BACKGROUND AND AIM: Forced-air drying (FAD) cabinets are recommended for storage of reprocessed endoscopes, but financial constraints prevent their universal application. The study aimed to determine bacterial contamination in flexible gastroscopes (FG) channels after storage, in a cabinet with filtered air and UV lights, but without FAD. METHODS: Eight FG in clinical use in an endoscopy service of a large Brazilian hospital were sampled: immediately "Time zero" (N = 50), 12 h "Time 1" (N = 25), and 60 h "Time 2" (N = 25) after reprocessing. Following a flush-brush-flush of channels, 40-mL sterile water and 3 cm of the brush were collected. Each sample was divided, filtered onto two 0.22-µm membranes, and incubated in media without or with disinfectant neutralizer. Automated method was used for identification and antibiotic resistance test of isolated bacteria. RESULTS: Bacterial contamination in times "1" and "2" was 5.9 and 16.1 times greater than that of "Time zero," respectively. Number of positive cultures in media with and without neutralizer was similar at times "1" and "2," while media with neutralizer produced more positive cultures at "Time zero." Most bacteria isolated at "Time 2" were Gram-negative rods (52.3%) and showed resistance to one or more antibiotics (65%). CONCLUSION: Bacterial contamination was detected on reprocessed FG stored in non-FAD cabinets overnight (12 h) and increased with longer storage time (60 h). The contamination source is likely to be bacteria in biofilm which multiply in the absence of FAD. Evidence-based criteria should be available for storage time according to the cabinet available.

PURPOSE: To verify the psychometric properties of the Brazilian-Portuguese version of the Falls Behavioral (FaB-Brazil) Scale in Parkinson's disease (PD). MATERIAL AND METHODS: Participants (n = 96) were assessed by disease-specific, self-report and functional mobility measures. Internal consistency of the FaB-Brazil scale was evaluated using Cronbach's alpha and inter-rater and test-retest reliability using intraclass correlation coefficients (ICC). The standard error of measurement (SEM), minimal detectable change (MDC), ceiling and floor effects, and convergent and discriminative validity were evaluated. RESULTS: Internal consistency was moderate (α = 0.77). Excellent inter-rater (ICC = 0.90; p < 0.001) and test-retest (ICC = 0.91; p < 0.001) reliability were found. The SEM was 0.20 and MDC was 0.38. Ceiling and floor effects were not found. Convergent validity was established by the positive correlations between the FaB-Brazil scale and age, modified Hoehn and Yahr, PD duration, Movement Disorders Society-Unified Parkinson's Disease Rating Scale, Motor Aspects of Experiences of Daily Living, Timed Up & Go and 8-item Parkinson's Disease Questionnaire, and negative correlations between the FaB-Brazil scale and community mobility, Schwab & England, and Activities-specific Balance Confidence scale. Females showed greater protective behaviors than males; recurrent fallers showed greater protective behaviors than non-recurrent fallers (p < 0.05). CONCLUSIONS: The FaB-Brazil scale is reliable and valid for assessing people with PD.


Fall-related behaviors should be part of the fall risk assessment of community-dwelling people with Parkinson's disease.The Brazilian-Portuguese version of the Fall Behavioral (FaB-Brazil) Scale is reliable and valid for assessing everyday behaviors and actions related to falling in community-dwelling people with Parkinson's disease.The FaB-Brazil scale may be used to tailor individualized fall prevention programs.

BACKGROUND: Rotary cutting instruments (RCIs) are sterilized routinely. The authors aimed to analyze the structural integrity, presence of dirt, and microbial contamination of RCIs used in clinical practice after processing. METHODS: Eighty-four RCIs (42 carbide burs, 42 diamond burs) were divided into baseline, control, and test groups. The RCIs were evaluated by means of scanning electron microscopy and microbiological analysis. Evaluation criteria included presence of structural damage, dirt, biofilm, and isolated cells and their phenotypic profile. RESULTS: The carbide burs from all groups and diamond burs from the test groups had structural damage. Dirt was observed in the baseline and test groups. Three bacterial species were isolated from 4 RCIs (9.52%). An isolated cell was observed from 1 carbide bur. Biofilm was observed on 3 RCIs (7.14%). CONCLUSIONS: RCIs should not be subjected to multiple uses; after the first clinical use they accumulate structural damage and dirt that hampers the cleaning step, causing failure in the sterilization process. PRACTICAL IMPLICATIONS: The presence of microorganisms and structural damage on the RCIs confirmed that they are not amenable to processing, a fact that characterizes them as a single-use health care product.

OBJECTIVE: Assess the accumulation of protein and biofilm on the inner surfaces of new flexible gastroscope (FG) channels after 30 and 60 days of patient use and full reprocessing. DESIGN: Clinical use study of biofilm accumulation in FG channels. SETTING: Endoscopy service of a public hospital. METHODS: First, we tested an FG in clinical use before the implementation of a revised reprocessing protocol (phase 1 baseline; n = 1). After replacement of the channels by new ones and the implementation of the protocol, 3 FGs were tested after 30 days of clinical use (phase 2; n = 3) and 3 FGs were tested after 60 days of clinical use (phase 3; n = 3), and the same FGs were tested in phase 2 and 3. Their biopsy, air, water, and air/water junction channels were removed and subjected to protein testing (n = 21), bacteriological culture (n = 21), and scanning electron microscopy (SEM) (n = 28). Air-water junction channels fragments were subjected to SEM only. RESULTS: For the FGs, the average number of uses and reprocessing cycles was 60 times. Extensive biofilm was detected in air, water, and air-water junction channels (n = 18 of 28). All channels (28 of 28) showed residual matter, and structural damage was identified in most of them (20 of 28). Residual protein was detected in the air and water channels of all FG evaluated (phases 1-3), except for 1 air channel from phase 2. Bacteria were recovered from 8 of 21 channels, most air or water channels. CONCLUSIONS: The short time before damage and biofilm accumulation in the channels was evident and suggests that improving the endoscope design is necessary. Better reprocessing methods and channel maintenance are needed.

Objective To perform the translation and cross-cultural adaptation to Brazilian Portuguese of the Non-Communicating Children's Pain Checklist - Postoperative Version (NCCPC-PV) instrument, which assesses acute pain in individuals with severe intellectual disability (ID) who present great cognitive impairment and inability to communicate (CIIC). Method In the adaptation process, the original NCCPC-PV was translated, back-translated, its versions were discussed by a committee of experts, and the resulting tool was tested in 20 health professionals and 20 caregivers of CCIC patients regarding its semantic clarity. Results Data from the present study and its participants were analyzed and their results were described. Thus, "Lista de Verificação de Dor em Crianças Não Comunicantes - Versão Pós-operatória" (Br-NCCPC-PV) was obtained as the final version in Brazilian Portuguese. Conclusion After the present study, the Br-NCCPC-PV was considered adequate for use in the Brazilian population.

A serious emerging problem worldwide is increased antimicrobial resistance. Acquisition of coding genes for evasion methods of antimicrobial drug mechanisms characterizes acquired resistance. This phenomenon has been observed in Enterobacteriaceae family. Treatment for bacterial infections is performed with antibiotics, of which the most used are beta-lactams. The aim of this study was to correlate antimicrobial resistance profiles in Enterobacteriaceae by phenotypic methods and molecular identification of 14 beta-lactamase coding genes. In this study, 70 exclusive isolates from Brazil were used, half of which were collected in veterinary clinics or hospitals Phenotypic methodologies were used and real-time PCR was the molecular methodology used, through the Sybr Green system. Regargding the results found in the tests it was observed that 74.28% were resistant to ampicillin, 62.85% were resistant to amoxicillin associated with clavalunate. The mechanism of resistance that presented the highest expression was ESBL (17.14%). The genes studied that were detected in a greater number of species were blaGIM and blaSIM (66.66% of the samples) and the one that was amplified in a smaller number of samples was blaVIM (16.66%). Therefore, high and worrying levels of antimicrobial resistance have been found in enterobacteria, and a way to minimize the accelerated emergence of their resistance includes developing or improving techniques that generate diagnoses with high efficiency and speed.

INTRODUCTION: Goiás State, which is in the midwest region of Brazil, has several urban forests. This fact, along with the expansion of urban areas within the limits of Forest Conservation Units, increases the contact between humans and wildlife, such as capuchin monkeys. The impulsive behavior of these animals and the scarcity of food cause them to vigorously search for food, leading to direct encounters with Park visitors, which can result in scratches and bites and making them potential disseminators of pathogenic microorganisms. METHODS: Ten specimens of bearded capuchin monkey (Sapajus libidinosus) were captured at the Onofre Quinan Environmental Park in Anápolis, Goiás, Brazil. Samples were collected from the monkeys, and the bacteria and fungi present in the samples were isolated and identified. Then, the identified microorganisms were subjected to antibiotic susceptibility testing. RESULTS: A total of 111 bacteria and 12 fungi were isolated, including two strict anaerobic bacteria of the genus Peptostreptococcus, 109 facultative anaerobic bacteria, and 12 yeasts. Among the facultative bacteria, enterobacteria and Staphylococcus were common. Resistance to tetracycline and ampicillin antibiotics was detected in the enterobacteria, and resistance to tetracycline, erythromycin, and clindamycin was detected in the Staphylococci. The other strains were sensitive to all tested antimicrobials. Cefoxitin showed 100% efficacy in all isolated bacteria. CONCLUSIONS: For bites from capuchin monkeys, we recommend performing complete hygiene and antibiotic therapy, according to medical recommendations. Given the 100% effectiveness of cefoxitin, it should be considered for this type of injury, especially in the study region.

BACKGROUND: The acquisition of reusable surgical instruments (RSIs) through loaner system is a worldwide phenomenon, particularly in orthopaedic surgeries. Loaner sets contain high-complex design RSIs, which are very difficult to clean, and also include single use implants, such as screws, that remain in the surgical tray and are subjected to multiple reprocessing until they are implanted. Inadequate cleaning of SI and various exposures of single-use implants to physical, chemical and biological agents can compromise their quality/safety and promote biofilm formation. The difficulty in cleaning is compounded in middle and low-income countries where automated cleaning infrastructure at sterilizing service units is often unavailable, and thus manual cleaning only is performed. Thus, we aimed to determine the condition of orthopaedic loaned sets when delivered to the hospital, assess the quality of complex-design RSIs reprocessed by manual cleaning, and evaluate the effect of multiple reprocessing on single-use implant (screw). METHODS: Flexible medullary reamers (FMRs), depth gauges and screws used for femur intramedullary nailing, in clinical use for >1 year, were obtained from three Brazilian loaner companies and assessed for residual ATP, protein, bacterial contamination, endotoxin and/or biofilm at delivery at the hospital, following cleaning and steam sterilisation. RESULTS: Before cleaning, blood was visible on a RSIs tray, and RSIs/screws were contaminated with high amounts of ATP, protein and bacteria. Visible soil was released during brushing of a FMR lumen and, following cleaning, the inner layer of 34/40 were visible soiled, and over 5/8 were protein test positive. Following sterilisation, biofilm and soil, including fragments appearing to be bone, were detected by scanning electron microscopy on RSIs/screws. A sterilised FMR revealed visible soil on the inner layer. Endotoxin tests were negative. CONCLUSION: The contaminated condition of loaned-complex-designed RSIs/screws upon arrival at the hospital and after reprocessing points to the insufficiency of manual reprocessing and management practices related to this instruments/implants. A multidisciplinary approach involving expert in design/manufacture, regulating, managing, reprocessing and surgeons is suggested to improve RSIs manufacture that enables complete decontamination and maintain the surgical patient safety.

Resumo Objetivo Avaliar a integridade da superfície e as condições microbiológicas de parafusos prontos para uso em bandejas ortopédicas após múltiplos processamentos. Métodos Após o processamento completo, as bandejas utilizadas em cirurgias de pequenos fragmentos, fornecidas por meio de sistema de consignação/comodato em um hospital brasileiro, foram selecionadas aleatoriamente durante quatro meses. Os parafusos mais utilizados (números 14, 16 e 18 - Grupo 1) e menos utilizados (números 10 e 38 - Grupo 2), portanto, os mais e menos expostos a agentes biológicos, químicos e físicos, foram aleatoriamente removidos e submetidos a inspeção visual (n=126), seguido de cultura bacteriana (n=6 parafusos/bandeja, 9 bandejas), teste de proteínas (n=6 parafusos/bandeja, 9 bandejas) e Microscopia Eletrônica de Varredura (MEV) (n=2 parafusos/bandeja, 9 bandejas). As culturas positivas foram submetidas a métodos automatizados de identificação bacteriana e suscetibilidade antimicrobiana. Resultados Foram detectadas ranhuras em 8,7% dos parafusos, predominantemente no Grupo 2 (8/11). Proteína residual foi detectada em 96,3%, e não houve diferença estatisticamente significativa na quantidade de proteína entre os grupos (P=0,07). Crescimento bacteriano foi identificado em 3/54 parafusos. Danos na superfície e presença de sujidade foram visualizados em todos os parafusos submetidos a MEV. Formação de biofilmes extensos foi detectada em oito parafusos, três do Grupo 1 e cinco do Grupo 2. Conclusão Recuperação de bactérias viáveis, acúmulo de biofilme e danos na superfície foram detectados nos parafusos prontos para uso. Os parafusos costumam permanecer nas bandejas cirúrgicas e serem submetidos a múltiplos processamento, sendo expostos a contaminação e danos repetidas vezes. Esses achados apontam para a necessidade de discutir e repensar a forma como esses implantes de uso único são atualmente disponibilizados para cirurgias.

Métodos Después del procesamiento completo, fueron seleccionadas aleatoriamente durante cuatro meses las bandejas utilizadas en cirugías de pequeños fragmentos, proporcionadas mediante el sistema de consignación/comodato en un hospital brasileño. Los tornillos más utilizados (números 14, 16 y 18 - Grupo 1) y menos utilizados (números 10 y 38 - Grupo 2), por lo tanto, los más y menos expuestos a agentes biológicos, químicos y físicos, fueron quitados aleatoriamente y sometidos a inspección visual (n=126), seguido de cultivo bacteriano (n=6 tornillos/bandeja, 9 bandejas), prueba de proteínas (n=6 tornillos/bandeja, 9 bandejas) y microscopía electrónica de barrido (MEB) (n=2 tornillos/bandeja, 9 bandejas). Los cultivos positivos fueron sometidos a métodos automatizados de identificación bacteriana y susceptibilidad antimicrobiana. Resultados Se detectaron ranuras en el 8,7 % de los tornillos, predominantemente en el Grupo 2 (8/11). Se detectó proteína residual en el 96,3 % y no se encontró diferencia estadísticamente significativa en la cantidad de proteína entre los grupos (P=0,07). En 3/54 tornillos se identificó crecimiento bacteriano. Se visualizaron daños en la superficie y presencia de suciedad en todos los tornillos sometidos a MEB. En ocho tornillos se detectó la formación de biopelículas, tres del Grupo 1 y cinco del Grupo 2. Conclusión Se detectó recuperación de bacterias viables, acumulación de biopelícula y daños en la superficie en los tornillos listos para uso. Los tornillos suelen permanecer en las bandejas quirúrgicas y son sometidos a múltiples procesamientos, donde están expuestos a contaminación y daños repetidas veces. Estos descubrimientos señalan la necesidad de discutir y repensar la forma como estos implantes de uso único se ponen a disposición para cirugía actualmente.


Abstract Objective Assess the surface integrity and microbiological conditions of patient-ready screws in orthopaedic trays that had been multiply reprocessed. Methods After full reprocessing, clinical trays used for small fragment surgery provided through a loaner system to a Brazilian hospital were randomly selected during four months. The most (numbers 14, 16 and 18 - Group 1) and least (numbers 10 and 38 - Group 2) frequently implanted screws, therefore, the ones estimated to be the most and least exposed to biological, chemical and physical agents, were randomly removed and subjected to visual inspection (n=126), followed by bacterial culture (n=6 screws/tray, 9 trays), protein test (n=6 screws/tray, 9 trays) and Scanning Electron Microscopy (SEM) (n=2 screws/tray, 9 trays). Positive cultures were subjected to automated bacterial identification and antimicrobial susceptibility tests. Results Grooves were detected on 8.7% screws, predominantly in Group 2 (8/11). Residual protein was detected on 96,3%, and there was no statistically significant difference in the amount of protein between the groups (P=0.07). Bacterial growth was identified in 3/54 screws. Surface damage and soil were visualized on all screws subjected to SEM. Extensive biofilms were detected on eight screws, three from Group 1 and five from Group 2. Conclusion Recovery of bacteria, biofilm accumulation and surface damage were detected on patient-ready screws. Screws frequently remain in surgical trays for multiple reprocessing; thus they are repeatedly exposed to contamination and damage. These findings point to the need to discuss and review the way these single-use implants are currently made available for surgeries.

Resumo Objetivo Realizar a tradução e a adaptação transcultural para o português falado no Brasil do instrumento "Non-Communicating Children's Pain Checklist - Postoperative Version" (NCCPC-PV), destinado a avaliar a dor aguda em indivíduos com deficiência intelectual (DI) grave que apresentam grande comprometimento cognitivo e incapacidade de comunicação (CCIC). Método No processo de adaptação utilizado, o NCCPC-PV original foi traduzido, retraduzido, suas versões foram discutidas por um comitê de especialistas, e a ferramenta resultante foi testada em 20 profissionais de saúde e 20 cuidadores de pacientes com CCIC quanto à sua clareza semântica. Resultados Os dados deste estudo e de seus participantes foram analisados, e seus resultados foram descritos. Dessa maneira, obteve-se a Lista de Verificação de Dor em Crianças Não Comunicantes - Versão Pós-operatória (Br-NCCPC-PV) como a versão final para o português falado no Brasil. Conclusão Após este estudo, a Br-NCCPC-PV foi considerada adequada para o uso na população brasileira.

A Escala de Mobilidade Hospitalar (EMH) avalia de forma específica a mobilidade de pacientes após AVC no ambiente hospitalar e em estudo prévio foi demonstrada a sua concordância interexaminadores, validade preditiva e a responsividade a mudanças na fase aguda. OBJETIVO: Avaliar a validade concorrente da EMH e a sua confiabilidade ao ser aplicada através de entrevista. MATERIAIS E MÉTODOS: Trata-se de um estudo de validação, desenvolvido com pacientes internados em uma Unidade de AVC na cidade de Salvador-Bahia. Para avaliar a validade concorrente da EMH comparamos o seu escore com a pontuação da Escala de Rankin modificada (ERm) utilizando o teste de Spearman. Ambas escalas foram aplicadas no quinto dia após o AVC pelo mesmo pesquisador, previamente treinado. Para avaliação da confiabilidade da EMH quando aplicada através de entrevista, a escala foi aplicada por dois examinadores em turnos diferentes, no mesmo dia. O primeiro examinador aplicou a escala através da observação do desempenho e o segundo examinador através de entrevista. Utilizamos para esta análise o Coeficiente de Correlação Intraclasse (CCI). RESULTADOS: A pontuação total da EMH apresentou uma correlação positiva muito forte com a ERm (r=0,90) e também foi encontrada uma correlação significativa entre os subitens da EMH e a ERm. Ao comparar a aplicação da EMH através da observação do desempenho e aplicação por entrevista, observamos uma excelente concordância interexaminadores (CCI > 0,90). CONCLUSÃO: A Escala de Mobilidade Hospitalar, projetada especificamente para pacientes após AVC, mostrou um alto grau de validade concorrente e se mostrou confiável quando aplicada através de entrevista.


The Hospital Mobility Scale (HMS) evaluates the mobility of stroke patients in the hospital environment and in a previous study showed its inter-examiner agreement, predictive validity and responsiveness to changes in the acute phase. OBJECTIVE: To evaluate the concurrent validity of HMS and its reliability when applied by interview. METHODS: This is a validation study, that was developed with patients admitted in a stroke unit, in the city of Salvador, Bahia, Brazil. In order to assess the concurrent validity of the HMS, we compared its score with the modified Rankin Scale (mRS) score by using the spearman test. The same researcher applied both scales on the fifth day after stroke. To assess the reliability of HMS when applied by interview, two examiners applied the scale in different shifts, on the same day. The first evaluation was face-to-face and the second was performed by interview. For this analysis, we used the intraclass correlation coefficient (ICC). RESULTS: The HMS showed a very strong positive correlation with the mRS (r = 0.90) and a significant correlation was also found between the sub-items of the HMS and the mRS. We found excellent inter-examiner agreement between face-to-face and interview assessment (ICC>0.90). CONCLUSION: The hospital mobility scale, that was developed specifically for stroke patients, showed a high degree of concurrent validity and was reliable when applied by interview.

This cross-sectional study, performed in an oncology hospital in Goiania, aimed to characterize the prevalence of oral colonization and antimicrobial susceptibility of Pseudomonas spp. isolated from the saliva of healthcare workers. Microorganisms were subjected to biochemical tests, susceptibility profile, and phenotypic detection. Of 76 participants colonized with Gram negative bacilli, 12 (15.8%) harbored Pseudomonas spp. Of all isolates, P. aeruginosa (75.0%), P. stutzeri (16.7%), and P. fluorescens (8.3%), were resistant to cefoxitin, and therefore likely to be AmpC producers. The results are clinically relevant and emphasize the importance of surveillance to minimize bacterial dissemination and multiresistance.

The evaluation of workers as potential reservoirs and disseminators of pathogenic bacteria has been described as a strategy for the prevention and control of healthcare-associated infections (HAIs). The aim of this study was to evaluate the presence of Enterobacteriaceae in the oral cavity of workers at an oncology hospital in the Midwest region of Brazil, as well as to characterize the phenotypic profile of the isolates. Saliva samples of 294 workers from the hospital's healthcare and support teams were collected. Microbiological procedures were performed according to standard techniques. Among the participants, 55 (18.7%) were colonized by Enterobacteriaceae in the oral cavity. A total of 64 bacteria were isolated, including potentially pathogenic species. The most prevalent species was Enterobacter gergoviae (17.2%). The highest rates of resistance were observed for ß-lactams, and 48.4% of the isolates were considered multiresistant. Regarding the enterobacteria isolated, the production of ESBL and KPC was negative. Nevertheless, among the 43 isolates of the CESP group, 51.2% were considered AmpC ß-lactamase producers by induction, and 48.8% were hyper-producing mutants. The significant prevalence of carriers of Enterobacteriaceae and the phenotypic profile of the isolates represents a concern, especially due to the multiresistance and production of AmpC ß-lactamases.

Abstract INTRODUCTION: Goiás State, which is in the midwest region of Brazil, has several urban forests. This fact, along with the expansion of urban areas within the limits of Forest Conservation Units, increases the contact between humans and wildlife, such as capuchin monkeys. The impulsive behavior of these animals and the scarcity of food cause them to vigorously search for food, leading to direct encounters with Park visitors, which can result in scratches and bites and making them potential disseminators of pathogenic microorganisms. METHODS: Ten specimens of bearded capuchin monkey (Sapajus libidinosus) were captured at the Onofre Quinan Environmental Park in Anápolis, Goiás, Brazil. Samples were collected from the monkeys, and the bacteria and fungi present in the samples were isolated and identified. Then, the identified microorganisms were subjected to antibiotic susceptibility testing. RESULTS: A total of 111 bacteria and 12 fungi were isolated, including two strict anaerobic bacteria of the genus Peptostreptococcus, 109 facultative anaerobic bacteria, and 12 yeasts. Among the facultative bacteria, enterobacteria and Staphylococcus were common. Resistance to tetracycline and ampicillin antibiotics was detected in the enterobacteria, and resistance to tetracycline, erythromycin, and clindamycin was detected in the Staphylococci. The other strains were sensitive to all tested antimicrobials. Cefoxitin showed 100% efficacy in all isolated bacteria. CONCLUSIONS: For bites from capuchin monkeys, we recommend performing complete hygiene and antibiotic therapy, according to medical recommendations. Given the 100% effectiveness of cefoxitin, it should be considered for this type of injury, especially in the study region.

The work conditions of dental surgeons (DS), associated with low compliance to precautionary standards, often lead to the colonization and dissemination of infectious agents. To assess the epidemiological and microbiological aspects of nasal colonization by Gram-negative bacteria in DS while teaching (dentistry teachers). The data were collected by application of a questionnaire and a nasal swab. The biochemical identification, the susceptibility profile and the detection of ß-lactamases were carried out in Vitek 2 compact®. 41 (77.3%) DS participated in the study, nine of them (22.0%) presented nasal colonization by Enterobacteriaceae, the participants were predominantly men (27/65.9%), over 50 years of age (24/58.5%). All of them confirmed using procedure gloves and a surgical mask while attending patients, with frequent (95.5%) hand washing although a statistical difference was found regarding personal habits (p=0.03). Enterobacter aerogenes (60.0%) was the most prevalent species, followed by Citrobacter koseri (20.0%). The intrinsic production of AmpC ß-lactamase by E. aerogenesspecies, which is multiresistant to antimicrobials, was present in the nasal cavity of 14.6% of the DS. There were high levels of nasal colonization by Enterobacteriaceae in teaching DS (22.0%), 14.6% had been colonized by multiresistant microorganisms and the results were associated with inadequate personal habits.

The aim of the study was to analyze epidemiological and microbiological aspects of oral colonization by methicillin-resistant Staphylococcus of health care workers in a cancer hospital. Interview and saliva sampling were performed with 149 health care workers. Antimicrobial resistance was determined by disk diffusion and minimum inhibitory concentration. Polymerase Chain Reaction, Internal Transcribed Spacer-Polymerase Chain Reaction and Pulsed Field Gel Electrophoresis were performed for genotypic characterization of methicillin-resistant Staphylococcus. Risk factors were determined by logistic regression. Methicillin-resistant Staphylococcus colonization prevalence was 19.5%, denture wearing (p = 0.03), habit of nail biting (p = 0.04) and preparation and administration of antimicrobial (p = 0.04) were risk factors identified. All methicillin-resistant Staphylococcus were S. epidermidis, 94.4% of them had mecA gene. Closely related and indistinguishable methicillin-resistant S. epidermidis were detected. These results highlight that HCWs which have contact with patient at high risk for developing infections were identified as colonized by MRSE in the oral cavity, reinforcing this cavity as a reservoir of these bacteria and the risk to themselves and patients safety, because these microorganisms may be spread by coughing and talking.

Resumo Objetivo: Determinar a carga microbiana de tubos de silicone imediatamente após a limpeza e em diferentes intervalos de armazenamento. Métodos: Estudo experimental que analisou tubos de silicone oriundos da assistência ao paciente cirúrgico. Foi conduzido após aprovação do Comitê de Ética (protocolo n° 1.277.077), no período de setembro a novembro de 2015, com tubos oriundos do Centro de Material e Esterilização (CME) de um hospital geral de grande porte da região Centro-Oeste do Brasil. Os tubos foram segmentados: extremidade 01, 02 e meio e novamente segmentados, conforme intervalos de tempo preestabelecidos em zero, 12 e 24 horas. Os fragmentos foram preenchidos com água estéril, vedados e submetidos a cinco minutos de sonicação. A água foi filtrada em Millipore 0,45 µm e as membranas incubadas a 35°C por 24 horas em ágar nutriente. As membranas foram removidas e dispostas em tubos de ensaio, contendo 1mL de solução salina, que foram agitadas por cinco minutos e submetidos a técnica de alça calibrada. Resultados: Houve aumento da carga microbiana na ordem de uma grandeza na escala logarítmica a cada 12 horas (p<0,05), nas condições de limpeza e armazenamento proporcionados pela instituição, nos grupos experimental e controle positivo, e não houve diferença quando comparados o meio e extremidades dos tubos de silicone (p>0,05) nos períodos zero, 12 e 24 horas. Conclusão: A depender da carga microbiana inicial, o aumento da ordem uma grandeza pode resultar no insucesso da esterilização, achados que ratificam a não permanência de PPS na área limpa aguardando o processamento.

Métodos: Estudio experimental en el que se analizaron tubos de silicona propios de la asistencia al paciente quirúrgico, en el período de septiembre a noviembre de 2015. Los tubos provenían del Centro de Material y Esterilización (CME) de un hospital general de gran tamaño de la región Centro-Oeste de Brasil. Los tubos fueron segmentados así: extremo 01, 02 y medio y nuevamente segmentados según intervalos de tiempo preestablecidos en cero, 12 y 24 horas. Los fragmentos se llenaron con agua estéril, fueron sellados y sometidos a cinco minutos de sonicación. El agua fue filtrada en Millipore 0,45 μm y las membranas incubadas a 35°C por 24 horas en agar nutriente. Las membranas fueron removidas y dispuestas en tubos de ensayo que contenían 1mL de solución salina, fueron agitados durante cinco minutos y sometidos a técnica de alza calibrada Resultados: Se observó un aumento de la carga microbiana en el orden de una magnitud en la escala logarítmica cada 12 horas (p <0,05), en las condiciones de limpieza y almacenamiento proporcionadas por la institución, en los grupos experimental y de control positivo. No hubo diferencia cuando se compararon el medio y los extremos de los tubos de silicona (p> 0,05) en los períodos cero, 12 y 24 horas. Conclusión: Dependiendo de la carga microbiana inicial, el aumento del orden de una magnitud puede resultar en el fracaso de la esterilización. Estos hallazgos ratifican la no permanencia de PPS en el área limpia mientras se aguarda el procesamiento.


Abstract Objective: To determine the microbial load of silicone tubes, immediately after cleaning, and at different storage intervals. Methods: Experimental study that analyzed silicone tubes from surgical patient care, conducted after approval by the Ethics Committee (protocol no. 1,277,077), from September to November of 2015, with tubes from the Central Processing Department (CPD) of a large general hospital in the West Central region of Brazil. The tubes were segmented (end 1 and 2, and the middle) and were then segmented again, according to established time intervals (zero, 12, and 24 hours). The fragments were filled with sterile water, sealed, and exposed to five minutes of sonication. The water was filtered via 0.45μm Millipore, and the membranes were incubated at 35°C for 24 hours, on nutrient agar. The membranes were removed and placed in test tubes containing 1mL of saline, which were mixed for five minutes, and subjected to a calibrated loop technique. Results: An increase in microbial load was identified, in the order of a logarithmic magnitude every 12 hours (p<0.05), in the cleaning and storage conditions provided by the institution, in the experimental and positive control groups, and no difference was identified when comparing the middle and ends of the silicone tubes (p> 0.05) at periods zero, 12, and 24 hours. Conclusion: Depending on the initial microbial load, an increase in the order of magnitude can result in sterilization failure, which corroborates the need to not maintain healthcare products in the storage place while awaiting processing.

Objetivou-se determinar a prevalência de trabalhadores colonizados por Staphylococcus sp., identificar preditores à colonização e avaliar a suscetibilidade aos antimicrobianos dos isolados. Realizou-se aplicação de questionário e coleta de saliva de 130 trabalhadores de áreas de apoio de uma instituição oncológica. Análise microbiológica foi realizada segundo procedimentos padronizados. A prevalência de trabalhadores colonizados por Staphylococcus sp. foi 37,7%, sendo a maioria dos isolados Staphylococcus coagulase-negative. Resistência à meticilina foi detectada em 35,1% dos Staphylococcus coagulase-negative, sendo 12 S. epidermidis e um S. haemolyticus, destes 92,3% possuíam gene mecA. Todos S. aureus foram sensíveis à meticilina. Turno e setor de trabalho foram identificados como preditores para colonização. A colonização de trabalhadores de áreas de apoio que assistem pacientes oncológicos indica que também sejam alvos de políticas de saúde ocupacional que deveriam incluir medidas pré e pós-colonização a serem investigadas/discutidas em estudos futuros com vistas à segurança do trabalhador e do paciente


The objectives were to determine the prevalence of workers colonized by Staphylococcus sp., to identify colonization predictors and, to assess their susceptibility to antimicrobials. A questionnaire was applied and saliva was collected in 130 workers from support areas of an oncology institution. Microbiological analysis was conducted following standard procedures. The prevalence of workers colonized by Staphylococcus sp. was 37.7%, and the isolated majority was Staphylococcus coagulase-negative. Resistance to methicillin was detected in 35.1% of Staphylococcus coagulase-negative, being 12 epidermidis and one S. haemolyticus. From those. 92.3% had the mecA. gene. All S. aureus were sensitive to methicillin. Working shift and sector were identified as predictors for colonization. The colonization in workers from support areas assisting oncology patients indicates that workers should be target of occupational health policies, and it should include pre- and post-colonization measures in investigations/discussions of future studies aiming at worker and patient safety

Objetivo: Identificar a presença de contaminação em torniquetes para punção intravenosa periférica e caracterizar o perfil dos Staphylococcus spp. e leveduras isolados. Métodos Estudo transversal que inseriu análise de 18 torniquetes para punção intravenosa periférica em uso no hospital. Os torniquetes foram imersos em caldo BHI por 24h e cultivados em meios seletivos para isolamento e identificação de Staphylococcus spp. e leveduras. O método disco-difusão foi empregado para analisar o perfil de suscetibilidade dos Staphylococcus spp. aos antimicrobianos. Resultados Treze (72,2%) torniquetes apresentaram crescimento de algum micro-organismo sendo 11 (52,4%) Staphylococcus coagulase-negativo, dois (9,5%) Staphylococcusaureus, quatro (19%) Rodothorulamucilaginosa, três (14,3%) Candidaalbicans. 61,5% dos Staphylococcus spp. apresentaram resistência a oxacilina. Os profissionais da equipe não relataram protocolos para limpeza, desinfecção ou substituição controlada destes materiais na instituição. Conclusão Foi identificada a contaminação de torniquetes por micro-organismos patogênicos com perfil de resistência aos antibióticos muito utilizados em instituições hospitalares.


Objectives: To identify the presence of contamination on tourniquets for peripheral intravenous puncture and to characterize the profile of the Staphylococcus spp. and the isolated yeasts. Methods Cross-sectional study in which 18 tourniquets for peripheral intravenous puncture in use at a hospital were analyzed. The tourniquets were immersed in BHI broth for 24h and cultivated in selective media for isolation and identification of Staphylococcus spp. and yeasts. The disk-diffusion method was employed to analyze the susceptibility profile of the Staphylococcus spp. to the antimicrobial agents. Results The growth of some microorganism was identified on 13 (72.2%) tourniquets: 11 (52.4%) coagulase-negative Staphylococcus, two (9.5%) Staphylococcus aureus, four (19%) Rodothorula mucilaginosa, three (14.3%) Candida albicans. 61.5% of the Staphylococcus spp. were oxacillin-resistant. The team professionals did not mention protocols for cleaning, disinfection or controlled replacement of these materials at the institution. Conclusion The contamination of tourniquets by pathogenic microorganisms was identified, with a resistance profile to the antibiotics that are frequently used in hospitals.

A investigação de trabalhadores dos serviços de saúde como reservatório e disseminadores de bactérias patogênicas tem sido referida como estratégia de prevenção e controle das infecções relacionadas à assistência à saúde. Este estudo buscou avaliar a presença de Enterobacteriaceae na cavidade bucal de trabalhadores de hospital oncológico do Centro-Oeste brasileiro, bem como caracterizar o perfil fenotípico dos isolados. Foi coletada amostra de saliva de 294 trabalhadores pertencentes às equipes de saúde e de apoio. Procedimentos microbiológicos foram realizados segundo técnicas referendadas. Dentre os participantes, 55 (18,7%) estavam colonizados por Enterobacteriaceae na cavidade bucal. Foram isoladas 64 bactérias, incluindo espécies potencialmente patogênicas. A espécie mais prevalente foi Enterobacter gergoviae (17,2%). As maiores taxas de resistências foram observadas para os β-lactâmicos e 48,4% dos isolados foram considerados multirresistentes. Para as enterobactérias pesquisadas, a produção de ESBL e KPC foi negativa. Porém, dentre os 43 isolados do grupo CESP, 51,2% foram considerados produtores de β-lactamase AmpC por indução e 48,8% mutantes hiperprodutores. Considera-se a prevalência de portadores de Enterobacteriaceae significativa e o perfil fenotípico dos isolados preocupante, especialmente pela multirresistência e produção de β-lactamases AmpC.
