RESUMO

In this 12-month multicenter Scandinavian study, 78 maintenance heart transplant (HTx) recipients randomized to everolimus with reduced calcineurin inhibitor (CNI) exposure or continued standard CNI-therapy underwent matched virtual histology (VH) examination to evaluate morphological progression of cardiac allograft vasculopathy (CAV). Parallel measurement of a range of inflammatory markers was also performed. A similar rate of quantitative CAV progression was observed in the everolimus (n = 30) and standard CNI group (n = 48) (plaque index 1.9 ± 3.8% and 1.6 ± 3.9%, respectively; p = 0.65). However, VH analysis revealed a significant increase in calcified (2.4 ± 4.0 vs. 0.3 ± 3.1%; p = 0.02) and necrotic component (6.5 ± 8.5 vs. 1.1 ± 8.6%; p = 0.01) among everolimus patients compared to controls. The increase in necrotic and calcified components was most prominent in everolimus patients with time since HTx >5.1 years and was accompanied by a significant increase in levels of von Willebrand (vWF) factor (p = 0.04) and vascular cell adhesion molecule (VCAM) (p = 0.03). Conversion to everolimus and reduced CNI is associated with a significant increase in calcified and necrotic intimal components and is more prominent in patients with a longer time since HTx. A significant increase in vWF and VCAM accompanied these qualitative changes and the prognostic implication of these findings requires further investigation.

A 77-year-old woman suffering from progressive dyspnea and chest pain for 2 1/2 years was admitted to hospital. There were no ECG changes at exercise test and a dobutamine stress echocardiography was normal. At catheterization, right-sided pressures were within normal limits. Coronary angiography revealed a congenital coronary fistula, 3-4 mm in diameter, from the left anterior descending artery to the proximal pulmonary artery. There was no significant rise in blood oxygen saturation in the pulmonary artery. Transcatheter coil embolization was performed in the distal part of the tortuous fistula. Flow ceased within minutes, demonstrating the feasibility and efficacy of this technique for treating large fistulas.

A report on 105 cases of ocular trauma hospitalized in St. Joseph's Hospital in Reykjavik during a 12-year period is presented. Of these patients 94 were males and 11 females. Thirty-nine patients were children, and only six patients were older than 50 years. Most common were occupational accidents (43 cases) and of these 29 were connected to industry. Other main causes were traffic accidents, sport and dangerous play by children. Of the total 105 cases, 70 had perforation of the bulb. All but nine underwent some kind of operation, and enucleation was performed in 10 patients. Nearly two thirds of the patients were hospitalized within six hours of the accident and 84% within 24 h.

The validity and reliability of interview reports on rheumatic disorders in a Swedish nationwide health survey were studied. Two samples, including altogether 157 individuals, who had reported rheumatic disorders, were medically examined 6 to 24 months after the original interview. The reproducibility of the original interview was studied by the means of two re-interviews. The proportional distribution of rheumatic disorders according to diagnostic group was similar to that in the original interview, in the two re-interviews and at the medical examination. In a subject-to-subject comparison the overall agreement between the medical examination and the lay re-interview was 84% (kappa 0.35) regarding presence of rheumatic disorders, and 57% (kappa 0.41) regarding matching of rheumatic diagnostic groups. In the original interview and the lay re-interview the same individual rheumatic disorder was reported by 38% of the subjects; 58% agreed regarding rheumatic diagnostic groups. The interview reports on rheumatic disorders in the health survey can therefore be considered to have acceptable validity and reliability only with regard to the proportional distribution of groups of rheumatic disorders, based on aggregated data.

For diagnosis of pulmonary embolism (PE), ventilation/perfusion lung scintigraphy is routinely used; approximately one-third of the patients will have the diagnosis "intermediate probability of PE" (inconclusive). In this group only about 33% are found to have pulmonary emboli if examined with pulmonary angiography. To evaluate the diagnostic, therapeutic, and economic consequences of ultrasound of the legs as a complementary diagnostic investigation to "intermediate probability", 72 consecutive patients were investigated with bilateral ultrasound of the proximal deep veins of the legs and pulmonary angiography in a prospective study. Ten patients had PE, of whom 7 had deep venous thrombosis, and 62 had no PE, of whom 2 had deep venous thrombosis. The negative predictive value of ultrasound was 0.95. In view of the importance of adequate treatment and rational use of public health care expenditure, complementary diagnostics should be performed, and ultrasound is an adequate complementary investigation.

In nine healthy adult volunteers, pulmonary magnetic resonance angiography was performed with the blood pool agent NC100150 injection combined with respiratory gating with a navigator echo. With increasing doses of the contrast agent, higher signal intensities and vessel branch order visualization were achieved. No motion artifacts were seen. The blood pool agent NC100150 injection in combination with respiratory navigator gating permitted acquisition of high-quality MR angiograms of the pulmonary vasculature during continuous breathing.
