RESUMO

PURPOSE: To investigate the efficacy of pattern visual evoked potentials (VEPs) in evaluating objective visual acuity (VA) and discriminating malingerers. METHODS: Two hundred and forty-nine eyes of 249 patients aged 20-65 years were included. There were 147 eyes with macular diseases (group 1) and 102 eyes with optic nerve diseases (group 2). Amplitudes and latencies were analyzed and correlated with best-corrected visual acuity by a regression analysis. We found the best-correlated mode of pattern VEP, determined the relations, and then calculated the pattern VEP-estimated VA (PVEP-VA) of all 249 eyes, another 30 malingering eyes, 13 eyes with macular diseases, and 17 eyes with optic nerve diseases, and used a receiver operating characteristic (ROC) curve to determine a cutoff for acceptable variance between PVEP-VA and subjective VA to discriminate malingerers. RESULTS: The best correlation was between the amplitude of 50' checkerboard size (Amp50') and VA in every group. Significant correlation was between Amp50' and VA, where p < 0.0001 in group 1 and p = 0.020 in group 2. A logarithmic curve best fitted the correlation in the regression analysis, where y = 1.731 - 1.569x (R(2) = 0.611, p < 0.0001) in group 1 and y = 2.413 - 2.169x (R(2) = 0.531, p < 0.0001) in group 2 [x: log(Amp50'), y: PVEP-VA (logMAR)]. By using the relations and ROC curve, we determined a variance value of 0.4041 (logMAR) with 100% sensitivity and 94.0% specificity in group 1 and 0.3658 with 70.6% sensitivity and 50.5% specificity in group 2 to discriminate malingerers. CONCLUSIONS: The pattern VEP amplitude of 50' checkerboard size was useful to assess VA and can be helpful in discriminating malingering from real disability.

PURPOSE: To evaluate the refractive development of premature infants with retinopathy of prematurity (ROP) after treatment with laser photocoagulation or intravitreal injection of bevacizumab (IVB). METHODS: The medical records of patients with ROP treated between 2003 and 2012 who underwent yearly follow-ups were retrospectively reviewed. Patients with residual ROP abnormalities were excluded. The cycloplegic refraction at 3 years of age, assessed using an autorefractometer, was recorded. RESULTS: In total, 54 eyes from 54 patients were enrolled. Patients were divided into 4 groups: group 1, including 14 eyes of 14 patients treated with laser therapy; group 2, 15 eyes of 15 patients treated with IVB; group 3, 13 eyes of 13 patients with non-type 1 ROP under conservative follow-up, and group 4, 12 eyes of 12 premature patients without ROP. The mean spherical equivalent at 3 years of age was -1.71 ± 1.27 dpt in group 1, -1.53 ± 2.20 dpt in group 2, 0.63 ± 1.37 dpt in group 3, and 0.41 ± 1.95 dpt in group 4. The mean refractive error differed significantly among the 4 groups (p < 0.001). Patients in groups 1 and 2 were more prone to myopia compared with those in groups 3 and 4. Furthermore, patients with type 1 ROP treated by laser photocoagulation (group 1) and those treated by IVB (group 2) had similar refraction (p = 1). CONCLUSIONS: The results of this study suggest that treatment-demanding ROP eyes are susceptible to more severe myopia with age compared with eyes without ROP or those with spontaneously regressed ROP. In addition, the myopic status between laser and IVB treatment did not differ statistically.

Herein, we report a case of bilateral simultaneous central retinal vein occlusion (CRVO) secondary to coronavirus disease 2019 (COVID-19). A 48-year-old man, with hypertension, type 2 diabetes mellitus, and stage 4 chronic kidney disease, diagnosed with COVID-19 1 month ago presented to the ophthalmology department with blurred vision in both eyes for 2 weeks. Ocular examination revealed a classic clinical presentation of CRVO in both eyes. Optical coherence tomography revealed increased central macular thickness with intraretinal and subretinal fluid in both eyes. Laboratory data revealed elevated D-dimer and C-reactive protein (CRP) levels. The levels of other hypercoagulability markers were normal. The patient received intravitreal anti-vascular endothelial growth factor therapy in both eyes, followed by regular follow-up every month until complete resolution of symptoms and gradual improvement of the retinal vascular appearance. COVID-19 can cause a variety of coagulation abnormalities and thromboembolic complications such as bilateral simultaneous CRVO. Clinicians and patients should be aware of ocular symptoms and presentations that are probably associated with COVID-19.

OBJECTIVE: To evaluate the role of cytomegaloviral or Pneumocystis jiroveci pneumonia (CMV/PJP) in systemic lupus erythematosus (SLE) patients with pulmonary hemorrhage (PH). METHODS: We retrospectively examined hospital records for 27 SLE patients with PH who received bronchoalveolar lavage fluid (BALF) analyses. Clinical profile and mortality rates were compared between groups with and without CMV/PJP. Risk factors for PH-related mortality were analyzed. RESULTS: Among 27 SLE patients with PH, 15 had pathogens from BALF samples, and 8 had CMV/PJP. Although CMV/PJP was treated, the RR for 90- and 180-day mortality rates of SLE patients with CMV/PJP were higher than those without these infections (5.94, 95% CI 1.44-24.48; 7.13, 95% CI 1.81-28.06, respectively). Risk factors for 90- and 180-day mortality were presence of CMV/PJP (OR 14.2, 95% CI 1.83-109.9; OR 25.5, 95% CI 2.91-223.3, respectively) and use of pulse methylprednisolone for PH treatment (OR 12.0, 95% CI 1.48-97.2; OR 8.5, 95% CI 1.13-63.9, respectively). Factors increasing the 90-day mortality rate were duration of mechanical ventilation exceeding 14 days (OR 11.1, 95% CI 1.11-112.0) and use of aggressive immunosuppression close to PH onset (OR 7.56, 95% CI 1.09-52.4). Three of the 7 patients receiving aggressive immunosuppression died with the presence of CMV/PJP. CONCLUSION: Owing to the high prevalence of CMV/PJP and its association with mortality, routine BALF analysis is recommended for all suitable SLE patients with PH. Use of aggressive immunosuppression does not benefit SLE patients with opportunistic infections during PH attack.

RESUMO

We report a case of ocular injury after bungee jumping. A 29-year-old woman presented with bilateral subconjunctival hemorrhage and periorbital petechiae after bungee jumping. Her best-corrected visual acuity was 20/20 in both eyes. Ocular examinations revealed no other intraocular hemorrhages. She received conservative treatment. One week later, the subconjunctival hemorrhage and periorbital petechiae resolved spontaneously. Her vision remained stable, and no detectable ocular complications were found during the follow-up. In conclusion, bungee jumping can cause ocular injuries. Further studies are required to identify their causes and potential risk factors.

PURPOSE: We report a case of rapid cataract progression after Nd:YAG vitreolysis for vitreous floaters. CASE REPORT: A 55-year-old man presented with acute onset of blurred vision following Nd:YAG vitreolysis for symptomatic floaters in the left eye. His initial best corrected visual acuity (BCVA) was 20/1,000 in the left eye. Ocular examinations showed frost-like opacities of the lens and a suspected break of the posterior capsule in the left eye. There were no detectable retinal lesions. Cataract surgery was then arranged. Posterior capsular rupture and vitreous loss occurred during surgery, which required a subsequent pars plana vitrectomy. After the surgery, BCVA in the left eye gradually improved to 20/20 and was maintained during a 1-year follow-up period. CONCLUSION: Crystalline lens injuries and rapid cataract progression may occur following Nd:YAG vitreolysis. While dealing with this type of complicated cataract, clinicians should be aware of the possibility of posterior lens capsule rupture during surgery and the need for combined vitrectomy.
