RESUMO

Acute abdominal aortic thrombosis is a rare and potential fatal event, which occurs in adult subjects. We present the case of a 72-year-old-man, who referred to the emergency Department of our hospital because of persistent severe abdominal and perineal pain. Doppler ultrasounds and computerized tomography angiography revealed the acute thrombosis of the abdominal aorta. Immediate revascularization through aortic thrombo-endoarterectomy resolved the disease.

In order to confirm the efficacy and safety of posatirelin (L-pyro-2-aminoadipyl-L-leucyl-L-prolinamide), a synthetic peptide having cholinergic, catecholaminergic and neurotrophic activities, a multicentre, double-blind, controlled study versus placebo was planned in elderly patients suffering from Alzheimer's disease and vascular dementia, according to National Institute of Neurological and Communicative Disorders and Stroke/Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA) and National Institute of Neurological Disorders and Stroke/Association Internationale pour la Recherche et l'Enseignement en Neurosciences (NINDS-AIREN) criteria, respectively. The trial consisted of a 2-week run-in phase with placebo administered once a day orally, followed by a double-blind period of 3 months, with posatirelin or placebo administered once a day intramuscularly. Efficacy was assessed using the Gottfries-Bråne-Steen (GBS) Rating Scale (primary variable) and the Rey Memory Test (secondary variable). Laboratory tests, vital signs and adverse events were monitored. A total of 360 patients were randomized, the intent-to-treat sample (ITT) being made up of 357 patients and the per protocol sample (PP) of 260 patients. Both pragmatic and explanatory analyses showed significant differences between treatment groups in the GBS Rating Scale and the Rey Memory Test, with no difference in the two types of dementia. No difference between treatments was observed in safety variables, the incidence of adverse events in the posatirelin group being 7.3%. The study confirms previous results showing that treatment with posatirelin can improve cognitive and functional abilities of patients suffering from degenerative or vascular dementia.

Circulating immune complexes (CIC) of the IgG class were detected in sera from 37 (35.58%) out of 104 of the preoperated breast cancer patients studied using a CIq solid-phase enzymatic assay. Sera of positive patients were then tested to identify the IgG subclasses which are part of the complexes with the aim of showing a better correlation between CIC and disease evolution. IgG1- and IgG3-CIC were indirectly detected by the above test using monoclonal antibodies against the mentioned subclasses. Curves obtained with aggregated IgG1 and/or IgG3 showed good sensitivity and specificity of the test. Complexes containing both IgG1 and IgG3 were present in sera from the majority of patients; however, the relative amounts of these subclasses varied from patient to patient.

Morphological aspects and the most important vectorial parameters of the VCG (Frank method) of normal children, aged 2 to 12 years, were analyzed. The total group consisted of 110 subjects (10 for each )ear) subdivided into three subgroups of progressive age. Semeiologic evaluation of the QRS in the frontal and sagittal planes was not significant because of the stereotypes of the figures. In the horizontal plane the majority of cases showed typical triphasic development of the vectorial groups, while biphasic morphology was found in the minority. The triphasic loop presented 1) a balance between anterior and posterior forces which was more frequent in the youngest children, 2) a posterior orientation which was more frequent in the oldest children or 3) an anterior orientation which was infrequent. Since the anterior orientation of triphasic QRS was not related to the age -- it may also be seen in normal adults -- it was attributed to a variation of ventricular activation. The biphasic loop was attributed to immaturity of ventricular conduction. The evaluation of quantitative parameters was in agreement with the results of other studies. The changes of initial and intermediate vectors were probably due to the more important development of the left ventricle. The QRS duration increased with age. The T loop was posterior in the majority of the youngest children and progressively advanced with age, although it remained posterior in an important number of cases of children aged twelve.

To assess the incidence of coronary artery disease in patients with valvular aortic stenosis and its implication on peak systolic valvular gradient, 31 consecutive patients who underwent cardiac catheterization were examined. Associated significant coronary artery disease (> 50% reduction in luminal diameter evaluated in proximal segments and right dominant circulation) was present in 54.8% of patients. There was no difference in the distribution of risk factors among patients with and without significant luminal narrowings. The prevalence of coronary artery disease was found not to be significantly correlated with age (p = 0.276). There was no relationship between typical angina pectoris and the presence of coronary artery disease (p = 0.063). Fourty-seven percent of cases resulted free of chest pain. Ejection fraction was found to be significantly lower in patients with coronary artery disease (45 +/- 14.2%) than in patients without coronary artery disease (65.1 +/- 3.9%; p = 0.03) and a reverse relationship was observed between the presence of coronary artery disease and peak systolic valvular gradient (p = 0.006) which, in turn, correlated significantly with ejection fraction (r = 0.68; p = 0.023). These data demonstrate that the value of peak systolic valvular gradient, as the only index for the evaluation of the severity of aortic stenosis, is greatly limited in patients with associated coronary artery disease. Moreover, confirming the guidelines of the American College of Cardiology and of the American Heart Association task force, these data also stress the necessity of performing coronary angiography regardless angina pectoris is present or not.

Various electrocardiographic and vectorcardiographic (ECG-VCG) patterns of ventricular conduction disturbances are inducible by premature right atrial stimulation (PRAS). These patterns are a consequence of different degrees of refractoriness in the specialized ventricular conduction system. We observed that the intermediate phase of left bundle branch block (LBBB) induced by PRAS in 20 subjects with normal basal QRS (complexes) were similar to those of left ventricular hypertrophy (LVH). In 18 patients with basal ECG-VCG signs of LVH, right bundle branch block (RBBB) induced by PRAS produced a progressive disappearance of these signs and the "normalization" of the tracings. The initial maximum QRS vector decreased, disappeared or remained absent in the patients with LBBB induced by PRAS, and appeared (when absent in the basal VCG) and remained unchanged (when present) in patients with RBBB induced by PRAS. In this paper we discuss the electrogenetic implications of these data. The ECG-VCG sings of LVH are probably dependent on a slowed conduction in the left bundle branch system, while anatomical hypertrophy per se probably plays a less important role.

Anterior displacement (AD) of the QRS horizontal loop (Frank VCG method) was induced by programmed right atrial stimulation (PRAS) in 15 cases. When AD occurred we noticed changes of the terminal QRS vectors and of the T loop similar to those observed in incomplete right bundle branch block (RBBB). The increasingly anticipated extrastimuli induced progressively the AD and then progressive degrees of RBBB. The anterior shifting of the efferent limb never appeared after the induction of RBBB. A left conduction disturbance never appeared after the AD. In cases of supposed incomplete left bundle branch block (i.e. left ventricular hypertrophy) the QRS duration decreased when the AD was induced. Therefore, the AD induced by PRAS and probably those observed in some clinical cases are due to a right ventricular conduction disturbance.

The purpose of the present study was to investigate the prognostic value of circulating immune complexes (CIC) in surgically treated breast cancer patients as compared with other well-known prognostic factors. CIC of IgG and IgM classes were determined by a C1q immunoenzymatic assay in serum samples of 122 patients before mastectomy and 51 of them were found positive for IgG. The other class of CIC was virtually absent. No relevant differences of distribution of other prognostic parameters such as estrogen and progesterone receptors, histological grading, nodal and menopausal status were found according to CIC levels. Level of IgG CIC was affected by surgical removal of the tumor since significant reduction of it was observed 2 weeks after mastectomy; however, this reduction did not show prognostic significance. The patients included in the present study were subjected to a 7-year follow-up; eventually a significant association was observed between preoperative IgG CIC and relapse of the disease. Patients with positive values of immune complexes relapsed more frequently than those with negative values. Serial determinations of IgG CIC were carried out within the 24 months following mastectomy and statistically evaluated for their prognostic use. No significant association was found between increase of IgG CIC level and relapse of the disease.
