OBJECTIVE: A wound is defined as chronic when it requires more than 6 weeks to heal. The link between chronic wounds and depression was first pointed out by House and Hughes in 1996 and later evaluated by other numerous studies. Several studies demonstrate that among chronic skin wounds causing chronical physical disease, the leg ulcers are the most frequently associated with depression. The aim of our study is to evaluate the prevalence of depression in patients with different types of chronic wounds. PATIENTS AND METHODS: We enrolled a total of 33 patients with chronic wounds and 33 healthy controls matched by sex and age. Both patients and controls underwent a BDI II survey. We evaluated 33 patients, with a mean age of 71 years (range 25-87), and 33 controls, with a mean age of 73 years (range 31-88). RESULTS: The average score at the BDI II questionnaire was 14.5 and 8, respectively. CONCLUSIONS: The depression among patients with chronic wounds has a multifactorial origin that should be treated with a multidisciplinary approach. Since the improvement of the psycho-emotional state means better compliance of the patient, we can also expect a better result in terms of efficacy in chronic wound treatment.

INTRODUCTION: Macromastia is a health problem that requires the coordination of surgical and medical specialists. Goals of reduction mammaplasty are to alleviate physical, emotional and psychosocial discomforts and to restore a conical-shaped breast, maintaining scars as short as possible. We report our approach for reduction mammaplasty with superior pedicle. MATERIALS AND METHODS: Our method combines advantages of round block with vertical scar, using a dermal flap that is fixed to the new mammary crease. We analyzed skin and glandular resection customizing the mammaplasty. RESULTS: The dermal flap works against the weight of residual tissue, maintaining the crease at the desired position with a natural result. Benefits are an excellent projection, short scar, suitable reshaping and patient satisfaction. CONCLUSIONS: This technique can be used for mild to severe hypertrophy with various degrees of ptosis. It results in a successful aesthetic outcome with minimal scarring, suitable breast remodeling and natural long-lasting projection.

OBJECTIVE: The reduction mammaplasty is indicated for patients with symptomatic breast hypertrophy. Although surgery is considered the gold standard treatment, it is still debated which is the complication rate and whether or not there is an increased complication rate with increased tissue resection per breast. The main objective of this retrospective study was to determine whether the rates of complications are higher in large reductions (≥2000 g per breast) as compared with smaller reductions (≤1999 g per breast) using the superior pedicle custom-made technique. PATIENTS AND METHODS: A retrospective study of 90 consecutive operated patients was performed. All the patients underwent a bilateral breast reduction for macromastia. 43 patients had a reduction of 1999 g or less and 47 patients had a reduction of 2000 g or more. RESULTS: There were no statistically significant differences in the rates of nipple necrosis, hematoma, seroma, wound dehiscence, wound infection, loss or reduction of nipple sensation, and a pathologic scar between the large and small resections. CONCLUSIONS: The large reduction patients pointed higher mean satisfaction rate than the small reduction patients. The superior pedicle custom-made technique is a safe method of breast reduction regardless of the degree of parenchymal resection, achieving a successful aesthetic outcome with minimal scarring and high satisfaction rate for both small and large breast reduction.

OBJECTIVE: The new lift is a procedure for facial rejuvenation with minimal incision, giving the patient a significant improvement of mid face and neck, with limited dissection and minimal scars. A further "One-stitch" of anchorage to the deep temporal fascia is required to hold tissues. By a minimal surgical access, we get a suitable cheek lift with the improvement of nose-labial folds, mandibular edge and neck contour. PATIENTS AND METHODS: Between February 2009 and June 2012, 32 patients underwent facial rejuvenation surgery called new lift at a mean age of 46 years (range 35-55 years). Seven of the patients had a previous facelift. 12 out of 32 patients (37.5%) had concomitant eyelid surgery and 4 (12.5%) neck contouring procedure. RESULTS: The technique we used is a safe and effective procedure with a high satisfaction rate; 28 patients (89%) were very satisfied with their result at 24-months follow-up. CONCLUSIONS: The surgical outcome was evaluated according to the analysis of photographs obtained before and after surgery and the analysis of pre- and postoperative measurements. Aesthetic results were evaluated also by patients themselves who indicated a high satisfaction rate at three months post-surgery questionary and by a surgeon not involved in the study using VAS (1-10) before and three months after surgery. Acquired data on the aesthetic result were statistically evaluated using Student t-test. This is a study aimed at assessing the effectiveness of the new-lift technique for facial rejuvenation. Based on the results of our study, the new-lift is a very effective surgery for rejuvenation of the face with mild to moderate aging. All patients healed uneventfully without any major postoperative problems. This technique responds to an increasing demand from a wide range of patients for less invasive, less expensive operations with faster healing time and fewer potential complications.

OBJECTIVE: To evaluate the palatability of antimicrobial agents effective against beta-lactamase-producing bacteria in American children. DESIGN: In a taste test of 4 antimicrobial agents, azithromycin (cherry flavored), cefprozil (bubble gum flavored), cefixime (strawberry flavored), and amoxicillin-clavulanic acid (banana flavored) were compared. SETTING: An urban inner-city primary care clinic. SUBJECTS: A volunteer sample of 30 healthy children (aged 5-8 years). INTERVENTION: Palatability was determined using a single-blind taste test of 4 flavored antimicrobial agents. The 4 antimicrobial agents used were azithromycin, cefprozil, cefixime, and amoxicillin-clavulanic acid. MAIN OUTCOME MEASURES: After each antimicrobial test dose, subjects rated the taste on a 10-cm visual analog scale incorporating a facial hedonic scale. Preference assessments for the best-tasting and worst-tasting agent were also conducted. RESULTS: Of the 20 children who expressed a preference, significantly more children (9 [45%], P<.05) selected the cefixime preparation as the best-tasting formulation compared with the other preparations. The cefixime preparation was also significantly the least likely to be selected as the worst-tasting preparation (2 [10%], P<.05). There were no significant differences between the other 3 preparations with respect to being selected as either the best or worst tasting. The mean (+/- SD) visual analog scale score for cefixime was highest (8.53 [2.49]) compared with the scores for azithromycin (6.78 [3.45]), cefprozil (6.26 [4.04]), and amoxicillin-clavulanic acid (6.24 [4.01]). CONCLUSION: The cefixime preparation was most commonly rated as best tasting by children.

Patient-reported data can be a valuable, adjunctive tool in helping physicians assess the quality-of-life and functional changes in patients with Parkinson's disease (PD). We analyzed and evaluated the largest PD patient-reported database (PROPATHtrade mark) relative to prescribing habits and changes in reported outcomes by therapy. Our analysis included 1436 patients, followed for 1 yr, who completed a series of questionnaires assessing medication therapy, daily activity scores adapted from the Unified Parkinson's Disease Rating Scale (UPDRS), global disease visual analogue scale, and health resources consumption. Our results indicated that physicians are prescribing Eldepryl(R) with increased frequency in patients with early and mild PD. Patients receiving Eldepryl alone or in combination with Sinemet(R) reported better outcomes than those receiving Sinemet monotherapy. There was a great deal of variability in the reported utilization of healthcare resources by patients in the PROPATH program and, thus, no statistical differences were noted for patients treated with different regimens. We conclude that the adjunctive use of longitudinal patient self-reported data programs such as PROPATH can help assess and improve overall patient outcomes. Future controlled studies should be conducted to further evaluate the roles of alternative therapies and patient-reported data in improving quality-of-life and outcomes for PD patients.

PURPOSE: This study involved identifying resource use and assigning monetary value to the diagnostic work-up and management of viral meningitis. METHODOLOGY: Using a previously established decision analytic framework, various resources were identified as part of routine management of viral meningitis. Secondary database analyses were used to quantify resources and assign a monetary value as a part of routine management of viral meningitis requiring use of the resource units identified in the decision analytic framework. Discharge data sources from the states of California, Florida, and Illinois, and Medicaid data sources from the state of Pennsylvania, were used for the purpose of analysis. PRINCIPAL FINDINGS: Physician visits, emergency room visits, hospital admissions, procedures, and medications were identified as the major resource used. Lumbar punctures, CT scans, and antibiotics were identified as the major procedures and medications utilized. No significant difference was found in the major resources used between the states' discharge data and the Medicaid data sources. The mean total charges for patient admissions with CT scans were significantly higher than for patient admissions without CT scans ($11,531.80 vs $7,841.30, P < 0.05). The mean lengths of stay for patients with CT scan were significantly higher than for patient admissions without CT scans (4.71 days vs. 3.88 days, P < 0.05). The patient readmission rate was 10.7 percent, while the readmission rate for episodes with more than one hospitalization was 11.1 percent. The mean charge associated with readmission was $12,200.

Squamous cell carcinoma (SCC) is a malignant tumor of epithelium that shows squamous cell differentiation. It is the second most common cancer of the skin and usually occurs in areas exposed to the sun but it can rarely arise within the conjunctival epithelium with a deep component. We describe a woman with a history of chronic blepharoconjunctivitis unresponsive to topical medications. Examination disclosed a hyperaemic translucent patch with blurred margins of the upper palpebral conjunctiva. Tarsoconjunctival biopsy revealed intraepithelial squamous cell carcinoma. Management consisted of complete tumor excision with removal of the entire posterior lamella of the left upper eyelid and reconstruction. Histopathologic analysis confirmed primary squamous cell carcinoma arising from conjunctival epithelium, involving the underlying tarsus. Patients with unexplained chronic unilateral blepharoconjunctivitis or papillary hypertrophy of the palpebral conjunctiva should be considered for biopsy to rule out neoplasia, even when there is no sign of an evident mass.

Facial expressions play a key role in affective and social behavior. However, the temporal dynamics of the brain responses to emotional faces remain still unclear, in particular an open question is at what stage of face processing expressions might influence encoding and recognition memory. To try and answer this question we recorded the event-related potentials (ERPs) elicited in an old/new recognition task. A novel aspect of the present design was that whereas faces were presented during the study phase with either a happy, fearful or neutral expression, they were always neutral during the memory retrieval task. The ERP results showed three main findings: An enhanced early fronto-central positivity for faces encoded as fearful, both during the study and the retrieval phase. During encoding subsequent memory (Dm effect) was influenced by emotion. At retrieval the early components P100 and N170 were modulated by the emotional expression of the face at the encoding phase. Finally, the later ERP components related to recognition memory were modulated by the previously encoded facial expressions. Overall, these results suggest that face recognition is modulated by top-down influences from brain areas associated with emotional memory, enhancing encoding and retrieval in particular for fearful emotional expressions.

This age-matched case-control study assessed total and segmental lean muscle mass in classic or ovulatory polycystic ovary syndrome (PCOS) patients and investigated whether lean mass is associated with hormone and metabolic features. Participants underwent anthropometric and clinical evaluation. Habitual physical activity was assessed with a digital pedometer, and body composition by dual-energy X-ray absorptiometry. Laboratory measurements included total cholesterol, cholesterol fractions, triglycerides, glucose, total serum testosterone, serum insulin, estradiol, luteinizing hormone, and SHBG. Energy intake was calculated using a food frequency questionnaire. Classic PCOS patients had higher body mass index (BMI), waist circumference, testosterone and lipid accumulation product values than ovulatory PCOS and controls. Energy consumption, homeostasis model assessment index, SHBG, free androgen index and triglycerides, total and trunk lean mass were higher only in classic PCOS women vs. controls. Arm, leg, trunk, total or limb lean masses were not correlated with hormone levels in any of the groups. However, in PCOS women lipid accumulation product was positively correlated with total (r=0.56, p=0.001), trunk (r=0.59, p=0.001), arm (r=0.54, p=0.001), leg (r=0.44, p=0.03) and limb (r=0.48, p=0.001) lean masses. BMI was positively correlated with all lean mass segments and independently associated with total lean mass. Lipid accumulation product and BMI were independently associated with trunk lean mass variation. The increase in lean mass in classic PCOS appears to be associated with insulin resistance and central obesity rather than with energy intake, physical activity or androgens.

The pharmacokinetics of cefonicid in the serum and tissues of patients undergoing open heart surgery, vaginal hysterectomy, and hip replacement was studied and compared with that of cefazolin and cefoxitin. Cefonicid achieved serum and tissue concentrations that were higher and more prolonged than those of cefazolin and cefoxitin. For cefonicid parallelism was observed between the serum and tissue concentration-time curves. Correlations of these properties to clinical efficacy were not attempted.
