OBJECTIVE: To generate and to evaluate ex vivo a novel model of bioengineered human bladder mucosa based on fibrin-agarose biomaterials. METHODS: We first established primary cultures of stromal and epithelial cells from small biopsies of the human bladder using enzymatic digestion and selective cell culture media. Then, a bioengineered substitute of the bladder lamina propria was generated using cultured stromal cells and fibrin-agarose scaffolds, and the epithelial cells were then subcultured on top to generate a complete bladder mucosa substitute. Evaluation of this substitute was carried out by cell viability and histological analyses, immunohistochemistry for key epithelial markers and transmission electron microscopy. RESULTS: The results show a well-configured stroma substitute with a single-layer epithelium on top. This substitute was equivalent to the control bladder mucosa. After 7 days of ex vivo development, the epithelial layer expressed pancytokeratin, and cytokeratins CK7, CK8 and CK13, as well as filaggrin and ZO-2, with negative expression of CK4 and uroplakin III. A reduction of the expression of CK8, filaggrin and ZO-2 was found at day 14 of development. An immature basement membrane was detected at the transition between the epithelium and the lamina propria, with the presence of epithelial hemidesmosomes, interdigitations and immature desmosomes. CONCLUSIONS: The present results suggest that this model of bioengineered human bladder mucosa shared structural and functional similarities with the native bladder mucosa, although the epithelial cells were not fully differentiated ex vivo. We hypothesize that this bladder mucosa substitute could have potential clinical usefulness after in vivo implantation.

OBJECTIVE: To report a case of GIST type retroperitoneal tumor with spontaneous rupture to the abdominal cavity causing acute abdomen secondary to hemoperitoneum. METHODS/RESULTS: We report the case of an 84 year-old man with history of BPH and chronic atrial fibrillation. He presented to the Emergency Department with diffuse abdominal pain, syncope and accompanying vegetative symptoms. Diagnostic work up showed a 19 cm retroperitoneal mass dependent of the left kidney with active bleeding and secondary hemoperitoneum. Left radical nephrectomy was performed with pathology report of gastrointestinal stromal tumor attached to the renal capsule. CONCLUSIONS: Spontaneous hemoperitoneum is a rare entity and it has various etiologies. It is rarely described in retroperitoneal tumors.

Nanoplastics and engineering nanomaterials (ENMs) are contaminants of emerging concern (CECs), increasingly being detected in the marine environment and recognized as a potential threat for marine biota at the global level including in polar areas. Few studies have assessed the impact of these anthropogenic nanoparticles in the microbiome of marine invertebrates, however combined exposure resembling natural scenarios has been overlooked. The present study aimed to evaluate the single and combined effects of polystyrene nanoparticles (PS NP) as proxy for nanoplastics and nanoscale titanium dioxide (nano-TiO2) on the prokaryotic communities associated with the gill tissue of the Antarctic soft-shell clam Laternula elliptica, a keystone species of marine benthos Wild-caught specimens were exposed to two environmentally relevant concentrations of carboxylated PS NP (PS-COOH NP, ∼62 nm size) and nano-TiO2 (Aeroxide P25, ∼25 nm) as 5 and 50 µg/L either single and combined for 96h in a semi-static condition.Our findings show a shift in microbiome composition in gills of soft-shell clams exposed to PS NP and nano-TiO2 either alone and in combination with a decrease in the relative abundance of OTU1 (Spirochaetaceae). In addition, an increase of gammaproteobacterial OTUs affiliated to MBAE14 and Methylophagaceae (involved in ammonia denitrification and associated with low-quality water), and the OTU Colwellia rossensis (previously recorded in polluted waters) was observed. Our results suggest that nanoplastics and nano-TiO2 alone and in combination induce alterations in microbiome composition by promoting the increase of negative taxa over beneficial ones in the gills of the Antarctic soft-shell clam. An increase of two low abundance OTUs in PS-COOH NPs exposed clams was also observed. A predicted gene function analysis revealed that sugar, lipid, protein and DNA metabolism were the main functions affected by either PS-COOH NP and nano-TiO2 exposure. The molecular functions involved in the altered affiliated OTUs are novel for nano-CEC exposures.

OBJECTIVE: To describe one case of hematocele secondary to rupture of an abdominoscrotal hydrocele in an adult patient. METHODS AND RESULTS: We report a huge hematocele in a patient with this unusual type of hydrocele that suffered a minimal scrotal trauma. It was a hydrocele that extended through the inguinal canal to the retroperitoneal space. CONCLUSIONS: Abdominoscrotal hydrocele is a rare condition in children and even rarer in adults. The presence of a hematocele requires early surgical treatment.

The Antarctic toothfish (Dissostichus mawsoni) is the largest notothenioid species in the Southern Ocean, playing a keystone role in the trophic web as a food source for marine mammals and a top predator in deep-sea ecosystems. Most ecological knowledge on this species relies on samples from areas where direct fishing is allowed, whereas in areas closed to fishing, such as the Antarctic Peninsula (AP), there are still key ecological gaps to ensure effective conservation, especially regarding our understanding of its trophic relationships within the ecosystem. Here, we present the first comprehensive study of the feeding behavior of Antarctic toothfish caught in the northern tip of the AP, during two consecutive fishing seasons (2019/20 and 2020/21). Stomach content was analyzed according to size-classes, sex and season. Macroscopic morphological analysis was used to identify prey, whereas DNA analysis was used in highly digested prey items. Fatty acid analysis was conducted to determine the prey's nutritional composition. The diet mainly consisted of Macrouridae, Cephalopoda, Anotopteridae, and Channichthyidae. Other prey items found were crustaceans and penguin remains; however, these were rare in terms of their presence in stomach samples. Sex had no effect on diet, whereas size-class and fishing season influenced prey composition. From 27 fatty acid profiles identified, we observed two different prey groups of fishes (integrated by families Anotopteridae, Macrouridae and Channichthyidae) and cephalopods. Our results revealed a narrow range of prey items typical of a generalist predator, which probably consumes the most abundant prey. Understanding the diet and trophic relationships of Antarctic toothfish is critical for a better comprehension of its role in the benthic-demersal ecosystem of the AP, key for ecosystemic fisheries management, and relevant for understanding and predicting the effect of climate change on this species.

We report a case of a rare congenital malformation, a urogenital sinus cyst in a young patient with non-specific symptoms for several months. A 21-year-old male presented with vague and intermittent abdominal pain. Ultrasound scan showed a retrovesical hypoechoic collection (approximately 6 cm) and left renal agenesis, compatible with a possible congenital malformation. These findings were confirmed by MRI. A laparoscopic excision of the cyst was performed with no complications. Pathology report confirmed a urogenital sinus cyst. The postoperative recovery was uneventful and the patient was discharged within 48 h. Urogenital sinus cysts are rare entities with few cases described in the literature. Imaging techniques such as ultrasound, CT or MRI may help with diagnosis. Therapeutic modalities range from observation to needle aspiration or surgical removal. In our case, we chose a laparoscopic approach to minimize morbidity and achieve an early recovery.

Fast climate changes in the western Antarctic Peninsula are reducing krill density, which along with increased fishing activities in recent decades, may have had synergistic effects on penguin populations. We tested that assumption by crossing data on fishing activities and Southern Annular Mode (an indicator of climate change in Antarctica) with penguin population data. Increases in fishing catch during the non-breeding period were likely to result in impacts on both chinstrap (Pygoscelis antarcticus) and gentoo (P. papua) populations. Catches and climate change together elevated the probability of negative population growth rates: very high fishing catch on years with warm winters and low sea ice (associated with negative Southern Annular Mode values) implied a decrease in population size in the following year. The current management of krill fishery in the Southern Ocean takes into account an arbitrary and fixed catch limit that does not reflect the variability of the krill population under effects of climate change, therefore affecting penguin populations when the environmental conditions were not favorable.

Internal marketing involves the development of organizational strategies that promote the welfare of the employees who, in turn, play a transcendental role in achieving institutional goals. Ecuadoran co-operativism lacks relevant studies of this construct and, because of this, this research intends to analyze the dimensions of internal marketing, through the validation of a measuring instrument that has been adapted to this sector of the social economy, in a developing country. The methodology is based on the completion of field work, where a structured questionnaire for a representative sample of 2,499 employees and officers of the operatives of Ecuador was applied. The suitability of the scale was determined through the means of a model of Ecuadoran structural equations. The results show that internal marketing is a multi-dimensional construct and it can be measured in six dimensions: Identify Value Exchange (IVE), Internal Market Segmentation (IMS), Internal Communication (IC), Management Concern (MC), Training (TR), and Work/Family Balance (WFB).

The continuous dynamism that tourism suffers makes the motivations of tourists change, making them search for new experiences. In this sense, gastronomy is positioned as a key element of union between tourism and culture, thus developing gastronomic tourism. The analysis presented here will address gastronomic motivations and their influence on the value perceived by foreign tourists in Oruro (Bolivia), based on a total sample of 370 and through a covariance-based structural equation modelling (CB-SEM). Among the main results, it is worth highlighting the positive influence of gastronomic motivations on the value perceived by tourists. These results are also very useful for public and/or private entities for the creation of new strategies to promote local gastronomy.

Gastronomy allows tourists to discover the cultural roots of the destination, thus being one of the motivations for its choice and even becoming a fundamental factor in the analysis of the overall satisfaction of the tourist experience. This research is focused on the analysis of the gastronomic experiences of foreign tourists who visit the city of Oruro (Bolivia). The main results of this research show a segmentation of travellers on the basis of their perceptions regarding the gastronomic experiences in this city into three different groups (survivors, enjoyers, and experiencers). Additionally, the study concludes that these gastronomic experiences are highly related to the motivations that the travellers have to discover a specific destination, and that their level of satisfaction with these experiences is highly connected to the relationship that gastronomy has with the cultural heritage of the local community.

Introducción: Procesos como la mutagénesis, la carcinogénesis y la teratogénesis son producto de la interacción de agentes de origen endógeno como exógeno que interactúan con la molécula de ADN en forma crónica produciendo rupturas en la doble hélice, y en cromosomas completos resultando en la inestabilidad genómica. El estrés oxidativo al que se encuentran sometidas las células al formarse las especies reactivas de oxígeno (ROS) y también las especies reactivas de nitrógeno (RNS), que pueden provenir de radicales producidos a consecuencia de la diabetes o en estados iniciales de la enfermedad renal crónica o como respuesta a procesos inflamatorios en estados avanzados de estas patologías, actúan como agentes genotóxicos endógenos.Objetivos: Esta investigación tuvo como objetivo determinar el daño basal en la molécula de ADN de pacientes diabéticos hemodializados, a través del ensayo del Cometa, como un bioindicador de inestabilidad genómica., durante seis meses de tratamiento. Materiales y métodos: Se planteó un estudio longitudinal prospectivo de cohorte para comparar los diferentes niveles de daño antes y durante los primeros seis del tratamiento de hemodiálisis. Se evaluó con el test del cometa o electroforesis de células individuales, el daño basal en muestras de sangre venosa de pacientes diagnosticados con Diabetes de tipo II como control negativo y en pacientes diabéticos con enfermedad renal crónica antes de iniciar el tratamiento de diálisis y luego durante el tratamiento. Se utilizó el test de t- Student para muestras independientes y emparejadas. Resultados: Se observó un aumento significativo de daño basal y oxidativo en el material genético de pacientes diabéticos con enfermedad renal crónica, comparados con los controles negativos (p< 0.005) y se observó, además, que el daño celular aumenta con el tratamiento de hemodiálisis (p<0.005). Conclusión: Los resultados obtenidos en esta investigación permiten concluir que el estrés oxidativo tiene un efecto genotóxico y que el nivel de daño genético es un buen bioindicador del avance de la enfermedad renal crónica y que la hemodiálisis induce a un aumento de daño a nivel del material genético, aumentando el riesgo de carcinogénesis.


Introduction: Processes such as mutagenesis, carcinogenesis and teratogenesis are the product of the interaction of agents of endogenous and exogenous origin that interact with the DNA molecule in a chronic way producing ruptures in the double helix, and in complete chromosomes resulting in genomic instability. The oxidative stress to which the cells are subjected when reactive oxygen species (ROS) and reactive nitrogen species (RNS) are formed, which may come from radicals produced as a result of diabetes or in initial stages of chronic kidney disease or in response to inflammatory processes in advanced stages of these pathologies, act as endogenous genotoxic agents. Objectives: This research aimed to determine the basal damage in the DNA molecule of hemodialyzed diabetic patients, through the Comet assay, as a bioindicator of genomic instability, during six months of treatment. Materials and methods: For this research, a prospective longitudinal cohort study was proposed to compare the different levels of genetic damage before and during the first six of hemodialysis treatment. Baseline damage was evaluated with the comet test or single cell electrophoresis, in venous blood samples from patients diagnosed with Type II Diabetes as a negative control and in diabetic patients with chronic kidney disease before starting dialysis treatment and then during treatment. Results: A significant increase in basal and oxidative damage was observed in the genetic material of diabetic patients with chronic kidney disease, compared to negative controls (p< 0.005) and it was also observed that cell damage increases with hemodialysis treatment (p<0.005). The t-Student test was used for independent and paired samples. Conclusion: The results obtained in this research allow us to conclude that oxidative stress has a genotoxic effect and that the level of genetic damage is a good bioindicator of the progression of chronic kidney disease and that hemodialysis induces an increase in damage at the level of the genetic material, increasing the risk of carcinogenesis.

La fosfomicina es un antibiótico natural, que actúa sobre la síntesis de la pared celular, con actividad bactericida y de amplio espectro. En este trabajo se evaluó la sensibilidad in vitro de aislados de Escherichia coli (E. coli), incluidos aquellos que producen Beta Lactamasa de Espectro Extendido (BLEE) obtenidos a partir de urocultivos, tomados en diferentes lapsos de colección de datos, en personas de ambos sexos. Fueron incluidos 260 muestras de orina con desarrollo de E. coli provenientes de pacientes que concurrieron al Laboratorio San Roque. El aislamiento e identificación bacteriana se realizó según métodos convencionales y la sensibilidad a los antimicrobianos por el método de difusión de disco. Para la detección de la sensibilidad frente a fosfomicina fueron utilizados discos de 200 µg con el agregado de 50 µg de glucosa 6-fosfato. Se observó frente a los antibióticos evaluados mayor sensibilidad a fosfomicina (98,5%) y nitrofurantoína (97,7%). Ciprofloxacina, trimetoprima y la combinación sulfametoxazol/trimetoprima exhibieron frente a los mismos aislados sensibilidad menor y muy similar entre ellos, con 64,2%, 61,2% y 61,2% respectivamente. En 44 (16,9%) de los aislados de E. coli se detectó la presencia de BLEE y es destacable la alta sensibilidad que mostraron fosfomicina y nitrofurantoína, aún frente a los aislados BLEE positivos, con frecuencias de 90,9% y 93,2%, respectivamente. En resumen, la alta sensibilidad demostrada en el presente estudio por E. coli ante la fosfomicina, abre la posibilidad de considerar a este antibiótico de primera elección en las infecciones urinarias bajas, aún en los casos de gérmenes productores de BLEE, en la población de nuestro país.


Fosfomycin is a natural antibiotic, which acts on the synthesis of the cell wall, with broad spectrum bactericidal activity. In this study, the in vitro sensitivity of Escherichia coli (E. coli) isolates was evaluated, including those that produce Extended Spectrum Beta Lactamase (ESBL) obtained from urine cultures taken at different data collection times, in people of both sexes. Were included 260 urine samples with development of E. coli from patients who attended the San Roque laboratory. Bacterian isolation and identification was carried out according to conventional methods and antimicrobial sensitivity by the disk diffusion method. For detection of sensitivity to fosfomycin, 200 µg discs were used with the addition of 50 µg of glucose 6-phosphate. A greater sensitivity for fosfomycin (98.5%) and nitrofurantoin (97.7%) was observed against the evaluated antibiotics. Ciprofloxacin, trimethoprim and the sulfamethoxazole / trimethoprim combination exhibited in front of the same isolated lower sensitivity and very similar among them, with 64.2%, 61.2% and 61.2% respectively. In 44 (16.9%) of the E. coli isolates the presence of ESBL was detected and the high sensitivity shown by fosfomycin and nitrofurantoin is noteworthy, even compared to the positive ESBL isolates, with frequencies of 90.9% and 93,2%, respectively. In summary, the high sensitivity demonstrated in the present study by E. coli to fosfomycin opens the possibility of considering this first-choice antibiotic in lower urinary infections, even in ESBL-producing germs, in the population of our country.

Paraguay is a landlocked country located in South America with a total population of 5,884,491. Most of the population (95%) is mestizo, a mixture of Spanish and American/Indian races. The total number of indigenous people in the country has increased from 38,703 in 1981 to 85,674 in 2002. The gross domestic product per capita was US $932.00 annually per person in the year 2002. Between 1992 and 1997, there were 380 patients on chronic dialysis in Paraguay and 75 patients received renal transplants, mostly living-related. The prevalence of renal replacement therapy was 87 patients per million, and the incidence of renal disease continues to rise. Seventy percent of cases of ESRD are of unknown etiology and 15% have diabetes-related renal disease. Only citizens covered by the employee's national health insurance have complete coverage for dialysis and transplantation. The remainder of the population has to apply to public hospitals when the need for hemodialysis arises. At such hospitals, they can receive hemodialysis coverage from the National Institute of Nephrology or from other medical foundations to obtain entrance to these programs. They must otherwise use their own resources to pay for treatment. Seventy percent of patients on chronic dialysis turn to public hospitals for treatment. Hospital hemodialysis is the method most widely used. Home dialysis is rarely performed and there are very few programs for ambulatory peritoneal dialysis. Thus, a large number of patients are not able to enter chronic dialysis programs. In a recent survey of 4655 ill children registered, the distribution of main renal disease was acute glomerulonephritis in 42 cases (9 per 1000), nephrotic syndrome in 40 cases (8.5 per 1000), systemic lupus erythematosis in 28 cases (6 per 1000), and hematuria alone in 11 cases (2.3 per 1000). In ambulatory pediatric practice, urinary tract infection is the leading reason for seeking medical advice. Two thirds of such cases are associated with urinary tract anomalies. Children with ESRD are able to enter hemodialysis programs, but there are not sufficient resources to transplant them. Over 60% of the children with ESRD are hospitalized with terminal renal failure; malformations of the urinary tract are the usual cause. One study of 9880 adults aged 18 to 74 years reported that 39.1% of the women and 26.8% of the men examined were found to have hypertension. Almost half who were found to have raised blood pressure in this study were not previously known to have hypertension. In another cross-sectional study of the urban and suburban mestizo population of Asuncion among patients between 20 and 74 years of age, the overall prevalence of diabetes mellitus was 6.5%, impaired glucose tolerance 13.5%, hypertension 17%, and obesity 31.6%. Extrapolating from this data, we can assume that 178,000 patients with hypertension in Paraguay need medical treatment. To face the problem of growing numbers of patients with end-stage renal failure, it is necessary to carry out basic epidemiologic research to detect and quantify cases early in the course of disease, and thus propose treatments designed to slow the progress of the disease. Without this type of data, it would be difficult to establish an efficient action plan for improving the development of the treatment of renal disease. Thus, we are recommending the establishment of early detection and treatment campaigns for chronic renal disease, especially in individuals at risk. It is also desirable to promote renal transplantation using related live donors. We need to cooperate with government authorities to increase the insurance coverage of patients on chronic dialysis and find the most practical ways to establish long-term dialysis programs. A major question that is hard to answer in practice is whether there should be universal insurance for dialysis and transplantation for all who need it from the outset, as opposed to implementation in successive stages, which gives priority to only a minority of the population; which could be better adapted to our financial possibilities.

RESUMO

La función renal (FR) es medida por varios métodos. La más utilizada es el aclaramiento de creatinina (ClCr), que reflejaría, el filtrado glomerular (FG). Para su medición se recurre a la recolección de orina durante 24 horas o a la utilización de fórmulas, siendo la más utilizada, la ecuación CKD-EPI (Chronic kidney disease Epidemiology Collaboration). La disfunción renal se clasifica en 5 estadios. El estadio 5 (cuando el ClCr es igual o inferior a 15 ml/min), es cuando los pacientes están prontos a recibir tratamiento sustitutivo renal (TSR). La hemodiálisis (HD), es una técnica muy utilizada como TSR y normalmente es llevada a cabo 3 veces a la semana, por 4 horas cada una. Un paciente de 59 años, quién debido a la distancia entre su domicilio y el Centro de diálisis decide (consentimiento firmado), someterse a solo 2 sesiones de HD por semana. Al inicio del tratamiento, el paciente presentaba todos los datos clínicos y bioquímicos de la Enfermedad renal Crónica Avanzada, estadio 5 y un volumen diurético (VD) ≥ 1 litro/día. En caso de empeoramiento clínico y/o bioquímico, las sesiones de HD serían 3 veces por semana. Al año, los parámetros clínicos, bioquímicos y la función renal residual (FRR), permanecen óptimos. Si FRR (medido por el aclaramiento de urea (Kru) y el VD), declina serán necesarias 3 sesiones de HD, semanales. El mantenimiento de la FRR, está relacionado con la mortalidad y la HD incremental, al preservar mejor la FRR, mejora la sobrevida del paciente.


Several methods are useful to measure renal function (RF). In clinical practice, the creatinina cleareance (CrCl), is widely used, which approximately reflects the glomerular filtration rate (GFR). The 24 hs urine volume collection is required to measure CrCl, however, thanks different formulas we can have a precise CrCl value. The CKD-EPI (chronic kidney disease epidemiology collaboration), is the equation frequently used. According to glomerular filtration rate (GFR), the renal function has been classified in 5 stages. At advances stages (stage 5), (CrCl: 15 ml/min), the patient is faced to receive renal replacement therapy (RRT). Hemodialysis (HD) method is often used. It is carry out 3 times per week (4 hours each). A 59 years old male, due to the distance between the Capital City and his home, decided to receive HD only two times per week (signed consent). At the beginning of the treatment the patient presents all the clinical and biochemical data corresponding to Chronic Renal Failure stage 5. His diuretic volume (DV), ≥ 1 lt/day. In the case of RF deterioration reflected clinical and/or biochemically, the HD session would be 3 times a week. At year, presents adequate residual renal function (RRF) and clinics, biochemical parameters as well. If the RRF (measured by urea clareance (Kru) and the DV) decline, 3 sessions per week will be necessary. The RRF maintenance is related to mortality, therefore, its preservation thanks incremental HD, improve the patient survival.

RESUMO

A 33 year-old male, who five years earlier was operated for a nodular melanoma in vertical growth phase of 1.5mm thick (Breslow), Clark II-III, presented to the Emergency department with a progressive increase of the left testicle. Ultrasound scan examination revealed a 2 x 2 cm solid mass in the left testis, with normal right testis.With the diagnosis of testicular neoplasm, we requested Alpha-fetoprotein and beta HCG levels, and patient underwent radical orchiectomy.

Introducción: El propósito del estudio es determinar en los urocultivos, la prevalencia de los mecanismos enzimáticos de resistencia encontrados in vitro. Materiales y métodos: Se realizó un estudio retrospectivo, de ambos sexos, mayores de 18 años, que acudieron al Consultorio Externo de Clínica Médica y Urgencias por síntomas de infección urinaria. Se incluyeron todos los urocultivos en los que se aislaron uropatógenos con recuento ≥ 105 UFC/mL. Se excluyeron los urocultivos polimicrobianos, los que no contaban con antibiograma o aquellos con datos clínicos incompletos. Resultados: Se identificaron 1031 urocultivos que cumplieron con los criterios establecidos para la realización del estudio. El 56% correspondió al sexo femenino y el 43% al masculino. La edad media de las mujeres fue de 52± 20 años y el de los hombres fue de 62±16 años. Los uropatógenos más frecuentes fueron Escherichia coli 553 (52% en promedio) seguida de Klebsiella pneumoniae con 148 (14% en promedio). Urocultivos de varones: El principal mecanismo de resistencia de Escherichia coli fueron las BLEE, 55 aislamientos (91%); seguida de las MBL, 3 aislamientos (5%) y KPC, 2 aislamientos (3%). En Klebsiella pneumoniae en 53 aislamientos se puedo observar: BLEE, 31 aislamientos (58%); seguida de las KPC 13 aislamientos (25%) y MBL, 9 aislamientos en (16%). Urocultivos en mujeres: Las enzimas de Escherichia coli fueron 81 aislamientos, de los cuales fueron BLEE, 79 aislamientos (97%); seguido de las KPC, 1 aislamiento (1%) y las MBL, 1 aislamiento (1%). En Klebsiella pneumoniae se pudo observar los siguientes mecanismos enzimáticos en base a 35 aislamientos; BLEE, 19 aislamientos (54%), seguida de las KPC, 12 aislamientos (34%) y por último, MBL, 4 aislamientos (13%). Conclusión: En las IVU de nuestro estudio, Escherichia coli y Klebsiella pneumoniae fueron las principales bacterias que originan resistencia a los antibióticos y la BLEE fue la enzima más frecuentemente identificada en ambos sexos.


Introduction: The objective of this study was to assess frequency of enzymatic resistance mechanisms isolated from community urinary tract infections (UTI) determined in vitro in urine cultures. Objectives: This is a retrospective study, a total of 1031 urine samples were included from patients with urinary tract infection who had consulted at the Outpatient Clinic and Emergencies Services. The following information was recorded, age, sex, urine sample. All urine cultures in which pathogens with a count of ≥ 105 CFU / mL were included. Were excluded polymicrobial urine cultures, those without an antibiogram or those with incomplete clinical data. Results: A total of 1031 urine samples met inclusion criteria, 56% of patients were female and 43% male. The mean age of the women was 52 ± 20 years and in men was 62 ± 16 years. 553 (52%) E. coli and 148 (14%) Klebsiella strains were isolated from community samples. Male urine cultures: The main resistance mechanism of Escherichia coli was ESBLs, 55 isolates (91%); followed by MBL, 3 isolates (5%) and KPC, 2 isolates (3%). In Klebsiella pneumoniae (53 isolates); ESBL, 31 isolates (58%); followed by KPC 13 isolates (25%) and MBL, 9 isolates in (16%). Female urine culture: Escherichia coli enzymes 81 isolates, of which ESBLs were 79 isolates (97%); followed by KPC, 1 isolate (1%) and MBL, 1 isolate (1%). In Klebsiella pneumoniae the following enzymatic mechanisms could be observed based on 35 isolates; ESBL, 19 isolates (54%), followed by KPCs, 12 isolates (34%) and finally, MBL, 4 isolates (13%). Conclusion: The result of our study showed high prevalence of Escherichia coli and Klebsiella pneumoniae causing resistance to antibiotics in culture bacteria from urine samples of patients with UTI, and ESBL was the main ß-lactamase resistance mechanism in Klebsiella and E. coli isolates in both male and female.
