RESUMO

Oral vancomycin is utilized in the treatment of severe Clostridium difficile infection (CDI). We prospectively measured serum vancomycin concentrations (SVC) in patients treated with oral vancomycin. The SVC was measured by immunoassay prior to, and at least 3 days after, the administration of oral vancomycin 125 mg every 6 h. Patients treated with intravenous vancomycin were excluded. Fifty-seven patients with a mean age of 74 y (± 18) were enrolled. There was no detectable SVC in 56 patients (98%); 1 patient had a transient SVC of 6.7 µg/ml that was not detectable on subsequent testing. The severity of the CDI and/or renal failure did not have an effect on SVC. Orally administered vancomycin at 125 mg 4 times daily was not absorbed from the gastrointestinal tract.

RESUMO

Angioedema is a well-demarcated localized edema involving the deeper layers of the skin, subcutaneous tissue, mucosa, and submucosa. It is a well-recognized side effect of angiotensin-converting enzyme (ACE) inhibitor therapy. Prolonged angioedema is rare. In most cases, the angioedema is self-limited and responds to cessation of ACE inhibitor therapy. ACE inhibitor-related angioedema occurs more commonly in African American patients and tends to be particularly severe in elderly African American women. We report 2 cases of extremely severe angioedema of prolonged duration that required tracheostomy to stabilize the airway.

A 53-year-old dialysis patient was admitted with symptoms of a respiratory tract infection, abdominal pain and vomiting. She aspirated and required intubation. A nasogastric tube was placed with slight difficulty and the patient developed severe epistaxis. The bleeding could not be controlled with mechanical pressure and nasal packing. Angiography revealed extravasation from a pseudoaneurysm arising from the inferior pharyngeal branch of the ascending pharyngeal trunk. The vessel was successfully embolised with cessation of bleeding. We emphasise that even a seemingly easy procedure like insertion of a nasogastric tube, can lead to a life-threatening complication.

Glycosylated haemoglobin (HbA1c) is a measurement commonly performed in patients with diabetes. Factors causing a change in the life span of the red blood cell (RBC) can affect the measurement of HbA1c. Thus haemolysis is an important factor that may affect the HbA1c level determination. Haemolysis has been shown to cause a falsely low HbA1c. A 62-year-old man with a history of autoimmune haemolytic anaemia was admitted for severe haemolytic anaemia and an Hb of 2.9 g/dL. HbA1c tested during hospitalisation was unrecordable due to the extremely low Hb. The patient was treated with intravenous steroids, immunoglobulin, fluids and RBC transfusions but continued to haemolyse and eventually expired. We emphasise that an extremely low HbA1c level can serve as a marker of haemolysis and an unrecordable HbA1c level may point towards fatal haemolysis.

RESUMO

The Platypnea-Orthodeoxia syndrome is characterised by dyspnoea and deoxygenation accompanying a change from the recumbent to the upright position. An 81-year-old woman had an elective paraesophageal hernia repair. She developed dyspnoea and hypoxemia post-operatively that was worse when upright. An agitated saline echocardiogram revealed a right-to-left shunt through a patent foramen ovale that increased when the patient was upright. Over 3 weeks the patients' shunt, dyspnoea and hypoxemia improved and she was discharged home.

RESUMO

ß-blocker and calcium channel blocker toxicity generally present with bradycardia and hypotension. A 69-year-old woman presented after a suicide attempt with a ß-blocker and calcium channel blocker overdose. Her blood pressure was 69/35 mm Hg and her HR was in the 40s. She was treated with calcium chloride, glucagon, a dextrose-insulin infusion and three vasopressors, but remained hypotensive. She suffered two cardiac arrests and required a transvenous pacemaker. When all interventions failed, she was started on a methylene blue infusion for refractory vasodilatory shock which resulted in a dramatic improvement in her blood pressure. The patient was successfully weaned off all vasopressors and from mechanical ventilation without any end-organ damage.

RESUMO

Aspirin is one of the most commonly used medications. We report a patient who presented with severe weakness, altered mental status and complete heart block requiring temporary pacing. Despite the patient's family denying that the patient used aspirin, an arterial blood gas that revealed a respiratory alkalosis and metabolic acidosis suggested the diagnosis of salicylate toxicity. The salicylate level was extremely elevated and the patient was successfully treated with haemodialysis. Our case illustrates that salicylate toxicity should be considered in a patient with a combined metabolic acidosis and respiratory alkalosis. A prompt salicylate level should be obtained. This is also the first case of salicylate toxicity causing complete heart block in an adult. The heart block resolved with treatment of the salicylate toxicity.

A 20-year-old woman admitted with an aspirin overdose was encephalopathic and was intubated for airway protection. Under sonographic guidance, a right internal jugular central venous catheter was inserted for volume resuscitation. She was agitated and required treatment with a propofol infusion. Her salicylate levels improved and she was successfully extubated 12 h after intubation. One day later she developed severe dyspnoea and a right-sided pleural effusion and required immediate reintubation. The effusion was drained and was consistent with a chylothorax. A CT scan of the chest revealed that the central venous catheter traversed the mediastinum with its tip in the right pleural space. The propofol infusion was draining into the pleural space. A video-assisted thoracoscopic surgery was performed to drain the pleural effusion and repair the punctured vessel. She was extubated within a day and discharged home 1 day later without any pleural fluid.

RESUMO

Pulmonary artery (PA) catheterisation has been performed for over four decades. It is utilised in intensive care units and perioperatively to titrate fluid and pressor therapy. Major and minor complications have been reported. The catheter can infrequently loop and coil in the right atrium, right ventricle and superior vena cava .We report a case of coiling of the PA catheter in the main PA. A 33-year-old woman was admitted with multiorgan failure and shock. She required mechanical ventilation, was in severe pulmonary oedema and required vasopressors for shock. We placed a PA catheter to assist in the management of her haemodynamic collapse. The catheter coiled in the main pulmonary trunk, which is extremely rare. The catheter was removed without any complications.

An 85-year-old woman with failure to thrive due to poor oral intake was admitted owing to dehydration. A nasogastric (NG) tube was inserted for the initiation of enteral feedings. The tube position was confirmed by gastric auscultation after insufflating air through the tube. A chest X-ray revealed that the NG tube traversed the right main stem bronchus with its tip ending in the right costophrenic angle adjacent to the pleura. No pneumothorax was identified. The tube was removed and a short while later the patient developed mild chest discomfort. A repeat chest X-ray revealed significant pneumothorax on the right side. She was treated conservatively with 100% oxygen with successful resolution of the pneumothorax.

Mixed dust pneumoconiosis secondary to domestic wood smoke exposure is a cause of pneumoconiosis in women from developing countries, but is rarely seen in the USA. An elderly female non-smoker, who immigrated to the USA from Pakistan 10 years previously, presented with a worsening non-productive cough and dyspnoea on exertion. She did not have any occupational or environmental exposures other than utilising firewood for cooking while living in Pakistan. Radiographs revealed multiple bilateral pulmonary nodules associated with hilar and mediastinal adenopathy. A video-assisted thoracoscopic biopsy revealed ill-defined nodules in a perivascular subpleural deposition, carbon pigment deposition around the terminal bronchioles, dust macules and negatively birefringent needles on polarised light microscopy with mixed dust nodules outnumbering the silicotic nodules consistent with mixed dust pneumoconiosis. This case illustrates the need for awareness of this condition among physicians caring for women who lived in areas where biomass exposure is common.
