RESUMO

Resistance exercise is deemed safe for women recovering from conventional breast cancer therapies but few clinicians are aware that dragon boat racing, as a form of resistive exercise, is available to the breast cancer community. The objectives of this study were to 1) increase clinician awareness of dragon boat racing (DBR) in breast cancer survivors as a community-based physical activity, and 2) evaluate quality of life (QOL) in breast cancer survivors with or without lymphedema who participate in DBR. This prospective, observational study surveyed 1,069 international breast cancer dragon boat racers from eight countries to compare function, activity, and participation in women with and without selfreported lymphedema using the Lymph-ICF questionnaire. Seventy-one percent of women (n=758) completed the questionnaires. Results revealed significantly higher Lymph-ICF scores in the lymphedema participants, signifying reduced QOL, when compared to the nonlymphedema participants (p<0.05), except for "go on vacation" for which no statistical difference was reported (p=0.20). International breast cancer survivors with lymphedema participating in DBR at an international competition had reduced function, limited activity, and restricted participation compared to participants without lymphedema. Clinicians should consider utilizing DBR as a community-based activity to support exercise and physical activity after a breast cancer diagnosis.

The methods available for the isolation of Yersinia enterocolitica from foods are generally considered to be less than optimal, and methods for estimation of numbers are lacking. Such methods are needed to understand better the significance of foodborne yersiniosis and to provide data for exposure assessment. We describe a method for the detection and enumeration of Y. enterocolitica containing the pYV virulence plasmid (YeP+) in samples from pork surfaces. The method uses a multiplex PCR targeting the ail and virF genes to detect Y. enterocolitica after incubation of surface swabs in Yersinia enrichment broth according to Ossmer. Enumeration was achieved by adapting the enrichment to a most probable number (MPN) method format. A presumptive result was available within 24 h of sample receipt, and YeP+ isolates were confirmed within four days. The presence/absence and MPN methods were evaluated in a pilot survey of 34 packs of raw pork meat purchased from retail outlets in Christchurch, New Zealand. YeP+ was detected by PCR on meat from 32% of the packs, and YeP+ isolates were obtained from 18% of the samples. YeP+ were present at numbers ranging from 0.30 to 5.42 MPN/cm(2). This improved method for the detection and enumeration of YeP+ from meat samples can be used for microbiological surveys to obtain data for assessments of consumer exposure to virulent Y. enterocolitica, and in outbreak investigations.

A capillary column gas chromatographic/high resolution mass spectrometric method was developed for the determination of monobromopolychlorodibenzo-p-dioxins (BPCDDs) in environmental samples. The mass spectrometer was operated at high resolution (> 10,000) in the selected ion monitoring mode with magnet switching among tetra through octa groups to achieve low or subpicogram detection limits. Standard BPCDDs (tetracongener through octacongener groups) were utilized to measure accurately the mass spectrometric relative response factors for these compounds, and linear calibrations were achieved by using these standard compounds at different concentrations. The method detection limit is low or sub-parts-per-trillion levels for BPCDDs, the accuracy better than 80% and the precision better than ± 10%. Quantification of BPCDDs and preliminary identification of some specific BPCDD isomers in fly ash samples are also reported.

Recent improvements in the LC-MS interface have increased the sensitivity and selectivity of this instrument in the analysis of polar and thermally-labile aqueous constituents. Determination of RDX, nitroso-RDX metabolites, and other munitions was enhanced using LC-MS with solid-phase extraction, 15N3-RDX internal standard, and electrospray ionization (ESI) in negative ion mode. ESI produced a five-fold increase in detector response over atmospheric pressure chemical ionization (APCI) for the nitramine compounds, while the more energetic APCI produced more than twenty times the ESI response for nitroaromatics. Method detection limits in ESI for nitramines varied from 0.03 microgram l-1 for MNX to 0.05 microgram l-1 for RDX.

A method using liquid chromatography-tandem mass spectrometry has been developed for determination of trace levels of tetracycline antibiotics in ground water and confined animal feeding operation waste water. Oxytetracycline (OTC), tetracycline (TC), and chlortetracycline (CTC) were extracted from water samples using both polymeric and C18 extraction cartridges. The addition of a buffer containing potassium phosphate and citric acid improved tetracycline recoveries in lagoon water. Method detection limits determined in reagent water fortified with 1 microg l(-1) OTC, TC, and CTC were 0.21, 0.20, and 0.28 microg l(-1). Method detection limits in lagoon water samples fortified at 20 microg l(-1) for OTC, TC, and CTC were 3.6, 3.1, and 3.8 microg l(-1). Variability in recovery from laboratory fortified blanks ranged from 86 to 110% during routine analysis.

Eleven medial subtalar dislocations, two of them open, were treated during the decade from 1969 to 1978 in four metropolitan Detroit hospitals. There was one open lateral dislocation. Eight patients were available for follow-up study. None of the dislocations was associated with a major fracture of the talus. The average time between injury and our follow-up was twenty-eight months. A simple qualitative method of measuring range of motion of the subtalar joint was devised. All of the patients had some loss of subtalar motion, but the lateral dislocation was the only one causing important disability.

AIMS: Little information exists regarding the microbiology of two traditional Maori food preparation processes which may involve fermentations. Preliminary microbiological and chemical analyses were carried out on these two foods in order to identify the fermentations involved (if any). METHODS AND RESULTS: Testing was carried out on freshly-prepared foods and on those that had been processed and stored. Kina (sea urchins, Evechinus chloroticus) are harvested and then stored either under fresh water or buried underground. The most frequently-occurring process appeared to be an alkaline fermentation. Large numbers of Clostridium perfringens were detected in one set of samples prepared outside of the traditional season, but this was the only pathogen detected. In Kina stored in buried plastic bottles during the traditionally-accepted time of the year, bacterial numbers decreased. Tiroi is prepared from mussels and Puha (sow thistle, Sonchos asper) that have been cooked to some degree, combined and stored. Of three methods used to prepare and store Tiroi, the results for one indicated the possible involvement of a lactic acid fermentation, but the other two methods were effectively only cooking and bottling processes. CONCLUSIONS: In the case of Kina, the use of an alkaline fermentation to prepare a seafood for consumption is unusual. One method of Tiroi production is a lactic acid fermentation. SIGNIFICANCE AND IMPACT OF THE STUDY: If these foods are produced as described and are not either eaten immediately or cooked before consumption, then growth, and intoxication by, Clostridium botulinum might occur.

Capillary column gas chromatography/high-resolution mass spectrometry was used in two different selected-ion monitoring modes to analyze trace levels of monobromopolychlorodibenzo-p-dioxins and dibenzofurans (BPCDD/Fs) in municipal waste incinerator (MWI) flyash. The mass profile monitoring mode is well-suited for identification of unknown compounds in uncharacterized matrices because it has superior diagnostic capability. Owing to its high sensitivity, conventional peak top monitoring was used to quantify, on the basis of polychlorodibenzo-p-dioxin and dibenzofuran (PCDD/F) standards, the BPCDD/Fs in the sample. The results were compared with those obtained by using two commercial BPCDDs as standards, and the latter results are 4 times greater, indicating the need for appropriate standards. The high certainty and sensitivity obtained from these two mass spectrometric techniques combined with the resolving power of capillary gas chromatography enabled us to compare for the first time the isomer distribution patterns between BPCDD/Fs and their PCDD/F analogues at a high confidence level. The comparison indicates BPCDD/Fs and PCDD/Fs found in MWI flyash are closely related and that many BPCDD/Fs with a 2,3,7,8-substitution configuration may be present in MWI flyash.

The advantages of gas chromatography/mass spectrometry (GC/MS) selected-ion monitoring (SIM) in the mass profile (MP) mode at medium mass resolving power were investigated for analyses requiring detection of low-picogram quantities of analytes in complex mixtures. The mass profile monitoring provides a certainty at least 10 times greater than that achieved by conventional GC/MS-SIM in the peak-top monitoring mode, and it can be operated at lower mass resolving power to compensate for the loss of sensitivity in the MP mode. The examination of mass profile peak shape, central mass shift, and sequential changes during GC elution not only reveals the presence of interfering compounds but also results in accurate mass measurement for those interferences. The latter feature takes the MP mode beyond the target mass analysis that GC/MS-SIM was originally designed for. This additional dimension of information is particularly useful for those complex and incompletely characterized matrices that are frequently encountered in environmental and biological sample analyses.
