Aim: Previous studies showed that granulocyte-colony stimulating factor (G-CSF) improved heart function in a mice model of Chronic Chagas Cardiomyopathy (CCC). Herein, we report the interim results of the safety and efficacy of G-CSF therapy vs. placebo in adults with Chagas cardiomyopathy. Methods: Patients with CCC, New York Heart Association (NYHA) functional class II to IV and left ventricular ejection fraction (LVEF) 50% or below were included. A randomization list using blocks of 2 and 4 and an allocation rate of 1:1 was generated by R software which was stratified by functional class. Double blinding was done to both arms and assessors were masked to allocations. All patients received standard heart failure treatment for 2 months before 1:1 randomization to either the G-CSF (10 mcg/kg/day subcutaneously) or placebo group (1 mL of 0.9% saline subcutaneously). The primary endpoint was either maintenance or improvement of NYHA class from baseline to 6-12 months after treatment, and intention-to-treat analysis was used. Results: We screened 535 patients with CCC in Salvador, Brazil, of whom 37 were randomized. Overall, baseline characteristics were well-balanced between groups. Most patients had NYHA class II heart failure (86.4%); low mean LVEF was 32 ± 7% in the G-CSF group and 33 ± 10% in the placebo group. Frequency of primary endpoint was 78% (95% CI 0.60-0.97) vs. 66% (95% CI 0.40-0.86), p = 0.47, at 6 months and 68% (95% CI 0.43-0.87) vs. 72% (95% CI 0.46-0.90), p = 0.80, at 12 months in placebo and G-CSF groups, respectively. G-CSF treatment was safe, without any related serious adverse events. There was no difference in mortality between both arms, with five deaths (18.5%) in treatment vs. four (12.5%) in the placebo arm. Exploratory analysis demonstrated that the maximum rate of oxygen consumption during exercise (VO2 max) showed an improving trend in the G-CSF group. Conclusion: G-CSF therapy was safe and well-tolerated in 12 months of follow-up. Although prevention of symptom progression could not be demonstrated in the present study, our results support further investigation of G-CSF therapy in Chagas cardiomyopathy patients. OBJECTIVE: Mucopolysaccharidosis is a rare genetic disease characterized by the intralysosomal deposition of glycosaminoglycans. Cardiovascular impairment is a common feature. Cardiac signs and symptoms are underestimated due to the disease involvement in other organs. Enzyme replacement therapy can be used in mucopolysaccharidosis I, II, IV, and VI. Thus, the knowledge about the use of new echocardiography tools is relevant to improve the care of this population. This study aimed to describe left ventricular function assessment by conventional echocardiography and left ventricular global longitudinal strain analysis and compare the alterations in patients receiving enzyme replacement therapy and who had different ages at the start of therapy. METHOD: Outpatient-based descriptive study. The patients were submitted to conventional echocardiography and left ventricular global longitudinal strain measurement. RESULTS: Sixteen patients were evaluated; median age of 14.2 years (SD=5.2 years). Left ventricular hypertrophy was found in nine patients (56.2%). All patients had preserved left ventricular systolic function (Simpson and Teichholz). Nine (56.2%) patients showed alterations in left ventricular global longitudinal strain. The study showed a positive association between left ventricular hypertrophy and alteration in the left ventricular global longitudinal strain, and late start of enzyme replacement therapy and alteration in the left ventricular global longitudinal strain. CONCLUSION: Echocardiographic alterations in patients with mucopolysaccharidosis were frequently observed, especially alterations in the left ventricular geometry and subclinical dysfunction. Patients who had a late enzyme replacement therapy start showed an association with worse left ventricular global longitudinal strain values, reinforcing the need for early diagnosis and treatment. The use of new echocardiographic tools may improve the follow-up of these patients.

BACKGROUND: Syndecan-4 is a transmembrane glycoprotein associated with inflammation and fibrosis. Increased syndecan-4 levels were previously detected after acute myocardial infarction and in subjects with heart failure. However, the levels of syndecan-4 in subjects with Chagas disease have not so far been investigated. The aim of this study was to investigate the potential role of serum sydencan-4 as a novel biomarker for myocardial fibrosis and cardiac dysfunction in subjects with Chagas disease. METHODS: This study comprised subjects with Chagas disease (n = 56), being 14 (25%) with the indeterminate form, 16 (29%) with the cardiac form without ventricular dysfunction, and 26 (46%) with the cardiac form with ventricular dysfunction. RESULTS: Syndecan-4 serum concentrations did not correlate with presence or absence of myocardial fibrosis (P = 0.386) nor disease severity in subjects with Chagas disease (P = 0.918). Additionally, no correlation was found either between the degree of myocardial fibrosis and serum syndecan-4 [r = 0.08; P = 0.567] or between left ventricular ejection fraction and syndecan-4 [r = 0.02; P = 0.864]. In contrast, NT-proBNP levels correlated with ejection fraction and myocardial fibrosis. CONCLUSIONS: Our results demonstrate the lack of correlations between serum syndecan-4, myocardial fibrosis and cardiac dysfunction in subjects with Chagas disease. Further studies are required to show if syndecan-4 concentrations can be marker for prognosis assessment or disease progression.

Resumo Objetivo: A mucopolissacaridose é uma doença genética rara, caracterizada por depósito intralisossômico de glicosaminoglicanos. O comprometimento cardiovascular é frequente. Sinais e sintomas cardíacos são subestimados pelo envolvimento da doença em outros órgãos. A terapia de reposição enzimática pode ser usada em mucopolissacaridose I, II, IV e VI. Assim, o conhecimento da aplicação de novas ferramentas de ecocardiografia é relevante para melhorar a assistência dessa população. Este estudo visou descrever a função do ventrículo esquerdo pelo ecocardiograma convencional e pela análise do strain global longitudinal do ventrículo esquerdo e comparar as alterações em pacientes que fazem uso da terapia de reposição enzimática e que tiveram idades distintas de início da terapia. Método: Estudo descritivo de base ambulatorial. Os pacientes foram submetidos à ecocardiografia convencional e medida do strain global longitudinal do ventrículo esquerdo. Resultados: Foram avaliados 16 pacientes; mediana de 14,2 anos (desvio: 5,2 anos). Hipertrofia do ventrículo esquerdo foi encontrada em nove pacientes (56,2%). Todos os pacientes tiveram função sistólica do ventrículo esquerdo preservada (Simpson e Teichholz). Nove (56,2%) pacientes apresentaram alteração no strain global longitudinal do ventrículo esquerdo. O estudo mostrou associação positiva entre hipertrofia do ventrículo esquerdo e alteração no strain global longitudinal do ventrículo esquerdo e início tardio da terapia de reposição enzimática e alteração no strain global longitudinal do ventrículo esquerdo. Conclusão: Alterações ecocardiográficas em pacientes com mucopolissacaridose foram frequentes, especialmente alterações na geometria e disfunção subclínica do ventrículo esquerdo. Pacientes que iniciaram tardiamente a terapia de reposição enzimática apresentaram associação com piores valores de strain global longitudinal do ventrículo esquerdo, o que reforça a necessidade do diagnóstico e tratamento precoces. O uso de novas ferramentas de ecocardiografia pode melhorar o acompanhamento desses pacientes.

BACKGROUND: One of the most challenging issues of chronic Chagas disease is to provide earlier detection of heart involvement. Two-dimensional speckle tracking (2-D ST) echocardiography, a new imaging modality with useful applications in several cardiac diseases, has been validated for subjects with myocardial infarction against cardiac magnetic resonance (CMR). Here we hypothesize that the longitudinal global strain (LGS) has an incremental value to ejection fraction for predicting myocardial fibrosis in subjects with Chagas disease. METHODS: This observational study comprised 58 subjects with Chagas disease, confirmed by two positive serologic tests. All subjects underwent conventional Doppler echocardiogram plus speckle tracking strain, and cardiac magnetic resonance. RESULTS: The ROC curve analysis revealed that both LGS (area under the curve: 0.78, p = 0.001) and ejection fraction (area under the curve: 0.82, p < 0.001) were significant predictors of myocardial fibrosis. Regarding the percentage of fibrosis, a high correlation was observed with both ejection fraction assessed by echocardiography (r = 0.70, p < 0.001) and LGS (r = 0.64, p < 0.001). However, when adjusted through multiple linear regression, the LGS lost statistical significance as a predictor of myocardial fibrosis (p = 0.111). CONCLUSIONS: LGS has no incremental value to conventional ejection fraction measurement in the prediction of myocardial fibrosis in subjects with Chagas disease.

Este estudo estabelece a soroprevalência de Neospora caninum e Toxoplasma gondii em rebanhos caprinos no Sul de Minas Gerais e verifica a sua associação com o desempenho reprodutivo dos rebanhos analisados, uma vez que não há dados sobre neosporose em caprinos neste Estado. Amostras de soro de 401 caprinos provenientes de 11 propriedades foram submetidas à reação de imunofluorescência indireta anti-N. caninum e T. gondii. A prevalência média de animais positivos foi de 10,7% e 21,4%, respectivamente. Houve diferença significativa entre as faixas etárias. Neste quesito os maiores índices de positividade, para ambos os parasitas, foram observados nos caprinos acima de três anos de idade. Estes apresentaram uma probabilidade 2,6 e 4,8 maior, em relação aos animais de até um ano de idade, para N. caninum e T. gondii, respectivamente. No teste T não houve associação positiva entre as médias de prevalência de N. caninum (p=0,553) e T. gondii (p=0,098) com a ocorrência de problema reprodutivo nos rebanhos. Porém, propriedades com histórico de problemas reprodutivos apresentaram uma média de reagentes para N. caninum de 18,64%, se comparadas a 13,97% de criatórios que não relataram essas alterações. Quando os problemas reprodutivos foram separados em categorias a relação entre a ocorrência de aborto e a soroprevalência de N. caninum e de T. gondii foi significativa (p<0,05) por meio dos testes Exato de Fisher e Qui-quadrado. Não houve diferença (p>0,05) entre as médias de prevalência de T. gondii e o acesso de gatos ao rebanho no teste T. Entretanto, a média de prevalência nas propriedades em que os gatos tinham acesso foi de 31,01%, se comparada a 17,34% em criatórios onde não existiam gatos. Foi constatado o acesso de cães em 100% das propriedades. Os resultados obtidos neste estudo demonstram que N. caninum e T. gondii infectam caprinos no Sul de Minas Gerais e que estes parasitas podem estar associados à ocorrência de abortos nessa espécie animal.


This study determines the seroprevalences of Neospora caninum and Toxoplasma gondii in caprine herds in southern Minas Gerais state and investigates their association with the reproductive performance of the herds analyzed since no data concerning neosporosis in goats in this state are available. Serum samples from 401 goats from 11 farms were subjected to indirect immunofluorescence reaction for the detection of N. caninum and T. gondii. The mean prevalence rates of animals that tested positive were 10.7% and 21.4%, respectively. Significant differences between different age groups were observed. The highest frequencies of positive cases for both parasites were observed in goats over three years of age; in this group, the probability of seropositivity for N. caninum and T. gondii was 2.6 and 4.8 times higher, respectively, when compared to animals up to one year of age. No positive correlation was found between the mean prevalence rate of N. caninum (p-0,553) and T. gondii (p=0.098) and the occurrence of reproductive problems in the herds using T-test. However, farms with a history of reproductive problems exhibited an average of 18.64% animals reactive to N. caninum, compared to 13.97% from farms where those problems were not reported. When reproductive problems were separated in category, the relationship between occurrence of abortion and seroprevalence of N. caninum and T. gondii was significant (p<0.05) using Fisher’s exact test and chi-square test. No difference was seen (p>0.05) between the mean prevalence rate of T. gondii of herds to which cats had access as compared to herds that had no contact with cats using a Ttest. However, the mean prevalence rate in farms where cats had access to the herd was 31.01% as compared to 17.34% in farms where there were no cats. Access of dogs was observed in 100% of the farms...
