Pesticides containing diazinon are frequently used in agriculture in the Vietnamese Mekong delta region leading to their potential residual occurrence in the environment. Under laboratory conditions, exposure to diazinon has been shown to result in adverse inhibition of cholinesterase enzyme (ChE) activity and subsequent death in several fish species. This study investigated a 96-h median lethal concentration (LC50) of diazinon concentrations from 0.08 to 1.25 mg/L on juvenile giant freshwater prawns (Macrobrachium rosenbergii de Man, 1879) in tanks in the laboratory. Inhibition of ChE in the flesh and in the eyes of the tested shrimps after exposed to diazinon concentrations of 2.7, 27, 67.5 µg/L equivalent to 1%, 10%, 25% LC50-96 h was calculated. The results indicated that diazinon was highly toxic to giant freshwater prawn with a low LC50-96 h of 270 µg/L. The activity of ChE in the flesh was more sensitive to diazinon than that in the eyes. Furthermore, in the future, the activity of ChE in the flesh or in the eyes of shrimps has potential to be used as biomarker for rapid recognition of diazinon contamination in water.

Oncology inpatients are at high risk of malnutrition. Identification of at risk patients by nutrition screening requires a practical and easy to use tool. The aim of this study was to determine the validity of the Bach Mai Boston Tool (BBT) compared to a 'gold standard' full nutrition assessment using the Patient-Generated Subjective Global Assessment (PG-SGA). A cross-sectional study was conducted on 270 oncology inpatients from January to December 2016. Cohen's Kappa, sensitivity, specificity and ROC analyses were performed. 270 inpatients were included in this study with a mean age of 56.3 ± 12.1 years old. Of these patients, 51.8% were male, and 74.1% had gastrointestinal cancer. The mean body mass index of patients was 20.6 ± 3.0 kg/m2. The PG-SGA tool identified 146 (54.1%) malnourished patients, while the BBT identified 105 (39.9%) malnourished patients. The BBT had a medium consistency, with a Kappa value of 0.6. Using a cut-off point of ≥ 4, the BBT had a sensitivity of 87.7% and a specificity of 72.6%. On the other hand, a BBT with a cut-off point ≥ 5 resulted in a sensitivity of 67.1%, a specificity of 94.4%, and an AUC of 0.81. The BBT is a practical, informative and valid tool for detecting malnutrition in hospitalized oncology patients. We recommend using a cut-off point of 4 for screening the risk of malnutrition for oncology inpatients.

The high use of pesticides in intensive rice farming in the Mekong Delta constitutes a potential hazard to the environment and to people's health. Chlorpyrifos ethyl (CPF) is a commonly used organophosphate (OP) insecticide, but information about its potential negative impacts on the aquatic environment in the Mekong Delta is scarce. Both acute and subacute toxicity tests were performed in a static nonrenewable system to investigate the effects of CPF on brain acetylcholinesterase (AChE) activity in native climbing perch fingerlings (Anabas testudineus, Bloch, 1972). Environmental parameters, such as dissolved oxygen, water temperature, and pH, were similar to field conditions in the Mekong Delta. In a 96-h lethal concentration (LC50) test, fingerlings of climbing perch were randomly exposed to five levels of CPF ranging from 0.8 to 4.5 ppm. Five sublethal levels of CPF (1, 5, 10, 15, and 20 % of the 96-h LC50 value) were tested to assess the sensitivity and recovery of the brain AChE activity in climbing perch fingerlings exposed to CPF. The results showed that CPF were moderately toxic to climbing perch with a 96-h median LC50 of 1.73 ppm. CPF also caused long-term AChE inhibition with 70 % inhibition remaining after 96 h for the four highest test concentrations. The recovery of brain AChE activity in fish placed in CPF-free water was very slow, and after 7 days the brain AChE activity was still significant lower in fish from the four highest concentrations compared with the control. The results from this study indicate that OP insecticides, such as CPF, can have long-lasting sublethal effects on aquatic species in the Mekong Delta.

Urbanization and economic development cause water pollution in the inner-city canals and rivers globally. Bung Xang canal in Can Tho city of Vietnam is facing problems with water pollution due to the lack of centralized wastewater treatment plants and low public awareness on environmental protection. Perception of local residents was collected using structured questionnaires including both qualitative and quantitative information. Regression analysis was employed to evaluate the factors affecting the decision of respondents on the willingness to pay (WTP) to improve water quality in the Bung Xang canal. Knowledge about the environmental protection fee for domestic wastewater (10% of the VAT-excluded from the selling price of 1 m3 of tap water purchased), age of the respondents and their education levels affected the WTP positively, while respondents' perception on water quality affected the WTP negatively. There was 58.33% of the respondents showed the WTP for improved water quality in the canal. They agreed to pay a small fee of VND 10,000 to 15,000 (equivalent to USD 0.42-0.63)/month (1 US$= 23,700 VND). The result indicates that environmental education is the only way forward for a successful sustainable urban city.

The combined effect of Vitashield 40EC (chlorpyrifos ethyl-CPF) and Bassa 50EC (fenobucarb-F) was compared with the effects from exposure to the two pesticides separately, by measuring the brain acetylcholinesterase (AChE) activity in climbing perch fingerlings (Anabas testudineus). The experiment was conducted under controlled laboratory conditions and included three treatments containing 0.173 mg/L of CPF, 1.137 mg/L of F, 0.173 mg/L of CPF + 1.137 mg/L of F (M), and a control. The inhibition of the brain AChE activity in fish exposed to F was weaker and shorter than in fish exposed to CPF. The inhibition by the mixture of CPF and F was significantly lower and less prolonged than the inhibition by only CPF but significantly higher than the inhibition by only F.

Organophosphates (e.g. chlorpyrifos ethyl) and carbamates (e.g. fenobucarb) are commonly used to control a wide range of pests in rice fields of the Mekong Delta in Vietnam. This study assesses the combined effect of chlorpyrifos ethyl (CPF) and fenobucarb (F), applied at concentrations used by rice farmers, on the brain acetylcholinesterase (AChE) activity in climbing perch fingerlings from rice fields in the Mekong Delta. It also investigates if Pyridine-2-aldoxime methiodide (2-PAM) can be used to reactivate cholinesterase that has been blocked by CPF. Three days after spraying, the water concentrations of both insecticides decreased quickly below the detection levels. However, the brain AChE activity in fish was inhibited for more than 7 days. The results indicate a quicker but less prolonged inhibition of the brain AChE activity by the mixture than by only CPF. The inhibition levels were above 70 % only during the first 48 h, which could explain why all fish survived. 2-PAM resulted in a significant reactivation of the cholinesterase blocked by a combination of CPF and F, and it is proposed that the reactivation by 2-PAM could provide a way to assess the AChE inhibition levels in organisms, when no unexposed individuals are available as controls. The results indicate that the current use of CPF and F in rice farming in the Mekong Delta is likely to cause negative effects on non-target organisms. Many of these effects may be sub-lethal, and there is a need to develop biomarkers that are relevant, inexpensive and easy to apply. The results show that brain AChE in climbing perch fingerling is a relevant biomarker for monitoring of exposure to, and sub-lethal impacts from organophosphates and carbamates under tropical conditions in developing countries.

With the expansion of agricultural areas within the Mekong River Delta in Vietnam, a concurrent, dramatic increase has occurred in agrochemical usage. To date, little consideration has been given to the negative impacts of this agricultural activity on the aquatic resources of the region. Both acute toxicity and subacute effects on brain cholinesterase (ChE) of two of the most commonly used insecticides, diazinon and fenobucarb, on adult native snakehead (Channa striata) were evaluated in a static, nonrenewable system, the environmental parameters of which, such as dissolved oxygen, water temperature, and pH, fluctuated similarly to field conditions. Four levels of insecticides, from 0.008 to 0.52 mg/L (for diazinon) and from 0.11 to 9.35 mg/L (for fenobucarb), were tested to assess the effects on the brain ChE activity of the snakehead up to 30 and 10 d for diazinon and fenobucarb, respectively. Diazinon was highly toxic to this fish species, with a 96-h median lethal concentration (LC50) of only 0.79 mg/L, and it also caused long-term ChE inhibition, with activity still significantly inhibited by 30% after 30 d for the three highest concentrations. Fenobucarb was less toxic to this species, with a 96-h LC50 of 11.4 mg/L. Fenobucarb caused more rapid ChE inhibition but also rapid recovery. The results of the present study indicate an urgent need to regulate the usage of these pesticides in the Mekong River Delta.
