Background and Objectives: Sepsis represents a global health challenge and requires advanced diagnostic and prognostic approaches due to its elevated rate of morbidity and fatality. Our study aimed to assess the value of a novel set of six biomarkers combined with severity scores in predicting 28 day mortality among patients presenting with sepsis in the Emergency Department (ED). Materials and Methods: This single-center, observational, prospective cohort included sixty-seven consecutive patients with septic shock and sepsis enrolled from November 2020 to December 2022, categorized into survival and non-survival groups based on outcomes. The following were assessed: procalcitonin (PCT), soluble Triggering Receptor Expressed on Myeloid Cells-1 (sTREM-1), the soluble form of the urokinase plasminogen activator receptor (suPAR), high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), and azurocidin 1 (AZU1), alongside clinical scores such as the Quick Sequential Organ Failure Assessment (qSOFA), Systemic Inflammatory Response Syndrome (SIRS), the Sequential Organ Failure Assessment (SOFA), the Acute Physiology and Chronic Health Evaluation II (APACHE II), the Simplified Acute Physiology Score II and III (SAPS II/III), the National Early Warning Score (NEWS), Mortality in Emergency Department Sepsis (MEDS), the Charlson Comorbidity Index (CCI), and the Glasgow Coma Scale (GCS). The ability of each biomarker and clinical score and their combinations to predict 28 day mortality were evaluated. Results: The overall mortality was 49.25%. Mechanical ventilation was associated with a higher mortality rate. The levels of IL-6 were significantly higher in the non-survival group and had higher AUC values compared to the other biomarkers. The GCS, SOFA, APACHEII, and SAPS II/III showed superior predictive ability. Combining IL-6 with suPAR, AZU1, and clinical scores SOFA, APACHE II, and SAPS II enhanced prediction accuracy compared with individual biomarkers. Conclusion: In our study, IL-6 and SAPS II/III were the most accurate predictors of 28 day mortality for sepsis patients in the ED.

OBJECTIVES: The success of ridge preservation techniques in reducing bone resorption is well documented, but no clear guidelines have been provided regarding the type of the biomaterial or technique to be used. This experimental animal study aimed at comparing the effectiveness of two ridge preservation techniques. MATERIALS AND METHODS: Following the extraction of the distal roots of the mandibular second and fourth premolars of four dogs, the sockets were preserved using a combination of a collagen membrane intimately covering the socket plus a collagen matrix or a collagen membrane alone. The mandibular quadrants were randomly assigned to one of the treatment groups. Histomorphometrical analyses as well as microscopic observations were performed. RESULTS: After 5 months of healing, the histological analysis revealed a similar picture of bone formation in both groups. No significant differences between the buccal and lingual dimensions were calculated between the two treatment groups. The mucosa covering the alveolar ridges is significantly more abundant in post-extraction sockets preserved with the double-layered approach. CONCLUSIONS: The double-layered approach used to treat post-extraction sockets may result in a better preservation of the mucosal seal than the single-layered approach. CLINICAL RELEVANCE: The use of the new collagen matrix associated with a collagen membrane could be a clinical option to preserve post-extraction ridges, especially when an improvement in soft tissue dimension and quality is desired. However, the cost-benefit ratio of this approach should be thoroughly evaluated in further studies.

This research aimed to observe the behavior of mesenchymal stem cells (MSCs) isolated from periodontal granulation tissue (gt) when manipulated ex vivo to induce three-dimensional (3D) spheroid (aggregates) formation as well as when seeded on two bone scaffolds of animal origin. Periodontal gt was chosen as a MSC source because of its availability, considering that it is eliminated as a waste material during conventional surgical therapies. 3D aggregates of cells were generated; they were grown for 3 and 7 days, respectively, and then prepared for transmission electron microscopic analysis. The two biomaterials were seeded for 72 h with gtMSCs and prepared for scanning electronic microscopic observation. The ultrastructural analysis of 3D spheroids remarked some differences between the inner and the outer cell layers, with a certain commitment observed at the inner cells. Both scaffolds showed a relatively smooth surface at low magnification. Macro- and micropores having a scarce distribution were observed on both bone substitutes. gtMSCs grew with relative difficulty on the biomaterials. After 72 h of proliferation, gtMSCs scarcely covered the surface of bovine bone scaffolds, demonstrating fibroblast-like or star-like shapes with elongated filiform extensions. Our results add other data on the possible usefulness of gtMSC and could question the current paradigm regarding the complete removal of chronically inflamed gts from the defects during periodontal surgeries. Until optimal protocols for ex vivo manipulation of MSCs are available for clinical settings, it is advisable to use biocompatible bone substitutes that allow the development of progenitor cells.

The present study was designed in the context of the uncertain circumstances related to the best therapeutic option for ridge preservation. The research aimed to investigate the quality of early healing processes developed in the former sockets preserved with a collagen matrix alone or associated with a bone substitute in comparison with naturally-healed sockets, using an animal model previously validated. In both quadrants of the mandible of two dogs, the distal sockets of the second and fourth premolars served as experimental sites. Two sockets healed naturally, three sockets were preserved with the collagen matrix and three sockets were preserved with the collagen matrix plus a bone substitute. After one month of healing, the samples were harvested and histologically processed. The soft tissue covering the preserved ridges displayed an obviously thicker epithelial layer containing mostly areas of parakeratinized epithelium alternating with keratinized ones in comparison with naturally-healed sockets. In the apical third of the sockets, a mature bone structure was recorded for all three types of post-extraction approaches. While in naturally-healed and collagen matrix-preserved specimens the central third of the sockets contained bone with a mature aspect, in collagen matrix plus bone substitute-preserved sockets an immature appearance was observed. In the external third of the sockets, only in matrix-preserved alveoli a well-developed cancellous bone was present. The bovine bone substitute seemed to delay hard tissue development. The use of the collagen matrix could be a clinical option to preserve post-extraction ridges especially when an improvement in soft tissue quality is desired.

The aim of the present animal study was to investigate the early healing processes developing in the post-extraction sockets preserved with a new-marketed collagen matrix as, to our knowledge, such investigations have not been reported so far. In both quadrants of the mandible of a mongrel dog, the distal sockets of the second premolars served as experimental sites for ridge preservation. The experimental site 1 was protected with a resorbable membrane and then with the collagen matrix. The experimental site 2 was filled with a xenograft and then covered with the collagen matrix. The samples were harvested after one month of healing. In both experimental sites, the bundle bone lining the inner surface of the alveolus was replaced with trabecular bone containing areas of woven bone. A continuous layer of osteoblasts could be observed on the surface of woven bone areas. Osteoclasts encased within resorptive lacunae lined the outer portions of bone walls for the experimental site 1. The trabecular bone occupied only the apical third of the socket in experimental site 1, but it was obviously more abundant in the experimental site 2, occupying also the central compartment of the socket. Moreover, the trabeculae of the bone occupying the inner area of the alveolus were thicker for the experiment site 2 than for experiment site 1, suggesting an increased osseous deposition in the latter situation. Our preliminary results suggest that the association collagen matrix plus xenograft may be a valuable method for ridge preservation.
