RESUMO

Objectives: To evaluate the clinical value of intravesical gemcitabine combined with immunotherapy in patients with non-muscle-invasive bladder carcinoma (NMIBC) after transurethral resection of bladder tumor (TURBT). Methods: Eighty patients with non-muscle-invasive urothelial carcinoma treated in Baoding No.1 Hospital from November 2016 to November 2019 were randomly divided into two groups, with 40 patients in each group. Both groups underwent TURBT. After surgery, the research group was treated with intravesical chemotherapy using gemcitabine combined with ubenimex, while the control group was given 40 mg pirarubicin by intravesical instillation. Postoperative condition was evaluated by cystoscopy every three months in both groups. The recurrence six months, one year and two years after treatment, the incidence of lower urinary tract symptoms such as dysuria, hematuria and frequent urination, general adverse drug reactions such as rashes, liver function damage and gastrointestinal reaction, as well as the changes in CD3+, CD4+, CD8+ and CD4+/CD8+ T lymphocyte subsets before and after treatment were comparatively analyzed between the two groups. Results: The recurrence rate showed no statistical significance between the two groups 6 months after treatment (p=0.17), but significant differences one year (p=0.04) and two years (p=0.03) after treatment, which were significantly lower in the research group than the control group. The incidence of adverse drug reactions was 22.5% in the research group and 7.5% in the control group, without significant difference (p=0.36). The incidence of lower urinary tract symptoms was 32.5% and 55%, respectively, in the research group and the control group. The incidence of lower urinary tract symptoms in the research group was significantly lower compared with the control group, with a statistically significant difference (p=0.04). After treatment, CD3+, CD4+ and CD4+/CD8+ levels in the research group increased significantly than those in the control group, with statistically significant differences (CD3+, p=0.01; CD4+, p=0.00; CD4+/CD8+, p=0.00). Conclusions: For NMIBC patients receiving bladder-preserving surgery, intravesical gemcitabine combined with immunotherapy can reduce the recurrence rate, relieve lower urinary tract symptoms, increase the tolerance of patients to intravesical chemotherapy and significantly improve the function of T lymphocytes, without obvious increase in adverse drug reactions. Therefore, it is safe and effective, and has certain clinical value.

RESUMO

The COVID-19 pandemic has hit the hospitality industry hard globally, resulting in millions of employees being laid off. Drawing upon the conservation of resources theory, this study aims to empirically examine how and when COVID-19-induced layoff influences employees' in-role and extra-role performance in the hospitality industry. We tested this model by using field data collected from 302 employees and their supervisors in China across two waves. Results revealed that COVID-19-induced layoff increases survivors' COVID-19-related stress, which in turn leads to decreased in-role and extra-role performance. The strength of these indirect effects is mitigated by perceived family support against COVID-19. Unexpectedly, perceived organizational support against COVID-19 intensifies these indirect effects. The theoretical and practical implications of this study are further discussed.

RESUMO

AIMS: The aim was to examine the reciprocal relationships of emotional labour strategies with emotional exhaustion and professional identity. DESIGN: This study adopted a four-wave cross-lagged panel design. METHODS: Survey data were collected in 2018 from a sample of 171 newly hired nurses from 58 hospitals in 11 provinces of China. Nurses' emotional labour (i.e., deep acting and surface acting), emotional exhaustion and professional identity were repeatedly measured. Cross-lagged panel analyses were conducted to examine the reciprocal relationships we hypothesized. RESULTS: We found that emotional exhaustion was positively related to surface acting (but not vice versa); deep acting was negatively related to emotional exhaustion (but not vice versa); professional identity was positively related to deep acting (but not vice versa). CONCLUSION: Our findings suggest that deep acting and professional identity may decrease the level of emotional exhaustion, whereas emotionally exhausted nurses are more likely to employ surface acting strategies. IMPACT: This research finding will have an impact on the nursing management. Healthcare managers may consider workshops or training and development programs that promote nurses' professional identify to promote nurses' use of deep acting and consequently reduce their level of emotional exhaustion, which has been associated with a variety of negative consequences, such as low quality of patient service, high medical accidents, and turnover rate.

RESUMO

OBJECTIVE: To assess the feasibility, the safety and short-term outcomes of laparoscopic radical rectal surgery for rectal cancer patients with increased body mass index (BMI) . METHODS: Retrospectively data reviews were conducted for 405 consecutive patients undergoing laparoscopic surgery for rectal cancer from June 2008 to June 2012. They were classified as normal-weight (NW, BMI 18.6-22.9 kg/m(2), n = 165), overweight (OW, BMI 23.0-24.9 kg/m(2), n = 125), and obese (OB, BMI ≥ 25.0 kg/m(2), n = 115)groups according to the categories as proposed by 2007 Chinese Obesity Surgery Treatment Guidelines. The differences of oncologic, intraoperative and postoperative status, postoperative complications, number of resected lymph nodes and short-term survival rates were compared among three groups. The data were analyzed by χ(2) or Fisher exact test. The Mann-Whitney U test or analysis of variance (one-way ANOVA) was used for parametric comparisons. The survival curve was drawn by Kaplan Meier method and the survivals of 3 groups were by the Log-rank test. RESULTS: The comorbidity of patients in the NW and OW groups were less than that in the OB group(27.9% (46/165) and 30.4% (38/125) vs 47.0% (54/115), χ(2) = 12.066, P < 0.05). No significant difference existed among the groups in terms of conversion rate (9.1% (15/165), 10.4% (13/125) and 12.2% (14/115)), the rate of postoperative complications (20.6% (34/165), 21.6% (27/125) and 24.3% (28/115) ), intraoperative volume of blood loss ((105 ± 30), (110 ± 25) and (115 ± 45) ml), first flatus( (2.8 ± 1.2), (2.9 ± 1.1) and (3.1 ± 1.4) d), postoperative hospital stays ((13.7 ± 5.5), (14.3 ± 7.5) and (14.1 ± 8.5) d, all P > 0.05), and the mean number of retrieved lymph nodes(P > 0.05). While the operation duration in the OB group were longer than that in the NW and OW groups ((250 ± 35) vs (205 ± 20) and (210 ± 30) min, F = 7.216, P < 0.05) . And 368 patients (90.9%) were followed up for a median of 24 months(2-48 months). As for survival curves, no significant difference existed among three groups (P > 0.05). CONCLUSIONS: It is both safe and feasible for obese patients with increased BMI to undergo laparoscopic radical rectal cancer. And there is no effect upon immediate survival.

RESUMO

Increased use and implementation of automation, accelerated by the COVID-19 pandemic, gives rise to a new phenomenon: occupation insecurity. In this paper, we conceptualize and define occupation insecurity, as well as develop an Occupation Insecurity Scale (OCIS) to measure it. From focus groups, subject-matter expert interviews, and a quantitative pilot study, two dimensions emerged: global occupation insecurity, which refers to employees' fear that their occupations might disappear, and content occupation insecurity, which addresses employees' concern that (the tasks of) their occupations might significantly change due to automation. In a survey-study sampling 1373 UK employees, psychometric properties of OCIS were examined in terms of reliability, construct validity, measurement invariance (across gender, age, and occupational position), convergent and divergent validity (with job and career insecurity), external discriminant validity (with organizational future time perspective), external validity (by comparing theoretically secure vs. insecure groups), and external and incremental validity (by examining burnout and work engagement as potential outcomes of occupation insecurity). Overall, OCIS shows good results in terms of reliability and validity. Therefore, OCIS offers an avenue to measure and address occupation insecurity before it can impact employee wellbeing and organizational performance.

RESUMO

OBJECTIVE: To investigate the safety, feasibility and short-term efficacy of laparoscopic surgery in patients with middle-low rectal cancer. METHODS: From January 2008 to January 2010, a total of 108 patients with middle-low rectal cancer at Department of Gastrointestinal Surgery, Affiliated Tumor Hospital, Xinjiang Medical University were divided into 2 groups by different treatments: laparoscopic resection group (LR, n = 63) and conventional open resection group (OR, n = 45). The choice of two surgical options was based on patient's desire. The outcomes of two patient groups with radical surgery were prospectively evaluated. RESULTS: Seven cases were converted into open surgery from the laparoscopic group. The operative durations in the LR and OR groups were (246 ± 57) min and (229 ± 53) min respectively (P > 0.05). The laparoscopic group had a lesser amount of blood loss during surgery ((51 ± 20) ml vs (110 ± 41) ml, P < 0.05). The time needed for the recovery of gastrointestinal functions in the laparoscopic group was significantly shorter than that in the open surgery group ((3.0 ± 0.8) d vs (3.7 ± 1.3) d, P < 0.05). In terms of the length of specimen, the range of radical surgery and the rate of complication, no significant difference were found between two groups (all P > 0.05). The numbers of dissected lymph nodes were 13 and 12 cases respectively (P > 0.05). No instance of incision implantation, local recurrence or short-term death was found in neither group. There was 2 cases with lung metastasis in the LR group and 2 cases with the metastasis of liver and lung in the OR group. CONCLUSION: As a safe, feasible, effective and mini-invasive with similar radical resection and satisfactory short-term outcomes, laparoscopic surgery for low-middle rectal cancer has better prospects in the future.

RESUMO

As research regarding the targets and initiators of workplace gossip is gaining traction, one perspective that remains overlooked is the gossip receiver. Organizational newcomers are a particularly relevant population to study the impact of receiving negative gossip on because they use social information to navigate an unfamiliar organizational terrain. We propose a parallel moderated mediation model in which receiving negative gossip has contradicting effects on newcomer job anxiety through perceived social inclusion and negative rumination, and agreeableness as a boundary condition of the effects of receiving negative gossip. We collected data from 202 newcomers using a four-wave time-lagged design and found that receiving negative gossip increased newcomer job anxiety via negative rumination but did not decrease job anxiety via perceived social inclusion. Further, agreeableness moderated the effect of receiving negative gossip on negative rumination (but not perceived social inclusion) such that the effect of receiving negative gossip on negative rumination was stronger for less agreeable newcomers. Lastly, the indirect effect of receiving negative gossip on job anxiety via negative rumination was stronger for less agreeable newcomers. Theoretical and practical implications specific to gossip and newcomers are discussed.

RESUMO

Using smartphone for work in off-work hours has been associated with negative employee outcomes. We postulate that: (1) organizational norm of connectivity precedes employees' work-related smartphone use after work, which predicts emotional exhaustion among employees; and (2) this process is moderated by employees' workaholism. We collected data from 399 full-time employees who completed six online surveys over 3 workdays to test our hypotheses. Multilevel path analysis of these data revealed that organizational norm of connectivity was more strongly related to smartphone use after work among employees with high workaholism compared to those with low workaholism. However, smartphone use was more strongly related to emotional exhaustion among those with low workaholism compared to those with high workaholism. In total, the indirect effect of organizational norm of connectivity on emotional exhaustion through work-related smartphone use after work was significant only for those with low workaholism. These findings highlight the role of organizational norm of connectivity and individual workaholism in the association of work-related smartphone use after work and negative employee outcomes. We discussed the theoretical and practical implications of these findings.

RESUMO

Today's workers around the world are experiencing growing uncertainty about their future employment. Living in the chronic threat to the continuity of their employment (i.e., job insecurity) has adverse consequences. To understand where job insecurity comes from, we take a resources-demands perspective to synthesize and meta-analyze 57 theoretical sources of job insecurity. Using 3-decade (1986-2018) data from 425 independent samples representing 219,190 individuals from 39 countries, we find that the vast majority of theoretical predictors explain meaningful variance in job insecurity. Interestingly, resources (facilitating goal attainment), compared with demands (hindering goal attainment) have stronger relationships with job insecurity. Moreover, individualism, gross domestic product, and egalitarianism at the country level strengthen the negative relationships between resources and job insecurity and attenuate the positive relationships between demands and job insecurity, whereas power distance, national unemployment rate, and income inequality at the country-level lessen the negative relationships between resources and job insecurity and aggravate the positive relationships between demands and job insecurity. Finally, organizational practices account for significantly more variance in qualitative job insecurity than quantitative job insecurity, whereas personal factors and organizational social indicators explain a similar amount of variance in qualitative and quantitative job insecurity. Results suggest that gathering personal and organizational resources is more important than removing demands in terms of reducing job insecurity; having access to more resources in an attempt to diminish job insecurity is especially functional in countries high in individualism, gross domestic product, and egalitarianism, or low in power distance, national unemployment rate, and income inequality. RESUMO

Hyperfibrinogenemia and cancer-associated systemic inflammatory response are strongly associated with cancer progression and prognosis. We aimed to develop a novel prognostic score (F-SII score) on the basis of preoperative fibrinogen (F) and systemic immunoinflammatory index (SII), and evaluate its predictive value in patients with resectable gastric cancer (GC). Patients diagnosed with GC between January 2012 and December 2016 were reviewed. The F-SII score was 2 for patients with a high fibrinogen level (≥ 3.37 g/L) and a high SII (≥ 372.8), whereas that for patients with one or neither was 1 or 0, respectively. A high F-SII score was significantly associated with older patient age, a high ASA score, large tumor size, large proportion of perineural invasion, and late TNM stage. Multivariate analysis indicated that the F-SII score, histological grade, and TNM stage were independent factors for overall survival (OS). The Harrell's concordance index (C-index) of a nomogram based on the F-SII score and several clinicopathological manifestations was 0.72, which showed a better predictive ability for OS than the TNM stage alone (0.68). In conclusion, preoperative F-SII may serve as a useful predictive factor for OS and refine outcome prediction for patients with resectable GC combined with traditional clinicopathological analysis.

RESUMO

OBJECTIVE: To investigate the clinicopathologic factors related to local recurrence following curative surgery for rectal cancer and explore the ways to improve the efficacies. METHODS: A total of 257 post-operative cases of rectal cancers from January 1995 to December 2002 were recruited. And the relation of each clinicopathologic characteristic and local post-operative recurrence was retrospectively investigated. The clinicopathologic factors and follow-up data were analyzed by uni and multi-factorial regression model by logistic regression. RESULTS: The total local recurrence rate of rectal cancer after radical operation was 18.7%, 75.0% (36/48) within 2 years and 95.8% (45/48) within 3 years, the median time was 14.6 months (range: 2-63). Univariate analysis showed that age, Dukes' stage, lymph node metastasis and level of CEA were significantly associated with the local post-operative recurrence (P<0.05). Multivariate analysis showed that age and lymph node metastasis were closely related with the local post-operative recurrence (P<0.05). CONCLUSION: Lymph node metastasis and age are very important prognostic factors for the local post-operative recurrence. It is possible to improve the prognosis of rectum cancer by making an early diagnosis and offering a scientific standard therapy.

RESUMO

Hyperspectral reflectance and green degree of Platanus orientalis L. and Populus tomentosa Carr. leaves were measured by the ASD portable spectrometer and the portable chlorophyll meter SPAD-502, respectively. The chlorophyll concentration retrieval models based on 10 common vegetation indexes were established, and the ANN-BP model which used wave bands with larger correlation coefficient as input variables was established for chlorophyll content estimation. Finally, the ANFIS model was established to inverse vegetation chlorophyll content using hyperspectral data. The results showed that normalized difference vegetation index can inverse chlorophyll content better than other vegetation index, and the determination coefficients R2 of models of Platanus orientalis L. and Populus tomentosa Carr. were 0.795 7 and 0.754 6, respectively. The determination coefficients R2 between the predicted and the measured chlorophyll content based on ANN-BP models of Platanus orientalis L. and Populus tomentosa Carr. were 0.935 2 and 0.917 1, respectively. ANFIS model which is a good method to be applied to hyperspectral data for estimation of vegetation chlorophyll concentration can greatly improve vegetation chlorophyll concentration estimation accuracy, and the determination coefficients R2 between the predicted and the measured chlorophyll content of Platanus orientalis L. and Populus tomentosa Carr. were 0.935 2 and 0.917 1, respectively.

RESUMO

In this study, we built and tested a contingency model linking leader daily empowering behaviors with employee daily job crafting. Drawing on the contingency leadership literature and the model of proactive motivation, we theorized employee daily work meaning and vigor as moderators of the above relationships. Daily data were collected from 103 Chinese employees for five consecutive days. Our findings suggest that leader dayT (a certain day) empowering behaviors are more strongly related to employee dayT+1 (next day after the certain day) job crafting when employee dayT work meaning is low and employee dayT+1 vigor is high. Our findings suggest that only under certain conditions can empowering leadership promote employee job crafting on a daily basis.

RESUMO

Reports on the correlation between the expression of Survivin/phosphatase and tensin homolog (PTEN) proteins and clinical factors in gastric cancer (GC) are varied, and the sample sizes were also not sufficient. The present study aimed to detect the expression of Survivin and PTEN proteins in GC patients on the basis of a greater number of specimens and to analyze the correlation with clinical features and survival. The results revealed that the Survivin expression rates in GC, normal tissues and metastatic lymph nodes were 72% (232/322), 5% (6/120) and 80% (36/45), respectively, while the PTEN expression rates were 34% (109/322), 92.5% (111/120) and 24.4% (11/45), respectively, and the differences between cancer and normal tissue or metastatic lymph nodes were significant for both proteins (P<0.05). The expression of Survivin was significantly associated with gross type, depth of invasion, distant metastasis, tumor, necrosis and metastasis (TNM) stage and vascular invasion, while PTEN expression was predominantly associated with age, tumor size, invasion depth, TNM stage and lymphatic invasion in GC patients (P<0.05). The expression of both was associated with postoperative metastasis and metastatic site (P=0.007 and P=0.011 for Survivin, and P=0.002 and P=0.005 for PTEN). There was a negative association between the expression levels of Survivin and PTEN (P=0.001, r=-0.524). The expression levels of both were also associated with prognosis. The expression of Survivin and PTEN protein exhibit opposing trends in GC, which may indicate adverse biological effects in the occurrence of GC. The Survivin and PTEN expression levels are likely to be an important molecular event in gastric tumorigenesis and may be considered as molecular markers of GC progression and reliable prognostic indicators of GC.

RESUMO

Colorectal cancer (CRC) is a frequently occurring lethal disorder with heterogeneous outcomes and drug responses. Recent studies have demonstrated that long non-coding RNAs (lncRNAs) play a critical role in carcinogenesis. Hence, the aim of this study was to investigate the role of lncRNA growth arrest-specific 5 (GAS5) in CRC cells via mediation of the microRNA-222-3p (miR-222-3p)/GAS5/phosphatase and tensin homolog (PTEN)-signaling pathway. HCT116 and SW480 cells were collected and treated with small interfering (si)-lncRNA GAS5, overexpressing (oe)-lncRNA GAS5, miR-222-3p mimic, miR-222-3p inhibitor, or si-lncRNA GAS5 + miR-222-3p mimic. The miR-222-3p level and mRNA and protein levels of GAS5, Beclin1, light-chain 3B (LC3B), PTEN, and Akt were detected. Besides, cell migration, invasion, and apoptosis as well as acidic vesicular organelles (AVOs) were examined respectively. Xenografts in nude mice were also performed to detect tumorigenesis in vivo. Results suggested that the downregulation of lncRNA GAS5 decreased the expressions of Beclin1, LC3B, and PTEN. When treated with oe-lncRNA GAS5 or miR-222-3p inhibitor, HCT116 and SW480 cells exhibited suppressed invasion and migration abilities and increased apoptotic cells and autophagosome and AVO activities. Moreover, overexpression of GAS5 inhibited the tumorigenesis of CRC cells in vivo. Taken together, lncRNA GAS5 upregulated the expression of PTEN by functioning as a competing endogenous RNA (ceRNA) of miR-222-3p, thus inhibiting CRC cell migration and invasion and promoting cell autophagy.

RESUMO

BACKGROUND: LncRNA and microRNA play an important role in the development of human cancers; they can act as a tumor suppressor gene or an oncogene. LncRNA GAS5, originating from the separation from tumor suppressor gene cDNA subtractive library, is considered as an oncogene in several kinds of cancers. The expression of miR-221 affects tumorigenesis, invasion and metastasis in multiple types of human cancers. However, there's very little information on the role LncRNA GAS5 and miR-221 play in CRC. Therefore, we conducted this study in order to analyze the association of GAS5 and miR-221 with the prognosis of CRC and preliminary study was done on proliferation, metastasis and invasion of CRC cells. In the present study, we demonstrate the predictive value of long non-coding RNA GAS5 (lncRNA GAS5) and mircoRNA-221 (miR-221) in the prognosis of colorectal cancer (CRC) and their effects on CRC cell proliferation, migration and invasion. METHODS: One hundred and fifty-eight cases with CRC patients and 173 cases of healthy subjects that with no abnormalities, who've been diagnosed through colonoscopy between January 2012 and January 2014 were selected for the study. After the clinicopathological data of the subjects, tissue, plasma and exosomes were collected, lncRNA GAS5 and miR-221 expressions in tissues, plasma and exosomes were measured by reverse transcription quantitative polymerase chain reaction (RT-qPCR). The diagnostic values of lncRNA GAS5 and miR-221 expression in tissues, plasma and exosomes in patients with CRC were analyzed using receiver operating characteristic curve (ROC). Lentiviral vector was constructed for the overexpression of lncRNA GAS5, and SW480 cell line was used for the transfection of the experiment and assigned into an empty vector and GAS5 groups. The cell proliferation, migration and invasion were tested using a cell counting kit-8 assay and Transwell assay respectively. RESULTS: The results revealed that LncRNA GAS5 was upregulated while the miR-221 was downregulated in the tissues, plasma and exosomes of patients with CRC. The results of ROC showed that the expressions of both lncRNA GAS5 and miR-221 in the tissues, plasma and exosomes had diagnostic value in CRC. While the LncRNA GAS5 expression in tissues, plasma and exosomes were associated with the tumor node metastasis (TNM) stage, Dukes stage, lymph node metastasis (LNM), local recurrence rate and distant metastasis rate, the MiR-221 expression in tissues, plasma and exosomes were associated with tumor size, TNM stage, Dukes stage, LNM, local recurrence rate and distant metastasis rate. LncRNA GAS5 and miR-221 expression in tissues, plasma and exosomes were found to be independent prognostic factors for CRC. Following the overexpression of GAS5, the GAS5 expressions was up-regulated and miR-221 expression was down-regulated; the rate of cell proliferation, migration and invasion were decreased.

RESUMO

To evaluate the soil quality in the main cotton growing regions of Xinjiang, 11 soil quality indices were measured in representative locations: Hami, Bole, Changji, Shihezi, Aksu, Kashgar and Kuitun. The indices included soil pH, salt, organic matter, total N, available P and available K for soil physicochemical properties, and Cr, Cu, Zn, As, and Pb for soil heavy metal pollution. Based on these indices, a comprehensive soil quality index (SQI) was developed to analyze the soil quality in the cotton fields of Xinjiang. The results showed that the soils in Xinjiang's cotton fields were alkaline, with an average pH of 7.87. The soils were mildly saline, with an average salt content of 3.44 g·kg-1. Soil organic matter and total N concentrations were generally low, whereas available P and available K concentrations were relatively high. Soil available P concentrations were significantly higher than that of the second national soil survey, whereas soil pH, salt content, organic matter, and total N were less. Soil available K was greater in some regions but lower in others compared with the second national soil survey. The average heavy metal concentrations were as follows: Cr, 45.88 mg·kg-1; Cu, 40.66 mg·kg-1; Zn, 68.30 mg·kg-1; As, 12.88 mg·kg-1; and Pb, 16.68 mg·kg-1. These values were all below the national standards. However, the Cu, Zn, and As concentrations were greater than the background values in Xinjiang, indicating the accumulation of those elements in the soils. When the Nemerow comprehensive pollution index (PN) of heavy metals was less than 0.5, the comprehensive soil quality improved as the soil physicochemical properties improved. Soil organic matter, total N, Cu, Zn, As and Pb were the main variables affecting soil quality in the main cotton cropping regions of Xinjiang. The cotton growing areas in Xinjiang generally had medium soil quality. Changji and Kuitun had the highest SQI (0.52) whereas Aksu had the lowest value (0.31). Soil quality was generally highest in northern Xinjiang, followed by western area, and then southern area.
