RESUMO

PURPOSE: Low physical activity in the academic workplace may increase the risk of cardiometabolic disease. This randomised controlled trial investigated the effect of 14 weeks of concurrent exercise training (CT) on components of metabolic syndrome, body composition, insulin resistance, and markers of systemic inflammation in inactive academics. METHODS: 59 inactive academics were randomised into a CT (n = 29) or wait-list control group (n = 30). CT performed supervised training at an onsite facility 3 times per week for 14 weeks and cardiometabolic health was assessed pre- and post-intervention. Aerobic capacity was measured via a metabolic cart. Dual-Energy X-ray Absorptiometry measured fat mass, lean mass, and central adiposity. Fasting blood samples were analysed for interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), Homeostatic Model Assessment for Insulin Resistance (HOMA-IR), glucose, and lipid profile. RESULTS: Following the intervention, there was a decrease in fat mass (mean ± SD; - 1.3 ± 1.4%), android fat mass (median (IQR); - 0.06 (0.27) kg), and visceral adipose tissue (median (IQR); - 66 (110) cm3) in CT, but not control. Lean mass (median (IQR); 1.35 (1.86) kg) and aerobic capacity (mean ± SD; 4.0 ± 3.1 mL/kg/min) increased in CT, but not in control. There were no changes in IL-6, TNF-a, HOMA-IR, glucose, or lipid profile in response to the intervention (P > 0.05). Changes in insulin resistance were positively associated with IL-6 in the control group only (coefficients [95%CI]; 5.957 [2.961, 8.953]). CONCLUSION: Implementing combined aerobic and resistance exercise training programs in academic institutions may be an appropriate intervention to increase physical activity and reduce risk factors associated with cardiometabolic disease. TRIAL REGISTRATION: The study was registered with the Australian New Zealand Clinical Trials Registry on the 23rd of April, 2019 (ACTRN12619000608167).

ISSUE ADDRESSED: Firefighting is physically and mentally taxing and recruits are expected to have optimal health and fitness. However, physical fitness tends to decline following initial training, placing firefighters at an increased risk for stress and injury. Efforts are needed to engage and support firefighters in maintaining adequate health and fitness to withstand the rigorous demands of their occupation. This study examined the feasibility of TARP, a pragmatic strength and conditioning intervention for metropolitan-based firefighters, delivered in collaboration with a professional National Rugby League club. METHODS: A mixed-methods approach was utilised to examine program implementation, recruitment and sample characteristics, intervention satisfaction and acceptability, and participants' response to the intervention. Evaluation measures included field notes taken during steering committee meetings, participant flow data, baseline and follow-up outcome measures, self-report questionnaires, and telephone interviews with a sample of participants. RESULTS: Participants (N = 113) were predominantly men (82%) with a mean age of 43 ± 9.3 years and BMI of 26.6 ± 2.9 kg/m2 . Program satisfaction was high (95% very satisfied or somewhat satisfied) among program completers (42% retention). Key strengths of the program included delivery through the professional sports club, quality of facilities and equipment, and scheduling flexibility. Future programs should consider incorporating education or training to support behaviour change maintenance and strategies to retain participants at follow-up. CONCLUSIONS: Results provide valuable insights into the design and delivery of interventions for firefighters and demonstrate the importance of strong partnerships between community stakeholders.

OBJECTIVES: Due to the nature of firefighting, most effective cooling interventions to reduce heat strain and optimise performance are not practically viable. This study quantified the effects of two practical cooling strategies, co-designed with subject-matter experts, on physiological strain and physical, perceptual, and visuo-motor performance during simulated firefighting in the heat. DESIGN: Randomised cross-over. METHODS: On three occasions 14 firefighters completed an 80-min simulation in a hot-humid environment (32.0[0.9]°C, 59[3]%RH) including two 20-min firefighting tasks in full protective clothing, each followed by 20-min seated recovery. Recovery involved removal of protective clothing and one of three interventions - control (CON; ambient-temperature water consumption), basic (BASIC; cool-water consumption, ambient-forearm immersion/towels, fan), and advanced (ADV; ice-slushy consumption, cool-forearm immersion/ice packs, misting-fan). Thermal (core temperature) and cardiovascular (heart rate, arterial pressure) responses were measured throughout, whilst physical (handgrip/balance), visuo-motor (reaction time/memory recall) and perceptual (fatigue/thermal sensation/comfort) measures were assessed pre- and post-trial. RESULTS: Compared to CON, core temperature was lower in BASIC and ADV following the second task (ADV: 37.7[0.4]; BASIC: 38.0[0.4]; CON: 38.3[0.4]°C) and recovery protocol (ADV: 37.5[0.3]; BASIC: 37.7 [0.3] CON: 38.3[0.4]°C). This was paralleled by lowered heart rate, rate pressure product, and thermal sensation following the recovery protocols, in the ADV and BASIC condition compared to CON (p < .05). No physical or visuo-motor outcomes differed significantly between conditions. CONCLUSION: Whilst these observations need to be extended to field conditions, our findings demonstrate that two novel cooling interventions developed in collaboration with subject-matter experts offered benefits for reducing thermal strain and optimising firefighter safety.

OBJECTIVES: This study examined how track cycling coaches, practitioners, and athletes: develop knowledge and practices; value performance areas; and, implement research into practice. DESIGN: Cross-sectional survey. METHODS: An online REDCap survey of track cycling coaches, practitioners, and athletes was conducted involving questions related to demographics, performance area importance, knowledge acquisition and application, research relevance, and research direction. RESULTS: A total of 159 responses were received from coaches (n = 55), practitioners (n = 29), and athletes (n = 75). Participants' highest track cycling competition level involvement ranged from local/regional (12.7%) to Olympic/Paralympic (39.9%). Respondents primarily develop practices by observing 'the sport' or 'others competing/working in it' (both 85.8%). Practitioners develop practices through self-guided learning (96.4%). The primary reason for practice use was prior experience (84.9%), whilst individuals were least likely to use practices resulting in marginal gains with potentially negative outcomes (27.3%). Areas of greatest perceived importance were Aerodynamics, Strength & Conditioning, and Tactics (all >96% agreed/strongly agreed). Scientific evidence for Tactics (30%) and Mental Skills (26%) was perceived to be lacking, resulting in greater reliance on personal experience (74% and 62%, respectively) to inform training decisions. The main barrier to implementing research into practice was athlete buy-in (84.3%). CONCLUSIONS: Within track cycling, informal learning was most popular amongst respondents. Greater reliance on personal experience within evidence-based practice for many performance areas aligns with limited existing research. Most respondents reported multiple barriers affecting research implementation in practice.

BACKGROUND: Track cyclists must develop mental, physical, tactical and technical capabilities to achieve success at an elite level. Given the importance of these components in determining performance, it is of interest to understand the volume of evidence to support implementation in practice by coaches, practitioners, and athletes. OBJECTIVE: The aim of this study was to conduct a systematic mapping review to describe the current scale and density of research for testing, training and optimising performance in track cycling. METHODS: All publications involving track cyclist participants were reviewed from four databases (PubMed, SPORTDiscus, Academic Search Complete, Cochrane Library) plus additional sources. Search results returned 4019 records, of which 71 met the inclusion criteria for the review. RESULTS: The review revealed most published track cycling research investigated athlete testing followed by performance optimisation, with training being the least addressed domain. Research on the physical components of track cycling has been published far more frequently than for tactical or technical components, and only one study was published on the mental components of track cycling. No true experimental research using track cyclists has been published, with 51 non-experimental and 20 quasi-experimental study designs. CONCLUSIONS: Research in track cycling has been growing steadily. However, it is evident there is a clear preference toward understanding the physical-rather than mental, tactical, or technical-demands of track cycling. Future research should investigate how this aligns with coach, practitioner, and athlete needs for achieving track cycling success. Heat is a major stressor during exercise, though its value in driving adaptation is not well understood. Muscle heating can upregulate pathways facilitating protein synthesis and could thereby enhance effects of exercise training, however, few studies have investigated this possibility. We examined whether heating active muscle during resistance training differentially affected physical and functional adaptations. Within a randomised contralateral-limb control study, ten healthy, resistance-untrained individuals (21 ± 3 y; 5 female) completed 30 sessions of progressive resistance training (12 weeks), performing 4 × 8 unilateral knee extensions at 70% of 1RM. One randomly-allocated thigh was heated during, and for 20 min after, each session using an electric pad eliciting muscle temperatures of >38 °C (HOT); the contralateral limb remained unheated (CON). Training intensity was progressed using 4-weekly strength assessments. Quadricep lean mass (measured using DXA) increased by 15 ± 7% in HOT (p = 0.00) and 15 ± 6% in CON (p = 0.00); the difference being trivial (p = 0.94). Peak isokinetic torque at 90°.s-1 increased by 30 ± 25% (HOT; p = 0.00) and 34 ± 33% (CON; p = 0.01), with no difference (p = 0.84) between limbs. Rate of torque development increased ∼40%, with no difference between limbs (p = 0.73). The increase in 3-RM strength was also similar in HOT (75 ± 16%) and CON (71 ± 14%; p = 0.80 for difference). No differences in mass or strength changes were evident between sexes. In conclusion, supplemental heating of active muscle during and after each bout of resistance training showed no clear positive (or negative) effect on training-induced hypertrophy or function.
