OBJECTIVE: The purpose of this study is to explore the biomechanical characteristics of the tibia after unicompartmental knee arthroplasty with different distributions of two-pin holes, and to optimize the two-pin holes scheme to reduce the risk of tibial fractures after unicompartmental knee arthroplasty. METHODS: Lower limbs model is segmented and reconstructed from computed tomography images. Four combinations of two pin holes created for tibial cutting guide placement are simulated with finite element analysis. RESULTS: In the third mode, the positioning hole at the proximal medial edge of the tibial plateau has the highest stress value, and the position of the positioning hole near the medial edge of the proximal tibial plateau appears stress concentration. CONCLUSIONS: The present study revealed that placing tibial cutting guide holding pins centrally would lower the risks of periprosthetic fracture of the medial tibial plateau.

OBJECTIVES: To explore the features of high-intensity zone (HIZ) in anterior annulus fibrosus and assess the association of anterior HIZ with low back pain (LBP). DESIGN, SETTING, AND SUBJECTS: A retrospective study of 5,940 discs in 1,188 individuals was conducted. METHODS: Subjects' information and LBP symptoms confirmed by an orthopedic surgeon were acquired from the medical record. Magnetic resonance (MR) image reading and analysis were performed by two experienced blinded radiologists. RESULTS: Two hundred eighty individuals exhibited 355 anterior HIZs in 355 discs. The prevalence was 23.57%; 88.45% were located in the inferior part of the annulus fibrosus. It frequently occurred in the middle and upper segments of lumbar spine, especially at L3/4 (45.63%). Of the 355 anterior HIZs, only 79 (22.25%) were consecutive-slides HIZ. Round type (63.38%) was the most common shape of anterior HIZs. The highest prevalence was found in individuals aged 60-69 years. LBP was confirmed in 141 anterior-HIZ individuals. The incidence of LBP in anterior-HIZ individuals was significantly higher than in non-HIZ subjects (50.36% vs 35.24%, χ2 = 18.314, P < 0.001). CONCLUSIONS: Anterior HIZ is a lower-prevalence, age-related sign on lumbar MR images. The spatial distribution of anterior HIZ can be distinguished from posterior HIZ. The number of consecutive anterior HIZ slides might suggest fewer Dallas grade 4 anterior annular disruptions in this sample. Anterior HIZ was correlated with LBP.

With the development of the internet of things (IoTs), big data, smart sensing technology, and cloud technology, the industry has entered a new stage of revolution. Traditional manufacturing enterprises are transforming into service-oriented manufacturing based on prognostic and health management (PHM). However, there is a lack of a systematic and comprehensive framework of PHM to create more added value. In this paper, the authors proposed an integrative framework to systematically solve the problem from three levels: Strategic level of PHM to create added value, tactical level of PHM to make the implementation route, and operational level of PHM in a detailed application. At the strategic level, the authors provided the innovative business model to create added value through the big data. Moreover, to monitor the equipment status, the health index (HI) based on a condition-based maintenance (CBM) method was proposed. At the tactical level, the authors provided the implementation route in application integration, analysis service, and visual management to satisfy the different stakeholders' functional requirements through a convolutional neural network (CNN). At the operational level, the authors constructed a self-sensing network based on anti-inference and self-organizing Zigbee to capture the real-time data from the equipment group. Finally, the authors verified the feasibility of the framework in a real case from China.

In the research of artificial joint biomechanics, it is a common method in the world to evaluate the biomechanical properties of the implanted fixtures through experiments in vitro. The domestic research started late, and the corresponding testing methods were lacking. There is still no unified standard. In this paper, a complete hardware test system was designed and built around the existing mechanical testing machine, including:binocular vision catcher, torque bearing clamp, film pressure sensor and so on. The system can accurately measure the relative motion angle value, external torque value, and inter pressure value of each joint. It has some reference significance for the forming and standardization of the artificial joints' evaluation system.

In the research of artificial joint biomechanics, it is a common method in the world to evaluate the biomechanical properties of the implanted fixtures through experiments in vitro. The domestic research started late, and the corresponding testing software were lacking. There is still no special software. In this paper, an integrated software test system was designed and built based on the existing hardware system, including:hardware control, data collection, data save, data processing and display. It can directly show the 3D motion trajectory and the angle curve of joints. The system can accurately measure the relative motion angle value, external torque value, and inter pressure value of each joint. It has some reference significance for the development of the artificial joints' evaluation system.

Cervical spondylosis is due to degenerative cervical disc and its stimulation or oppression of the adjacent nerves, spinal cord, spinal artery and other tissue caused by clinical symptoms. The cervical spine is an anatomical structure with activity, while the pillow has a certain plastic fixation effect on the cervical spine anatomy. Therefore, the pillow not only plays a health role in the cervical spine, but also plays an important role in restoring the normal physiological curvature of the cervical spine. Based on this, a multi-functional cervical vertebra treatment pillow is designed, which not only has the functions of traction, maintaining different positions of the cervical spine, correcting the cervical curvature and equipment exercises, but also has the functions of voice broadcast and network data terminal.

RESUMO

This paper constructs numerical models of bioprosthetic heart valve and blood. The fluid solid interaction is carried out using penalty function method. The mechanical property of the bioprosthetic heart valve during cardiac cycle is simulated with ANSYS software. Results show that the Von Mises stress concentrates at the junction of attachment edge and coaptation edge. The open time of bioprosthetic heart valve is consistent with that of actural measurement. The peak velocity of blood is in the range of physiology. This model provides more realistic mechanical property of bioprosthetic heart valve during cardiac cycle compared to pure solid model, and facilitates design and optimization of bioprosthetic heart valve.

Postoperative depression is a serious problem with under-optimized treatment. Our previous result demonstrated that single dose of propofol exposure in mice could cause depressive-like behavior. In the present study, we explored the possible preventive strategy that could ameliorate the depressive-like behavior in mice. Irisin was used for co-treatment with propofol in mice to test whether irisin could prevent the depressive-like behavior in mice. We found irisin could significantly reduce the immobility time in tail suspension and forced swimming tests in mice with propofol. We also found irisin was able to protect neurons against high concentration propofol treatment induced cell death in cultures. Meanwhile, irisin could significantly inhibit the increase of cytokines in astrocyte cultures exposed to propofol. These in vitro results suggested that interaction between astrocyte and neuron may be involved in the action mechanism that irisin prevented the mice from developing of depressive-like behavior. Last, we revealed that propofol caused increased surface expression of epidermal growth factor receptor (EGFR) but irisin could reduce the expression level of EGFR on cell surface.

To compare the effectiveness and safety of dynamic fixation (DF) and static fixation (SF) in distal tibiofibular syndesmosis injuries (DTSI) by a system review and meta-analysis. PubMed, Cochrane, and EMBASE were systematically searched by computer to select clinical randomized controlled trials (RCT) and cohort trials comparing DF and SF in treating patients with DTSI. RCT and cohort trials comparing DF and SF for patients with DTSI were included. Inclusion criteria: (i) prospective or retrospective study of patients with DTSI; (ii) patients were diagnosed as having DTSI by imageology and only received DF treatment or SF treatment; (iii) the study compared DF and SF in DTSI; and (iv) one or more of the following outcomes were reported: ankle joint functional score, surgical complications, malreduction of syndesmosis, and second operations. Exclusion criteria: (i) non-human studies; (ii) DTSI patients accompanied with other complications or other joints injuries; and (iii) full text unavailable. RevMan V5.3 software was used to perform the statistical analysis. Outcomes analyzed by Revman software showed that there were no statistically significant differences between DF and SF in the American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score (MD, 1.90; 95% CI, -0.23 to 4.03; P = 0.08; I2 = 0%), Olerud-Molander (OM) score (MD, 1.92; 95% CI, -7.96 to 11.81; P = 0.70; I2 = 55%), incidence of syndesmotic malreduction (RR, 0.19; 95% CI, 0.03 to 1.09; P = 0.06; I2 = 0%), and overall postoperative complication rate (RR, 0.30; 95% CI, 0.09 to 0.99; P = 0.05, I2 = 75%) and the rate of second procedure was significantly lower with DF (RR, 0.17; 95% CI, 0.07 to 0.43; P = 0.0002, I2 = 54%). Compared to SF, DF has an advantage, with a low rate of second procedures to treat DTSI.

BACKGROUND: Due to racial differences in the morphology of the knee joint and due to most prostheses available in the market being designed using measurements from Caucasians, the objective of this study was to provide the morphometric data of the patella for the southern Chinese population for total knee arthroplasty (TKA), patellar resurfacing, and prostheses design. METHODS: The CT slices of the knee joint were obtained from both knees of 40 Chinese volunteers (20 females, 20 males, and age from 20-25 years) by performing a computer tomographic scan. A 3D model was reconstructed by Mimics software based on the computed tomography images. Six metrical characteristics were measured by digital ruler. Statistical analysis was performed with the SPSS statistical program. RESULTS: The mean, standard deviation and P values of measurements and ratios were calculated using SPSS. All dimensions showed a significant gender difference with P < 0.05, but the six variables of the left and right knees had no statistical significance with P > 0.05. In addition, we studied the relationship between six couples (H-W, H-T, H-HAF, W-T, W-HAF, T-HAF) of the four variables (H: height, W: width, T: thickness and HAF: height of articulating facet) that were measured, which showed a significant correlation. CONCLUSIONS: Examination of the southern Chinese population revealed that males have larger patellae than women. In both genders, comparing data between left and right knees shows no statistically significant difference. Compared with Westerners in previous studies, the patella in our study was thin and small. There was a good linear regression correlation between measurements of the patella. The indirect measurement method on 3D models makes it easy to obtain anatomical data, and the results can provide a region and gender specific database for morphometric measurements of the patella, and can be helpful for designing implants suited for southern Chinese patients.
