AIMS: To investigate the effects of arbuscular mycorrhizal fungi (AMF) species on the absorption and distribution of mineral nutrients in soybean cultivars under drought stress, an experiment was carried out through a factorial method in the form of randomized complete blocks with six replicates in 2020. METHODS AND RESULTS: Experimental factors include: drought stress at three irrigation levels (well-watered [WW], medium stress [MS] and severe stress [SS], i.e., 100%, 70% and 40% FC), soybean cultivars at two levels (Sepideh and Williams), and mycorrhizae application at four levels (nonapplication, Funneliformis mosseae, Glomus hoi, Rhizophagus intraradices). The results indicated that drought stress increased the concentration of grain potassium (K) and sodium (Na), leaf K, stem and leaf Na, and decreased the concentrations of grain phosphorus (P), copper (Cu), and zinc (Zn), leaf P, stem and leaf Cu, and manganese (Mn). In addition, there was no significant difference in terms of concentrations of grain K. The highest reduction was observed under SS conditions (40% FC). G. hoi colonization increased the concentrations of grain P and Zn, leaf K and Cu and stem Mn under SS conditions (40% FC). In addition, compared to Sepideh cv., Williams cv. showed higher stress resistance. Seed oil content decreased in the plants exposed to drought stress. Severe drought treatments have a deleterious effect on seed fatty acid composition, resulting in enhanced linoleic, oleic and linolenic acids. CONCLUSION: AMF colonization is a useful tool for improving the plant nutrient uptake, fatty acid profile, efficiency of resource utilization and stabilizing yield, hence reducing the production risks of crops grown under drought stress conditions. It was concluded that AMF colonization should be employed to help alleviate the adverse effects of drought stress. SIGNIFICANCE OF STUDY: AMF colonization is an effective biotechnological strategy that can alter nutrient uptake and fatty acid composition and enhance oil quality in soybean cultivars under drought conditions.

BACKGROUND: To assess the impact of brachytherapy on macular microvasculature utilizing optical coherence tomography angiography (OCTA) in treated choroidal melanoma. METHODS: In this retrospective observational case series, we reviewed the recorded data of the patients with unilateral extramacular choroidal melanoma treated with ruthenium - 106 (106Ru) plaque radiotherapy with a follow-up period of more than 6 months. Automatically measured OCTA retinal parameters were analysed after image processing. RESULTS: Thirty-one eyes of 31 patients with the mean age of 51.1 years were recruited. Six eyes had no radiation maculopathy (RM). From 25 eyes with RM, nine eyes (36%) revealed a burnout macular microvasculature with imperceptible vascular details. Twenty-one non-irradiated fellow eyes from the enrolled patients were considered as the control group. Foveal and optic disc radiation dose had the highest value to predict the burnout pattern (ROC, AUC: 0.763, 0.727). Superficial and deep foveal avascular zone (FAZ) were larger in irradiated eyes in comparison to non-irradiated fellow eyes (1629 µm2 vs. 428 µm2, P = 0.005; 1837 µm2 vs 268 µm2, P = 0.021; respectively). Foveal and parafoveal vascular area density (VAD) and vascular skeleton density (VSD) in both superficial and deep capillary plexus (SCP and DCP) were decreased in all irradiated eyes in comparison with non-irradiated fellow eyes (P < 0.001). Compared with non-irradiated fellow eyes, irradiated eyes without RM had significantly lower VAD and VSD at foveal and parafoveal DCP (all P < 0.02). However, these differences at SCP were not statistically significant. CONCLUSION: The OCTA is a valuable tool for evaluating RM. Initial subclinical microvascular insult after 106Ru brachytherapy is more likely to occur in DCP. The deep FAZ area was identified as a more critical biomarker of BCVA than superficial FAZ in these patients.

BACKGROUND: Artificial intelligence systems have been employed for the development of predictive models that estimate many agricultural processes. RESULTS: In present study, the predictive capabilities of artificial neural networks (ANNs) were evaluated with respect to assessing fruit firmness as a postharvest life index, with determinations made at four stages of storage: 1, 60, 120 and 180 days after harvesting. Single concentrations of nitrogen (N), potassium (K), calcium (Ca) and magnesium (Mg) on fruit (D1 ), all of these nutrient concentrations (D2 ), the ratios of the nutrient concentrations alone (D3 ), and a combination of nutrient concentrations and their ratios (D4 ), were considered. CONCLUSION: The results obtained showed that fruit firmness at 1 and 60 days after harvesting was not influenced by nutrients. However, the ANN model estimated fruit firmness of 120 and 180 days, respectively, for D1 and D3 more accurately than for the D2 and D4 datasets. Application of D3 (nitrogen/calcium ratio) as the input dataset improved predictions of fruit firmness, with a correlation coefficient of 0.85 between the measured and estimated data. © 2019 Society of Chemical Industry.

We constructed a food-grade expression system harboring a F1S1 fusion protein of Bordetella pertussis to be produced in Lactococcus lactis NZ3900 as a new oral vaccine model against whooping cough, caused by B. pertussis. F1S1 was composed of N-terminally truncated S1 subunit of pertussis toxin and type I immunodominant domain of filamentous hemagglutinin which are both known as protective immunogens against pertussis. The recombinant L. lactis was administered via oral or intranasal routes to BALB/c mice and the related specific systemic and mucosal immune responses were then evaluated. The results indicated significantly higher levels of specific IgA in the lung extracts and IgG in sera of mucosally-immunized mice, compared to their controls. It was revealed that higher levels of IgG2a, compared to IgG1, were produced in all mucosally-immunized mice. Moreover, immunized mice developed Th1 responses with high levels of IFN-γ production by the spleen cells. These findings provide evidence for L. lactis to be used as a suitable vehicle for expression and delivery of F1S1 fusion protein to mucosa and induction of appropriate systemic and mucosal immune responses against pertussis.

Rose is an ornamental plant which contains one of the world's top cut flowers. Vase life of cut rose flower is short. Extracts of Mentha pulegium and 8-hydroxy quinoline sulphate (8-HQS) were used as two preservative solutions, aiming to extend the vase life of cut rose (Rosa hybrid L.) flowers. Rose flowers were treated with a vase solution containing the extract of M. pulegium, at concentrations of 0, 10, 20 and 30%, in combination with 8-HQS at concentrations of 0, 200, 400 and 600 mg l(-1). Longevity of cut roses flowers was determined on the basis of wilting and chlorophyll retention. Cut roses flowers were kept at room temperature (20 ± 2 degrees C) under normal day light and natural ventilation. The vase life of cut flowers studied was prolonged by all 8-HQS and extract treatments. The best concentration of 8-HQS and extractwere 400 mg l(-1) and 10%, respectively. Our results indicated that the flowers treated with the extract and 8-HQS had longer vase life, higher rate of solution uptake and lower SPAD value (total chlorophyll) compared to the control. Also, cut flowers treated with the extract and 8-HQS had least bacterial colonies. The greatest longevity of vase life by 11.20 and 10.25 days was related to 400 mg I(-1) 8-HQS and 10% of extract, respectively. These treatments improved cut vase life more than the control treatment. The maximum solution uptake (1.85 ml g(-1) f.wt.) and minimum SPAD value (2.19) were calculated in 30% extract along with 200 mg l(-1) 8-HQS, and 200 mg l(-1) 8-HQS, respectively. The lowest number of bacterial colonies (55.75) was obtained in treatment of 600 mg l(-1) 8-HQS. Flower quality of specimens treated with extract and 8-HQS was better than those of the control. The experiments were repeated three times with three replicates and a completely randomized design had been used. The present study concludes that it would be possible to use preservative solutions containing extract of M. pulegium L. and 8-HQS to extend vase life of cut rose (R. hybrida L.) flowers.

The present study was conducted to study the individual and combined effect of bio-fertilizer (Barvar-2) and chemical phosphate fertilizer on the floral quality of marigold (Tagetes erecta L.). A factorial experiment was carried out which consisted of two factors: i) inoculation of seed, root and seed + root with bio-fertilizer (Barvar-2) and control; application of chemical phosphorus at 100 mg I(-1), 200 mg l(-1), 300 mg l(-1) and 400 mg l(-1) levels. In this study, flowering time, display life, fresh and dry weight of flower, available soil phosphorus, shoot phosphorus and carotenoid content were evaluated. Results showed that the combined effect of bio- and chemical fertilizer was insignificant (p < 1 and 5%) for most of the characteristics studied except for shoot phosphorus and carotenoid content in petals. The lowest time to flowering (64.67 days) was obtained in seeds and transplant roots inoculation to bio-fertilizer x 400 mg I(-1) P. Maximum display life (25.35), fresh weight (16.20 g), carotenoid content (3.903 mg g(-1) d. wt.) and concentration of P in shoots (0.352%) were observed in transplant roots inoculation to bio-fertilizer x 400 mg I(-1) P.

This diversity necessitates a range of delivery strategies, each tailored to specific needs. We delve into various delivery cargos that are pivotal in ophthalmic care. These cargos encompass biodegradable implants that gradually release medication, nonbiodegradable implants for sustained drug delivery, refillable tools allowing flexibility in treatment, hydrogels capable of retaining substances while maintaining ocular comfort, and advanced nanotechnology devices that precisely target eye tissues. Within each cargo category, we explore cutting-edge research-level approaches and FDA-approved methods, providing a thorough overview of the current state of ophthalmic drug delivery. In particular, our focus on nanotechnology reveals the promising potential for gene delivery, cell therapy administration, and the implantation of active devices directly into the retina. While previous studies have addressed the desirable effects of biochar (BC) or magnesium nanoparticles (Mg NPs) on salinity stress individually, there is a research gap regarding their simultaneous application. Additionally, the specific mechanisms underlying the effects of BC and Mg NPs on salinity in Physalis alkekengi L. remain unclear. This study aimed to investigate the synergistic effects of BC and Mg NPs on P. alkekengi L. under salinity stress conditions. A pot experiment was conducted with salinity at 100 and 200 mM sodium chloride (NaCl), as well as soil applied BC (4% v/v) and foliar applied Mg NPs (500 mg L-1) on physiological and biochemical properties of P. alkekengi L. The results represented that salinity, particularly 200 mM NaCl, significantly reduced plant yield (58%) and total chlorophyll (Chl, 36%), but increased superoxide dismutase (SOD, 82%) and catalase (CAT, 159%) activity relative to non-saline conditions. However, the co-application of BC and Mg NPs mitigated these negative effects and improved fruit yield, Chl, anthocyanin, and ascorbic acid. It also decreased the activity of antioxidant enzymes. Salinity also altered the fatty acid composition, increasing saturated fatty acids (SFAs) and polyunsaturated fatty acids (PUFAs), while decreasing monounsaturated fatty acids (MUFAs). The heat map analysis showed that fruit yield, anthocyanin, Chl, and CAT were sensitive to salinity. The findings can provide insights into the possibility of these amendments as sustainable strategies to mitigate salt stress and enhance plant productivity in affected areas.

BACKGROUND: Sildenafil is a selective phosphodiesterase type 5 inhibitor used for the treatment of erectile dysfunction and pulmonary hypertension. It is available over the counter in many countries. While there have been a few reports of retinal vascular occlusion following sildenafil consumption, most cases have other comorbidities as risk factors for the disease, and the exact causal role of this drug in these conditions remains unclear. CASE PRESENTATION: We present the case of a healthy 32-year-old Iranian man who developed combined central retinal vein occlusion and retinal artery occlusion following sildenafil exposure. The patient underwent a hypercoagulative state workup for possible underlying risk factors. Additionally, we conducted a literature search on PubMed using the keywords: retinal vein occlusion AND Sildenafil OR Viagra, retinal artery occlusion AND Sildenafil OR Viagra, retinal vascular occlusion AND Sildenafil OR Viagra. To obtain more objective results in the reviews, we employed an adverse drug reaction possibility algorithm. The patient was found to be otherwise healthy, and ancillary tests were unremarkable. A literature review identified seven reports of retinal vascular occlusion following sildenafil use. In most of these cases, the role of sildenafil was not clearly established. To the best of our knowledge, our case achieved the highest score based on the algorithm compared with previous reports. CONCLUSION: Sildenafil may be associated with severe retinal vascular accidents in otherwise healthy young individuals.

To compare the choroidal neovascular features of individuals with pachychoroid neovasculopathy (PNV) and neovascular age-related macular degeneration (nAMD) with and without shallow irregular pigment epithelial detachment (SIPED). Using optical coherence tomography angiography, the choroidal neovascular complexes of 27 patients with PNV, 34 patients with nAMD and SIPED, and 15 patients with nAMD without SIPED were analyzed with FIJI and AngioTool software. PNV compared to nAMD with SIPED had a greater vessel percentage area (P = 0.034), junction density (P = 0.045), average vessel length (P < 0.001), and fractal dimension (P < 0.001). PNV, compared to nAMD without SIPED, had a greater total vessel length (P = 0.002), total number of junctions (P < 0.001), junction density (P = 0.034), and fractal dimension (P = 0.005). nAMD with SIPED, compared to nAMD without SIPED, had greater vessel area, total number of junctions, total vessel length, and average vessel length (all P values < 0.001). Patients with nAMD plus SIPED and individuals with nAMD without SIPED have similar fractal dimension values (P = 0.703). Biomarkers of choroidal neovascular complexity, such as fractal dimension, can be used to differentiate PNV from nAMD with or without SIPED.

Tomatoes (Lycopersicun esculentum L.) are an important group of vegetable crops that have high economical and nutritional value. The use of fertilizers and appropriate substrates is one of the important strategies that can assist in increasing the yield and quality of fruits. The present study aimed to investigate the effects of exogenous seaweed extract (Nizamuddinia zanardinii), silicon (Na2SiO3), and selenium (Na2SeO3) on quality attributes and fruit yield (FY) of tomato under palm peat + perlite and coco peat + perlite substrates. Seaweed extract significantly improved several of the fruit quality attributes such as total carbohydrate content, total soluble solids (TSS), and pH as well as the FY. The results showed that silicon (Si) (75 mg) was the best foliar spray treatment to enhance the fruit firmness (30.46 N), fruit volume (196.8 cm3), and FY (3320.5 g). The highest amount of plant yield (3429.33 g) was obtained by the interaction effects of silicon (75 mg L-1) under the effect of palm peat. The use of selenium (Se) led to improvements in flavor index (TSS/TA). Also, the application of palm peat + perlite substrate caused an increase in vitamin C (16.62 mg/100g FW), compared to other substrates (14.27 mg/100g FW). The present study suggested that foliar spray with seaweed extract and Si had beneficial effects on the quality and FY of tomatoes. Also, the palm peat substrate can be used as a good alternative to the coco peat substrate in the hydroponic system.

PURPOSE: To present the success rate of nonsurgical management of full-thickness inflammatory macular hole (IMH). METHOD: Retrospective case series of five patients with IMH. RESULT: Five eyes from five patients with IMH enrolled in the current case series. All five eyes had successful closure with corticosteroid in the form of topical, periocular, or intravitreal injections. Systemic immunomodulatory treatment was employed for two patients, in addition to local therapy. For local therapy, one patient received topical eye drops, subtenon injection of corticosteroid, and intravitreal injection of combination of corticosteroid and anti-VEGF was performed in two patients. The closed macular hole reopened in one patient after two years, which required pars plana vitrectomy and anatomical and visual success achieved. CONCLUSION: Inflammatory macular holes can be closed with non-surgical interventions, although reopening may occur which requires surgery.

PURPOSE: To compare the anatomical and refractive outcomes of transscleral diode versus transpupillary laser photocoagulation for the treatment of zone II type 1 retinopathy of prematurity (ROP). METHODS: In this prospective comparative interventional case series, infants with type 1 ROP in zone II were assigned to either transpupillary or transscleral laser based on the surgeons' expertise area. The rate of regression, need for retreatment, and structural and biometric outcomes at month 6 were evaluated and compared between the two treatment groups. RESULTS: In total, 209 eyes were enrolled; 145 eyes of 77 infants and 64 eyes of 33 infants and were in transscleral and transpupillary groups, respectively. There was no significant difference in baseline characteristics between the groups. There was no significant difference in retreatment rates (1.6% vs. 3.4%; P = 0.669) and progression to stage 4 (1.6% vs. 2.8%; P = 0.999) between the transpupillary and transscleral groups, respectively. At month 6, the mean spherical equivalent was 0.31 ± 3.57 and 0.44 ± 2.85 diopters, and the axial length was 18.28 ± 6.22 and 18.36 ± 6.87 mm in the transpupillary and transscleral groups, respectively, without a significant difference between groups. There was no significant difference in the rate of myopia (43.8% vs. 33.8%; P = 0.169) and high myopia (4.7% vs. 4.8%; P = 0.965) in transpupillary and transscleral groups at month 6. CONCLUSION: The transpupillary and transscleral laser photocoagulation routes are both effective in the treatment of zone II type 1 ROP and show no significant differences in anatomical or refractive outcomes in relation to the route chosen.

PURPOSE: To evaluate acute changes in intraocular pressure (IOP) and their short-term effects on the peripapillary retinal nerve fiber layer (RNFL) thickness after intravitreal bevacizumab (IVB) injection. METHODS: Fifty-eight eyes of 37 patients with treatment-naïve diabetic macular edema or exudative age-related macular degeneration were included in the study. Patients were divided into two groups, and the participants of each group received 3 monthly injections of IVB. IOP was measured right before the injection, immediately after the injection, and 5 min and 20 min after each injection. Peripapillary, RNFL thickness was measured before the injection and 1 month after the third injection. In the second group, anterior chamber (AC) paracentesis was performed before IVB injection. RESULTS: IOP values after injection in all sessions were significantly higher in the first group (P < 0.001). The peripapillary RNFL thickness changes 1 month after the third injection was not statistically significantly different in each group (P = 0.816 and 0.773 for the first and second groups, respectively). CONCLUSION: AC paracentesis is an effective modality to reduce the acute rise in IOP. The effect of acute IOP elevation on the peripapillary RNFL thickness was not statistically significant.

PURPOSE: To assess the safety of biosimilar intravitreal aflibercept (CinnaGen Co., Iran) compared to the reference product (Eylea®; Bayer Schweiz AG, Zurich, Switzerland) in rabbit eyes through functional and histologic studies. METHODS: Forty New Zealand albino rabbits were recruited to the study and were divided into four groups to be sacrificed at 48 hours, one, two, and four weeks after injections. In each group, five rabbits received 0.05 mL (2 mg) biosimilar aflibercept in the right eye and 0.05 mL saline in the left eye as the control, and in a similar manner, the remaining five rabbits received the reference drug in the right eye and saline in the left eye. All the rabbits underwent comprehensive ophthalmic examination and electroretinography (ERG) tests at baseline and also just before enucleation at the specific predefined time points. The enucleated eyes were prepared for retinal toxicity histological examination. RESULTS: No retinal toxicity was observed based on histologic and ERG findings in all groups. Choroidal congestion was revealed after 1 week in an eye that was injected with biosimilar aflibercept, although the similar finding was detected in the contralateral eye which received saline. Also, one subject which received the reference drug showed chronic vitritis and lymphoplasmocytic reaction of the optic disc at week 4. The remaining subjects showed no histologic changes. CONCLUSION: The 2 mg intravitreal injection of biosimilar aflibercept (CinnaGen Co., Iran) was found to be nontoxic in rabbit eyes in the short-term period. Further studies are required to warrant the efficacy and safety profile of the drug in human subjects.

BACKGROUND: Lactic acid bacteria such as Lactococcus (L.) lactis are powerful tools that can function as live delivery vectors and heterologous protein expression hosts in development of novel vaccines. Pertussis toxin (PT) and filamentous hemagglutinin (FHA) are important virulence factors of Bordetella (B.) pertussis and constitute the major components of commercially available acellular pertussis (aP) vaccines. The purpose of the present study was to express F1S1 fusion protein, consisted of the N-terminal region of S1 subunit from PT and FHA type 1 immunodominant domain by L. lactis and to evaluate its immunogenicity. METHODS: The fusion gene composed of sequences encoding the F1S1 and the signal peptide of usp45 fragments (SECF1S1) was codon optimized for protein production in L. lactis and was synthesized and inserted in-frame inside pNZ8149 plasmid. The resulting pNZ8149-SECF1S1 construct was introduced by electroporation into L. lactis cells (LL-F1S1). BALB/c mice were subcutaneously immunized with LL-F1S1 or commercial DTaP vaccine. The immune responses were investigated. RESULTS: The LL-F1S1-immunized mice produced significant levels of specific IFN-g compared to their respective controls and DTaP-immunized mice. The F1S1- specific IgG antibody response was lower in LL-F1S1-immunized mice while the IgG2a/IgG1 ratio was higher in this group compared to the DTaP-immunized mice. Moreover, anti-F1S1 IgA antibodies were only detected in the lung homogenates of the LL-F1S1-immunized mice, suggesting the induction of a mucosal immune response. CONCLUSION: These results indicate the feasibility of expression of F1S1 fusion protein in L. lactis. This recombinant bacterium could induce mucosal and Th1-type systemic immune responses following subcutaneous administration.

PURPOSE: The aim of this study was to detect the genetic defects in a large pedigree of affected individuals with various phenotypes of ocular anomalies including partial aniridia, congenital cataract, and nystagmus. METHODS: The entire coding region of paired box gene 6 (PAX6) was amplified by polymerase chain reaction (PCR), sequenced, and compared with a GenBank database. RESULTS: A novel mutation (c.1170 C > T; p.Gln297X) was found in the proband and all affected members. This nonsense mutation leads to PAX6 protein truncation. CONCLUSIONS: Our findings suggest that this novel mutation is most likely responsible for the pathogenesis of the congenital aniridia, cataract, and nystagmus in this pedigree. To the best of our knowledge, this is the first report of this mutation of PAX6 gene in a kindred pedigree with various ocular abnormalities.

OBJECTIVES: After decades of containment, pertussis disease, caused by Bordetella pertussis seems to be re-emerging and still remains a major cause of reported vaccine-preventable deaths worldwide. The current licensed whole-cell vaccines display reactogenicity while acellular vaccines are expensive and do not induce Th1-type immune responses that are required for optimum protection against the disease. Thus, there is an urgent need to develop new vaccines and the recombinant technology seems to be the method of choice for this purpose. The present study was an attempt to develop a new, simplified, cost-effective and well-defined vaccine against Bordetella pertussis, with capacity to induce a Th1 response. MATERIALS AND METHODS: A fusion DNA fragment encoding the N-terminal region of pertussis toxin S1 subunit and filamentous hemagglutinin type 1 immunodominant domain was constructed and the corresponding fusion protein (F1S1) was produced in Escherichia coli. F1S1 in conjunction with imiquimod was administered by subcutaneous (SC) and intranasal (IN) routes to BALB/c mice. RESULTS: This vaccine formulation could elicit high levels of IFN-γ, serum IgG (with higher IgG2a/IgG1 ratio) and lung IgA after the SC and, to a lesser extent, following the IN administration. CONCLUSION: Our results indicate that the above-mentioned important proteins of B. pertussis could be successfully produced in E. coli as a single fusion protein. Furthermore, this protein could induce proper systemic and mucosal immune responses after administration via SC or IN routes.

PURPOSE: To classify subjects with primary angle closure into clusters based on features from anterior segment optical coherence tomography (ASOCT) imaging and to explore how these clusters correspond to disease subtypes, including primary angle closure suspect (PACS), primary angle closure glaucoma(PACG), acute primary angle closure (APAC) and fellow eyes of APAC and reveal the factors that become more predominant in each subtype of angle closure. METHOD: A cross-sectional study of 248 eyes of 198 subjects(88 PACS eyes, 53 PACG eyes, 54 APAC eyes and 53 fellow eyes of APAC) that underwent complete examination including gonioscopy, A-scan biometry, and ASOCT. An agglomerative hierarchical clustering method was used to classify eyes based on ASOCT parameters. RESULTS: Statistical clustering analysis produced three clusters among which the anterior segment parameters were significantly different. Cluster 1(43 eyes) had the smallest anterior chamber depth(ACD) and area, as well as the greatest lens vault (p<0.001 for all). Cluster 2(113 eyes) had the thickest iris at 2000 microns(p = 0.048), and largest iris area(p<0.001), and the deepest ACD (p<0.001). Cluster 3(92 eyes) was characterized by elements of both clusters 1 and 2 and a higher iris curvature(p<0.001). There was a statistically significant difference in the distribution of clusters among subtypes of angle closure eyes(p<0.001). Although the patterns of clusters were similar in PACS and PACG eyes, with the majority of the eyes classified into cluster 2(55%, and 62%, respectively), the highest proportion of APAC and fellow eyes were assigned to clusters 1(44%) and 3 (51%), respectively. CONCLUSION: Hierarchical cluster analysis identified three clusters with different features. Predominant anatomical components are different among subtypes of primary angle closure.

PURPOSE: To determine whether early amniotic membrane transplantation improves the outcomes among patients with bacterial keratitis. PATIENTS AND METHODS: In this prospective randomized clinical trial 49 eyes from 49 patients with bacterial keratitis received routine antibiotic therapy followed by double-layer amniotic membrane transplantation during 2-5 days after start up of the medications and the second group of 50 eyes from 50 patients only received routine antibacterial therapy. The main outcomes including visual acuity, corneal healing time, scar size, hypopyon size and duration as well as the depth of corneal opacity and neovascularization were followed for a six months period. In addition, Langerhans cells density and tectonic procedures were documented during the healing period. RESULTS: Our results indicated statistically significant better best corrected visual acuity (p < 0.001), uncorrected visual acuity (p < 0.001), as well as contact lens corrected visual acuity (p < 0.001) at six months in the case group compared to control group. Corneal healing time, the depth of corneal opacity, hypopyon size and duration were not significantly different in the case group while the scar size (p < 0.001) was significantly smaller. Also significantly less corneal vascularization at six months (p < 0.001) and Langerhans cells density (p < 0.001) at one month were observed in the case group. CONCLUSION: The early use of amniotic membrane transplantation in treatment of patients with bacterial keratitis was associated with better outcomes compared to antibiotic therapy alone. More comparative studies to investigate the probable side effects as well as the cost benefit of this method are needed.
