RESUMO

Detecting cardiac sarcoidosis; a potentially life-threatening condition is challenging and requires a multimodality imaging approach using echocardiography, PET/CT and CMR. Although 18F-FDG is the recommended PET tracer for evaluating cardiac sarcoidosis, it is limited by physiological cardiac FDG uptake and requires stringent patient preparation/ dietary modifications before imaging. We hereby present a case of cardiac sarcoidosis demonstrating myocardial FAPI uptake on cardiac PET, highlighting the potential role of 68Ga-FAPI PET in the evaluation of cardiac sarcoidosis.

RESUMO

ABSTRACT: Prostate cancer is the fifth leading cause of death in the male population worldwide. 68 Ga-PSMA PET/CT has proved to be an excellent modality with greater accuracy for nodal and bone/visceral metastases staging than bone scintigraphy and CT scan, with high sensitivity and specificity. Common sites of metastasis include bone (84%), lymph nodes (10.6%), liver (10.2%), lung, and pleura (9.1%); however, metastasis to the skin is quite rare (≤0.36%). The present case demonstrates PSMA-avid perineal metastasis in a patient of prostate cancer postchemoradiotherapy on 68 Ga-PSMA PET/CT scan.

RESUMO

ABSTRACT: Uterine fibroids are benign tumors originating from the smooth muscle cells of the myometrium seen in approximately 20%-50% of women of reproductive age. The Arg-Gly-Asp (RGD) binds to αvß3 integrin expressed on the surface of angiogenic blood vessels or tumor cells. 18 F-FDG PET/CT has been used to evaluate uterine fibroids, with moderate 18 F-FDG uptake. However, angiogenesis imaging in uterine fibroids has not been evaluated. The present case presents a rare finding of RGD uptake in the uterine fibroid on 68 Ga-DOTA-RGD-2 PET/CT in a patient who underwent angiogenesis imaging for left ankle joint pain and swelling.

RESUMO

ABSTRACT: Prostate-specific membrane antigen PET imaging has revolutionized the role of prostate cancer diagnosis and management, with very high sensitivity and specificity. To prevent misdiagnosis and for accurate therapy planning, prostate-specific membrane antigen (PSMA) uptake in nonprostatic diseases needs to be recognized correctly. Besides the physiological PSMA expression, 68 Ga-PSMA-11 uptake has been mentioned in multiple oncological and nononcological lesions. The present case report exhibits 68 Ga-PSMA-11 uptake in fibroadenoma in a male accessory breast in the right axillary region.

RESUMO

Neuroendocrine tumors (NETs) up to 80% may have metastatic disease to lymph nodes, liver, and bones upon diagnosis due to their indolent course and benign nature. However, metastasis to the breast from gastropancreatic-neuroendocrine tumors (GEP-NETs) is unusual and rarely reported. Furthermore, such metastases may mimic a primary breast carcinoma clinically and radiologically. This case report illustrates an unusual presentation of metastasis to the right breast in addition to liver, pancreas, and lymph nodal metastases in a patient with ileal NET who was operated upon 5 years back. The metastases were detected by somatostatin receptor-based imaging and post-therapy scan which was confirmed by cytology and immunocytochemistry.

RESUMO

ABSTRACT: Neuroendocrine tumors (NETs) are rare tumors associated with the overexpression of somatostatin receptors owing to their origin from neural crest cells. The somatostatin receptor-based molecular imaging of NETs with 68Ga-DOTANOC is extensively used to diagnose primary and metastatic disease with high diagnostic accuracy. Unlike conventional octreotide imaging, physiological gallbladder uptake is not seen on 68Ga-DOTANOC PET/CT imaging. The present case report exhibits the rare physiological 68Ga-DOTANOC uptake in the gall bladder and bile duct.

RESUMO

Mantle cell lymphoma (MCL) is a rare subtype of B-cell non-Hodgkin's lymphoma with an annual incidence of 5%, common in men with a median age of 60-70 years. Renal involvement is seen in about 0.7% of the extranodal lymphomas. The bilateral presentation is seen in 10%-20% of cases. There have been many case reports of MCL with renal involvement; however, primary renal involvement by MCL is extremely rare. We hereby describe the 18F-fluorodeoxyglucose positron emission tomography/computed tomographic findings in primary MCL involving the bilateral kidneys.

RESUMO

OBJECTIVE: To determine the frequency of medication errors by caregivers at home in neonates discharged from the neonatal intensive care unit and to identify the associated risk factors. METHODS: A descriptive, cross-sectional study was conducted in the High Risk Newborn Follow-up Clinic of our institute, on a sample of 166 children, <3 months old. The medications prescribed (syrup preparations of vitamin D, multivitamins, calcium, iron and levetiracetam, tablet L-thyroxine and ursodeoxycholic acid and human milk fortifier powder) were noted from the discharge summary. The caregiver who usually administered the medicines to the child at home was asked the names of the medications, frequency of their administration and to show in a measuring cup/syringe/dropper the dose of the medication. The names, doses and frequency of the drugs as reported were matched against those actually prescribed in the discharge summary. Various risk factors probably associated with medication errors, were noted. RESULTS: The frequency of medication errors by caregivers in infants discharged from the neonatal intensive care unit was 66.3%. Dose administration error, that is, measurement of a dose different from what was prescribed was the most common error (54%). A prescription containing more than three drugs was found to have statistically significant association (OR 4.19, CI 1.59 to 11.07, p=0.00). CONCLUSION: Medication errors by caregivers in infants less than 3 months of age are very common, dose administration error being the most common type. A prescription of more than three drugs increases the odds of an error.
