RESUMO

OBJECTIVE: To study the influence of adjuvant androgen suppression and bisphosphonates on incident vertebral and non-spinal fracture rates and bone mineral density (BMD) in men with locally advanced prostate cancer. PATIENTS AND METHODS: Between 2003 and 2007, 1071 men with locally advanced prostate cancer were randomly allocated, using a 2 × 2 trial design, to 6 months i.m. leuprorelin (androgen suppression [AS]) before radiotherapy alone ± 12 months additional leuprorelin ± 18 months zoledronic acid (ZdA), commencing at randomization. The main endpoint was incident thoraco-lumbar vertebral fractures, which were assessed radiographically at randomization and at 3 years, then reassessed by centralized review. Subsidiary endpoints included incident non-spinal fractures, which were documented throughout follow-up, and BMD, which was measured in 222 subjects at baseline, 2 years and 4 years. RESULTS: Incident vertebral fractures at 3 years were observed in 132 subjects. Their occurrence was not increased by 18 months' AS, nor reduced by ZdA. Incident non-spinal fractures occurred in 72 subjects and were significantly related to AS duration but not to ZdA. Osteopenia and osteoporosis prevalence rates at baseline were 23.4 and 1.4%, respectively, at the hip. Treatment for 6 and 18 months with AS caused significant reductions in hip BMD at 2 and 4 years (P < 0.01) and ZdA prevented these losses at both time points. CONCLUSION: In an AS-naïve population, 18 months of ZdA treatment prevented the sustained BMD losses caused by 18 months of AS treatment; however, the study power was insufficient to show that AS duration or ZdA influenced vertebral fracture rates.

A new sensitive method to determine polonium-210 (210Po) and lead-210 (210Pb) in a diversity of environmental samples was developed. For fresh and marine waters, Po was pre-concentrated using a titanium (III) hydroxide (Ti(OH)3) co-precipitation. Solid environmental samples were digested with nitric acid (HNO3) and hydrogen peroxide (H2O2). The alpha thin layer source was prepared using CuS micro-precipitation and 210Po was measured by alpha spectrometry. Lead-210 was left to decay for up to a year and indirectly measured via its progeny, 210Po. The chemical recoveries for 210Po and 210Pb were high, 90% and 97%, respectively, for a large variety of samples and a very low minimum detectable activity (MDA) was obtained. The method was validated using standardized solutions and certified reference materials.

We evaluated the effects of chronic exposure to environmental radiological contamination on the reproductive fitness of sensitive fern (Onoclea sensibilis) by quantifying viability in haploid gametophytes of spores collected from ferns from background and contaminated areas of the Chalk River site. Dose rates measured in situ at field sites ranged from 60 to 849 µGy h-1, with effects possible at the more contaminated sites (greater than 400 µGy h-1). Fern spores were also irradiated from 1 to 1000 Gy to develop dose-response curves. We found no effects on gametophyte viability at the most contaminated areas of the Chalk River site, where we estimated growing season doses of 0.3-3.7 Gy. Dose-response curves show evidence of hormesis, with an increase in gametophyte viability up to 10 Gy, followed by a rapid decline to no viable gametophytes at doses of 1000 Gy. The sensitive fern is not a radiosensitive plant species, but effects do occur within the normal range (10-1000 Gy) of most plant species, making it useful as a sentinel species from a community perspective. Sensitive fern spore germination is high and stable over field dose ranges, with effects primarily on gametophyte viability. This method shows promise as an effects monitoring tool for sites with radiological contamination.

Several cesium and strontium bioaccumulation models are used widely in national and international guidance for ecological and human health risk assessments for radiocesium (134Cs and 137Cs) and radiostrontium (90Sr), but have not been used to make predictions of radiological risk from nuclear accidents under variable environmental conditions on broad geographical scales. In this paper, we first present models for predicting the bioaccumulation of cesium and strontium by aquatic biota based on ambient concentrations of dissolved potassium and calcium, respectively, and then test these models using independent data from aquatic ecosystems at Canadian nuclear sites. Secondly, models yielding the best predictions across a wide range of input parameters were selected to estimate bioaccumulation factors (BAFs) for cesium and strontium in aquatic ecosystems across Canada, using trophic level of organisms and dissolved potassium for cesium and calcium concentrations for strontium as predictor variables, and presented as contour maps of radiological risk. The models show that risk from bioaccumulation of cesium and strontium can vary by several orders of magnitude depending on site-specific environmental conditions and trophic ecology. Overall, our results suggest that single-parameter approaches taken by regulatory standards may either over- or under-predict radiological risk at many locations, and are thus not readily suitable to inform siting decisions for new nuclear developments.

Strontium (Sr) is a chemical element that is often used as a tracer in hydrogeochemical studies, and is ubiquitously distributed as a radioactive contaminant in nuclear sites in the form of strontium-90 (Sr-90). At the interface between groundwater and surface water, wetlands possess unique hydrogeochemical properties whose impact on Sr transport has not been investigated thoroughly. In this study, the adsorption and desorption of Sr was investigated on six natural wetland substrates and two mixes of exogenous media and wetland sediment: winter and summer wetland sediments, decayed cattails, wood, leaf litter, moss, bone charcoal, and clinoptilolite. The composition of the organic matter was characterized using carbon-13, solid phase Nuclear Magnetic Resonance analysis. The range of the substrates' adsorption coefficients obtained could be explained by factors indicative of proteins in the organic matter, which were shown to support strong and poorly reversible Sr adsorption. In contrast, the proportion of carbohydrates and lignin were found to be indicative of lower adsorption coefficients and higher desorption. The implications of these results for Sr pollution remediation in wetlands are discussed.

OBJECTIVE: The purposes of this essay are to illustrate the causes of FLAIR hyperintensity in the subarachnoid space and to outline the mechanisms of the findings. CONCLUSION: FLAIR subarachnoid space hyperintensity may be encountered with both pathological conditions and artifacts. Knowledge of these conditions and appearances coupled with any associated findings may suggest the cause of the FLAIR subarachnoid space hyperintensity. A diffuse distribution and a lack of ancillary findings often remain nonspecific and may require clinical correlation and CSF analysis.

Selenium (Se), an essential micro nutrient, is toxic to aquatic life at slightly higher water concentrations. Watersheds receiving leachate from selenium rich sources require large-scale, long-term treatment to mitigate Se toxicity. We applied the principles of anaerobic bacterial bioreactors, previously successful in small scale Se mitigation, to a whole end-pit lake ecosystem. Fertilization of the lake with N and P increased primary production, creating a meromictic, anoxic layer, and enhanced the habitat for locally present, anaerobic, Se and sulfur reducing bacteria. Within two years, Se concentrations were reduced ten-fold, reaching water-quality guideline values. The successful experiment demonstrated a novel treatment of large volumes of Se-contaminated water, and introduced an inexpensive method to mitigate a persistent aquatic pollutant of global concern.

Data from published studies and World Wide Web sources were combined to develop a regression model to predict (137)Cs concentration ratios for saltwater fish. Predictions were developed from 1) numeric trophic levels computed primarily from random resampling of known food items and 2) K concentrations in the saltwater for 65 samplings from 41 different species from both the Atlantic and Pacific Oceans. A number of different models were initially developed and evaluated for accuracy which was assessed as the ratios of independently measured concentration ratios to those predicted by the model. In contrast to freshwater systems, were K concentrations are highly variable and are an important factor in affecting fish concentration ratios, the less variable K concentrations in saltwater were relatively unimportant in affecting concentration ratios. As a result, the simplest model, which used only trophic level as a predictor, had comparable accuracies to more complex models that also included K concentrations. A test of model accuracy involving comparisons of 56 published concentration ratios from 51 species of marine fish to those predicted by the model indicated that 52 of the predicted concentration ratios were within a factor of 2 of the observed concentration ratios.

Two stigmatizing attitudes related to dangerousness and personal responsibility may undermine the opportunities of persons with serious mental illness. This study set out to examine path models that explain how these attitudes lead to discriminatory behavior and to assess the impact of antistigma programs on components of personal responsibility and dangerousness models. Two hundred thirteen persons were randomly assigned to one of five antistigma conditions: education on personal responsibility, education on dangerousness, contact with a person with serious mental illness where personal responsibility is discussed, contact where dangerousness is discussed, or no change. Persons completed an attribution questionnaire (AQ) representing personal responsibility and dangerousness path models at pretest, posttest, and 1-week followup. They also completed tasks that represented helping behavior. Goodness of fit indexes from linear structural modeling were mixed for both models but suggested that fear of dangerousness was a key attitude leading to discriminatory behavior. Results also showed that subjects who had contact with persons with serious mental illness experienced greater changes than subjects in the education or control groups did on measures of attribution and helping behavior.

In this study, we build on previous work by developing and estimating a model of the relationships between causal attributions (e.g., controllability, responsibility), familiarity with mental illness, dangerousness, emotional responses (e.g., pity, anger, fear), and helping and rejecting responses. Using survey data containing responses to hypothetical vignettes, we examine these relationships in a sample of 518 community college students. Consistent with attribution theory, causal attributions affect beliefs about persons' responsibility for causing their condition, beliefs which in turn lead to affective reactions, resulting in rejecting responses such as avoidance, coercion, segregation, and withholding help. However, consistent with a danger appraisal hypothesis, the effects of perceptions of dangerousness on helping and rejecting responses are unmediated by responsibility beliefs. Much of the dangerousness effects operate by increasing fear, a particularly strong predictor of support for coercive treatment. The results from this study also suggest that familiarity with mental illness reduces discriminatory responses.

Data from published studies and World Wide Web sources were combined to produce and test a regression model to predict Cs concentration ratios for freshwater fish species. The accuracies of predicted concentration ratios, which were computed using 1) species trophic levels obtained from random resampling of known food items and 2) K concentrations in the water for 207 fish from 44 species and 43 locations, were tested against independent observations of ratios for 57 fish from 17 species from 25 locations. Accuracy was assessed as the percent of observed to predicted ratios within factors of 2 or 3. Conservatism, expressed as the lack of under prediction, was assessed as the percent of observed to predicted ratios that were less than 2 or less than 3. The model's median observed to predicted ratio was 1.26, which was not significantly different from 1, and 50% of the ratios were between 0.73 and 1.85. The percentages of ratios within factors of 2 or 3 were 67 and 82%, respectively. The percentages of ratios that were <2 or <3 were 79 and 88%, respectively. An example for Perca fluviatilis demonstrated that increased prediction accuracy could be obtained when more detailed knowledge of diet was available to estimate trophic level.
