPHARMACOLOGICAL RELEVANCE: The chosen plant and its extracts have been an alternative in the treatment of several inflammatory and oxidant diseases, and is therefore a viable option for the treatment of hepatic fibrosis. AIM OF THE STUDY: This study aimed to use Moquiniastrum polymorphum subsp. polymorphum, mainly the ethanolic extract and fractions, in the treatment of hepatic fibrosis. MATERIALS AND METHODS: Extracts were prepared from dried leaves in 100% ethanol (ET) and fractionated with an increased polarity solvent (dichloromethane to methanol). The quantification of compounds in the extracts was characterized by GCMS. The decrease in cell proliferation and the cytotoxicity of the extracts were evaluated together with the mechanisms of apoptosis and autophagy. The expression of genes associated with decreased fibrosis and cell cycle control was assessed and the production of lipid droplets was quantified by Oil Red O staining. RESULTS: The experiments showed that treatment with ET and fraction 1 (F1) inhibited the expression of CDKIs (CCDN1, CDK2, CDK4 and CDK6) through an increase in p27, related to an increase in autophagic vesicles. The extract and F1 were able to decrease proliferation and revert the activated state of GRX cells to their quiescent state. CONCLUSION: Our results suggest that extracts obtained from Moquiniastrum polymorphum subsp. polymorphum have a potential therapeutic effect against liver fibrosis.

Acute lung injury (ALI) is a disease of high prevalence and is characterized by the excessive production of inflammatory mediators in the lungs of people sick. Inflammation is the major characteristic of ALI and studies report that inhibition of inflammatory cytokines could be an alternative treatment. Statins such as Simvastatin (SV) are known to their use for cholesterol reduction but also for inflammatory and immunoregulatory processes. In this study, we evaluated the effects of SV on LPS-induced alveolar macrophages and in ALI mice model. Our study has demonstrated the protective effects of SV on LPS-activated alveolar macrophages RAW 264.7 and LPS-induced ALI in mice. SV treatment significantly inhibited the alveolar macrophages activation by decreasing the iNOS, IL-1ß, and IL-6 gene expression in vitro and in vivo. The treatment also decreased the inflammatory cells migration and the cytokines gene expression. Our findings suggest that SV can act as an anti-inflammatory agent for acute lung injury.

In pulmonary fibrosis, the proliferation of fibroblasts and their differentiation into myofibroblasts is often caused by tissue damage, such as oxidative damage caused by reactive oxygen species, which leads to progressive rupture and thus destruction of the alveolar architecture, resulting in cell proliferation and tissue remodeling. Bezafibrate (BZF) is an important member of the peroxisome proliferator-activated receptor (PPARs) family agonists, used in clinical practice as antihyperlipidemic. However, the antifibrotic effects of BZF are still poorly studied. The objective of this study was to evaluate the effects of BZF on pulmonary oxidative damage in lung fibroblast cells. MRC-5 cells were treated with hydrogen peroxide (H2O2) to induce oxidative stress activation and BZF treatment was administered at the same moment as H2O2 induction. The outcomes evaluated were cell proliferation and cell viability; oxidative stress markers such as reactive oxygen species (ROS), catalase (CAT) levels and thiobarbituric acid reactive substances (TBARS); col-1 and α-SMA mRNA expression and cellular elasticity through Young's modulus analysis evaluated by atomic force microscopy (AFM). The H2O2-induced oxidative damage decreased the cell viability and increased ROS levels and decreased CAT activity in MRC-5 cells. The expression of α-SMA and the cell stiffness increased in response to H2O2 treatment. Treatment with BZF decreased the MRC-5 cell proliferation, ROS levels, reestablished CAT levels, decreased the mRNA expression of type I collagen protein (col-1) and α-smooth muscle actin (α-SMA), and cellular elasticity even with H2O2 induction. Our results suggest that BZF has a potential protective effect on H2O2-induced oxidative stress. These results are based on an in vitro experiment, derived from a fetal lung cell line and may emerge as a possible new therapy for the treatment of pulmonary fibrosis.

Background: hip fracture is the major cause of morbidity and mortality. Geriatric fracture programs promise to improve the quality of care, health outcomes and reduce costs. Objective: To describe the results related to the Geriatric fracture programs implementation in two Colombian institutions to assess reproducibility. Methods: We performed A retrospective descriptive study of the patients treated under the Geriatric fracture programs in two institutions in Colombia. The information was collected from the initial year of implementation until 2018. Demographic characteristics, length of stay, hospitalization complications, readmissions and mortality were described. Consumption of healthcare resources was defined using base cases determined with local experts and costs were estimated using standard methods. Results: 475 patients were included in the Geriatric fracture programs. We observed an increase in the number of patients. The length of stay decreased between 8.5% and 26.1% as did the proportion of total complications, with delirium having the greatest reduction. A similar situation was seen for first year mortality (from 10.9% to 4.7%), in-hospital deaths and readmissions. Estimates of costs of stay and complications showed reductions in all scenarios, varying between 22% and 68.3%. Conclusions: The present study presents the experience of two institutions that implemented the Geriatric fracture programs with increase in the number of patients treated and reductions in the time of hospital stay, the proportion of complications, readmissions, mortality and estimated costs. These are similar between both institutions and with other published implementations. This could hint that geriatric fracture program may be implemented with reproducible results.


Antecedentes: las fracturas de cadera son una causa importante de morbilidad y mortalidad. Los programas de fracturas geriátricas prometen mejorar la calidad de la atención, los desenlaces clínicos y reducir costos. Objetivos: Describir los resultados relacionados con la implementación de un programa de fracturas geriátricas en dos instituciones en Colombia para evaluar la reproducibilidad. Métodos: Realizamos un estudio retrospectivo descriptivo de los pacientes atendidos en el programa de fracturas geriátricas en dos instituciones en Colombia. Se obtuvo información desde el año de implementación hasta 2018. Se describió información demográfica, de estancia hospitalaria, de complicaciones, de rehospitalizaciones y de mortalidad. Se determinó un caso base de consumo de recursos con expertos y se costeó usando métodos estandarizados. Resultados: Se incluyeron 475 pacientes. Se observó un aumento en la cantidad de pacientes. La duración de estancia disminuyó entre 8.5% y 26.1%, así como la frecuencia de complicaciones. Se encontraron reducciones en mortalidad en el primer año (entre 10.9% y 4.7%), muertes hospitalarias y rehospitalizaciones. Los costos estimados mostraron reducciones entre 22% y 68.3%. Conclusiones: El presente estudio muestra la experiencia de implementación de programa de fracturas geriátricas en dos instituciones en Colombia, mostrando aumento en cantidades de pacientes y reducciones en estancia hospitalaria, frecuencia de complicaciones, rehospitalizaciones, mortalidad y costos estimados. Los resultados fueron similares en las dos instituciones y comparables con otros descritos en literatura. Esto sugiero que los programas de fracturas geriátricas se pueden implementar con resultados reproducibles.

Potassium 5-cyano-4-methyl-6-oxo-1,6-dihydropyridine-2-olate (CPBMF65) is a potent inhibitor of the uridine phosphorylase 1 (UPP1) enzyme. Its non-ionized analog has already demonstrated biological properties by reducing adverse effects caused by the chemotherapeutic 5-fluorouracil (5-FU). In addition, it has been demonstrated that uridine inhibits inflammation and fibrosis in bleomycin lung injury, decreasing collagen production. The purpose of this study was to investigate the in vitro and in vivo effects of CPBMF65 on activated hepatic stellate cells (HSC) and on carbon tetrachloride-induced liver fibrosis in mice. After incubation with CPBMF65, decreased cell proliferation and phenotype reversion were observed in vitro. In addition, CPBMF65 promoted a protective effect on tetrachloride-induced liver fibrosis in mice, demonstrated by its antifibrotic and anti-inflammatory actions. The results of the present study indicate that the UPP1 inhibitor (CPBMF65) may have potential as a novel therapeutic agent for the treatment of liver fibrosis.

Octyl gallate (OG) is an antioxidant commonly used in food, although there is no definition of its acceptable daily intake. There are reports in vitro and in vivo showing that food additives and drugs can alter lipid metabolism. Lipid droplet accumulation in hepatic cells is one of the main findings in the unregulated lipid metabolism and is strongly related to the development of nonalcoholic fatty liver disease (NAFLD). In this study, we investigated the effects of OG on lipid metabolism in the hepatocellular carcinoma cell line (HepG2). The results have shown, for the first time, that treatment with OG increased the overall amount of lipids, the triglyceride concentration, the lipid droplet area, and SREBP-1c and PPAR-γ gene expression. Taken together, the findings indicate that OG induces lipid droplet accumulation in HepG2 cells through the regulation of SREBP-1c and PPAR-γ gene expression without involving mTOR/S6K1 and may contribute to NAFLD when used as a food additive.

The incidence of hepatocellular carcinoma (HCC) keeps rising year by year, and became the second leading cause of cancer-related death. Some studies have found that liraglutide, a GLP-1 analog, may decrease the tumor cells proliferation. Due to this, the aim of this work is to investigate the antiproliferative potential of exenatide, another GLP-1 analog. Cell proliferation was assessed by direct count with Trypan blue dye exclusion. Flow cytometry was used to determinate autophagy and nuclear staining. Morphometric analysis was used to verify senescence and apoptosis. The mechanism that induced cell growth inhibition was analyzed by Western Blot. Treatment with exenatide significantly decreases cell proliferation and increases autophagy, both in relation to control and liraglutide. In addition, mTOR inhibition was greater in cells treated with exenatide. In relation to chronic treatment, exenatide does not allow cellular regrowth by preventing some resistance mechanism that the cells can acquire. These results suggest that exenatide has a potent anti-proliferative activity via mTOR modulation and, among the GLP-1 analogs tested, could be in the future an alternative for HCC treatment.

OBJECTIVE: This study investigated the effect of topical application of 0.12% chlorhexidine, 10% povidone-iodine and 50% erythromycin on the optimization of healing process of traumatic ulcers made on ventral tongue of rats. DESIGN: Forty-Eight Wistar rats were randomly divided into four groups: control, chlorhexidine (Chx), povidone-iodine (PvI) and erythromycin (Er). An ulcer of 5 mm in diameter was made on the ventral tongue of the animals. After 24 h, a microbiological sample was taken and daily application of the substances started. Six animals each group were euthanized at 4 days and the others at 8 days postoperative, totaling three and seven days of treatment. Prior to euthanasia, a new microbiological collection was performed. RESULTS: The experimental groups showed less area of residual ulcer. A significant difference was seen between the PvI and Chx in relation to the control after three days of treatment (p < 0.05). Although the experimental groups displayed greater newly formed epithelial area, there was no significant difference compared to the control (p > 0.05). Er exhibed the lowest inflammation scores after seven days of treatment (p < 0.05). PvI showed reduction of microorganisms at both times and under aerobic (p < 0.01 at 3 days and p < 0.001 at 7 days) and microaerophilic (p < 0.05) conditions. Er significantly reduced the count of microorganisms in aerobic condition when compared to control group (p < 0.05 at 3 days and p < 0.01 at 7 days). CONCLUSIONS: All drugs promoted reduction of the microorganisms at the site of the injury, which may have a direct effect on the tissue repair process.

Octyl gallate (OG) is an antioxidant that has shown anti-tumor, anti-diabetic and anti-amyloidogenic activities. Mitochondria play an important role in hepatocellular carcinoma, mainly by maintaining accelerated cellular proliferation through the production of ATP. Thus, the mitochondria may be a target for antitumor therapies. Here, we investigated the effects of OG in the hepatocarcinoma cell line (HepG2) and the mechanisms involved. We report, for the first time, that treatment with OG for 24h inhibited HepG2 cell growth by decreasing mitochondrial activity and mass, which led to the reduction of ATP levels. This reduction in the energy supply triggered a decrease in Ki67 protein expression, leading cells to cycle arrest. In addition, treatment with two doses of OG for 48h induced loss of mitochondrial functionality, mitochondrial swelling and apoptosis. Finally, we report that HepG2 cells had no resistance to treatment after multiple doses. Collectively, our findings indicate that metabolic dysregulation and Ki67 protein reduction are key events in the initial anti-proliferative action of OG, whereas mitochondrial swelling and apoptosis induction are involved in the action mechanism of OG after prolonged exposure. This suggests that OG targets mitochondria, thus representing a candidate for further research on therapies for hepatocarcinoma.

These are similar between both institutions and with other published implementations. This could hint that geriatric fracture program may be implemented with reproducible results.


Resumen Antecedentes: las fracturas de cadera son una causa importante de morbilidad y mortalidad. Los programas de fracturas geriátricas prometen mejorar la calidad de la atención, los desenlaces clínicos y reducir costos, Objetivos: Describir los resultados relacionados con la implementación de un programa de fracturas geriátricas en dos instituciones en Colombia para evaluar la reproducibilidad Métodos: Realizamos un estudio retrospectivo descriptivo de los pacientes atendidos en el programa de fracturas geriátricas en dos instituciones en Colombia. Se obtuvo información desde el año de implementación hasta 2018. Se describió información demográfica, de estancia hospitalaria, de complicaciones, de rehospitalizaciones y de mortalidad. Se determinó un caso base de consumo de recursos con expertos y se costeó usando métodos estandarizados. Resultados: Se incluyeron 475 pacientes. Se observó un aumento en la cantidad de pacientes. La duración de estancia disminuyó entre 8.5% y 26.1%, así como la frecuencia de complicaciones. Se encontraron reducciones en mortalidad en el primer año (entre 10.9% y 4.7%), muertes hospitalarias y rehospitalizaciones. Los costos estimados mostraron reducciones entre 22% y 68.3%. Conclusiones: El presente estudio muestra la experiencia de implementación de programa de fracturas geriátricas en dos instituciones en Colombia, mostrando aumento en cantidades de pacientes y reducciones en estancia hospitalaria, frecuencia de complicaciones, rehospitalizaciones, mortalidad y costos estimados. Los resultados fueron similares en las dos instituciones y comparables con otros descritos en literatura. Esto sugiero que los programas de fracturas geriátricas se pueden implementar con resultados reproducibles.

Introducción: En Paraguay, la población de 65años y más, constituye el 7,4% de la población total.El cuidador primario es aquella persona que deforma cotidiana se hace cargo de las necesidadesbásicas y psicosociales del adulto mayor; y la sobrecargadel cuidador, es un estado psicológico queresulta de la combinación de trabajo físico, presiónemocional, y restricciones sociales. Objetivos:Determinar el grado de sobrecarga que padece elcuidador primario del adulto mayor. Metodología:Estudio observacional, descriptivo de cortetransverso con componente analítico, realizados encuidadores principales de adultos mayores, en losbarrios que forman parte del territorio de la USFClínicas 1 y 2, Asunción, de julio a setiembre del2015. Se utilizó la escala de Zarit, la escala de depresiónde Goldberg y expedientes clínicos. Resultados:Grado de sobrecarga se observó que el33,3% no lo presentó, mientras que el 50% sí; el68% fueron mujeres y el 65 % mayor de 40 años.Presentaron depresión 67%. Se encontró asociaciónentre sobrecarga del cuidador primario y vínculofamiliar, X2≥ 23,12 (p<0,05), y entre sobrecargadel cuidador y depresión en el mismo, X2≥17,18(p<0,05). Conclusión: El grado de sobrecargadel cuidador principal en esta serie fue predominante,presentando síntomas depresivos. El perfilsocio-demográfico corresponde a mujeres, más de40 años, casada, con escolaridad secundaria y nexofamiliar. Se encontró asociación con significaciónestadística entre grado de sobrecarga del cuidadorprimario y vínculo familiar, como también entregrado de sobrecarga del cuidador y depresión enel mismo.

Objective: To determine the degreeof overload the primary caregiver of older peoplesuffer. Methodology: Observational, descriptivestudy with a transversal analytic component, carriedout in primary caregivers of older adults, inneighborhoods that are part of the USF territory ofClinicas 1 and 2, Asunción, from July to September2015. The scales used were Zarit, the depressionscale of Goldberg, and medical records.Results: itwas observed that 33.3% of the population studieddid not show a degree of overload, while 50%did; 68% were women and 65% over 40 years old.67% showed depression. An association was foundbetween the overload of the primary caregiverand the family links with the patient, X2≥ 23,12(p<0,05), and between the overload of the primarycaregiver and depression, X2≥17,18 (p<0,05).Conclusion: The degree of overload in the primarycaregiver in this series was predominant, andincluded symptoms of depression. The social-demographicprofile is: women, over 40 years old,married, with secondary school studies, and family links. Association was found with statistical significancebetween the degree of overload in the primarycaregiver and their family links, as well asbetween their degree of overload and depression.

Introducción: el envejecimiento poblacional caracteriza a las últimas décadas. En Paraguay la población mayor de 60 años constituye el 10%, con edad promedio de 70 años. La funcionalidad geriátrica es la capacidad para realizar independientemente actividades básicas de la vida. Objetivos: determinar perfil clínico epidemiológico y prevalencia de dependencia funcional en adultos mayores del Barrio San Miguel de Asunción, Paraguay. Metodología: estudio observacional,de corte transverso, descriptivo con componente analítico, realizado en adultos mayores de 65 años del Barrio San Miguel, de abril a julio del año 2014. Variables estudiadas: edad, sexo, enfermedades crónicas, síndromes geriátricos y capacidad funcional. Se utilizó el índice de Katz, la escala geriátrica de depresión de Yesavage y expedientes clínicos. Resultados: se encontró 3% de prevalencia de adultos mayores de 65 años. El perfil sociodemográfico correspondió a un adulto mayor con predominio del sexo femenino, edad media de 71,8 años, casado, con escolaridad primaria. La enfermedad crónica con mayor frecuencia fue la HTA. El 79% de los adultos mayores presentan algún síndrome geriátrico, predominando la depresión (43%). El 41% de la población estudiada presenta dependencia funcional en las actividades básicas de la vida diaria. Se encontró asociación significativa entre depresión y funcionalidad básica Conclusión: la enfermedad crónica más prevalente es la hipertensión arterial. La prevalencia de dependencia funcional y de depresión es alta en los adultos mayores.


Introduction: The last decades are characterized by population aging. In Paraguay, the population older than 60 years old represents 10% with an average mean of 70 years. The geriatric functionality is the capacity to perform basic activities of life independently. Objectives: To determine the epidemiological-clinical profile and prevalence of functional dependence in the older adults from San Miguel neighborhood in Asunción, Paraguay. Methodology: This was a descriptive cross-sectional observational study with analytical component carried out in adults older than 65 years in San Miguel neighborhood from April to July, 2014. The studied variables were: age, sex, chronic diseases, geriatric syndromes and functional capacity. The Katz index, geriatric depression scale of Yesavage and clinical records were used. Results: There was 3% of prevalence of adults older than 65 years. The socio-demographic profile corresponded to an older adult with predominance of women, mean age of 71.8 years, married and with primary education. The most frequent chronic disease was AHT while 79% of the elderly presented a geriatric syndrome prevailing depression (43%). Forty one percent of the studied population presented functional dependence in the basic activities of daily living. A significant association was found between depression and basic functionality. Conclusion: The most frequent chronic disease was arterial hypertension. The prevalence of functional dependence and depression is high in older adults.

RESUMO

Este es el primer reporte de una línea de investigación que se inició sobre OSTEOGENESIS POR DISTRACCION EN LA MANDIBULA. Se presentan dos prototipos de distractor para cerdos, que permiten alargar la rama y el cuerpo mandibulares, simultánea e independientmente cuya activación es intraoral. El hueso neoformadopor distracción se comparó con un control simulado y un hueso control. Se evaluaron los cambios en la Articulación temporomandibular, los cambios dentales y la biocompatibilidad del aparato. Se encontró que el hueso neoformado por distracción está dispuesto en forma perpendicular al hueso maduro y es paralelo al vector de la tensión. El hueso control simulado está dispuesto paralelo al hueso maduro
