It is increasingly recognized that psychological stress is linked with type 2 diabetes mellitus and its late complications. Thus, the aim of the current study was to investigate the psychophysiological response to acute psychosocial stress in patients with type 2 diabetes. In total, 53 type 2 diabetes patients with complications, 16 type 2 diabetes patients without complications, and 47 age and gender matched non-diabetic participants underwent the Trier Social Stress Test. Subjective as well as biological parameters (i.e., blood levels of cortisol, adrenocorticotropic hormone (ACTH), norepinephrine, methylglyoxal) were assessed repeatedly before and after stress induction. Data were analyzed by means of multilevel regression. Patients with type 2 diabetes showed an exaggerated cortisol response to acute stress as compared to age matched control participants (diabetes*T2 est. = 1.23, p < .001), while stress-induced alterations of ACTH and subjective parameters did not differ. Norepinephrine levels were lower among patients (diabetes est. = -4.36, p = .044) and tended to decrease earlier than in controls. The subjective reaction of type 2 diabetes patients with complications was stronger than that of patients without complications (complication*T2 est. = -1.83, p = .032), while their endocrine response to stress was similar. Stress had no effect on methylglyoxal level, and there were no group differences regarding methylglyoxal response. These results show that the cortisol reactivity of patients with type 2 diabetes to acute psychosocial stress is increased compared to a control group. Thus, alterations of the hypothalamus-pituitary-adrenal axis - especially regarding its dynamic regulation - are a plausible link between psychological stress and type 2 diabetes and its complications.

In the limit of weak molecular interaction with an inorganic surface, noncovalent interactions between molecules dominate the nucleation and thin-film growth. Here, we report on the formation of three-dimensional triptycene clusters with a particularly stable structure. Once formed at the early stage of molecular adsorption, the clusters are stable for all temperatures until desorption. Furthermore, the clusters diffuse and nucleate as individual entities, therefore constituting building blocks for the later thin-film formation. High resolution scanning tunneling microscopy images indicate that the cluster is stabilized by C-H-pi interactions. The formation of such molecular structures at a surface is possible because the three-dimensional structure of the triptycene molecule leads to a very weak and mobile adsorption state. These results show that it is possible to investigate complex pathways in the formation of three-dimensional supramolecules at surfaces using a scanning tunneling microscope.

Carboxyhemoglobin (COHb%) and gamma-glutamyl-transferase (GGT) are indicators of tobacco and alcohol consumption; similarly, body weight broadly reflects dietary habits. Relationships between COHb%, GGT, relative body weight, heart rate and blood pressure were studied in 242 48-year-old men attending a general health screening program in Malmö, Sweden. All were without treatment for high blood pressure. Positive correlations were found between blood pressure and body weight, GGT, and pulse, and a reciprocal correlation between blood pressure and COHb%. Use of objective markers for known or suspected risk factors, such as alcohol consumption, smoking, or overweight, were studied to elucidate their usefulness for further prospective studies.

The antihypertensive effect of amiloride was compared to that of the calcium antagonist nitrendipine in 12 patients (8 males), aged 34-62 years, with essential hypertension WHO grade I-II (mean supine blood pressure 158/103, standing 155/106 mmHg) in a double-blind placebo-controlled cross-over study design. Amiloride was given 5 mg once daily for one month followed by 20 mg twice daily for another month. Amiloride 5 mg once daily significantly reduced supine and standing DBP but not SBP (supine 151/94, standing 149/97 mmHg), whereas 10 mg once daily decreased SBP as well as DBP (supine 145/98, standing 145/101 mmHg). Nitrendipine 20 mg once daily significantly reduced supine and standing SBP and standing DBP (supine 150/97, standing 148/98 mmHg), but on 20 mg twice daily only supine SBP was significantly reduced (supine 150/99, standing 151/106 mmHg). Heart rate was transiently increased by nitrendipine 20 mg once daily and unchanged following amiloride. Plasma noradrenaline was unaltered following amiloride 10 mg once daily as well as nitrendipine 20 mg twice daily, whereas plasma renin activity and aldosterone were elevated following amiloride. Serum electrolytes, blood glucose, plasma lipids and body weight were not altered by any of the drugs. Amiloride 5-10 mg daily has a mild to moderate BP lowering effect in patients with essential hypertension. The BP reduction following nitrendipine 20 mg once daily was comparable to that of amiloride 5 mg daily. Nitrendipine 20 mg twice daily gave no additional BP decrease.

RESUMO

The use of a symbol to prompt the adoption of correct lifting posture was examined in three studies. Study 1 used an Appropriateness Test to evaluate nine symbols designed to encourage the adoption of correct lifting posture. Four symbols met the appropriateness criteria and were tested for comprehension in Study 2. Study 3 examined the effect of the best performing symbol from Study 2 in a field setting which involved subjects lifting a small box. Results indicate significant increases in the adoption of the use of correct lifting posture when the symbol was present compared to a control condition. The study also identified the placement of a lifting criterion symbol onto packaging as a useful technique for communicating safety information.

The combination of organic linkers with metal atoms on top of inorganic substrates offers promising perspectives for functional electronic and magnetic nanoscale devices. Typically, coordination bonds between electron-rich end groups and transition-metal atoms lead to the self-assembly of metal-organic nanostructures, whose shape and electronic and magnetic properties crucially depend on the type of ligand. Here, we report on the site-selective bonding properties of Co atoms to the dichotomic dicyanoazobenzene molecule with its carbonitrile and diazo N-based moieties as possible ligands. Using low-temperature scanning tunneling microscopy (STM) and spectroscopy measurements, we resolve the formation of self-assembled metal-organic motifs. Cobalt atoms exhibit a clear spectroscopic fingerprint dependent on the different coordination site, which is further used to map their position, otherwise not clearly visible in the topographic STM images. Density functional theory corroborates the observed bonding patterns and evidences their coordinative nature.

Codeposition of C60 and the three-dimensional molecular hydrocarbon 1,3,5,7-tetraphenyladamantane (TPA) on Au(111) leads to the spontaneous formation of molecular nanostructures in which each fullerene is locked into a specific orientation by three surrounding TPA. Scanning tunneling spectroscopy shows that the electronic coupling of C60 with the surface is significantly reduced in these nanostructures, enhancing the free-molecule properties. As evidenced by density functional theory simulations, the nanostructures are stabilized by 18 local electrostatic forces between C60 and TPA, resulting in a lifting of the C60 cage from the surface.

There is a growing evidence for that in modern societies the function of the cellular sodium-potassium pump (membrane-bound Na+ K+ ATPase) in several tissues in man cannot respond adequately to demands. This is not seen in any other free-living vertebrates on this earth. The clearly unphysiological very high intake of sodium-chloride (salt) and also alcohol is definitely playing an important role in the development of the common degenerating metabolic aberrations, e.g. essential hypertension, diabetes II and severe over-weight, in man. The special and overall important role of the sodium-potassium pump for optimal cellular function and regeneration with special reference to the vascular tissues is presented and discussed.

Haemodynamic changes in the early phases of hypertensive diseases have mainly been described in so-called borderline hypertensives. Their cardiac output (CO) has mostly been found to be elevated, but the peripheral vascular resistance (PRI) normal. In this study of the offspring (26 subjects) of hypertensive individuals, CO and intravascular pressures were measured during rest, muscular work, psychological stress and cold pressure test. For comparison, the same measurements were performed in a control group of healthy volunteers without known hypertension in their families. The offspring had higher blood pressure at rest than the controls throughout the study except during the cold pressure test and the highest dynamic work loads. Although there was no statistically significant difference between offspring and controls as regards PRI, the resistance was slightly higher in the group of offspring throughout the study. Arterial blood pCO2 in offspring was significantly lower at rest, during psychological stress and dynamic muscle work. Possible mechanisms behind this finding are considered.

Thirty-six obese, pre-menopausal women were studied after an overnight fast and randomized to four different regimens of metabolic stimuli: group I (n = 12), 100 ml orange juice with dissolved K- and Mg-phosphates (K, 40 mmol; Mg, 17.5 mmol; HPO4, 35 mmol); group II (n = 8), 100 ml of water with electrolytes as in group I; group III (n = 8), 100 ml of orange juice; group IV (n = 8), injection of ephedrine intravenously, 0.25 mg/kg body weight. The women in groups I, II and III were further stimulated with ephedrine as in group IV and new measurements made. Thirty minutes after the first stimulus VO2, VCO2 and energy expenditure (EE) rose significantly (13.1-16.5 per cent) in groups I and IV only. In groups I and III the blood concentrations of glucose, insulin, and C-peptide rose significantly. After the second stimulus (ephedrine i.v.) no further increase in VO2, VCO2 and EE occurred in group I, but the increases from basal values became significant in group III. In all women at baseline, whole-body potassium was significantly correlated to VO2. Serum-magnesium was negatively correlated to A/I weight (actual/ideal weight). We conclude that the addition of K- and Mg-phosphates to glucose increases the postprandial thermogenesis in obese patients.

A total of 195 relatives of patients from families with essential hypertension in at least two generations have been examined in regard to blood pressure (BP) and heart rate (HR) during muscle work and psychological stress. They were compared to controls without heredity for hypertension. The differences in BP and HR at rest between the groups were maintained throughout all tests, but during heavy dynamic muscle work systolic BP rose less in relatives than in controls. When relatives and controls were divided into subgroups, we found that BP was more related to age in relatives than in controls, both at rest and during the provocation tests. The differences in BP between the groups could thus depend on a genetic factor with greater penetration with increasing age.

Erythrocyte sodium, potassium, zinc and magnesium were measured in 56 male offspring to essential hypertensive patients and compared with 46 matched controls. The same parameters were measured again 4-6 months apart in 31 offspring and eight controls. Intracellular sodium was significantly higher in offspring (P less than 0.001). There was a highly significant correlation between the two intra-individual measurements of intracellular sodium, potassium and zinc but not for magnesium. Sodium in erythrocytes is probably an indicator of the activity of the sodium-pump and could possibly be used as a genetic marker in the future investigations of individuals with heredity for severe, essential hypertension.

Fresh erythrocytes from 35 young normotensive individuals (20 males), the offspring of patients with essential hypertension (EH), and from 24 normotensive controls (14 males) were incubated at 37 degrees C in an isotonic buffer with 22Na. After 80 min, the net influx (net accumulation) of 22Na was not significantly different in either group, but after 140 min it was significantly higher (8.2%, p less than 0.01) in erythrocytes from male offspring as opposed to male controls. After both 80 and 140 min of incubation, the net influx of 22Na was significantly higher in males than in females in the respective groups. During a 60-minute period the net efflux of 22Na from preincubated (180 min) erythrocytes in 14 males in the offspring group was not significantly different from that found in 10 male controls. These findings could indicate that little differences exists in vitro between offspring and controls in the undirectional fluxes across the membrane. The difference between the male groups in the net influx after 140 min could indicate a higher Na content in the intracellular space in the offspring group, as assumption subsequently verified in a follow-up. The net influx of 22Na after 140 min was significantly correlated (R=0.58, p less than 0.05) to total plasma aldosterone male controls, but not in the offspring. No differences in the mean concentrations of total aldosterone, plasma and urinary electrolytes or in serum gamma glutamyl transferase were noted between offspring and controls. Abnormal cellular handling of sodium at least in the male offspring of patients with EH for at least two generations may be an important predisposing factor for EH.
