BACKGROUND: The vast majority of ovarian mucinous carcinomas are metastatic tumours derived from nonovarian primary cancers, typically gastrointestinal neoplasms. Therapy targeting claudin18.2 might be used in gastric, gastroesophageal junction and pancreatic cancers with high expression of claudin18.2. In this study, we aimed to profile the expression of claudin18.2 in primary ovarian mucinous carcinoma (POMC) and metastatic gastrointestinal mucinous carcinoma (MGMC). METHODS: Immunohistochemistry was used to detect claudin 18.2 expression in whole tissue sections of ovarian mucinous carcinomas, including 32 POMCs and 44 MGMCs, 23 of which were derived from upper gastrointestinal primary tumours and 21 of which were derived from lower gastrointestinal primary tumours. Immunohistochemical studies for claudin18.2, SATB2, PAX8, CK7 and CK20 were performed in all 76 cases. RESULTS: Among 76 primary and metastatic mucinous carcinomas, claudin18.2 was expressed in 56.6% (43/76) of cases. MGMCs from the upper gastrointestinal tract, including 22 derived from primary stomach tumours and one derived from a pancreas tumour, were positive for claudin 18.2 in 69.5% (16/23) of cases. MGMCs from the lower gastrointestinal tract, including 10 derived from primary appendiceal cancer and 11 derived from colorectal cancers, showed no claudin18.2 expression (0/21). The expression rate of claudin18.2 in primary ovarian mucinous neoplasms, including 22 primary ovarian mucinous carcinomas and 10 primary ovarian borderline mucinous tumours, was 84.4% (27/32). The common immunophenotypic characteristics of POMCs, upper gastrointestinal tract-derived MGMCs, and lower gastrointestinal tract-derived MGMCs were claudin18.2 + /PAX8 + /SATB2- (17/32), claudin18.2 + /PAX8-/SATB2- (16/23) and claudin18.2-/PAX8-/SATB2 + (19/21), respectively. CONCLUSION: Claudin18.2 is highly expressed in POMCs and MGMCs derived from upper gastrointestinal tract primary tumours; therefore, claudin18.2-targeted therapy might serve as a potential therapeutic strategy for POMCs and MGMCs from the upper gastrointestinal tract.

Objective: To assess the utility of magnetic resonance imaging (MRI) medical technology in the perioperative management of brain gliomas and its impact on anesthesia and prognosis. Methods: An observational, retrospective comparative study was conducted. We selected 60 patients who underwent glioma resection at our hospital from January 2019 to January 2020. Patients were divided into two groups based on admission order: the experimental group (EG) and the control group (CG), with 30 cases each. Patients in the CG underwent conventional intracranial tumor surgery, while those in the EG underwent supratentorial craniotomy for tumor resection with the assistance of MRI medical technology. We compared perioperative parameters, hemodynamic indices, tumor resection outcomes, postoperative complications, and postoperative physical function between the two groups. Results: Compared to the CG, the EG had significantly longer surgery preparation time, anesthesia time, and surgery time (P < .001). However, there were no significant between-group differences in infusion volume and intraoperative blood loss (P > .05). Postoperative hemodynamic indicators were significantly higher in the EG than in the CG (P < .001), and postoperative tumor volume was markedly smaller in the EG (P < .001). The EG also achieved a significantly larger volume of tumor resection and a higher tumor resection rate (P < .001), a significantly lower total incidence of postoperative complications (P < .05), and notably higher Karnofsky Performance Status (KPS) scores (P < .001). Conclusions: Compared to conventional intracranial tumor surgery, the utilization of MRI medical technology in brain glioma surgery, although it prolongs surgery and anesthesia times, enhances the tumor resection rate, and offers significant advantages in improving the prognosis of patients with brain glioma.

In order to solve the problems of powder g-C3N4 catalysts being difficult to recycle and prone to secondary pollution, floating network porous-like sponge monolithic structure g-C3N4 (FSCN) was prepared with a one-step thermal condensation method using melamine sponge, urea, and melamine as raw materials. The phase composition, morphology, size, and chemical elements of the FSCN were studied using XRD, SEM, XPS, and UV-visible spectrophotometry. Under simulated sunlight, the removal rate for 40 mg·L-1 tetracycline (TC) by FSCN reached 76%, which was 1.2 times that of powder g-C3N4. Under natural sunlight illumination, the TC removal rate of FSCN was 70.4%, which was only 5.6% lower than that of a xenon lamp. In addition, after three repeated uses, the removal rates of the FSCN and powder g-C3N4 samples decreased by 1.7% and 2.9%, respectively, indicating that FSCN had better stability and reusability. The excellent photocatalytic activity of FSCN benefits from its three-dimensional-network sponge-like structure and outstanding light absorption properties. Finally, a possible degradation mechanism for the FSCN photocatalyst was proposed. This photocatalyst can be used as a floating catalyst for the treatment of antibiotics and other types of water pollution, providing ideas for the photocatalytic degradation of pollutants in practical applications.

The discharge of emerging pollutants in the industrial process poses a severe threat to the ecological environment and human health. Photocatalytic self-Fenton technology combines the advantages of photocatalysis and Fenton oxidation technology through the in situ generation of hydrogen peroxide (H2O2) and interaction with iron (Fe) ions to generate a large number of strong reactive oxygen species (ROS) to effectively degrade pollutants in the environment. Graphite carbon nitride (g-C3N4) is considered as the most potential photocatalytic oxygen reduction reaction (ORR) photocatalyst for H2O2 production due to its excellent chemical/thermal stability, unique electronic structure, easy manufacturing, and moderate band gap (2.70 eV). Hence, in this review, we briefly introduce the advantages of the photocatalytic self-Fenton and its degradation mechanisms. In addition, the modification strategy of the g-C3N4-based photocatalytic self-Fenton system and related applications in environmental remediation are fully discussed and summarized in detail. Finally, the prospects and challenges of the g-C3N4-based photocatalytic self-Fenton system are discussed. We believe that this review can promote the construction of novel and efficient photocatalytic self-Fenton systems as well as further application in environmental remediation and other research fields.

In this study, a Z-Scheme WO3/CoO p-n heterojunction with a 0D/3D structure was designed and prepared via a simple solvothermal approach to remove the combined pollution of tetracycline and heavy metal Cr(VI) in water. The 0D WO3 nanoparticles adhered to the surface of the 3D octahedral CoO to facilitate the construction of Z-scheme p-n heterojunctions, which could avoid the deactivation of the monomeric material due to agglomeration, extend the optical response range, and separate the photogenerated electronhole pairs. The degradation efficiency of mixed pollutants after a 70 min reaction was significantly higher than that of monomeric TC and Cr(VI). Among them, a 70% WO3/CoO heterojunction had the best photocatalytic degradation effect on the mixture of TC and Cr(VI) pollutants, and the removing rate was 95.35% and 70.2%, respectively. Meanwhile, after five cycles, the removal rate of the mixed pollutants by the 70% WO3/CoO remained almost unchanged, indicating that the Z-scheme WO3/CoO p-n heterojunction has good stability. In addition, for an active component capture experiment, ESR and LC-MS were employed to reveal the possible Z-scheme pathway under the built-in electric field of the p-n heterojunction and photocatalytic removing mechanism of TC and Cr(VI). These results offer a promising idea for the treatment of the combined pollution of antibiotics and heavy metals by a Z-scheme WO3/CoO p-n heterojunction photocatalyst, and have broad application prospects: boosted tetracycline and Cr(VI) simultaneous cleanup over a Z-scheme WO3/CoO p-n heterojunction with a 0D/3D structure under visible light.

The study area is a part of the North China Plain, where groundwater is heavily abstracted for drinking and irrigation purposes. Groundwater quality is adversely affected due to rapid economic development and urbanization. Therefore, the purposes of this study were to evaluate the suitability of groundwater for drinking and irrigation purposes and to quantify the associated human health risks in the southern part of Gu'an County, North China Plain. The matter-element extension method based on entropy weight was used to evaluate the water quality for drinking, while sodium adsorption ratio, sodium percentage (%Na), residual sodium carbonate and magnesium hazard were used to evaluate the water quality for irrigation. Non-carcinogenic and carcinogenic health risks via different exposure ways were evaluated for different age groups. The study found that the quality of both deep and shallow groundwater in this area was generally suitable for drinking. Deep water quality has better quality than the shallow water. However, 8.70% and 73.92% of water samples pose non-carcinogenic health risks on adults and children, respectively. Children and adults are also at cancer risk due to Cr6+ and As in drinking groundwater in this area. The main responsible parameters for non-carcinogenic risk are Cr6+, F- and Fe, and Cr6+ is also responsible for carcinogenic risk. These toxic elements are mainly from industries. Therefore, deterioration of groundwater quality can be prevented by strengthening the sewage management of various industries.

BACKGROUND: The prognosis of glioma is poor, despite recent advances in diagnosis and treatment of the disease. It is important to investigate the clinical characteristics and prognostic factors of glioma so as to provide basis for treatment and management of patients. METHOD: A total of 335 patients with glioma were included in this study. These patients were admitted to the medical center between November 2015 and December 2018. The clinical data, including demographic data, tumor characteristics, treatment strategy, expression pattern of tumor markers, and survival data, were retrospectively reviewed. Survival data were analyzed using Kaplan-Meier curves with log-rank test, while multivariate analysis Cox regression model was used to investigate risk factors for mortality. RESULTS: In this patient cohort, glioblastoma (40%), diffuse glioma (14.6%) and oligodendroglioma (9.6%) were the most common pathological types. The expression of Ki-67 was associated with several clinicopathological parameters (e.g. tumor type, grade, and number of lesions). In addition, Ki-67 correlated with the mortality within the first year of the post-treatment follow-up (P <  0.001). Kaplan-Maier analysis revealed that older patients (≥ 45 years) displayed worse prognosis than those aged under 45 years (P = 0.038). Dismal prognosis was also associated with clinical parameters, including high tumor grade, multiple lesions, and Karnofsky performance score (KPS). Multivariate analysis showed that low KPS (< 85) increased the risk of mortality by 2.3 folds with a 95% CI of 1.141 to 4.776 (P = 0.020). Low tumor grade (grade 1-2) oppositely reduced the mortality risk by 0.22 folds (95% CI, 0.065 to 0.763, P = 0.0168). CONCLUSION: KPS and tumor grade were independent prognostic factors in patients with gliomas.

The ZnWO4/Ag3PO4 nanocomposites synthesized by simple precipitation processes were characterized by powder X-ray diffraction (XRD), scanning electron microscopy (SEM), transmission electron microscopy (TEM), high-resolution transmission electron microscopy (HRTEM) and UV-vis diffuse reflectance spectra. The results indicated that the ZnWO4 nanorods dispersed well on the surface of Ag3PO4 particles and ball-and-rod structure p-n heterojunctions were successfully fabricated. In subsequent degradation experiments of methyl orange (MO), ZnWO4/Ag3PO4 composites showed the highest photocatalytic activity compared to pure Ag3PO4 and ZnWO4, due to the presence of ZnWO4/Ag3PO4 heterojunctions, which could separate and transfer the electron-hole pairs generated by visible light and enhance the photocatalytic performance of the catalysts. The band gap structure and degradation mechanism of the enhanced photocatalytic materials are also discussed in this article. In conclusion, the ZnWO4/Ag3PO4 composite is a promising and excellent photocatalyst for the degradation of dye wastewater under visible light irradiation.

Backgroud: The tumor immune microenvironment influences the efficiency of concurrent chemoradiotherapy (CCRT) in high-grade glioma (HGG). This study investigated peripheral blood T lymphocyte subsets as clinical indicators of therapeutic response and prognosis in pediatric high-grade glioma (pHGG). Methods: This retrospective study included 77 patients with postoperative pHGG who were treated concurrently with temozolomide and external beam radiotherapy between January 1, 2012, and December 31, 2018. The median follow-up was 26 (range: 5-106) months. Peripheral venous blood samples were collected before and after CCRT. The proportions of peripheral blood T lymphocytes and their association with treatment outcome and survival were determined. Results: Sixty-four (83.1%) patients achieved complete remission, partial remission, and stable disease, and 13 (16.9%) patients had progressive disease. Higher CD3+ T cell, CD4+ T cell, and CD8+ CD28+ T cell ratios were predictive of better response, while a higher CD8+ CD28- T cell ratio was predictive of poorer response. Binary logistic regression analysis showed that the CD8+ CD28+ T cell ratio was a significant independent predictor of CCRT response (odds ratio [OR] = 53.521, 95% confidence interval [CI] = 4.294-667.119, P = .002). Univariate and multivariate analysis of prognostic factors associated with survival showed that the CD8+ CD28+ T lymphocyte ratio was a significant independent predictor of progression-free survival (hazard ratio [HR] = 1.80, 95% CI = 1.06-3.08, P = .03), but none of the subsets were significantly associated with overall survival. Conclusion: Peripheral blood T lymphocytes have potential as predictors of CCRT response and prognosis in pHGG.

OBJECTIVE: The aim of this study is to explore whether preoperative hemoglobin levels could serve as a prospective biomarker for early brain injury in patients with aneurysmal subarachnoid hemorrhage (aSAH). This investigation seeks to discern its association with postoperative complications and unfavorable clinical outcomes. METHODS: We conducted a comprehensive analysis of data derived from the LongTeam registry, including patients with aSAH diagnosed between January 2015 and September 2021. These patients were stratified into three distinct groups based on their hemoglobin levels: anemic, standard, and elevated HGB. We employed logistic models featuring spline transformations to assess the relationship between HGB levels and in-hospital complications. Furthermore, a multivariate Cox proportional hazard model was employed to estimate the impact of elevated hemoglobin levels on the hazard function, which was elucidated through Kaplan-Meier curves. RESULTS: Our study comprised a total of 988 patients, among whom 115 (11.6%) presented preoperative anemia, and 63 (6.4%) exhibited elevated preoperative HGB levels. Following adjustments for potential confounding factors, no significant disparity in risk was evident between anemic patients and those with standard HGB levels. However, individuals with elevated HGB levels displayed a heightened incidence and an increased risk of developing deep vein thrombosis (DVT, odds ratio [OR] = 2.39, 95% confidence interval [CI] = 1.16-4.91, p = 0.018; hazard ratio [HR] = 2.05, 95% CI 1.08-3.92, p = 0.015). Aberrant HGB concentrations did not demonstrate an association with other clinical outcomes. CONCLUSION: Our findings emphasize that abnormal HGB levels show no association with adverse outcomes at the 90 days mark after accounting for clinical confounding factors in patients with aSAH. Simultaneously, the study illuminates the potential of HGB as an early indicator for identifying patients at a heightened risk of developing DVT.

Introduction: The relationship between different puncture points and perioperative complications and length of stay in hospital (LOS) in SCCAG patients has rarely been reported. Aim: To compare the curative effect and safety of the transradial artery approach and the transfemoral artery approach in combined heart-brain angiography. Material and methods: 120 patients who received combined cardio-cerebral angiography in our hospital were selected and divided into a transradial artery approach group (TRA) and a transfemoral artery approach group (TFA) according to a random number table. The postoperative efficacy and safety of the 2 groups were compared. Results: There was no statistically significant difference in puncture time and operation time between the 2 groups (p > 0.05). Postoperative bed rest time, hospitalization time, and X-ray exposure time in the TRA group were shorter than those in the TFA group, and the difference was statistically significant (p < 0.05). Before operation and 3 days after operation, there was no significant difference in left ventricle ejection fraction between the 2 groups (p > 0. 05). The overall incidence of complications in the TFA group was higher than that in the TRA group. The incidence between haematoma and pseudoaneurysm in the TFA group was higher, and the difference was statistically significant (p < 0.05). Conclusions: For simultaneous heart-brain angiography, interventional therapy via radial artery and femoral artery has good curative effect and can improve cardiac function. However, interventional therapy through the radial artery can shorten the postoperative bed rest time and hospitalization time, and reduce the incidence of complications.

The rapid development of the social economy and the influence of human activities can lead to aggravated groundwater pollution. Groundwater safety is the premise of residents' health. Therefore, studying the sustainable utilization and health risks of groundwater quality is important. The groundwater quality and potential health risks were evaluated in the Shijiazhuang area, which is located in the North China Plain in this paper. Based on 159 groundwater samples collected in the study area, the potential health risks of As, Cr6+, Ni, Pb, F-, and NO3 - to humans were evaluated from oral and skin contact. Results of the human health risk assessment showed that the average carcinogenic risk and non-carcinogenic risk of children are higher than those of adults. According to the spatial distribution of the total risk value, adults and children in the southwest of the study area face higher risks. Because of the uncertainty of USEPA, Monte Carlo simulation was used to calculate the probability of health risk assessment and prioritization of contaminant treatment. The results of the Monte Carlo simulation showed that the acceptable range for children is 6.82%, and the acceptable range for adults is 18.07%. According to the HRWM model, carcinogenic pollutants mainly include As, Cr6+, and Ni. The most important chemical species of As is HAsO4 2-, followed by H2AsO4 -. Similarly, CrO4 2- and Ni2+ are the main forms of Cr6+ and Ni. The results of this study can provide data support for the protection and management of groundwater quality in the North China Plain. PRACTITIONER POINTS: Children are more susceptible to carcinogenic risk than adults. After calculation, the main influencing elements are Ni and Cr. Metal morphology analysis was carried out, and the results showed that HAsO4 2-, CrO4 2-, and Ni2+ were the main types.

The recent use of PARP inhibitors (PARPi) in the maintenance treatment of ovarian tumor has significantly improved the survival rates of cancer patients. However, the current oral administration of PARP inhibitors fails to realize optimal therapeutic effects due to the low bioavailability in cancerous tissues, and often leads to a range of systemic adverse effects including hematologic toxicities, digestive system reactions, and neurotoxicities. Therefore, the demand for an advanced drug delivery system that can ensure effective drug administration while minimizing these unfavorable reactions is pressing. Injectable hydrogel emerges as a promising solution for local administration with the capability of sustainable drug release. In this study, we developed an injectable hydrogel made from aminated hyaluronic acid and aldehyde-functionalized pluronic127 via Schiff base reaction. This hydrogel exhibits excellent injectability with short gelation time and remarkable self-healing ability, and is applied to load niraparib. The drug-loaded hydrogel (HP@Nir hydrogel) releases drugs sustainably as tested in vitro as well as displays significant anti-proliferation and anti-migratory properties on human epithelial ovarian cancer cell line. Notably, HP@Nir hydrogel effectively suppresses the growth of ovarian cancer, without significant adverse reactions as demonstrated in animal studies. Additionally, the developed hydrogel is gradually degraded in vivo for around 20 d, while maintaining good biocompatibility. Overall, the injectable hydrogel loaded with niraparib provides a secure and efficient strategy for the treatment and management of ovarian cancer.

Solar energy is the inevitable choice to achieve the low-carbon, green, and circular development of society, and photocatalysis technology is one of the shining pearls. To make full use of the solar spectrum and solve the shortcomings of the recovery difficulty of powdery materials and the loss of activity due to the influence of the external environment, it is possible to construct floating materials using melamine sponges to recover photocatalytic materials quickly. At the same time, floating materials can absorb oxygen in the air for the generation of active groups, effectively solving the problem of less O2 in the water. The carbon-based materials have excellent light absorption properties, high thermal conductivity, and excellent photothermal conversion efficiency and are ideal for constructing floating photothermal photocatalytic systems. As an example, we combined a cheap melamine sponge with urea, prepared a hollow porous network structure g-C3N4 (HPNCN) with a high specific surface area by direct thermal shrinkage method, and then attached the CoO to its surface by hydrothermal method to form a heterojunction with a suitable band gap. Various characterization tests verified the photothermal-photocatalytic properties. Among them, 30% CoO/HPNCN has the best photocatalytic degradation effect on tetracycline (TC), and the removal rate is 88.1%. After five cycles, the removal rate is only 5% lower than the initial, indicating that it has good stability and recyclability. We conducted an active ingredient capture experiment, ESR, and LC-MS analysis to clarify the intermediates and reaction mechanism of TC photocatalytic degradation. On this basis, the ECOSAR program and QSAR method were used to analyze the environmental toxicity of TC and its intermediate products. These results provide a broad prospect for the potential application of the floating photothermal-photocatalysis system in antibiotic pollution control and its application in other fields.

The step-scheme (S-scheme) heterojunction has excellent redox capability, effectively degrading organic pollutants in wastewater. Combining S-scheme heterojunction with activated persulfate advanced oxidation process reasonably can further enhance the degradation of Emerging Contaminants. Herein, a novel zero-dimensional/one-dimensional (0D/1D) CoO-CuBi2O4 (CoO-CBO) photocatalyst with S-scheme heterojunction was designed by hydrothermal and solvothermal methods. The band structure and electron and hole transfer pathway of CoO-CBO were analyzed using the ex-situ and in-situ X-ray photoelectron spectroscopy (XPS), Ultraviolet and Visible Spectrophotometer (UV-Vis) and optical radiation Kelvin probe force microscope (KPFM), and the formation of S-scheme heterojunction was demonstrated. The photocatalytic activity of ·S-scheme CoO-CBO heterojunction was carried out by degrading tetracycline (TC) with activating potassium monopersulfate triple salt under visible light. Compared with pure CuBi2O4 and pure CoO, 30%CoO/CuBi2O4 catalyst exhibited the highest TC degradation performance after activating persulfate, degrading 89.5% of TC within 90 min. On the one hand, the S-scheme heterojunction formed between CoO and CBO had a high redox potential. On the other hand, the activation of persulfate by Co and Cu could accelerate redox cycles and facilitate the generation of active radicals such as SO4-, O2- and OH, promoting the separation of the photogenerated e- and h+ in the composite, enhancing the peroxydisulfate (PDS) activation performance and improving the degradation effect of TC. Then, a gradual decrease in the toxicity of the intermediates in the TC degradation process was detected by ECOCER. In all, this study provided an S-scheme CoO/CuBi2O4 heterojunction that can activate PDS to degrade TC efficiently, which provided a new idea for the study of novel pollutant degradation and environmental toxicology.

Domestically and internationally, the effect of fracture flowing water and transferring heat on the temperature field of surrounding rock in high-level radioactive waste repositories is a popular research area. Compared with straight fracture flowing water and transferring heat, there are few relevant literatures about the heat transfer of curved fracture water flow. Based on the conceptive model of flowing water and transferring heat in curved fractured rock mass, the influence of flowing water and transferring heat in "I", "L", , and shaped fractures on the temperature field of rock mass is calculated by using discrete element program. The findings indicate that: When the model goes into a stable state under four working conditions, the rock on the x = 0-2 m mostly forms a heat transfer path from left to right; the x = 2-4 m primarily forms a heat transfer path from bottom to top, and the temperature gradient reveals that the isotherm of 40-45 °C is highly similar to the shape of four different fractures, indicating that flowing water and transferring heat in the fracture configuration dominate the temperature field of the right side rock mass. The direction of the flowing water and transferring heat of the fracture exerts a dominant effect on the temperature of the rock mass than the length.

Objective: This study employs case data analysis to elucidate alterations in hormone levels pre and post-surgery among patients with pituitary adenoma. Moreover, it investigates the influence of various associated factors on endocrine function. Methods: A retrospective analysis was conducted on clinical data from 428 patients who underwent surgical treatment within a short period at a single center. Statistical methods were employed to examine detailed hormone level fluctuations before and after surgery in patients with pituitary adenoma, along with their interrelations with different factors. Results: Between January 2016 and October 2022, a total of 428 consecutive patients with pituitary adenoma underwent surgical treatment. Of these, 232 were males and 196 were females, with an average age of 45.91 years (range: 16-84, standard deviation: 12.18). Univariate analysis indicated that females exhibited a higher susceptibility to preoperative endocrine dysfunction (p < 0.05). Conversely, males, patients with larger tumor volumes, and older patients were more prone to preoperative pituitary insufficiency. Postoperatively, the most substantial remission rate occurred in prolactin (77.4%), followed by growth hormone (GH) (71.8%) and cortisol (4/6). The highest recovery rate in functions was observed in growth hormone secretion function (80%), followed by pituitary-adrenal axis hormone secretion function (56.3%) and pituitary-thyroid axis hormone secretion function (47.5%). The most noteworthy incidence of newly developed postoperative endocrine deficiencies was found in the pituitary-adrenal axis (31.8%), while the occurrence rates of deficiencies in other axes were relatively low. The elevated postoperative remission rate of growth hormone correlated with a higher surgical resection rate and lower preoperative growth hormone levels. Additionally, lower preoperative prolactin levels corresponded to a higher remission rate of prolactin postoperatively. Furthermore, the restoration of postoperative thyroid hormone secretion function was associated with higher preoperative free thyroxine levels. Reduced postoperative cortisol secretion function was linked to multiple surgeries and an extended interval between hormone retesting and surgery. Conclusion: Surgical intervention effectively ameliorates endocrine disorders in pituitary adenoma patients, thereby mitigating symptoms and enhancing their quality of life. Preoperative management of growth hormone and prolactin levels facilitates an increased remission rate of these hormones post pituitary adenoma surgery. Patients displaying preoperative thyroid hormone secretion dysfunction should be considered for active supplementation therapy. Whenever feasible, complete tumor resection is recommended. For patients undergoing reoperation or multiple surgeries, vigilant postoperative cortisol monitoring and supplementation should be thoughtfully administered.

BACKGROUND: To describe and report the efficacy and safety of MR-guided laser interstitial thermal therapy (MRgLITT) in the treatment of drug-resistant epilepsy. METHODS: A retrospective review of all MRgLITT procedures in our hospital was performed. All procedures were performed using a surgical laser ablation system. Demographic and outcome data were compiled and analyzed. RESULTS: A total of 19 patients underwent MRgLITT procedures from June 2021 to November 2021. The average age at surgery was 18.1 years (3-61.4 years). The average length of hospitalization post-surgery was 4.95 days (4-7 days). Surgical substrates included 8 patients with hypothalamic hamartomas, 5 with medial temporal lobe epilepsy, 3 with deep focal cortical dysplasia, 1 with tuberous sclerosis, 1 with a cavernous malformation, and 1 with Lennox-Gastaut syndrome who underwent anterior corpus callosotomy. Complications occurred in three patients. After an average follow-up of 1 year, 6 patients were seizure-free (Engel I, 31.6%), 1 had significant seizure control (Engel II, 5.3%), 7 had seizure control (Engel III, 36.8%), and 5 had no improvement in their seizures (Engel IV, 26.3%). Fisher's exact tests did not reveal statistical significance for the association between Engel class outcome and epileptic disease. CONCLUSION: This study confirmed that MRgLITT, as a method for treating drug-resistant epilepsy, is minimally invasive, safe, and efficient and that it can reduce the incidence of surgery-related complications.

Brain arteriovenous malformations (AVMs) with a diffuse nidus structure present a therapeutic challenge due to their complexity and elevated risk of hemorrhagic events. This study examines the long-term effectiveness of interventional therapy versus conservative management in reducing hemorrhagic stroke or death in patients with ruptured diffuse AVMs. The analysis was conducted based on a multi-institutional database in China. Patients were divided into two groups: conservative management and interventional therapy. Using propensity score matching, patients were compared for the primary outcome of hemorrhagic stroke or death and the secondary outcomes of disability and neurofunctional decline. Out of 4286 consecutive AVMs in the registry, 901 patients were eligible. After matching, 70 pairs of patients remained with a median follow-up of 4.0 years. The conservative management group showed a trend toward higher rates of the primary outcome compared to the interventional group (4.15 vs. 1.87 per 100 patient-years, P = 0.090). While not statistically significant, intervention reduced the risk of hemorrhagic stroke or death by 55% (HR, 0.45 [95% CI 0.18-1.14], P = 0.094). No significant differences were observed in secondary outcomes of disability (OR, 0.89 [95% CI 0.35-2.26], P = 0.813) and neurofunctional decline (OR, 0.65 [95% CI 0.26 -1.63], P = 0.355). Subgroup analysis revealed particular benefits in interventional therapy for AVMs with a supplemented S-M grade of II-VI (HR, 0.10 [95% CI 0.01-0.79], P = 0.029). This study suggests a trend toward lower long-term hemorrhagic risks with intervention when compared to conservative management in ruptured diffuse AVMs, especially within supplemented S-M grade II-VI subgroups. No evidence indicated that interventional approaches worsen neurofunctional outcomes.
