RESUMO

A comparative analysis was made of efficacy of chronic tonsillitis (CT) treatment with high frequency ultrasound (HFU), low frequency ultrasound (LFU) and laser. The study covered 172 CT patients aged 7 to 35 (65 males and 107 females) with CT history 5-32 years. Clinical examination included micropharyngoscopy, blood count, urine tests, microbiological investigation of tonsillar lacune lavage and nasal mucus, immunological tests, consultation of the therapist. Tonsillar prints were analysed for 40 patients. A compensated CT form was diagnosed in 58 patients, a decompensated one--in 114. The treatment efficacy was evaluated in the course of therapy and 1, 3, 6, 12 and 24 months after it. Immediate results were the best in patients treated with LFU on the palatine tonsils (73%). Patients exposed to LFU and laser showed good response in 35-37% in equal number of unsatisfactory outcomes. Repeated courses of all three therapies improved results in all the groups: good and satisfactory results were 69%, 51% and 43% for LFU, HFU, laser, respectively. Thus, LFU (Tonsillor device) is the most efficient up-to-date conservative therapy of CT which provides mechanical elimination of pathogens from the tonsils, a bactericidal effect, recovery of the barrier function.

RESUMO

Basing on his long-term experience in otiatria, the author believes that further improvement in surgical management of otitis media purulenta chronica lies in advancing microsurgical techniques and skill of the surgeons, design of operations more effectively preserving and improving hearing, better understanding of microsurgical anatomy of the middle ear, wide introduction of low-frequency ultrasound and low-energy laser.

RESUMO

Taking into account the severity of middle ear pathology and the Eustachian tube function, three methods of "closed" surgery were applied to 132 patients with chronic purulent-caries epimesotympanitis. On post-operation days 2-15, 85 patients were exposed to internal or external laser therapy. As a result, the post-operation therapy period was 5-6 days shorter and the healing quality of the treated ear a year after was better than in the absence of laser therapy. It is concluded that the method of "closed" surgery should be chosen on an individual basis and that its combination with early post-operation laser therapy yields good results.

RESUMO

By means of biophotometry the coefficients of absorption and penetration depth of helium-neon laser radiation with a wavelength of 632.8 nm were measured. Measurements were made in the inflammatory mucous membrane of the tympanic cavity, mucous membrane of the tympanic-cavity infiltrated with the saline solution, cortical bone of the mastoid, cartilage of the auricle, blood, serum, exudate, 0.02% furacillin, 3% boric acid, 0.9% and 10% NaCl. These observations were used to develop an efficient method of laser therapy of the inflammatory and wound surface of the middle ear biological tissue, which formed the basis of a new procedure of the laser treatment of "closed" cavities of the middle ear in an early postoperative period after debridement or hearing repair surgery of suppurative otitis media which were filled with a transparent antiseptic solution.
