RESUMO

Recognizing the importance of following the International Nursing Association for Clinical Simulation and Learning Standards: Healthcare Simulation Standards of Best Practice to ensure a successful simulation-based experience, this study aimed to update an electronic workflow created to guide teachers in the development and conduct of a clinical simulation for undergraduate nursing students, according to the latest evidence published in the International Nursing Association for Clinical Simulation and Learning Standards; implement it for online access; and describe the results of its usability criteria evaluation. It is a technological production and study of quantitative nature, descriptive and exploratory. The workflow was reconfigured according to the Contextualized Instructional Design model. The usability criteria analysis occurred through the application of a questionnaire to 23 undergraduate nursing courses professors. It was possible to identify that the technology reached high levels of global classification among faculty, being classified as "excellent" (M [SD], 81.9 [14.9]) by the final score. In view of the results obtained, it is concluded that the Electronic Workflow for High-Fidelity Clinical Simulation in Nursing can serve as a reference to promote the best simulation-based learning for professional training in nursing.

Cerebrovascular accident is a serious public health problem and requires the attention of professionals who can detect, diagnose, and provide care in a timely fashion. A quantitative quasi-experimental study was conducted using a mobile app called mSmartAVC for clinical evaluation of nursing care at the bedside. The study aimed at measuring the knowledge of nurses and nursing students in the detection and care of cerebrovascular accident. In this study, a total of 115 nurses from health services in the South of Brazil and 35 nursing students of a community university participated. The stages focused on development, modeling of clinical cases, problem-based learning, pretest (before) app use, and posttest (after) use of the app. The results of the pretest and posttest corrections showed a substantial statistical difference (P < .001), indicating a significant knowledge gain after the use of the app, particularly in terms of the detection scales and interpretation of the imaging tests. The mSmartAVC app used at the bedside supported decision-making for detection and nursing care. It was possible to confirm that the use of mobile apps plays an essential role as a learning tool for nurses and nursing students.

OBJECTIVE: Analyzing the ergonomics and usability criteria of the Computerized Nursing Process based on the International Classification for Nursing Practice in the Intensive Care Unit according to International Organization for Standardization(ISO). METHOD: A quantitative, quasi-experimental, before-and-after study with a sample of 16 participants performed in an Intensive Care Unit. Data collection was performed through the application of five simulated clinical cases and an evaluation instrument. Data analysis was performed by descriptive and inferential statistics. RESULTS: The organization, content and technical criteria were considered "excellent", and the interface criteria were considered "very good", obtaining means of 4.54, 4.60, 4.64 and 4.39, respectively. The analyzed standards obtained means above 4.0, being considered "very good" by the participants. CONCLUSION: The Computerized Nursing Processmet ergonomic and usability standards according to the standards set by ISO. This technology supports nurses' clinical decision-making by providing complete and up-to-date content for Nursing practice in the Intensive Care Unit. OBJETIVO: Analisar os critérios de ergonomia e usabilidade do Processo de Enfermagem Informatizado a partir da Classificação Internacional para as Práticas de Enfermagem, em Unidade de Terapia Intensiva, de acordo com os padrões da InternationalOrganization for Standardization (ISO). MÉTODO: Pesquisa quantitativa, quase-experimental do tipo antes e depois, com uma amostra de 16 participantes, realizada em uma Unidade de Terapia Intensiva. Coleta de dados realizada por meio da aplicação de cinco casos clínicos simulados e instrumento de avaliação. A análise dos dados foi realizada pela estatística descritiva e inferencial. RESULTADOS: Os critérios organização, conteúdo e técnico foram considerados "excelentes", e o critério interface "muito bom", obtendo médias 4,54, 4,60, 4,64 e 4,39, respectivamente. Os padrões analisados obtiveram médias acima de 4,0, sendo considerados "muito bons" pelos participantes. CONCLUSÃO: O Processo de Enfermagem Informatizado possui padrões ergonômicos e de usabilidade de acordo com os padrões estabelecidos pela ISO. Esta tecnologia apoia a decisão clínica do enfermeiro fornecendo conteúdo completo e atualizado para a prática de Enfermagem em Unidade de Terapia Intensiva.

The aim of this work was to measure the cognitive workload to complete printed nursing process versus computerized nursing process from International Classification Practice of Nursing in intensive care units. It is a quantitative, before-and-after quasi-experimental design, with a sample of 30 participants. Workload was assessed using National Aeronautics and Space Administration Task-Load Index. Six cognitive categories were measured. The "temporal demand" was the largest contributor to the cognitive workload, and the role of the nursing process in the "performance" category has excelled that of computerized nursing process. It was concluded that computerized nursing process contributes to lower cognitive workload of nurses for being a support system for decision making based on the International Classification Practice of Nursing. The computerized nursing process as a logical structure of the data, information, diagnoses, interventions and results become a reliable option for health improvement of healthcare, because it can enhance nurse safe decision making, with the intent to reduce damage and adverse events to patients in intensive care.

OBJECTIVE: To analyze the usability of Computerized Nursing Process (CNP) from the ICNP(®) 1.0 in Intensive Care Units in accordance with the criteria established by the standards of the International Organization for Standardization and the Brazilian Association of Technical Standards of systems. METHOD: This is a before-and-after semi-experimental quantitative study, with a sample of 34 participants (nurses, professors and systems programmers), carried out in three Intensive Care Units. RESULTS: The evaluated criteria (use, content and interface) showed that CNP has usability criteria, as it integrates a logical data structure, clinical assessment, diagnostics and nursing interventions. CONCLUSION: The CNP is a source of information and knowledge that provide nurses with new ways of learning in intensive care, for it is a place that provides complete, comprehensive, and detailed content, supported by current and relevant data and scientific research information for Nursing practices.

RESUMO

A hybrid study combining technological production and methodological research aiming to establish associations between the data and information that are part of a Computerized Nursing Process according to the ICNP® Version 1.0, indicators of patient safety and quality of care. Based on the guidelines of the Agency for Healthcare Research and Quality and the American Association of Critical Care Nurses for the expansion of warning systems, five warning systems were developed: potential for iatrogenic pneumothorax, potential for care-related infections, potential for suture dehiscence in patients after abdominal or pelvic surgery, potential for loss of vascular access, and potential for endotracheal extubation. The warning systems are a continuous computerized resource of essential situations that promote patient safety and enable the construction of a way to stimulate clinical reasoning and support clinical decision making of nurses in intensive care.

RESUMO

The Nursing Process is a care technology that guides the sequence of clinical reasoning and improves the quality of care. This article reports the development of a Computerized Nursing Process (CNP) for the Intensive Care Unit. The study was conducted in three main steps: discussion and understanding of the International Organization for Standard 18104; theoretical expertise regarding the ICNP® 1.0; and the association of information with nursing diagnoses and interventions. The knowledge base was organized according to the ICNP® Version 1.0. The result was a restructuring of the CNP from the association of clinical evaluations to diagnosis and interventions that allow documentation of the clinical practice of nursing and provide support for decision-making. The methodological steps employed permitted the creation of an association between the clinical evaluation, diagnoses, interventions and results of the ICNP ® 1.0 with ISO 18104.

OBJECTIVES: to identify and confirm the priority nursing diagnosis of International Classification for Nursing Practice® for home nursing consultation to adults in Primary Health Care. METHODS: qualitative study, of methodological and validation type. The 5-point Likert scale was used, with a minimum Content Validity Index of 80% consensus among judges., considering the answers "priority" or "very priority" for the list of nursing diagnoses presented. 23 expert judges participated in this survey. RESULTS: a hundred and eleven nursing diagnoses of prepared statements lists have been grouped by human systems and sociodemographic characteristics. were grouped by human systems and sociodemographic characteristics. Eighty-three of them (74.77%) had a Content Validity Index equal or higher to 0.8; and 27 (32.5%) had an index of 1.0 (100%) among judges. CONCLUSIONS: nursing diagnosis validated can be used to assist clients in home nursing consultations in Primary Health Care.

RESUMO

In this qualitative study, we adapted photographic research methods from earlier nursing research to identify factors related to organization, storage and distribution that could lead to errors in the selection, preparation and administration of medications. The research excerpt presented here was developed in a clinical unit of an urban Brazilian public hospital. The research participants were nurses working at that unit and students from the two final semesters of the Undergraduate Nursing Course. We collected digital photographs of the medication system and subsequently used photo elicitation to review the images with research participants, so as to obtain their perceptions and narratives of working with medications in the unit. We report selected findings here on the organization, storage and distribution of medications, which indicate there is room to improve the safety of the medication system.

OBJECTIVE: to analyze the nursing research priorities in critical care in Brazil identified by specialists and researchers in the area, as well as to establish the consensus of the topics suggested by the experts. METHOD: a descriptive study, using the e-Delphi technique in three rounds. The research participants were 116 Brazilian nurses who are experts in critical care in the first round, ending up with 68 participants in the third round of the study. Descriptive statistics were used to analyze the demographic variables and the results of the research topics in the second round. In the final analysis, the Kappa agreement coefficient was calculated, comparing the answers between rounds two and three. RESULTS: 63 research topics were generated, grouped into 14 domains of intensive care practice in the first round, and consensus was settled in the subsequent rounds. Topics such as humanization of care (0.56), bloodstream infection control (0.54), and nursing care for polytrauma patients (0.51) were items rated above 0.50 in the agreement analysis between the topics in the two rounds using the Kappa coefficient. CONCLUSION: this study provides an important guideline for nursing research in critical care in Brazil, guiding for future research efforts in the area.

OBJECTIVES: To localize the textual interface of the SureWash Pocket® mobile health app and to present the methodological experience used in the localization process. METHOD: Software localization study to Brazilian Portuguese. The localization process was carried out between December 2018 and June 2019 in the city of Cuiabá/MT, through six steps: permission to localize the software; two independent translations; synthesis of translations; evaluation of the validity of the textual interface by a Committee of Expert Judges; back translation; and publication. RESULTS: The textual interfaces were organized into 107 items and analyzed by ten expert judges. In the first round, 64% of the items reached a pre-established minimum agreement level of 90%. During the second round, 39 remaining items were adapted according to consensual suggestions and the pre-final version was consolidated. CONCLUSION: The localization process suggests that the localized SureWash Pocket® is considered valid and can be used for an educational intervention on hand hygiene.

The leadership of the nurse in intensive care units faces challenges and paradoxes and is present in the planning of care, in the moments of ethical and bioethical conflicts, in the supervision of care, in capacity of making decisions quickly and safety, in knowing deal with the issues involving the process of living and dying.

RESUMO

We developed and evaluated the usability of Check-up Surgery, a surgery application from the mApp® Platform. The Usability Engineering Cycle was used as method of development. The results showed that the Check-up Surgery application had excellent usability criteria with an average of 4.63 to 4.70 by 81% of the evaluators. For all the interviewees there was statistical significance in relation to the usability criteria of the application, with pValue ≤.05.

Cerebrovascular and hypertensive diseases are among the leading causes of death in the world. This study was developed in two stages: the development of a dashboard with specific reports of cerebrovascular diseases and applied a preliminary predictive analysis model for cerebrovascular diseases. The results demonstrate the abitity to predict a citizen's chance acquiring a cerebrovascular disease.

ABSTRACT Objective: to demonstrate the application of some prompts and to problematize the use of ChatGPT® to guide the best answers for nursing students and teachers on septic shock in intensive care learning. Method: a methodological study where prompt technology was applied in ChatGPT® to support nursing learning in intensive care with an emphasis on septic shock. The study was organized in 3 stages, covering an understanding of ChatGPT® and models, as well as testing and exercising prompts. Results: applications of prompts were presented, based on a structure of pre-defined stages that made it possible to exemplify the answers given and to organize an output generation diagram as a way of summarizing the process of decision support in intensive care. Conclusion: ChatGPT® is a natural language processing model that uses deep learning approaches to generate human-like answers. However, the generation of prompts for the teaching-learning process in intensive care nursing requires in-depth association with the pillars of evidence-based practice.


RESUMEN Objetivo: demostrar la aplicación de algunos prompts y debatir cómo se utiliza ChatGPT® para orientar las mejores respuestas a estudiantes y profesores de Enfermería sobre shock séptico en la enseñanza de cuidados intensivos. Método: estudio metodológico en el que se aplicó la tecnología de prompts en ChatGPT® para respaldar la enseñanza de Enfermería en cuidados intensivos con énfasis en shock séptico. El estudio se organizó en 3 etapas, contemplando lo que se sabe sobre ChatGPT® y diversos modelos, al igual que pruebas y prácticas con prompts. Resultados: Se presentaron aplicaciones de prompts a partir de una estructura de etapas predefinidas que permitieron ejemplificar respuestas dadas por el sistema y organizar un diagrama de generación de salidas como una manera de resumir el proceso de apoyo a las decisiones que se toman en cuidados intensivos. Conclusión: ChatGPT® es un modelo de procesamiento del lenguaje natural que utiliza enfoques de aprendizaje profundo para generar respuestas semejantes a las humanas. Sin embargo, la generación de los prompts para el proceso de enseñanza-aprendizaje en Enfermería de cuidados intensivos requiere una profunda asociación con los pilares de la práctica basada en evidencias.


RESUMO Objetivo: Demostrar a aplicação de alguns prompts e problematizar o uso do ChatGPT® para guiar as melhores respostas aos estudantes e professores de enfermagem sobre choque séptico na aprendizagem em terapia intensiva. Método: Estudo metodológico, com aplicação de tecnologia de prompts no ChatGPT® para apoiar a aprendizagem de enfermagem em terapia intensiva com ênfase no choque séptico. O estudo foi organizado em 3 etapas, contemplando o entendimento sobre o ChatGPT®, modelos, bem como teste e exercício de prompts. Resultados: Foram apresentadas aplicações de prompts a partir de uma estrutura de etapas pré-definidas que permitiram exemplificar respostas dadas e organizar um diagrama de geração de saídas como uma forma de resumir o processo de apoio à tomada de decisão em terapia intensiva. Conclusão: O ChatGPT® é um modelo de processamento de linguagem natural que usa abordagens de aprendizagem profunda para gerar respostas semelhantes às humanas. Contudo, a geração dos prompts para o processo de ensino-aprendizagem em enfermagem de terapia intensiva requer associação profunda com os pilares da prática baseada em evidência.

Resumo Objetivo Descrever o processo de desenvolvimento e validação de um software web de apoio à gestão da assistência pré-natal. Métodos Pesquisa de produção tecnológica, realizada entre março e agosto de 2022, com base em princípios e técnicas do método Scrum. As funcionalidades do software web, com acesso e funções específicas a três diferentes usuários (Administrador, Gestor e Visitante), foram desenvolvidas de forma colegiada entre desenvolvedores, pesquisadores, gestores e profissionais da saúde, a partir de necessidades práticas relacionadas à gestão da assistência pré-natal. Resultados O software web é uma ferramenta de apoio à gestão da assistência pré-natal, com potencial para gerar infográficos que denotem intercorrências gestacionais e que permitem, de forma ágil e segura, à tomada de decisões estratégicas loco-regionais. Conclusão O processo de desenvolvimento e validação do software web de apoio à gestão da assistência pré-natal deu-se de forma colegiada entre desenvolvedores, pesquisadores, gestores e profissionais da saúde, a partir de necessidades práticas relacionadas à gestão da assistência pré-natal. A ferramenta foi desenvolvida de modo a possibilitar acesso e funções específicas a três diferentes usuários (Administrador, Gestor e Visitante), com vistas à tomada de decisões rápidas e seguras.


Resumen Objetivo Describir el proceso de desarrollo y validación de un software web de apoyo a la gestión de la atención prenatal. Métodos Investigación de producción tecnológica, realizada entre marzo y agosto de 2022, con base en principios y técnicas de la metodología Scrum. Las funcionalidades del software web, con acceso y funciones específicas para tres usuarios (administrador, gestor y visitante), se desarrollaron de forma colegiada entre desarrolladores, investigadores, gestores y profesionales de la salud, a partir de necesidades prácticas relacionadas con la gestión de la atención prenatal. Resultados El software web es una herramienta de apoyo a la gestión de la atención prenatal, con potencial para generar infográficos que indiquen complicaciones gestacionales y que permitan tomar decisiones estratégicas locorregionales de forma ágil y segura. Conclusión El proceso de desarrollo y validación del software web de apoyo a la gestión de la atención prenatal se realizó de forma colegiada entre desarrolladores, investigadores, gestores y profesionales de la salud, a partir de necesidades prácticas relacionadas con la gestión de la atención prenatal. La herramienta se desarrolló a fin de permitir el acceso y funciones específicas para tres diferentes usuarios (administrador, gestor y visitante), con el objetivo de tomar decisiones rápidas y seguras.


Abstract Objective To describe the development and validation process of a web software to support the management of prenatal care. Methods Technological development research, carried out between March and August 2022, based on principles and techniques of the Scrum method. Descriptive and exploratory study, aimed to develop an educational practice of Problem-Based Learning in CPR/BLS with 24 students in the third stage of the Nursing Undergraduate Course in a University in the Southern region of Brazil. The study used the PBL methodology, focused on problem situations of cardiopulmonary arrest, and was approved by the CONEP. The methodological strategies for data collection, such as participative observation and questionnaires to evaluate the learning, the educational practices and their methodology, allowed for grouping the results in: students' expectations; group activities; individual activities; practical activities; evaluation of the meetings and their methodology. The study showed that PBL allows the educator to evaluate the academic learning process in several dimensions, functioning as a motivating factor for both the educator and the student, because it allows the theoretical-practical integration in an integrated learning process.

This research-action type study aimed to establish clinical evaluation parameters essential to the patient subject to hemodynamic monitoring by Pulmonary Artery Catheter (PAC) and to develop a Nursing Care Protocol to critical care patients using catheter with the nurses of an ICU in Santa Catarina State. The population consisted of a non-probabilistic intentional sample of five nurses, and the data were collected through group meetings and questionnaire. The study is presented with descriptive statistics and qualitative analyses of the subjective questions. In conclusion, the clinical evaluation criteria, essential to the patient subjected to hemodynamic monitoring, are constituted by invasive and non-invasive parameters and the protocol grounds making clinical decisions when caring for patient using PAC.

Resumo Objetivo identificar as contribuições de aplicativos móveis para o design ou condução de simulação clínica de alta fidelidade em enfermagem, a fim de promover e facilitar o uso da simulação como estratégia educacional. Métodos revisão integrativa de estudos científicos publicados nas bases de dados indexadas BDENF, CINAHL, LILACS, MEDLINE/PubMed, Scopus, Web of Science e bibliotecas Cochrane e SciELO, guiada pelo Preferred Reporting Items for Systematic Reviews and Meta-Analyses . Foram incluídos estudos em inglês, português ou espanhol, disponíveis na íntegra e sem restrições de tempo. A busca resultou em 320 estudos e, após revisão dos textos completos e análise dos dados, foram selecionados quatro estudos que atenderam ao objetivo da pesquisa. Resultados foram encontrados quatro aplicativos móveis que podem contribuir com atividades de simulação clínica de alta fidelidade em enfermagem, porém um ainda está em desenvolvimento. Dois aplicativos estão disponíveis para acesso público, sendo que apenas um foi submetido a testes de validade, confiabilidade e usabilidade no estudo. Os resultados mostraram que três aplicativos podem contribuir para a facilitação da simulação e apenas um contribui para o design da simulação. Conclusão aprimorar o desenvolvimento de uma estratégia de simulação requer ferramentas práticas e de fácil acesso para auxiliar o corpo docente. Assim, a pesquisa permitiu a identificação de contribuições de aplicativos móveis para o design de simulação e facilitação, mas o número limitado de estudos encontrados e a falta de descrição de rigor metodológico e testes de avaliação no processo de desenvolvimento indicam a lacuna de aplicativos móveis disponíveis e baseados em evidências que contribuam diretamente com as necessidades dos provedores de simulação, o que poderia orientá-los a desenvolver uma experiência de aprendizado eficaz.

Resultados Se encontraron cuatro aplicaciones móviles que pueden contribuir con actividades de simulación clínica de alta fidelidad en enfermería, pero una todavía está siendo elaborada. Dos aplicaciones están disponibles para acceso público, de las cuales solo una fue sometida a pruebas de validez, fiabilidad y usabilidad en el estudio. Los resultados mostraron que tres aplicaciones pueden contribuir para facilitar la simulación y solo una contribuye al diseño de la simulación. Conclusión Mejorar el desarrollo de una estrategia de simulación requiere herramientas prácticas y de fácil acceso para ayudar al cuerpo docente. De esta forma, este estudio permitió identificar las contribuciones de aplicaciones móviles para el diseño de simulación y facilitación, pero el número limitado de estudios encontrados y la falta de descripción de rigor metodológico y pruebas de evaluación en el proceso de elaboración indican el vacío de aplicaciones móviles disponibles y basadas en evidencias que puedan contribuir directamente con las necesidades de proveedores de simulación, o que puedan orientarlos a desarrollar una experiencia eficaz de aprendizaje.


Abstract Objective To identify the contributions of mobile applications to the design or conduct of high-fidelity clinical simulation in Nursing in order to promote and facilitate the use of simulation as an educational strategy. Methods Integrative review of scientific studies published in indexed databases: BDENF, CINAHL, LILACS, MEDLINE/PubMed, Scopus, Web of Science, and Cochrane and SciELO libraries, guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Studies in English, Portuguese or Spanish were included, available in full and without time restrictions. The search yielded 320 studies and, after reviewing full texts and analyzed data, four studies were selected that met the research objective. Results Four mobile applications were found that can contribute to high-fidelity clinical simulation activities in Nursing, however one is still under development. Two applications are available for public access, and only one was submitted to validity, reliability and usability tests in the study. The findings showed that three applications can contribute to simulation facilitation, and only one contributes to the simulation design. Conclusion Improving the development of a simulation strategy requires practical and easily accessible tools to assist the faculty. Thus, the research allowed the identification of mobile applications contributions to simulation design and facilitation, however, the limited number of studies found and the lack of methodological rigor description and evaluation tests in the development process, indicates the gap of available and evidence-based mobile applications that directly contribute to the needs of simulation providers, which could guide them to develop an effective learning experience.
