RESUMO

A candidate live vaccine for avian pathogenic Escherichia coli (APEC) was constructed from a virulent field APEC 078 strain by mutation of the aroA gene. The mutant was highly similar to the parent wild-type strain in respect of colony morphology, motility, growth in suspension, hemagglutination, Congo Red binding, HEp-2 cell adhesion, and the elaboration of surface antigens type 1 fimbriae and flagella, although production of curli fimbriae was reduced marginally. The mutant proved avirulent when inoculated into 1-day-old chicks by spray application and when presented again in the drinking water at 7 days of age. Chickens and turkeys vaccinated with an 078 aroA mutant were protected against a challenge at 6 wk of age by virulent APEC strains.

Lymphocytes obtained from the blood, spleen, and bursa of normal chickens and of chickens infected with infectious bursal disease virus (IBDV) were analyzed for phenotypic expression of CT4, CT8, and immunoglobulin cell surface markers. Single-cell suspensions were stained with monoclonal antibodies by an indirect immunofluorescent assay, and percent staining was quantitated by flow cytometry. Although an appreciable decline from control levels in the percentage of lymphocytes expressing IgM was detected in the spleen and bursa of infected chickens, the relative proportions of lymphocytes expressing CT4 and CT8 in peripheral blood and spleen remained unchanged following infection. These results suggest that whereas humoral immune depression by IBDV may be associated with lysis of antibody-producing B cells, cellular immune depression is not associated with a detectable change in the proportion of helper or cytotoxic/suppressor subpopulations of T lymphocytes.

Packed cell volumes (PCVs) from 3-, 7-, 14-, 21-, 28-, and 35-day-old clinically healthy chicken anemia agent (CAA)-free and specific-pathogen-free (SPF) leghorn chicks were compared with PCVs from age-matched clinically healthy CAA-free broiler chicks. The PCVs of the SPF chicks regressed significantly (F = 20.6, df = 2/3, P < 0.025) on age in a linear fashion. The PCVs of the broiler chicks regressed significantly (F = 9.56, df = 2/3, P < 0.05) on age in a cubic parabola. The mean PCVs of the broiler chicks were significantly (P < 0.05) higher than PCVs of SPF chicks at every tested time interval. Results indicate that PCV values are higher in broiler chicks than in SPF leghorn chicks, and that PCVs increase as chicks age. Clinicians, diagnosticians, and investigators who intend to work with chicken blood must be aware of these differences.

With the emergence of very virulent Marek's disease virus (MDV) strains, vaccines based on herpesvirus of turkeys (HVT) appear to be not powerful enough to confer full protection, whereas in chicken flocks vaccinated with MDV CVI 988 strain protective immunity sometimes is generated not early enough for full protection. For this reason combination vaccines containing HVT as well as CVI 988 have been developed. In this paper the beneficial effect of combining both types of virus strains in one vaccine for early protection is shown in a vaccination challenge experiment, in which one-day-old chickens were vaccinated with suboptimal dosages of the monovalent vaccines and the same dosages in a combination vaccine. After 5 days the chickens were challenged with a very virulent MDV strain and subsequently observed for a period of approx. 50 days. It appeared that the combination vaccine provided better early protection than the monovalent vaccines. In addition, the combination vaccine was tested as vaccine administered in ovo. It appeared that after in ovo vaccination the vaccine conferred adequate protection against challenge with a very virulent MDV strain, 5 days after hatch, and that protection after in ovo vaccination was similar to that obtained after subcutaneous vaccination with the same combination vaccine.

RESUMO

Intravenous DSA was performed in 106 patients with symptoms of arterial insufficiency in the lower extremities, requiring treatment according to preceding clinical examination. The examinations were carried out using a 36.5 cm (14 in.) intensifier and a 5.5 F pigtail catheter located centrally. 71% of the examinations were carried out on out-patients. No complications were observed. In 96% of the cases, the intravenous DSA provided sufficient information to make decisions about treatment.

The EEGs of 13 experienced practitioners of transcendental meditation (TM) were recorded for 5 min preceding TM, during 20 min of TM and until 5 min after, as well as during closed-eyed wakefulness, drowsiness, sleep onset and sleep. Thirteen healthy volunteers matched for age served as control subjects. Computer period-amplitude analysis of F3-C3, T3-T5, P3-O1, F4-C4 and P4-O2 epochs of 50--100 sec duration resulted in a frequency and amplitude spectrum (0.5--28.6 c/sec), and the mean frequency and the mean voltage of each EEG lead. The EEG frequency spectra constituted a continuum with increasing theta and delta activity and decreasing alpha activity as the participants tended to fall asleep. The frequency spectrum during TM corresponded to a spectrum situated between that of wakefulness and drowsiness and remained virtually unchanged during the 20 min of meditation. The EEG mean frequency of the TM group was about 1 c/sec slower than that of the control group. Intra- or interhemispheric differences between quantities of EEG activity remained stable during TM, nor did we observe any theta bursts. There was no consistent EEG patter associated with a successful or unsuccessful meditation, nor did the EEGs of two meditators who stated they had felt drowsy during TM show a different pattern.

RESUMO

Spontaneous rupture of the diaphragm is rare. A case is presented in which spontaneous rupture of the left hemidiaphragm occurred as a result of physical exercise 4 months before admission to hospital. The patient, a 42-year-old woman, underwent transabdominal repair with uncomplicated postoperative course. The clinical manifestations, diagnosis and treatment of spontaneous diaphragmatic rupture are reviewed against the background of this case and the total of 12 cases in the available literature. All but one of the ruptures were left-sided. The importance of keeping this condition in mind in differential diagnosis is underlined by the fatal outcome in 2 of the known 13 cases.

The efficacy and safety of the non-ionic contrast medium iopentol, 250 mg I/ml (Imagopaque, Nycomed Imaging AS, Oslo, Norway) were evaluated and compared to those of the ionic contrast medium diatrizoate 219 mg I/ml (Urografin, Schering AG, Berlin, Germany). The trial was carried out as a randomized, double-blind comparative two-group study. One hundred and sixteen patients completed the study: 59 received iopentol and 57 received diatrizoate, the contrast medium used according to routine hospital procedure for ERCP. Demographic data and details of the ERCP procedure were comparable for the two contrast medium groups. Safety was assessed by monitoring serum pancreatic iso-amylase and by recording adverse events. Efficacy was evaluated through assessment of the diagnostic information and the quality of radiographs. Twenty-one of the patients receiving iopentol and 27 of the patients receiving diatrizoate reported adverse events during the ERCP procedure. Pain was the kind of procedure-related event most frequently reported. Three patients experienced serious adverse events with a fatal outcome 2, 10 and 12 days after the ERCP procedure, respectively. The principal investigator concluded for all three serious adverse events that any causal relationship with the contrast medium injected was unlikely. A mean change in serum pancreatic iso-amylase after contrast medium injection was noted in both contrast medium groups. However, statistical analysis did not show any significant difference between mean changes for the two groups. The efficacy results, both in terms of diagnostic information and quality of radiographic visualisation, were comparable for the two contrast media. In conclusion, the study did not show any statistically significant differences between the two contrast media as regards safety or efficacy in ERCP.

Prophylactic efficacy and safety of a low molecular weight (LMW) heparin against postoperative thromboembolic complications were investigated in a double-blind, randomized study. Totally, 210 consecutive patients undergoing total hip replacement were allocated to two groups. Patients in the heparin group received 50 IU anti-Xa per kilo body weight of Logiparin once daily, and patients in the placebo group received one daily injection of saline. Additional prophylaxis in all the patients was thigh-length compression stockings beginning on the day of the operation. Deep vein thrombosis was diagnosed by bilateral ascending phlebography between Days 8 and 10 after the operation. Twenty patients were excluded from the evaluation. Thirty of 93 patients in the heparin group compared with 45 of 97 patients in the placebo group suffered a thromboembolic complication during the study (P = 0.02). The postoperative blood loss and total number of blood transfusions in the heparin group were higher than in the placebo group. However, the observed differences were of no clinical importance. Adverse effects, including bleeding complications and wound hematomas, were observed in 13 heparin patients and 7 placebo patients (NS). One patient in each group died. Thrombo-prophylaxis with LMW heparin once daily was safe and more effective than the placebo in patients undergoing total hip replacement.
