OBJECTIVE: The treatment of congenitally missing maxillary lateral incisors is often complex and requires a multidisciplinary approach, which demands continuous collaboration among the different specialists involved in the rehabilitation. In young patients, implant therapy or the preparation of intact teeth for fixed prostheses is not indicated. CLINICAL CONSIDERATIONS: Two complex cases of double maxillary canine inclusion were treated with combined surgical, orthodontic, and conservative prosthetic treatments. Orthodontic therapy allowed the creation of the necessary interocclusal space to fabricate the all-ceramic resin-bonded fixed partial dentures without the preparation of the abutment teeth. CONCLUSIONS: Carefully planned and meticulously executed orthodontic and prosthetic therapies allowed for a noninvasive rehabilitation of patients with maxillary lateral incisor agenesis. CLINICAL SIGNIFICANCE: This article provides clinicians with a noninvasive solution for agenesis of lateral incisors associated with canine inclusions. Indications for the choice of material of all-ceramic resin-bonded fixed partial dentures are discussed and presented.

OBJECTIVE: The treatment of a diffuse and severe case of amelogenesis imperfecta represents a challenge for the dental team and in particular for the dentist who perform the prosthetic restorations. CLINICAL CONSIDERATIONS: The enamel alterations, which are characteristic of the disease, determine different problems that need to be carefully planned and solved before the beginning of the treatment, with particular regard to the materials used to perform the restorations. In order to create the best conditions to perform the prosthetic restoration, the treatment plan should be multidisciplinary. In addition, the young age of the patients usually affected by the disease imposes a very conservative and cautious approach. The main goals of the therapy are represented by the need to protect the occlusal dental surfaces from the abrasion and provide a satisfactory esthetic result. In particular, dental abrasions if not treated may determine the loss of the vertical dimension as well as a diffuse and marked dentinal hypersensitivity. CONCLUSIONS: A correct treatment plan and the use of the new adhesive ceramic materials made it possible to obtain good results both from a functional and esthetic point of view with a much more conservative approach compared with the traditional one. CLINICAL SIGNIFICANCE: This clinical report describes the prosthetic rehabilitation of an adult female patient with a diffuse amelogenesis imperfecta using feldspathic ceramics for the esthetic regions and monolithic lithium disilicate material for the posterior areas.

The treatment plan represents the final step in every diagnostic procedure and is the result of a series of assessments based on information gathered from a detailed clinical history. This clinical case reports the replacement of two central incisors that were lost because of a trauma. The advantages and disadvantages of the two treatment options (i.e. implantology or prosthetic restoration) were carefully evaluated in relation to the case in hand. Patient compliance and aesthetic requirements had been also considered. In this case, a conventional prosthetic restoration was performed using a metal-ceramic bridge, utilising full crowns between the right and left maxillary lateral incisors associated with minimal canine preparation. By this strategy, the treatment plan allows for a future implant restoration, should this become necessary.

Delayed replantation of an avulsed tooth may result in rapid root resorption or, more frequently, dental ankylosis with subsequent bone substitution. If this process develop slowly, it is possible to observe that tooth loss is characterized by a well conserved alveolus with regard to bone preservation, particularly in vertical dimension. This clinical case reports a dental trauma of a central incisor in a young boy characterized by tooth avulsion and its delayed replantation. After 10 years, dental ankylosis of the incisor was recorded and the patient underwent a prosthetic-orthodontic rehabilitation using CAD-CAM technology and no-prep veneers.

PURPOSE: Bisphosphonates are a class of drugs with several uses. Primarily, they are used to treat bone metastases, multiple myeloma, Paget's disease, and osteoporosis. However, the use of bisphosphonates can lead to osteonecrosis of the jaws. Here, we present our experience treating this widely discussed disease. PATIENT AND METHODS: We describe a patient who underwent surgical reconstruction of the mandible with a fibula free flap after total mandibulectomy, performed to treat osteonecrosis induced by the use of intravenous bisphosphonates complicated by a mandibular fracture. Endosseous implants were inserted simultaneously to optimize the length and outcome of surgery. RESULTS: Osteonecrosis of the jaws is a treatment challenge, and several approaches have been suggested. In this patient, the use of a fibula free flap to reconstruct the mandible was a winning strategy. The patient is now free from disease, and the esthetic results were optimal. CONCLUSIONS: With the simultaneous placement of endosseous dental implants, the functional outcome was optimal, with no significant complications or recurrence.

RESUMO

The introduction of fiber posts has improved the treatment of endodontically treated teeth, increasing retention, and distributing the stress along the root in order to reduce the risk of fracture. This clinical case describes the use of posts during the prosthetic rehabilitation of severely compromised teeth in the anterior segment.

RESUMO

Dental agenesis is the most commonly encountered dental anomaly in humans. Oligodontia, however, is a rare condition that involves the congenital absence of six or more teeth, excluding the third molars. Treatment of oligodontia requires an interdisciplinary approach. The prosthetic treatment plan should carefully consider esthetic and functional rehabilitation but should take a conservative approach. Adhesive techniques, combined with the new ceramic materials, permit functional and esthetic prosthetic restorations that are more conservative in comparison to those used in the past. Ultrathin occlusal veneers without tooth preparations may represent a good esthetic and conservative approach for oral rehabilitation of patients affected by severe hypodontia.

RESUMO

Removal of third molars is one of the most common operations performed in oral surgery. A well recognized serious complication of mandibular third molar extraction is injury to the inferior alveolar nerve (IAN). We describe a case of an unusual nerve passage discovered after the extraction of a completely erupted third molar. The likelihood of direct damage to the IAN can be predicted with more specific information obtained by the use of a preoperative computerized tomography scan when conventional radiography does not clearly show the nerve canal.
