BACKGROUND: Current approaches to profile the single-cell transcriptomics of human pancreatic endocrine cells almost exclusively rely on freshly isolated islets. However, human islets are limited in availability. Furthermore, the extensive processing steps during islet isolation and subsequent single cell dissolution might alter gene expressions. In this work, we report the development of a single-nucleus RNA sequencing (snRNA-seq) approach with targeted islet cell enrichment for endocrine-population focused transcriptomic profiling using frozen archival pancreatic tissues without islet isolation. RESULTS: We cross-compared five nuclei isolation protocols and selected the citric acid method as the best strategy to isolate nuclei with high RNA integrity and low cytoplasmic contamination from frozen archival human pancreata. We innovated fluorescence-activated nuclei sorting based on the positive signal of NKX2-2 antibody to enrich nuclei of the endocrine population from the entire nuclei pool of the pancreas. Our sample preparation procedure generated high-quality single-nucleus gene-expression libraries while preserving the endocrine population diversity. In comparison with single-cell RNA sequencing (scRNA-seq) library generated with live cells from freshly isolated human islets, the snRNA-seq library displayed comparable endocrine cellular composition and cell type signature gene expression. However, between these two types of libraries, differential enrichments of transcripts belonging to different functional classes could be observed. CONCLUSIONS: Our work fills a technological gap and helps to unleash frozen archival pancreatic tissues for molecular profiling targeting the endocrine population. This study opens doors to retrospective mappings of endocrine cell dynamics in pancreatic tissues of complex histopathology. We expect that our protocol is applicable to enrich nuclei for transcriptomics studies from various populations in different types of frozen archival tissues.

The pancreatic islet is the functional and structural unit of the pancreatic endocrine portion. Islet remodeling occurs in both normal development and pathogenesis of type 1 (T1D) and type 2 diabetes (T2D). However, accurately quantifying changes in islet cellular makeup and hormone expressions poses significant challenges due to large intra- and inter-donor heterogeneity and the limited scalability of traditional methods such as immunostaining. The cytometry by time-of-flight (CyTOF) technology enables simultaneous quantification of more than 30 protein markers at single-cell resolution in a high-throughput fashion. Moreover, with distinct DNA and viability markers, single live cells can be explicitly selected in CyTOF. Here, leveraging the CyTOF data generated by the Human Pancreas Analysis Program, we characterized more than 12 million islet cells from 71 donors. Our data revealed continued age-related changes in islet endocrine cell compositions, but the maturity of endocrine cells is reached by 3 years of age. We also observed significant changes in beta cell numbers and key protein expressions, along with a significant increase in bihormonal cells in T1D donors. In contrast, T2D donors exhibited minimal islet remodeling events. Our data shine a light on the islet dynamics during development and diabetes pathogenesis and suggest divergent pathogenesis processes of T1D and T2D. Our comprehensive approach not only elucidates islet plasticity but also establishes a foundation for integrated CyTOF analysis in islet biology and beyond.

In many cell types, disparate non-centrosomal microtubule-organizing centers (ncMTOCs) replace functional centrosomes and serve the unique needs of the cell types in which they are formed. In Drosophila fat body cells (adipocytes), an ncMTOC is organized on the nuclear surface. This perinuclear ncMTOC is anchored by Msp300, encoded by one of two Nesprin-encoding genes in Drosophila. Msp300 and the spectraplakin short stop (shot) are co-dependent for localization to the nuclear envelope to generate the ncMTOC where they recruit the microtubule minus-end stabilizer Patronin (CAMSAP). The fat body perinuclear ncMTOC requires Patronin, Ninein, and Msps (ortholog of ch-TOG), but does not require γ-tubulin for MT assembly. The Msp300 gene is complex, encoding at least eleven isoforms. Here we show that two Msp300 isoforms, Msp300-PE and -PG, are required and only one, Msp300-PE, appears sufficient for generation of the ncMTOC. Loss of Msp300-PE,-PG retains the presence of the other isoforms at the nuclear surface, indicating that they are not sufficient to generate the ncMTOC. Loss of Msp300-PE,-PG results in severe loss of localization of shot and Patronin, and disruption of the MT array. This results in nuclear mispositioning and loss of endosomal trafficking. Msp300-PE has an unusual domain structure including a lack of a KASH domain and very few spectrin repeats and appears therefore to have a highly derived function suited to generating an ncMTOC on the nuclear surface.

Functional autonomy (FA) is a critical factor in determining the quality of life of older adults (OA), especially in the case of older women (OW), as they face a decline in FA in their later years of life. FA should be assessed early, using valid, reliable, and low-cost tests. This study evaluated the test-retest reliability of GDLAM and GDLAM autonomy index (GI) in OW. Thirty-nine OW (71.2 ± 6.50 years) participated in the study. A repeated measures design was used to compare the interday test-retest reliability of the five GDLAM tests (seconds) and the GI (points). The five tests represent activities of daily living, such as dressing or wandering around the house, while the GI provides a weighting of the results of the five tests. The analysis consisted of the intraclass correlation coefficient (ICC), standard error of measurement (SEM), and coefficient of variation (CV). A CV ≤ 10% and an ICC ≥ 0.80 were considered acceptable reliability, whereas a CV ≤ 5% and an ICC ≥ 0.90 were considered high reliability. The outcome of the five tests, represented by the GI, showed high interday test-retest reliability (CV = 6.00% and ICC = 0.91). The results of this study demonstrate that the five tests of the GDLAM protocol and the GI have high interday test-retest reliability and good interday reproducibility. From a practical point of view, the GDLAM protocol allows the assessment of FA of community-dwelling OW, providing background for early diagnosis and, with it, the possibility of developing an individualized physical exercise prescription.

Dicrocoeliosis is a parasitic disease that mainly affects ruminants during grazing, caused by trematodes of the genus Dicrocoelium, with D. dendriticum being the most common species worldwide. This parasitosis is a chronic and generally subclinical process, with nonspecific signs, which makes its diagnosis challenging. This study aimed to determine the prevalence and seasonal dynamics of D. dendriticum infection in adult sheep from the Valencian Community, eastern Spain, as well as to evaluate the efficacy of flotation and sedimentation techniques when compared with the macroscopic exam of the liver. From February to May 2018, 2019, 2020, 2021, and 2022, a total of 290 adult sheep were examined. The animals were sourced from Castellón province, where a semi-intensive production system predominates. Each animal was euthanized and underwent a macroscopic examination of the liver, as well as a fecal analysis. Among the sampled animals, 117 (40.6%) tested positive for adult trematodes in their liver, while 87 (30%) showed evidence of trematode eggs in the coprological exam, reaching a total of 126 sheep parasitized, with a prevalence of 43.4%. The parasitic burden was established in 90.3 adults per animal when the liver was examined and in 54.5 eggs per gram when the sedimentation coprological exam was performed. No eggs were observed when the flotation technique was employed. A positive correlation was found between the number of adults in the liver and the fecal egg count. No significant differences were detected in the prevalence or parasitic burden throughout the study. Considering the difficulty in controlling the intermediate hosts and the complexity of the life cycle, effective diagnostic methods, combined with the adoption of other preventive measures, is crucial to achieving proper management of this parasitic disease.

It is estimated that approximately one in three women develop chronic venous disease (CVD) during pregnancy, a broad spectrum of morphofunctional disorders affecting the venous system in different regions of the body, including the lower limbs. A growing body of evidence supports the diverse maternofetal consequences derived from this condition, with the placenta being an organ particularly affected. Among other consequences, having CVD during pregnancy has been associated with systemic inflammation and altered cytokines and chemokine profiles in the maternal and fetal serum related to this condition. In the present work, we aimed to analyze if these inflammatory changes also occurred in the placental tissue of women with CVD, exploring by immunohistochemistry and real-time PCR (RT-qPCR) gene and protein expression of critical inflammatory markers like allograft inflammatory factor 1 (AIF-1), interleukin 10 (IL-10), IL-12A, and IL-18. Our results demonstrate an enhanced tissue expression of AIF-1, IL-12A, and IL-18, accompanied by a decrease in IL-10 in the placentas of women who had undergone CVD during pregnancy. Overall, our results suggest a possible pathophysiological role of inflammation in the placental tissue of women with CVD during pregnancy, although the precise consequences of this feature remain to be deeply analyzed.

Introducción: Las expectativas académicas representan aquello que los estudiantes esperan alcanzar durante su formación y son un factor predictivo del desempeño académico y social. Estas expectativas académicas son moldeadas por los padres, luego la educación de estos es una variable importante en el desarrollo académico de los hijos. Si bien los progenitores proyectan sus anhelos sobre sus hijos, los motivan a tener altos niveles de aspiración y son los responsables de crear ambientes intelectualmente más estimulantes; el objetivo de este estudio fue identificar las diferencias en las expectativas académicas de una muestra de estudiantes, según el nivel de escolaridad de sus padres. Método: Esta investigación contó con la participación de 261 estudiantes colombianos universitarios quienes están cursando su primer año de pregrado, entre los 16 y 54 años (media=21,9). Para este estudio se utilizó el Cuestionario de percepciones académicas-expectativas (CPA-E; Deaño et al., 2015); se obtuvieron los puntajes de cada participante, junto con sus datos sociodemográficos que estipulaban el nivel de escolaridad de los padres, y se llevaron a cabo los análisis descriptivos de cada grupo, y los análisis de varianza unidireccional (ANOVA). Resultados: Las expectativas académicas de los estudiantes con ambos padres sin educación superior (media = 189.176), se encuentran por debajo de la media, siendo significativamente diferentes a los puntajes de expectativas académicas de individuos con uno (media = 202.050) o ambos padres con estudios superiores (media = 204.286, p < 0,001). Discusión: Se demuestra la relevancia y necesidad del acceso a la educación para la mayoría de la población colombiana, entendiendo que este es un factor influyente en las expectativas académicas de una persona, y que debe tenerse un acompañamiento diferencial a los estudiantes con padres sin formación profesional.


Introduction: Academic expectations represent what students hope to achieve during their training and are a predictor of academic and social performance. These academic expectations are shaped by the parents, therefore their education is an important variable in the academic development of the children. Although parents project their aspirations on their children, motivate them to have high levels of aspiration and are responsible for creating more intellectually stimulating environments, the objective of this study was to identify the differences in the academic expectations of a sample of students, according to the level of education of their parents. Method: This research had the participation of 261 Colombian university students who are studying their first year of undergraduate, between the ages of 16 and 54 (mean = 21,9). For this study, the Academic Perceptions-Expectations Questionnaire (CPA-E; Deaño et al., 2015), was used, the scores of each participant were obtained, together with their sociodemographic data stipulating the parents' level of education, and descriptive data and one-way variance (ANOVA) analyzes were done. Results: The academic expectations of students with both parents without higher education (mean= 189.176), are below the average, being significantly different from the academic expectations scores of individuals with one (mean= 202.050) or both parents with higher education (mean= 204.286, p < 0,001). Discussion: The relevance and need of access to education for most of the Colombian population is demonstrated, understanding that this is an influential factor in the academic expectations of a person, and that there must be differential support for students with parents without training professional.

Introducción: Las alteraciones oftalmológicas asociadas al SARS-CoV-2 se pueden clasificar como efectos directos del virus, por reacción inmunológica o por la vacunación. Objetivo: Describir las alteraciones oftalmológicas en pacientes con COVID-19. Métodos: Se presenta una serie de casos con diagnóstico clínico y serológico de COVID-19 en el curso de la enfermedad, posterior a esta o por la vacunación. Fueron estudiados 7 casos (4 mujeres y 3 hombres), que acudieron de forma consecutiva a la consulta de Neuroftalmología del Hospital "Hermanos Ameijeiras" (HHA), remitidos por especialistas que evalúan el protocolo de casos "Pos-COVID-19" en esta institución con las siguientes alteraciones: neurorretinitis (NR), papilitis (P), uveítis y papilitis (U-P), membrana epirretiniana (MER) y parálisis facial periférica (PFP), en el periodo de agosto a noviembre de 2021. Resultados: La edad promedio fue de 44 años. La NR, P y MER unilateral predominaron en la serie estudiada. La etiología inmunológica fue la más frecuente. Los síntomas y signos referidos por los pacientes fueron: disminución visual y déficit en la visión de los colores. Los resultados terapéuticos no fueron satisfactorios en los casos 1 y 6, con un tiempo de evolución prolongado, desde el inicio de los síntomas hasta el diagnóstico y tratamiento con esteroides y vitaminas del complejo B. Conclusiones: Existe una asociación entre las enfermedades oftalmológicas encontradas en pacientes jóvenes con antecedentes de padecer la COVID-19 o relacionado con la vacunación. La efectividad terapéutica estuvo condicionada por el tiempo de evolución de la enfermedad(AU)


Introduction: Ophthalmologic alterations associated with SARS-CoV-2 can be classified as direct effects of the virus, by immunologic reaction or by vaccination. Objective: To describe the ophthalmologic alterations in patients with COVID-19. Methods: A series of cases with clinical and serologic diagnosis of COVID-19 during the course of the disease, after the disease or after vaccination is presented. Seven cases were studied (4 women and 3 men), who consecutively attended the Neurophthalmology Consultation of the "Hermanos Ameijeiras" Hospital (HHA), referred by specialists who evaluate the "Post-COVID-19" case protocol in this institution with the following alterations: neuroretinitis (NR), papillitis (P), uveitis and papillitis (U-P), epiretinal membrane (MER) and peripheral facial palsy (PFP), in the period from August to November 2021. Results: The average age was 44 years. NR, P and unilateral MER predominated in the series studied. Immunologic etiology was the most frequent. The symptoms and signs referred by the patients were visual impairment and color vision deficit. Therapeutic results were not satisfactory in cases 1 and 6, with a prolonged evolution time, from the onset of symptoms to diagnosis and treatment with steroids and B-complex vitamins. Conclusions: There is an association between the ophthalmologic diseases found in young patients with a history of suffering from COVID-19 or related to vaccination. Therapeutic effectiveness was conditioned by the time of evolution of the disease(AU)

RESUMEN Objetivo: Evaluar los valores de referencia de la dinámica circulatoria arterial ocular de los pacientes con hipertensión arterial esencial. Métodos: Se realizó un estudio descriptivo y transversal en 105 pacientes hipertensos y en un grupo de 33 sujetos no hipertensos, con edades comprendidas entre 18 y 60 años, sin antecedentes de padecer diabetes mellitus ni enfermedades oculares como glaucoma, o haber recibido tratamiento quirúrgico por catarata, hipertensión ocular u otras. A todos se les realizó la toma de la presión arterial sistémica, el examen clínico oftalmológico y el ultrasonido Doppler a color de carótida y de los vasos orbitarios. Resultados: Se encontró un predominio de mujeres de piel blanca, entre la cuarta y quinta década de la vida. Existió un incremento del pico de velocidad sistólica, la velocidad final diastólica y el índice de resistencia en la arteria oftálmica, que fue desde un rango normal en el grupo de los no hipertensos a valores promedios elevados en el grupo de hipertensos, los cuales fueron más altos en los casos descontrolados. No se encontraron modificaciones en el análisis de estos parámetros en las arterias centrales de la retina ni en las ciliares posteriores cortas. Conclusiones: En la casuística estudiada, el incremento del pico de la velocidad sistólica en la arteria oftálmica pudiera estar relacionado con áreas de obstrucción vascular localizadas o con vasoespasmo. Se encontró una asociación entre el descontrol de la presión arterial y los valores elevados del índice de resistencia en la arteria oftálmica(AU)


ABSTRACT Objective: Evaluate the reference values for ocular arterial circulation dynamics in patients with essential arterial hypertension. Methods: A descriptive cross-sectional study was conducted of 105 hypertensive patients and a group of 33 non-hypertensive subjects aged 18-60 years with no antecedents of diabetes mellitus or ocular conditions such as glaucoma or having undergone cataract surgery, ocular hypertension or others. All the patients underwent systemic arterial pressure measurement, clinical ophthalmological examination and color Doppler carotid and orbital ultrasonography. Results: A predominance was observed of the female sex, white skin color and age between the fourth and fifth decades of life. There was an increase in peak systolic velocity, end diastolic velocity and the resistive index in the ophthalmic artery, which ranged from normal in the non-hypertensive group to high average levels in the hypertensive group, higher in uncontrolled cases. Analysis of these parameters did not find any change in central retinal or short posterior ciliary arteries. Conclusions: In the cases studied, the peak systolic velocity increase in the ophthalmic artery could be related to localized vascular obstruction areas or vasospasm. An association was found between uncontrolled arterial pressure and high resistive index values in the ophthalmic artery(AU)

Abstract INTRODUCTION: Benznidazole is used for treating Chagas disease (CD). This cross-sectional study aimed to characterize the adverse drug reactions (ADRs) of benznidazole at a public hospital in Brazil's Federal District. METHODS: Medical records were analyzed and ADRs were categorized by type, intensity, seriousness, and causality. RESULTS: Of the 62 patients who started benznidazole treatment for CD, 41 (66%) presented with 105 ADRs; 23 (37%) discontinued the treatment. Most reactions were classified as probable (81%), severe (63%), serious (67%), and dose-dependent (56%). CONCLUSIONS: The high incidence of ADRs because of treatment withdrawal revealed the need for safer alternatives for CD treatment.

Resumen: Introducción: la hipertensión arterial (HTA) es uno de los factores de riesgo cardiovascular más prevalentes en el mundo. La detección temprana, el tratamiento apropiado y su control determinan importantes beneficios en materia de salud y economía. Dada la alta incidencia de consultas por complicaciones vinculadas a esta enfermedad, se crea en el año 2014 la policlínica de HTA en el Hospital Maciel, realizándose en ella tratamiento integral de pacientes hipertensos, fundamentalmente de aquellos de difícil control. Objetivo: caracterizar la población de hipertensos que asisten a esta policlínica, así como mostrar los resultados de su implementación respecto al control de las cifras tensionales. Material y método: estudio descriptivo, longitudinal, retrospectivo y prospectivo entre los años 2015 y 2017 que incluyó a los pacientes que se asisten en dicha policlínica, excluyendo a embarazadas, menores de 18 años, pacientes que no se controlaron en el último año y fallecidos. Resultados: se analizó un total de 122 pacientes, media de edad 59,8 años, predominio sexo femenino (1,8:1). Factores de riesgo cardiovascular asociados: sobrepeso/obesidad (88%), dislipemia (65%), tabaquismo (39%) y diabetes (29%). Prevalencia de daño de órgano blanco en el momento de ser derivados, 62%. El 98% recibe tratamiento farmacológico: 71% terapia combinada. Predomina la HTA grado III (68,9%). Conclusión: la población se caracteriza por una elevada incidencia de sobrepeso/obesidad, HTA grado III, y alta prevalencia de daño de órgano blanco en el momento de la consulta, lo cual conduce a pensar que los pacientes se derivan tardíamente. En el subgrupo de hipertensos grado III se logró un adecuado control promedio de cifras tensionales en el seguimiento anual.


Summary: Introduction: high blood pressure is one of the most prevalent cardiovascular risk factors in the world. Early detection, appropriate treatment and control determine important health and economic benefits. Given the high incidence of consultations due to complications of this disease, in 2014 the hypertension polyclinic was created in the Maciel Hospital, in which evaluation and treatment of the hypertensive patient were carried out in an integral way, fundamentally for those with difficult control. Objective: to characterize the population of hypertensive patients attending this polyclinic, as well as to show the results of its implementation regarding the figures control. Materials and methods: descriptive, longitudinal, retro and prospective study between 2015-2017 that includes patients who are attended by polyclinic, excludes pregnant women, young persons under 18 years old, those who were not controlled in the last year and deceased. Results: 122 patients were analysed, average age 59,8 years old, female sex 1.8:1. Associated cardiovascular risk factors: overweight/obesity (88%), dyslipidemia (65%), smoking (39%) and diabetes (29%). Prevalence of white organ damage 62% at the time of referral. 98% receive pharmacological treatment: 71% combined therapy. Hypertension grade III predominates (68,9%). Conclusion: the population is characterized by a high incidence of overweight/obesity, hypertension grade III as well as high prevalence of white organ damage at the time of consultation, which leads to think that the patients are derived late. In the subgroup of hypertensives grade III adequate average control of blood pressure figures was achieved in the annual follow-up.


Resumo: Introdução: a hipertensão arterial é um dos fatores de risco cardiovascular mais prevalentes no mundo. A detecção precoce, o tratamento e o controle apropriados determinam importantes benefícios econômicos e para a saúde. Dada a alta incidência de consultas relacionadas a complicações desta doença, foi criado em 2014 a policlínica de hipertensão em Hospital Maciel, realizando avaliação e tratamento de pacientes hipertensos de forma holística, principalmente daqueles de difícil controle. Objetivo: caracterizar a população de hipertensos atendidos nesta policlínica, bem como mostrar os resultados de sua implementação no controle de figuras. Materiais e métodos: estudo descritivo, longitudinal, retro e prospectivo, entre os anos de 2015 e 2017, que incluiu os pacientes que compareceram à policlínica, excluindo gestantes, menores de 18 anos, pacientes que não foram controlados no último ano e falecidos. Resultados: foram analisados 122 pacientes, idade média de 59,8 anos, sexo feminino 1,8: 1. Fatores de risco cardiovascular associados: sobrepeso/obesidade (88%), dislipidemia (65%), tabagismo (39%) e diabetes (29%). Prevalência de lesão de órgão branco 62% no momento do encaminhamento. 98% recebem tratamento farmacológico: 71% terapia combinada. Hipertensão grau III predomina (68,9%). Conclusão: a população é caracterizada por uma alta incidência de sobrepeso / obesidade, hipertensão grau III, bem como a alta prevalência de lesão de órgão branco no momento da consulta, o que leva a pensar que os pacientes são derivados tardiamente. No subgrupo de hipertensos grau III, o controle médio adequado dos valores da pressão arterial foi alcançado no seguimento anual.

RESUMEN Objetivo: Evaluar la eficacia de la técnica quirúrgica Supramáxima en la corrección de la ptosis palpebral superior moderada y grave. Métodos: Se realizó un ensayo clínico aleatorizado a simple ciega, en 95 pacientes (119 ojos), provenientes de la Consulta de Oftalmología del Hospital Hermanos Ameijeiras, desde febrero del año 2016 a noviembre de 2017, los cuales recibieron tratamiento quirúrgico con la técnica Supramáxima o la técnica Suspensión al frontal. La eficacia del tratamiento se evaluó en cuanto a: caída del párpado, altura del pliegue palpebral, distancia margen reflejo y satisfacción del paciente. Resultados: El promedio de edad fue de 59,35 años, con leve predominio del sexo femenino (56,8 por ciento) y de la ptosis palpebral grave (51,6 por ciento). La diabetes mellitus fue la enfermedad sistémica más frecuente (56,8 por ciento). La eficacia terapéutica de la TSM fue superior (96,7 por ciento) a la TSF (86,2 por ciento). Se constató una mayor satisfacción del paciente con la TSM (97,9 por ciento). Conclusiones: La técnica quirúrgica Supramáxima es más eficaz que la suspensión al frontal en la corrección de la ptosis palpebral superior moderada y grave(AU)


ABSTRACT Objective: Evaluate the efficacy of the Supramaximal surgical technique for correction of moderate and severe upper eyelid ptosis. Methods: A randomized single-blind trial was conducted with 95 patients (119 eyes) from the Ophthalmology Service of Hermanos Ameijeiras Hospital from February 2016 to November 2017. These patients had undergone surgery by either Supramaximal technique or frontal suspension technique. Efficacy of the treatment was evaluated in terms of eyelid ptosis, eyelid crease height, margin-reflex distance and patient satisfaction. Results: Mean age was 59.35 years, with a slight predominance of the female sex (56.8 percent) and of severe eyelid ptosis (51.6 percent). Diabetes mellitus was the most common systemic condition (56.8 percent). The therapeutic efficacy of SMT was higher (96.7 percent) than that of FST (86.2 percent). Patient satisfaction was higher with SMT (97.9 percent). Conclusions: The Supramaximal surgical technique is more effective than frontal suspension for correction of moderate and severe upper eyelid ptosis(AU)

ABSTRACT Patients with Bipolar Disorder (BD) usually display cognitive deficits with aging. However, the correlation between BD and dementia syndromes is inconclusive, despite the similarity with behavioral variant frontotemporal dementia. We report a 78-year-old female patient who had bipolar type 1 disorder since adolescence. Her symptoms ranged from apathy to psychotic mania. She had had three hospitalizations, and since her last stay 10 years ago, her symptoms had remained stable. However, in the past 2 years, she displayed different symptoms, such as irritability manifested as verbal and physical aggression, cognitive impairment, repetitive pattern of behavior, perambulation, persecutory delusions, disorientation, and hyporexia. Treatment with anticholinesterases or mood stabilizers promoted no improvement. She scored 17/30 points on the Mini-Mental State Examination. Neuropsychological assessment suggested deficits in executive function, attention, and memory. Neuroimaging tests revealed frontotemporal degeneration and hypoperfusion. Diagnostic and therapeutic approaches for this type of patient represent a significant challenge for clinicians.


RESUMO Pacientes com Transtorno Bipolar (TB) costumam apresentardéficits cognitivosao envelhecer. No entanto, a correlação com síndromes demenciais é inconclusiva, apesar da similaridade com a variante comportamental da demência frontotemporal (bvFTD). Nósrelatamos uma paciente de 78 anos de idade com TB tipo 1 desde a adolescência. Seus sintomas variavam de apatia a mania psicótica. A paciente passou por 3 internações, sendo a última há 10 anos, seguida de estabilização clínica. No entanto, nos últimos 2 anos, ela apresentou sintomas diferentes, como irritabilidade expressada por agressões verbal e física, comprometimento cognitivo, padrão repetitivo de comportamento, perambulação, delírios persecutórios, desorientação e hiporexia. O tratamento com anticolinesterásicos ou estabilizadores de humor não revelou melhora. Apresentou 17/30 pontos no miniexame do estado mental, a avaliação neuropsicológica sugeriu déficit de função executiva, atenção e memória. Os exames de neuroimagem demonstraram atrofia e hipoperfusão fronto-temporal. Abordagens diagnósticas e terapêuticas para este tipo de paciente representam um desafio significativo para os clínicos.

Resumen: Objetivos: evaluar la transferencia de adolescentes desde Cardiología Pediátrica (CP) a Cardiología de Adultos (CA) en el Hospital Dr. Hernán Henríquez de Temuco (HHHT), determinando si recibieron su primera atención en CA en la fecha requerida y si los controles se ajustaron al plazo indicado. Evaluar si la complejidad de la cardiopatía y ruralidad se asocian al cumplimiento en ambos objetivos, en los pacientes sin seguimiento adecuado. Material y Método: revisión de fichas de pacientes atendidos en CP del HHHT que, cumplida la edad requerida, fueron transferidos a CA, entre Octubre 2014 - Julio de 2017. Resultados: De 53 pacientes transferidos de CPCA, 12 (22.6%) tuvo traspaso no efectivo; de ellos, 6 presentaban complejidad moderada-alta (66,6%) y 3 eran rurales (25%). De 28 pacientes ingresados a CA, 14 (50%) tenían seguimiento inefectivo; de ellos 13 (92.6%) tenían cardiopatías de moderada-alta complejidad y 3 (21.5%) provenían de áreas rurales. Conclusiones: la transferencia de CP - CA fue inefectiva en 22.6% de los pacientes; la complejidad de la cardiopatía no se asoció al resultado de la transferencia en este grupo. En CA el seguimiento fue inefectivo en la mitad de los pacientes; de ellos, 92.8% tenían cardiopatías de moderada-alta complejidad, lo que fue estadísticamente significativo respecto de la complejidad en todos los pacientes traspasados. El domicilio no estuvo asociado a la pérdida de pacientes en la transferencia ni en el seguimiento. Estos hallazgos nos permitieron tomar medidas de mejoría en la transferencia y los controles en CA que esperamos mejoren los resultados futuros.


Abstracts: Aim: To evaluate the transfer of adolescents from Pediatric Cardiology (PC) to Adult Cardiology (AC) at Hospital Dr. Hernán Henríquez de Temuco (HHHT). We determined whether they got their first care in AC on the required date and whether controls were performed on the period prescribed. Also, to evaluate whether the complexity of cardiac disease and the patient's place of residence influenced the accomplishment of the objectives, in patients without adequate following. Material and Method: Retrospective review of clinical records of patients that were treated in PC at HHHT who, having the required age were transferred to AC, between October 2014 and July 2017. Results: Of 53 patients that were transferred from PC to AC, 12 (22.6%) had an non effective transfer; 6 of them had moderate to high complexity cardiac lesions (66.6%) and 3 lived in rural areas (25%). Of 28 patients admitted to AC, 14 (50%) had a non effective follow up and 13 (92.8%) had moderate to high complexity forms of heart disease; 3 (21.5%) came from rural areas. Conclusions: Transfer of patients from PC to AC was not effective in 22.6% of patients. The complexity of the disease was not related to the results of transfer in this group. At AC the follow-up was not effective in half the patients; from these, 92.6% had moderate to high complexity heart disease, which was significantly different from the complexity of all patients transferred. The place of residence was not relates to transfer neither to follow-up. These findings allowed us to make improvement in the transfer processes and the follow up at AC.

Introducción: la infección por Helicobacter pylori es muy común, particularmente en los países en vías de desarrollo, y juega un rol importante en la enfermedad ácido péptica. Objetivos: conocer la prevalencia y algunos datos clínico epidemiológicos, describir los hallazgos endoscópicos (gastritis eritematosa, nodular y úlcera duodenal), y relacionarlos con los hallazgos histológicos; describir la asociación entre los grados histológicos de la gastritis crónica y la positividad de la prueba de ureasa rápida y la presencia de Helicobacter pylori según histología, así como identificar la relación de los pacientes con gastritis crónica antral y úlcera duodenal por Hp, con la edad. Métodos: se realizó un estudio tipo descriptivo prospectivo durante 3 años se hizo una encuesta de síntomas a pacientes y/o padres y examen físico. Se realizó panendoscopia, previo consentimiento informado, y 2 biopsias de mucosa antral para prueba de ureasa rápida, así como estudio histopatológico (hematoxilina-eosina y Giemsa) para determinar el grado de gastritis y presencia de Helicobacter pylori según sistema Sydney. Se demostró la infección por uno de los métodos. Resultados: se seleccionaron 196 pacientes entre 7-18 años (media 14,6) de 471 (41,6 por ciento). El 53 por ciento era del sexo femenino, y el 59,7 por ciento tenía entre 10 y 14 años. Por endoscopia, el 49,5 por ciento tenía gastritis eritematosa antral, el 36,8 por ciento gastritis nodular astral, y el 13,7 por ciento úlcera duodenal con gastritis asociada. Con antecedentes familiares de úlcera péptica estuvo el 29,6 por ciento, con epigastralgia el 85,2 por ciento, con acidez el 46,4 por ciento con vómitos el 21 por ciento y con náuseas el 16,3 por ciento. En relación con el tiempo de evolución al diagnóstico, predominó el grupo de más de un año (24,5 por ciento), seguido del de 4-6 meses (22,4 por ciento). Todos presentaron gastritis crónica de diferentes grados: ligera (34,7 por ciento), moderada (37,2 por ciento) y severa (28,1 por ciento), con Helicobacter pylori por ureasa rápida 83,2 por ciento y 93,4 por ciento por histología, con coincidencia de ambos métodos en 150 (76,5 por ciento). Se observó presencia de nódulos linfoides en 41,8 por ciento. Conclusiones: la infección por Helicobacter pylori es frecuente en niños y adolescentes, causa lesiones inflamatorias gastroduodenales, en particular, gastritis nodular antral. Se encuentra una asociación importante del Helicobacter pylori por ambos métodos diagnósticos. Se recomienda seguimiento por biopsia, en especial, a los que continúan con infección crónica y presenten nódulos linfoides, por el riesgo de presentar linfoma gástrico de células B del tejido linfoide asociado a mucosa (linfoma MALT), si no se consigue la erradicación del Helicobacter pylori(AU)


Introduction: Helicobacter pylori infection is very common, particularly in developing countries, and plays an important role in acid peptic disease. Objectives: to know the prevalence and some clinical epidemiological data for describing the endoscopic findings (erythematous gastritis, nodular and duodenal ulcer), and to relate them with the histological findings; to describe the association among the histological degrees of chronic gastritis and the positivity of the rapid urease test, and the presence of Helicobacter pylori according to histology, as well as identifying by Hp with age the number of patients with chronic antral gastritis and duodenal ulcer. Methods: a prospective, descriptive study was carried out during 3 years, and a survey of symptoms was made to patients and / or parents and also physical examination. Panendoscopy was performed with prior informed consent, and also 2 antral mucosa biopsies for rapid urease test, as well as histopathological study (hematoxylin-eosin and Giemsa) to determine the degree of gastritis and the presence of Helicobacter pylori according to the Sydney system. Infection was demonstrated by one of the methods. Results: 196 patients among 7-18 years (mean 14.6) were selected of 471 (41.6 percent). 53 percent were female, and 59.7 percent were among 10 and 14 years old. By endoscopy, 49.5 percent had antral erythematous gastritis, 36.8 percent astral nodular gastritis, and 13.7 percent duodenal ulcer with associated gastritis. 29.6 percent had a family history of peptic ulcer, 85.2 percent of epigastralgia, 46.4 percent with acidity, 21 percent with vomiting, and 16.3 percent with nausea. In relation to the time of evolution to the diagnosis, the group of more than one year prevailed (24.5 percent followed by the one of 4-6 months (22.4 percent). All presented chronic gastritis of different degrees: light (34.7 percent), moderate (37.2 percent) and severe (28.1 percent), with Helicobacter pylori by rapid urease 83.2 percent, and 93.4 percent by histology with coincidence of both methods in 150 (76.5 percent). The presence of lymphoid nodules was observed in 41.8 percent. Conclusions: Helicobacter pylori infection is frequent in children and adolescents causing gastroduodenal inflammatory lesions, in particular antral nodular gastritis. An important association of Helicobacter pylori is found by both diagnostic methods. Follow-up by biopsy is recommended, especially those who continue with chronic infection and have lymphoid nodules due to the risk of presenting B-cell gastric lymphoma of the lymphoid tissue associated to the mucosa (MALT lymphoma), if Helicobacter pylori eradication is not achieved(AU)

Objetivo: evaluar los resultados terapéuticos obtenidos con la inyección de toxina botulínica en el músculo recto superior en pacientes con retracción palpebral moderada y grave en el curso de la orbitopatía tiroidea; determinar la influencia de algunos factores relacionados con estos e identificar las complicaciones y las reacciones adversas asociadas a su uso terapéutico. Métodos: se realizó un estudio descriptivo prospectivo en el Servicio de Oftalmología del Hospital Hermanos Ameijeiras en una serie de 21 casos con retracción palpebral moderada y grave en el curso de la orbitopatía tiroidea. La inyección de toxina botulínica en el músculo recto superior para la corrección de la retracción de la orbitopatía tiroidea se realiza por primera vez en el país. Resultados: fueron satisfactorios en el 66,7 por ciento de los casos. La edad y la gravedad clínica de la retracción fueron los factores que influyeron sobre la respuesta terapéutica (p= 0,013 y p= 0,015 respectivamente). Conclusiones: la inyección de toxina botulínica en el músculo recto superior es efectiva en la mayoría de los casos tratados, sobre todo en adultos jóvenes y con menor gravedad clínica de la retracción. La hipercorrección es la complicación más temida de este procedimiento(AU)


Objective: to evaluate the therapeutic results of the botulinum toxin injection in the upper rectus muscle in patients with moderate and severe eyelid retraction in the course of thyroid orbitopathy, and to determine the influence of some factors related to these patients and to identify the complications and adverse reactions associated to its therapeutic use. Methods: prospective and descriptive study was carried out at the ophthalmological service of Hermanos Ameijeiras hospital in a 21 case series study with moderate and severe eyelid retraction in the course of thyroid orbitopathy. The injection of botulinum toxin into the upper rectus muscle for the correction of retraction in thyroid orbitopathy was performed for the first time in the country. Results: in this group, 66.7 percent of patients had satisfactory results. Age and clinical severity of retraction were the factors having influence on the therapeutic response (p= 0.013 and p= 0.015 respectively). Conclusions: the botulinum toxin injection into the upper rectus muscle is effective in most of treated cases, mainly in young adults with less clinical retraction severity. Hypercorrection is the most fearful complication in this procedure(AU)

ABSTRACT Current dengue vector control strategies, focusing on reactive implementation of insecticide-based interventions in response to clinically apparent disease manifestations, tend to be inefficient, short-lived, and unsustainable within the worldwide epidemiological scenario of virus epidemic recrudescence. As a result of a series of expert meetings and deliberations, a paradigm shift is occurring and a new strategy, using risk stratification at the city level in order to concentrate proactive, sustained efforts in areas at high risk for transmission, has emerged. In this article, the authors 1) outline this targeted, proactive intervention strategy, within the context of dengue epidemiology, the dynamics of its transmission, and current Aedes control strategies, and 2) provide support from published literature for the need to empirically test its impact on dengue transmission as well as on the size of disease outbreaks. As chikungunya and Zika viruses continue to expand their range, the need for a science-based, proactive approach for control of urban Aedes spp. mosquitoes will become a central focus of integrated disease management planning.


RESUMEN Las estrategias actuales de control de vectores del dengue, centradas en la ejecución reactiva de intervenciones con insecticidas en respuesta a la aparición de cuadros clínicos evidentes de la enfermedad, suelen ser ineficientes, de duración limitada e insostenibles en el contexto epidemiológico mundial, caracterizado por la recrudescencia de las epidemias virales. Como resultado de una serie de reuniones y deliberaciones entre expertos, está en proceso un cambio de paradigma y ha surgido una nueva estrategia, que consiste en estratificar el riesgo de cada ciudad para concentrar y mantener los esfuerzos proactivos donde hay un alto riesgo de transmisión. En este artículo, los autores 1) describen esta estrategia de intervención específica y proactiva dentro del contexto de las características epidemiológicas del dengue, la dinámica de su transmisión y las estrategias actuales de control de Aedes y 2) fundamentan con fuentes bibliográficas la necesidad de demostrar empíricamente las repercusiones de esta estrategia sobre la transmisión del dengue y el tamaño de los brotes. Dado que los virus del chikunguña y el Zika siguen ampliando su alcance, uno de los objetivos primordiales de la planificación de la atención integrada de estas enfermedades estará determinado por la necesidad de adoptar un enfoque científico y proactivo del control urbano de los mosquitos del género Aedes.

El mucocele es una lesión quística benigna, que se desarrolla en el interior de los senos perinasales por la obstrucción de su drenaje natural en el curso de los procesos inflamatorios, traumas y cirugías. Se presentan tres casos con mucocele frontoetmoidal y expansión intraorbitaria, los cuales fueron atendidos en el Hospital Hermanos Ameijeiras entre los meses de febrero a diciembre del año 2013. El propósito del presente estudio es el análisis de los resultados terapéuticos obtenidos y determinar la posible influencia de la vía de abordaje utilizada sobre la efectividad terapéutica y la recidiva tumoral en cada uno de estos pacientes(AU)


Mucocele is a benign cystic lesion that emerges inside the perinasal sinuses due to the obstruction of their natural drainage in inflammatory processes, traumas and surgeries. Here are three cases with frontoethmoidal mucocele and intraorbital expansion, which were treated at Hermanos Ameijeiras hospital from February to December, 2013. The objective of this study was to analyze the therapeutic results and to determine the possible influence of the approach path on the therapeutic effectiveness and the tumor recurrence in each of the patients(AU)
