PURPOSE: In the last decades, different criteria have been developed for detecting inappropriate prescription in older patients. In Spain, translations and adaptations of international lists are available but it would be necessary a national list which could cope with the peculiarities of our health system, existing pharmaceutical market, and prescription habits. We propose in this project the creation of a Spanish potentially inappropriate drugs list which could be applicable in our clinical scenario. METHODS: We use a Delphi method involving 25 experts from different backgrounds (Clinical Pharmacology, Geriatrics, Rational Use of Drugs and Pharmacy, Primary Care and Pharmacoepidemiology, and Pharmacovigilance) that were asked to participate in two-round questionnaires. For analysis, current recommendations of Worth and Pigni were applied, and every statement was classified into one of three groups: strong, moderate, or low agreement. Statements with strong agreement were accepted to be part of the inadequate prescription list. Moderate agreement statements were selected to enter the second questionnaire, and statements with low agreement were further analyzed to determine if it was due to heterogeneity or due to dispersion in the answers. RESULTS: The first questionnaire consisted of 160 proposed sentences, of which 106 reached a high agreement, 32 a moderate agreement, and 22 a low agreement. All sentences proposed in the second questionnaire reached a strong agreement. The total accepted sentences were 138. CONCLUSIONS: We offer a list of inadequate prescription in older patients adapted to the Spanish pharmacopeia and according to the prescription habits in our environment.

Barley (Hordeum vulgare L.) possesses a large and highly repetitive genome of 5.1 Gb that has hindered the development of a complete sequence. In 2012, the International Barley Sequencing Consortium released a resource integrating whole-genome shotgun sequences with a physical and genetic framework. However, because only 6278 bacterial artificial chromosome (BACs) in the physical map were sequenced, fine structure was limited. To gain access to the gene-containing portion of the barley genome at high resolution, we identified and sequenced 15 622 BACs representing the minimal tiling path of 72 052 physical-mapped gene-bearing BACs. This generated ~1.7 Gb of genomic sequence containing an estimated 2/3 of all Morex barley genes. Exploration of these sequenced BACs revealed that although distal ends of chromosomes contain most of the gene-enriched BACs and are characterized by high recombination rates, there are also gene-dense regions with suppressed recombination. We made use of published map-anchored sequence data from Aegilops tauschii to develop a synteny viewer between barley and the ancestor of the wheat D-genome. Except for some notable inversions, there is a high level of collinearity between the two species. The software HarvEST:Barley provides facile access to BAC sequences and their annotations, along with the barley-Ae. tauschii synteny viewer. These BAC sequences constitute a resource to improve the efficiency of marker development, map-based cloning, and comparative genomics in barley and related crops. Additional knowledge about regions of the barley genome that are gene-dense but low recombination is particularly relevant.

Background: Solid pseudopapillary neoplasm, or Frantz-Gruber tumor, is a diagnostic challenge for the surgical pathologist. It is classified by the WHO as a malignant epithelial tumor of the pancreas, its prevalence is low, it occurs in only 1 to 2% of all malignant tumors of the pancreas, it usually affects mainly young women, its origin is still unclear, it is generally It presents as a solitary, encapsulated lesion, without invasion of peripancreatic tissues with rare cases of metastasis, which is why it is considered a low-grade malignant tumor by the WHO. The objective of this article is to present three clinical cases and to evaluate the epidemiology, clinical manifestations, morphology and immunohistochemical expression of the tumor in a review of the bibliography, as well as to compare it with the cases already reported on the subject. Clinical case: Three cases of Frantz tumor were diagnosed by the pathology department of a tertiary hospital are presented, which correspond to two women aged 17 and 34, as well as a 52-year-old man whose presentation by age and sex is rare. Conclusions: After the bibliographical review and the analysis of the cases presented, we verified the difficulty to make a correct diagnosis, since its presence is rare in the daily practice of the surgical pathologist. The morphological patterns of the solid pseudopapillary tumor are varied and can often be reminiscent of neuroendocrine tumors of the pancreas, whose presentation rate is higher.

A biomaterial made of coir and Multi-Walled Carbon Nanotubes (MWCNTs) is presented which exhibits a relatively high-Temperature Coefficient of Resistance (TCR) and thermal insulation properties. Bolometers usually offer acceptable thermal isolation, electrical resistance, and high TCR. Fibers from agricultural waste materials such as coir has a synergistic effect as thermal insulating material and noise reducer. Based on it, powdered coir pills were used as pilot samples, as well as 2 other samples with different dispersions of MWCNTs, sodium dodecyl benzene sulfonate (SDBS) and polyvinylpyrrolidone (PVP) solution. The 3 kinds of samples were thermo-electrically characterized to determine their bolometric performance. Thermal conductivity of k = 0.045 W/m K was obtained by solving the Fourier's law substituting the data into the equation describing heat flux on the sample around room temperature. Results show that adding different concentrations of MWCNT to powdered coir will lead to films with lower electrical resistance, therefore the thermal conductivity increases while thermal resistance decreases. Finally, the bolometric performance shows a maximum peak with a relatively high TCR of - 40.4% at a temperature of 300.3 K, this synthesized material outperforms by almost 1 order of magnitude larger than commercial materials. Results in this work also indicate that it is possible to tune bolometric parameters of this kind of samples and to use them as thermal insulators in the construction industry, when building roofs and walls.

Introduction: Aortic root dilatation is a frequent disease affecting mostly young patients that often requires surgical repair. Surgical techniques in young patients include aortic valve-sparing procedures to avoid prosthetic valve implant. Objective: The aim of this paper is to describe the results obtained in three patients with aortic root dilatation using the Florida Sleeve technique. Methods: From November 2015 to January 2017, three patients with severe aortic regurgitation due to aortic anuloectasic were intervened applying the Florida Sleeve technique. Results: Excellent postoperative results were obtained in the three cases including freedom of aortic regurgitation and any cause re-operation during three years of follow-up. Conclusion: The Florida Sleeve technique is a safe, reproducible technique with a learning curve and lower surgical times than traditional techniques. The medium-term clinical outcomes in terms of morbidity and mortality are good.


Introducción: La dilatación de la raíz aórtica es una patología frecuente que afecta a pacientes jóvenes y su tratamiento es quirúrgico. Las técnicas quirúrgicas de elección en pacientes jóvenes son las de preservación de la válvula aórtica para evitar el implante de una prótesis valvular. Objetivo: El presente trabajo tiene como objetivo describir la técnica de Florida Sleeve y los resultados de nuestra experiencia. Método: Entre noviembre de 2015 y enero de 2017 fueron intervenidos en nuestro centro tres casos de pacientes con insuficiencia aórtica severa secundaria a anulectasia aórtica en los que se realizó la técnica de Florida Sleeve. Resultados: En los tres casos se obtuvieron excelentes resultados postoperatorios, libres de insuficiencia aórtica residual y de reoperación por cualquier causa, resultados que se mantuvieron a los tres años de seguimiento. Conclusiones: La técnica de Florida Sleeve es una técnica segura, reproducible con una curva de aprendizaje y tiempos quirúrgicos inferiores a las técnicas tradicionales. Los resultados clínicos a medio plazo en términos de morbimortalidad son buenos.

Hybridization using overgo probes is an established approach for screening arrayed bacterial artificial chromosome (BAC) libraries. We have improved the use of overgos by increasing the yield of positive clones using reduced levels of radioisotopes and enzyme. The strategy involves labeling with all four radiolabeled nucleotides in a hot pulse followed by a cold nucleotide chase and then extending the exposure time to compensate for reduced specific activity of the probes. The resulting cost savings and reduced human exposure to radiation make the use of highly pooled overgo probes a more attractive approach for screening of BAC libraries from organisms with large genomes.

PURPOSE: A heterophilic ganglioside cancer vaccine was developed by combining NeuGcGM3 with the outer membrane protein complex of Neisseria meningitidis to form very small size proteoliposomes (VSSP). A phase I clinical trial was performed to determine safety and immunogenicity of this vaccine. PATIENTS AND METHODS: Stage III to IV breast cancer patients received up to 15 (200 micro g) doses of the vaccine by intramuscular injection. The first five doses (induction phase) were given at 2-week intervals, with the remaining treatment (maintenance) administered on a monthly basis. RESULTS: Twenty-one patients, 11 of whom had metastatic disease, were included. Main toxicities included erythema and induration at the injection site, sometimes associated with mild pain, and low-grade fever (World Health Organization grades 1 and 2). All treated patients who completed the induction phase developed anti-NeuGcGM3 antibody titers between 1:1,280 and 1:164,000 immunoglobulin G (IgG), and 1:640 and 1:164,000 IgM. Noteworthy specific IgA antibodies were induced by vaccination in all stage III patients and in three stage IV patients. Serum antibody levels were higher in the stage III patients, with the larger increases observed after week 32. The antiganglioside IgG subclasses were mainly IgG1 and IgG3. Hyperimmune sera increased complement-mediated cytotoxicity versus P3X63 myeloma cells and a marked IgG differential reactivity against human mammary ductal carcinoma samples. CONCLUSION: NeuGcGM3/VSSP/Montanide ISA 51 is an unusual immunogenic ganglioside vaccine and also seems to be safe in this small trial. Immunologic surrogates of activity indicate that this reagent warrants further investigation.

Objetivo: Determinar la frecuencia de agresividad y conducta antisocial en estudiantes universitarios con dependencia al teléfono móvil. Materiales y métodos: Se incluyeron 66 estudiantes universitarios entre 18 a 25 años (54,5 % eran mujeres). Los participantes completaron la escala Mobile Phone Problematic Use Scale para la dependencia del teléfono móvil y la escala del Inventario Multifásico de la Personalidad de Minnesota, versión 2-RF para comportamiento antisocial y la agresividad. Resultados: Se encontró dependencia del teléfono móvil en el 22,7 % de la muestra (punto de corte: percentil 80). Además, se observó una significación estadística (p <0.01) en 8 escalas del MPPI-2-RF, que incluían CR4, AG y AGGR-r, relacionadas con la agresión y el comportamiento antisocial. Conclusiones: Existe una relación entre la dependencia del teléfono móvil y las escalas de agresividad y comportamiento antisocial. Postulamos que la dependencia del teléfono móvil podría ser un factor criminógeno.


Objective: To determine the frequency of aggression and antisocial behavior in university students with mobile phone dependence. Materials and methods: Sixty-six (66) university students from 18 to 25 years old (54.5 % females) participated in the study. They completed the Mobile Phone Problematic Use Scale (MPPUS) to assess the mobile phone dependence, and the Minnesota Multiphasic Personality Inventory 2-RF (MMPI-2-RF) to assess the antisocial behavior and aggression. Results: Twenty-two point seven percent (22.7 %) of the sample showed mobile phone dependence (cutoff point: 80th percentile). Additionally, a statistical significance (p <0.01) was observed in eight scales of the MPPI-2-RF, including CR4, AG and AGGR-r, which are related to aggression and antisocial behavior. Conclusions: There is a relationship between mobile phone dependence and the scales assessing aggression and antisocial behavior. We postulate that mobile phone dependence could be a factor conducive to crime.

BACKGROUND: This study was conducted to compare the antiseptic efficacy of sodium hypochlorite of electrochemical production with that of povidone-iodine in human voluntaries. METHODS: Three areas of 25 cm(2) each were selected from the forearms; 1 was designated as control to determine the baseline bacterial count; and 2 more were selected to swab 10% povidone-iodine or 10% sodium hypochlorite. Every volunteer was studied on 3 separated occasions. Quantitative skin cultures were performed on agar plates containing a neutralizer. RESULTS: Forty-eight healthy subjects were enrolled for a total of 144 determinations for every antiseptic and control. The bacterial counts from the control areas showed a median of 1500 colony-forming units (CFU)/cm(2). For the areas treated with sodium hypochlorite, the median was 192 CFU/cm(2). For the areas treated with povidone-iodine, the median was 231 CFU/cm(2). When the colony counts for the areas treated with antiseptics were compared with those of the controls, the difference was significant (Kruskal-Wallis test (H) = 55.7, P < .001). The difference in counts between the areas treated with antiseptics was not significant (difference in z values <1960). CONCLUSION: The present study did not find differences in antiseptic action between 10% povidone iodine and 10% sodium hypochlorite.

The prognosis of patients with advanced head and neck cancer remain dismal. For this tumor type, elevated levels of EGFR are associated with a shorter disease free survival and time to treatment failure, reflecting a more aggressive phenotype. Nimotuzumab is a humanized monoclonal antibody that recognizes domain III of the extracellular region of the EGFR, within an area that overlaps with both the surface patch recognized by cetuximab and the binding site for EGF. In order to assess the efficacy of nimotuzumab in combination with radiotherapy, a controlled, double blind, randomized clinical trial was conducted in 106 advanced squamous cell carcinoma of the head and neck patients, mostly, unfit for chemoradiotherapy. Control patients received a placebo and radiotherapy. Treatment was safe and the most frequent adverse events consisted on grade I or II asthenia, fever, headache and chills. No skin rash was detected. A significant complete response rate improvement was found in the group of patients treated with nimotuzumab as compared to the placebo. In the intent to treat analysis, a trend towards survival benefit for nimotuzumab treated subjects was found. The survival benefit became significant when applying the Harrington-Fleming test, a weighted log-rank that underscores the detection of differences deferred on time. In addition, a preliminary biomarker investigation showed a significant survival improvement for nimotuzumab treated patients as compared to controls for subjects with EGFR positive tumors. All patients showed a quality of life improvement and a reduction of the general and specific symptoms of the disease.

Low temperature and drought have major influences on plant growth and productivity. To identify barley genes involved in responses to these stresses and to specifically test the hypothesis that the dehydrin (Dhn) multigene family can serve as an indicator of the entire transcriptome response, we investigated the response of barley cv. Morex to: (1) gradual drought over 21 days and (2) low temperature including chilling, freeze-thaw cycles, and deacclimation over 33 days. We found 4,153 genes that responded to at least one component of these two stress regimes, about one fourth of all genes called "present" under any condition. About 44% (1,822 of 4,153) responded specifically to drought, whereas only 3.8% (158 of 4,153) were chilling specific and 2.8% (119 of 4,153) freeze-thaw specific, with 34.1% responsive to freeze-thaw and drought. The intersection between chilling and drought (31.9%) was somewhat smaller than the intersection between freeze-thaw and drought, implying an element of osmotic stress response to freeze-thaw. About 82.4% of the responsive genes were similar to Arabidopsis genes. The expression of 13 barley Dhn genes mirrored the global clustering of all transcripts, with specific combinations of Dhn genes providing an excellent indicator of each stress response. Data from these studies provide a robust reference data set for abiotic stress.

NeuGcGM3 ganglioside is especially attractive because it is expressed on melanoma cells but it is minimally or not expressed at all on most normal human tissues. A Phase Ib/IIa clinical trial was carried out in patients with advanced cutaneous and ocular malignant melanomas, to evaluate immunogenicity and toxicity of an intramuscularly administered cancer vaccine and composed by NeuGcGM3 in a proteoliposome of Neisseria meningitides with Montanide ISA 51 as adjuvant. Twenty two patients were included, twelve at dose level of 200 microg and 10 at 400 microg. The first five doses were administered every other week and then monthly until 9 doses. 12 patients received additional immunizations. Vaccination induced specific anti-NeuGcGM3 IgM, IgG and IgA antibodies responses. Titers of IgM were greater for the highest vaccine doses. Vaccination also elicited DTH response in 45.5% of patients in the lower doses and 77.8% in the higher doses. Toxicities were mostly grade 1 or 2, according CTC-NCI criteria. Interestingly, 3 patients developed vitiligo at the lower dose (none in the highest dose) although the nominal antigen NeuGcGM3 is not present in melanocytes. Survival analysis was not the goal of this Phase I trial; nevertheless, the fact that seven patients are alive for more than 2 years after inclusion is noteworthy. Safety and immunogenicity with NeuGcGM3 vaccine treatment in advanced melanoma patients were established. The prognostic value of autoimmunity and the possibilities of dissociating anti-tumor immunity from autoimmunity deserve further research.

Se realizó un estudio descriptivo transversal con el objetivo de determinar la frecuencia y los factores involucrados en la infección por Chlamydia trachomatis, con un universo de 90 mujeres sexualmente activas, que acudieron a consulta de patología de cuello uterino en el Hospital Territorial Dr Mario Muñoz Monroy, de Colón, en el tiempo comprendido del 1 de abril al 31 de junio de 2009. Se obtuvieron muestras de exudado endocervical, para realizar el ensayo inmunocromatográfico CHLAMY-CHECK-1 para Chlamydia trachomatis. Se aplicó una planilla de recolección de datos sobre algunos aspectos sociodemográficos. La prevalencia total fue de 67,7 por ciento y se destacan los parámetros: edad igual o menor de 25 años (93,3 por ciento), nuliparidad (27,8 por ciento) y no uso de condón (85,2 por ciento). Estos resultados sugieren que la infección se presenta con más frecuencia en las mujeres sexualmente activas, con 25 años o menos de edad, nulíparas y que no usan condón.


We made a transversal descriptive study with the objective of determining the frequency and the facts implicated in the infection by Chlamydia trachomatis, in a universe of 90 sexually active women who assisted the consultation of uterine neck pathology in the Hospital Dr Mario Muñoz Monroy of Colon, in the period from April 1st to June 31st 2009. We obtained samples of endocervical exudates to make the immuno-chromatographic assay CHLAMY-CHECK-1 for C.trachomatis. We applied a form for data collection on some socio-demographic aspects. The total prevalence was 67,7 percent and the following parameters stand out: age equal or less than 25 years (93,3 percent), nulliparity (27,5 percent) and no use of the condom (85,2 percent). These results suggest that the infection is present more frequently in sexually active women, aged 25 years old or less, nulliparas, and who do not use condom.

More than 2,000 genome-wide barley single nucleotide polymorphisms (SNPs) were developed by resequencing unigene fragments from eight diverse accessions. The average genome-wide SNP frequency observed in 877 unigenes was 1 SNP per 200 bp. However, SNP frequency was highly variable with the least number of SNP and SNP haplotypes observed within European cultivated germplasm reflecting effects of breeding history on genetic diversity. More than 300 SNP loci were mapped genetically in three experimental mapping populations which allowed the construction of an integrated SNP map incorporating a large number of RFLP, AFLP and SSR markers (1,237 loci in total). The genes used for SNP discovery were selected based on their transcriptional response to a variety of abiotic stresses. A set of known barley abiotic stress QTL was positioned on the linkage map, while the available sequence and gene expression information facilitated the identification of genes potentially associated with these traits. Comparison of the sequenced SNP loci to the rice genome sequence identified several regions of highly conserved gene order providing a framework for marker saturation in barley genomic regions of interest. The integration of genome-wide SNP and expression data with available genetic and phenotypic information will facilitate the identification of gene function in barley and other non-model organisms.

Although cold and drought adaptation in cereals and other plants involve the induction of a large number of genes, inheritance studies in Triticeae (wheat [Triticum aestivum], barley [Hordeum vulgare], and rye [Secale cereale]) have revealed only a few major loci for frost or drought tolerance that are consistent across multiple genetic backgrounds and environments. One might imagine that these loci could encode highly conserved regulatory factors that have global effects on gene expression; therefore, genes encoding central regulators identified in other plants might be orthologs of these Triticeae stress tolerance genes. The CBF/DREB1 regulators, identified originally in Arabidopsis as key components of cold and drought regulation, merit this consideration. We constructed barley cDNA libraries, screened these libraries and a barley bacterial artificial chromosome library using rice (Oryza sativa) and barley Cbf probes, found orthologs of Arabidopsis CBF/DREB1 genes, and examined the expression and genetic map location of the barley Cbf3 gene, HvCbf3. HvCbf3 was induced by a chilling treatment. HvCbf3 is located on barley chromosome 5H between markers WG364b and saflp58 on the barley cv Dicktoo x barley cv Morex genetic linkage map. This position is some 40 to 50 cM proximal to the winter hardiness quantitative trait locus that includes the Vrn-1H gene, but may coincide with the wheat 5A Rcg1 locus, which governs the threshold temperature at which cor genes are induced. From this, it remains possible that HvCbf3 is the basis of a minor quantitative trait locus in some genetic backgrounds, though that possibility remains to be thoroughly explored.

We generated the 1E10 gamma-type anti-idiotype mAb (Ab2) specific to an Ab1 mAb able to react specifically with N-glycolyl-containing gangliosides and with Ags expressed on human melanoma and breast carcinoma cells. This Ab2 mAb induced an Ab response in animal models sharing immunochemically defined idiotopes with the Ab1. The treatment of tumor-bearing mice with 1E10 mAb induced a strong antitumor activity. A clinical trial was conducted in 20 patients with advanced malignant melanoma. Patients were treated with six intradermal injections of aluminum hydroxide-precipitated 1E10 anti-Id mAb given at 2-wk intervals. Sixteen of the 17 patients who received at least four doses of the anti-Id vaccine develop Ab3 Abs capable of inhibiting Ab2 binding to Ab1 (Ab3Id+). In contrast to the incapacity of 1E10 mAb to generate Ab3 Abs with the same antigenic specificity as the Ab1 mAb in mice, a very specific and strong Ab3 response against N-glycolyl-containing gangliosides was induced in 16 patients (Ab3Ag+). No evidence of serious or unexpected adverse effects has been observed in this clinical trial. 1E10 anti-Id vaccine was safe, well tolerated, and immunologically effective, with most patients being able to generate a specific immune response against 1E10 and Neu-glycolyl-GM(3) ganglioside.

La Genética médica en Cuba evolucionó con los estudios de carácter nacional y la formación de másteres en asesoramiento genético, con ello la salud de los adultos cambió. La hipertensión arterial representa un signo clínico, una enfermedad y un factor de riesgo. Está entre las enfermedades no transmisibles que afectan la población adulta del mundo, de Cuba, la provincia de Matanzas y el municipio de Colón, con incrementos en la morbimortalidad. Por ello se realizó un estudio analítico transversal sobre hipertensión arterial en gemelos, en Colón, desde el 3 de febrero hasta el 10 de mayo del 2007. Sus resultados fueron: predominio de gemelos dicigóticos; mayor concordancia tanto para el par como para el caso índice en gemelos monocigóticos; edad promedio de aparición de la enfermedad de 44 años, en el segundo gemelo apareció aproximadamente dos años después. El género femenino predominó en los monocigóticos, en los dicigóticos no hubo diferencias. Prevaleció la piel de color blanco. El antecedente de hipertensión arterial en familiares de primero y segundo grados aportó los mayores valores. El sedentarismo fue el factor de riesgo modificable más significativo para los concordantes; el tabaquismo y el inadecuado consumo de frutas y vegetales resultaron para los discordantes. Se evidenció el componente genético de la hipertensión arterial.


Medical genetics in Cuba evolved with studies of national character and with the formation of masters in Genetic Counseling, promoting a change in adults’ health. Arterial hypertension is a clinical sign, a disease and a risk fact. It is among the non-transmissible diseases affecting adult population of the world, of Cuba, of the province of Matanzas and of the municipality of Colón, with increases in morbidity and mortality. That is why we carried out a transversal analytical study on arterial hypertension in twins, in Colon, from February 3rd to May 10th 2007. The results were: predominance of dizygotic twins; a bigger concordance as for the pair as for the case of the index in monozygotic twins; 44 years as average age of disease beginning; in the case of the second twin the disease began two years later. Among monozygotic twins, there was prevalence of the female sex, and among the dizygotic ones, there were not differences. There were a larger proportion of white persons. The antecedent of arterial hypertension in first and second grade relatives showed the greatest values. The most significant modifiable risk fact was lack of exercises for concordants; smoking and the inadequate consume of fruits and vegetables was the most significant for discordants. It was evident the genetic component of the arterial hypertension.
