RESUMO

OBJECT: To evaluate the subjective outcome and quality of life (QoL) of patients who suffered from aneurysmal subarachnoid hemorrhage and underwent endovascular coiling or microsurgical clipping in a single center. METHODS: For this retrospective single-center study, we included patients who underwent aneurysm occlusion at the Cantonal Hospital of Aarau between January 2000 and December 2006. The QoL, the functional status, and the level of independence were assessed by means of the Short Form (SF)-12 Health Survey, the modified Rankin Scale (mRS), and the Barthel Index. The questionnaires were sent to and completed by the patients. A total of 104 patients with a mean age of 53.14 years (range, 18-80 years) were included in the study. In 63 (60.6 %) of the cases, the aneurysm was clipped; in 41 (39.4 %) of the cases, endovascular coiling was performed. RESULTS: The SF-12 scores for the PCS (Physical Component Summary) and MCS (Mental Component Summary) were similar for both clipped (PCS 45.35; MCS 46.55) and coiled (PCS 46.31; MCS 47.87) patients. The mean values were, on average, 4.17 points lower for the PCS and 2.79 points lower for the MCS when compared with the mean of the US population, with a mean of 50 (standard deviation (SD) 10). The mean Barthel Index for the entire group was 92.26 (SD 16.8) and was almost identical for both the clipped (92.54; SD 16.21) and coiled (91.83; SD 17.9) patients (p = 0.56). The mean mRS did not differ between the coiled and clipped patients (coiled 1.63; clipped 1.56; p = 0.97) CONCLUSIONS: There were no significant differences in the functional and mental health scores between the two groups of clipped and coiled patients who were treated at our center, but both groups were lower than population-based scores. Although the neurologic condition and the imaging results on admission were worse in the coiled group, the long-term results did not differ significantly.

RATIONALE AND OBJECTIVES: This cooperative multicenter human study was designed to evaluate the safety, magnetic resonance (MR) imaging characteristics, and clinical response to a single gadolinium contrast agent: gadopentetate dimeglumine. MATERIAL AND METHODS: Ninety-five patients (age range: 1 month to 78 years; sex: 50 males, 45 females) were included in this prospective study. The patients presented clinically with a variety of cranial or spinal signs and symptoms for which an intrathecal contrast myelogram or cisternogram was requested by clinical staff. Via lumbar puncture (20-25 g needle), 3 to 5 mL/ml of cerebrospinal fluid were withdrawn and mixed with a single volume of 0.5 (n = 63), 0.7 (n = 13), 0.8 (n = 12), or 1.0 (n = 7) cc/mL of gadopentetate dimeglumine (Magnevist; Schering, Berlin, Germany). This was then injected into the subarachnoid space, and the needle was removed. Immediate and delayed (up to 96 hours) T1- and T2-weighted MR imaging was performed on super conductive, high-field (1.0-1.5 tesla) imaging units in two or three planes. All patients were hospitalized for an observation period of 24 hours following the procedure, and follow-up neurologic examinations were performed serially for 6 to 12 months afterward. RESULTS: No patient manifested gross behavioral changes, neurologic alterations, or seizure activity at any time following the procedure. Nineteen patients (20%) experienced postural postlumbar puncture headache, six patients had nausea (6%), and two patients had episodes of vomiting (2%), all which resolved within the first 24 hours of the lumbar puncture with conservative bed rest. CONCLUSION: This cooperative study demonstrates the general safety and feasibility of low dose (0.5-1.0 mL/ml) intrathecal gadopentetate dimeglumine administration. The potential useful clinical applications include the evaluation of obstructions and communications of the various subarachnoid spaces, spontaneous or traumatic/postsurgical craniospinal cerebrospinal fluid leaks, and subarachnoid space CSF flow and parenchymal CNS interstitial diffusion dynamics. This worldwide cooperative study seeks to progressively perform human studies for further definitive evaluation of the practical clinical applications, of the relationship of this technique to other imaging studies and modalities, and the long-term safety of the procedure in a larger number of subjects.

AIMS: To determine cerebrospinal fluid (CSF) dynamics between intracranial CSF spaces and CSF in the subarachnoid space (SAS) of optic nerves (ONs) in 10 patients with papilloedema. METHODS: Prospective assessment of 10 patients with papilloedema and two control subjects using CT cisternography and analysis of CSF for the presence of lipocalin-like prostaglandin D synthase (betatrace protein). RESULTS: CT cisternography showed a progressively reduced influx of contrast-loaded CSF from intracranial CSF spaces into the SAS. The lowest concentration of contrast-loaded CSF was found in the region of the ON immediately behind the globe, where the ON sheath was widened (possibly by unfolding) in all patients compared with normal subjects. The concentration of lipocalin-like prostaglandin D synthase differed between the spinal CSF and the CSF in the SAS, with a markedly higher concentration in the SAS. CONCLUSION: The results of this study suggest that CSF turnover in the SAS of the ON is reduced in patients with papilloedema from various causes and that the composition of CSF differs between spinal CSF and that surrounding the ON.
