RESUMO

Atopic dermatitis (AD) is a common chronic skin disease caused by immune dysfunction, specifically the hyperactivation of Th2 immunity. AD is a complex disease with multiple factors contributing to its development; however, the interaction between these factors is not fully understood. In this study, we demonstrated that the conditional deletion of both the forkhead box p3 (Foxp3) and B-cell lymphoma 6 (Bcl6) genes induced the spontaneous development of AD-like skin inflammation with hyperactivation of type 2 immunity, skin barrier dysfunction, and pruritus, which were not induced by the single deletion of each gene. Furthermore, the development of AD-like skin inflammation was largely dependent on IL-4/13 signaling but not on immunoglobulin E (IgE). Interestingly, we found that the loss of Bcl6 alone increased the expression of thymic stromal lymphopoietin (TSLP) and interleukin (IL)-33 in the skin, suggesting that Bcl6 controls Th2 responses by suppressing TSLP and IL-33 expression in epithelial cells. Our results suggest that Foxp3 and Bcl6 cooperatively suppress the pathogenesis of AD. Furthermore, these results revealed an unexpected role of Bcl6 in suppressing Th2 responses in the skin.

The International Council of Nurses (2012) and the Council for International Organizations of Medical Sciences (2016) determined and published ethical standards relevant to nursing researchers and practitioners; based on these standards, nurses are expected to participate in committees where decisions on ethical issues are made. While clinical practitioners and nursing educators actively serve on research ethics committees, their precise role in these platforms has yet to be elucidated. In this study, medical, humanities/social science, lay, and nursing members in research ethics committees across Japan were invited to participate in a semi-structured interview; data were analyzed through a qualitative analysis method. Specifically, we interviewed 23 research ethics committee members in Japan to clarify the role of nursing members in the committee. Our qualitative analysis yielded six themes: share perspectives and experiences in nursing, protect research participants, evaluate the research design, represent the voice of research participants, confirm the informed consent documents and ascertain research participants' free will. The analyses revealed a slight difference between what other committee members expected of the role of nursing members and nursing members' recognition of their own role. Nursing members make an important and independent contribution to ethics committees on deliberations and decision-making regarding research ethics. Within the context of research ethics committees, member selection and training are essential issues, and this study contributes to the literature by showing how these topics relate to the role of the research ethics committees and of their members.

For reducing a risk of pressure sore caused by long period sitting on a wheelchair, a patient is required to depressurize buttocks by changing the posture suitably. Thus, we have been developed an automatic depressurization motion assistance system for a seated patient on a wheelchair. In our previous work, we developed a sitting surface coordination system which can lift or incline and assists a depressurization motion of a patient. However, in some cases, it is difficult to change his posture suitably for patients who do not have enough physical strength in their upper body using our proposed device. Therefore, in this paper, we propose a novel posture coordination assistance system for a wheelchair user. Our system consists of the adjustable sitting surface and novel proposed adjustable backrest. These two devices coordinate the sitting posture of the patient automatically. Our key ideas are two topics. One topic is mechanical design of the adjustable backrest for practical use. We realize small mechanism which enables easy implementation to a general wheelchair without special reconstruction. The other topic is a natural posture coordination scheme using the sitting surface and the backrest. We analyze buttocks depressurization operation by a nursing specialist and drive our proposed devices for realizing it. The performance of our system is verified by experiments using our prototype.

For reducing the risk of pressure sore caused by long period sitting on a wheelchair, we develop a depressurization motion assistance system which is low cost and suitable for practical use. Our developing system consists of a seating cushion which the patient sits on and four air cells which can lift or incline the seating cushion. Each air cell is actuated by small air compressor, which can drive using batteries on the wheelchair respectively, and each compressor has a pressure sensor on its body. In this paper, our key ideas are two topics. One topic is mechanical design for practical use. We realize thin mechanism which enables easy implementation to the general wheelchair. For realizing this thinly design, we develop the tilt mechanism using elasticity of acrylic resin and the controller which uses only pressure sensors for estimating its lifting height and inclination. The other topic is assistance control scheme based on the patient's depressurization operation for increasing a rehabilitation performance. For realizing the proposed control scheme, we analyze the hip depressurization operation by the nursing specialists and use its results for estimating the patient's condition. Using our system, the patient can depressurize by his own will on the general wheelchair easily. The performance of our system is verified by experiments using our prototype.
