RESUMO

PURPOSE: During open-wedge high tibial osteotomy, fracture occurring in the insufficient osteotomy before distraction of the osteotomy gap is an important complication. The objective of this study was to evaluate bone mineral density (BMD) around the proximal tibiofibular joint (PTFJ) and the osteotomy hinge. The hypotheses of this study were (1) BMD would be higher in the level of PTFJ, compared with that of above--or below--the level of PTFJ, (2) BMD of the posterolateral side of the hinge would be higher than that of the anterior or lateral side. METHODS: Computed tomography was used to determine the BMD of the lateral aspect of the proximal tibia around the PTFJ and the osteotomy hinge. The means and standard deviations of the regions of interest were measured. To verify the first hypothesis, a coronal reconstructed image showing the beginning of the fibula head was used and an axial reconstructed image showing the beginning of the fibula head was used for verification of the second hypothesis. RESULTS: BMD of the lateral aspect of the proximal tibia at the level of the PTFJ was significantly higher, compared with that of above (P = 0.04)-or below (P < 0.01)--the level of the PTFJ in male patients. In addition, it was also significantly higher, compared with that of below the level of the PTFJ (P < 0.01). BMD of the posterolateral area of the proximal tibia was significantly higher than that of the anterior or lateral area in both male and female patients (P < 0.01). CONCLUSION: BMD of the level of the PTFJ was higher, compared with that of above-or below-the level of the PTFJ and that of the posterolateral area of the proximal tibia was significantly higher, compared with that of the anterior or lateral area.

PURPOSE: The purposes of this study were (1) to evaluate the foot rotational effects on local and whole leg alignment and (2) to confirm the correlation between local and whole leg alignment. The hypotheses of this study were that (1) the alignment would become varus if the rotation of the foot changes from internal to external rotation, and (2) there would be some correlation between local and whole leg radiographs, and local knee radiographs could then be used indirectly for the assessment of whole leg alignment in patients with bilateral medial compartment knee osteoarthritis. METHODS: A total of 80 lower limbs with genu varum of patients who complained of medial knee pain were examined. The standing anterior-posterior view of whole leg radiographs was taken in the four foot positions, and a custom-made foot plate was used for the attainment of accurate radiographs: feet straight ahead with foot contact at the medial side (R: routine), feet straight ahead at shoulder width (N: neutral), 30° external rotated (ER) and 15° internal rotated (IR) position. In order to obtain a local radiograph of the knee, we took only whole leg radiographs and selected the area of interest on the whole leg radiograph. We evaluated the total width of the tibia plateau (Total), the length of the weight-bearing line, the ratio of weight-bearing line/Total and femorotibial angle (FTA). RESULTS: The absolute value of weight-bearing line was shifted laterally in the 30° ER position and shifted medially in the 15° IR position compared to the neutral position (1.8 mm lateral and 0.2 mm medial in the WLR; 3.5 mm lateral and 3 mm medial in the local radiograph). Significant statistical differences were observed in the local knee weight-bearing line; however, no significant statistical differences were observed in the weight-bearing line of the whole leg radiograph (n.s.). Results of the % (weight-bearing line/Total) were similar to those of weight-bearing line. The FTA of the local radiograph showed statistical differences, and it showed more valgus in the 30° ER position. In the correlation analysis between whole leg radiograph and local knee radiograph, moderate correlation (correlation coefficient = 0.67) was observed; however, significant statistical differences were observed in the comparison of weight-bearing line and % weight-bearing line/Total (p < 0.01 and < 0.01, respectively) between local knee and whole leg radiograph. CONCLUSIONS: Foot position of ER could show less varus alignment and the reverse could occur in the IR position, compared to the neutral foot position. The severity of varus alignment could be underestimated in the local radiograph, compared with that of whole leg radiograph.

INTRODUCTION: The purposes of this study were: (1) to compare tunnel locations using the conventional transtibial technique with reference data, and (2) to identify factors that make it difficult to position the femoral tunnel correctly or contribute to breakage of the bone bridge between the two tibial tunnels. MATERIALS AND METHODS: A prospective study was performed on 28 patients who underwent double bundle ACL reconstruction. Locations of each tunnel were determined using an anatomic coordinate axes method (ACAM). Measurements included: thickness of the bone bridge between tibial two tunnels (BB), height from the union (HU) point to expected joint surface, the ratio between the length of Blumensaat's line and the anterior-posterior length of the lateral femoral condyle (RBL), and the ratio between anterior-posterior and proximal-distal lengths of the medial wall of the lateral femoral condyle (RAPPD). RESULTS: The posterior-anterior direction of femoral AM tunnel, the proximal-distal direction of femoral PL tunnel, and the posterior-anterior direction of femoral PL tunnel were statistically significantly different from the reference data. In correlation analyses between BB or HU and other variables, the AM tibial tunnel and RBL showed a moderate negative correlation. The cutoff point for tunnel breakage was an RLB of 1.14, meaning that the possibility of bone bridge breakage would increases for RBL values of >1.14. CONCLUSIONS: It seems that conventional transtibial drilling technique used during double bundle ACL reconstruction does not reproduce correct tunnel locations compared with reference data. This problem was found to be related to the bony geometry of the medial wall of the lateral femoral condyle or the bone bridge between the two tibial tunnels. Our results indicate that RBL should be determined by pre-operative CT or plain lateral radiography, and that transtibial single bundle reconstruction or double bundle reconstruction using other methods should be attempted when the RBL exceeds 1.14.

BACKGROUND: The incidence of Achilles tendon rupture and its trend has not been studied in Asia. The purpose of this nationwide study was to analyze the trend of incidence and surgical treatment of tendon ruptures in South Korea based on sex, age, and income level of patients, as well as seasonal variation. METHODS: A descriptive epidemiologic study was performed based on the data collected retrospectively from the Korea National Health Insurance Service. Data of all outpatients and inpatients were collected from approximately 52 million residents of South Korea, primarily diagnosed with Achilles tendon rupture from 2009 to 2017. RESULTS: A total of 112,350 patients had Achilles tendon rupture, of which 44,248 patients underwent surgical treatment during the study period. The overall, age-specific, and sex-specific incidence of Achilles tendon rupture and surgical treatment showed an increasing trend. Patients in the age group of 41 to 50 years showed the highest increase in incidence. Regarding season, higher incidence was reported during spring and summer, whereas the lowest incidence was found in winter. Higher income level was associated with increased incidence of the condition. CONCLUSIONS: The incidence of Achilles tendon rupture and surgical treatments increased rapidly in patients between 41 and 50 years of age. Patients in the higher income quintile groups experienced more Achilles tendon injury than those in lower income groups, and fewer ruptures were observed during winter.

PURPOSE: We studied the effect of interleukin-8 (IL-8) as the factor for angiogenesis in the joint fluid of remnant-preserved anterior cruciate ligament reconstruction (RP-ACLR). MATERIALS AND METHODS: We measured 12 cytokines in joint fluid by multiplex assay and assessed the relationship between IL-8 and vascular endothelial growth factor (VEGF) concentrations. The signal intensity and mean sagittal diameter via postoperative magnetic resonance imaging (MRI) scans were evaluated and the stress X-ray image was analyzed at 3, 6, and 12 months after operation. RESULTS: The IL-8 concentration was highest 3 months postoperatively in those patients who underwent RP-ACLR. Clinical data also showed that the signal intensity and stress radiography of the knee graft were significantly better at the early postoperative stage. DISCUSSION: Our results show that IL-8 plays an important role in angiogenesis within 3 months after RP-ACLR. This effect yields better recovery after operation. RP-ACLR patients with high knee stability in clinical data were identical to those with high expression of IL-8 in experimental data. Therefore, IL-8 has been shown to help revascularization and ligamentization of the grafted tendon. These results indicate that IL-8 in RP-ACLR is an important factor for angiogenesis after operation. Unfortunately, the relationship of IL-8 and VEGF in vivo has not been studied. CONCLUSION: Our results showed that the IL-8 concentration was very high within 3 months after RP-ACLR operation. The increase in concentration of IL-8 over time was consistent with the increase in VEGF concentration. In the IL-8 clinical setting, MRI analysis showed that ACL synovialization and tension were better in patients who underwent the remnant preservation method. In addition, it was shown that RP-ACLR may be advantageous for early anterior stability within 1 year post operation and beneficial for tendon graft in the early stage post operation. Taken together, our findings suggest that IL-8 may contribute to angiogenesis which is helpful for revascularization and ligamentization of the graft tendon in the early stages of RP-ACLR.

Cyanobacterial blooms and associated microcystins in hypertrophic stagnant West-Nakdong River were investigated at weekly intervals from April to August, 1999. Microcystis spp. accounted for over 85% of the numeric abundance of total phytoplankton. Microcystins were present in the blooms sampled between May and August. The peak of total microcystin (microcystin-LR + -RR) levels (maximum level; 612 microg g(-1)) was detected in the initial stage of the bloom (mid May), at the same time as high N/P ratio and high Microcystis biomass. This study indicated that total microcystins of this regulated river was positively correlated to Microcystis biomass, absolute nutrient concentration and TN/TP ratio.

RESUMO

Chronic multifocal closed rupture of the extensor hallucis longus tendon is an extremely rare injury. Previously, chronic multifocal partial rupture of the extensor hallucis longus tendon had not been reported. This case study reports one case of autogenous graft repair of a chronic multifocal rupture of the extensor hallucis longus tendon using a semitendinous tendon autograft.
