RESUMO

OBJECTIVES: Antiretroviral therapy (ART) during acute HIV infection (AHI) restricts the HIV reservoir, but additional interventions are necessary to induce a cure. Intravenous immunoglobulin (IVIG) is not HIV-specific but is safe and temporarily reduces the HIV reservoir in chronic HIV infection. We present a randomized controlled trial to investigate whether IVIG plus ART in AHI reduces the HIV reservoir and immune activation compared with ART alone. METHODS: Ten men with AHI (Fiebig II-IV) initiated ART (tenofovir, entricitabine, ritonavir boosted darunavir and raltegravir) at HIV-1 diagnosis and were randomized to ART alone or ART plus 5 days of IVIG, once virally suppressed (week 19). Blood samples were evaluated for viral reservoir, immune activation, immune exhaustion and microbial translocation. Flexible sigmoidoscopy was performed at weeks 19, 24 and 48, and gut proviral DNA and cell numbers determined. RESULTS: IVIG was well tolerated and no viral blips (> 50 HIV-1 RNA copies/mL) occurred during IVIG therapy. From baseline to week 48, total HIV DNA in peripheral blood mononuclear cells (PBMCs) (cases: -3.7 log10 copies/106 CD4 cells; controls: -3.87 log10 copies/106 CD4 cells) declined with no differences observed between the groups (P = 0.49). Declines were observed in both groups from week 19 to week 48 in total HIV DNA in PBMCs (P = 0.38), serum low copy RNA (P = 0.57) and gut total HIV DNA (P = 0.55), but again there were no significant differences between arms. Biomarkers of immune activation, immune exhaustion and microbial translocation and the CD4:CD8 ratio were similar between arms for all comparisons. CONCLUSIONS: Although safe, IVIG in AHI did not impact total HIV DNA, immune function or microbial translocation in peripheral blood or gut tissue.

Sex researcher Alfred Kinsey's vision of sexual taxonomy continued to evolve after he published his first landmark volume on human sexuality, and his research into sexual subcultures went beyond his initial studies of homosexuality and prostitution. In the late 1940s and early 1950s, he developed a new interest in cross-dressing and cross-gender identification. This article outlines how and why he began to interview transvestites and transsexuals, and places his emerging vision of gendered behavior and gender identity within the scientific theories of his day. Kinsey rejected the prevailing views, preferring instead a behaviorist model of gender. He saw cross-dressing and crossgender identification as male phenomena and used them to speculate about sex differences in the capacity for psychological conditioning. In his usual style, he did not condemn transvestites or transsexuals, but he disapproved of the genital surgery requested by male-to-female transsexuals. It was here that Kinsey hit the limits of his well-known sexual liberalism in which he approved of all sexual variations that did not involve coercion.

RESUMO

The assumed etiology of adolescent illegitimate pregnancy is reviewed, and seven central concepts are indicated. The prognostic value of these, concepts, expressed as antecedent attitudes, is evaluated by secondary analysis of data drawn from a lengthy, survey of seventh graders. A multiple regression equation, of three factor-analysis terms, CR(2)=0.83, is described.

This study was undertaken to compare the effect of eugenol containing and non-eugenol temporary cements on the bond strength of three brands of luting cements to enamel. Flat enamel surfaces were prepared on 90 surgically removed, unerupted, human third molar teeth. The teeth were randomly divided into three groups of thirty. The flat enamel surfaces in two of the groups were treated with either a eugenol containing or non-eugenol temporary cement and the third group was left untreated. The teeth were stored in water for 7 days and the cements then removed and all surfaces etched. The teeth were divided into 9 sub-groups of 10 each and one of the three resin cements was then bonded to each sub-group. The relevant shear bond strengths were determined after 7 days. The results indicated that prior use of a eugenol containing temporary cement reduced the resin cement-enamel bond strengths. No differences were found between the bonds achieved by the three brands. It was concluded that eugenol containing temporary cements should not be used prior to bonding with resin luting cements.
