RESUMO

OBJECTIVE: To analyze the effects of Butylphthalide on cerebral vascular circulation, coagulation function, and neurological function in patients with acute severe ischemic stroke following intravenous thrombolysis. METHODS: Clinical efficacy, cerebral vascular circulation indicators [anterior cerebral artery (ACA), middle cerebral artery (MCA), vertebral artery (VA) blood flow velocity], coagulation function indicators [prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB)], neurological function indicators [Activities of Daily Living (ADL) score. RESULTS: The total effective rate of treatment in the control group was 76.47%, while in the observation group, it was 96.08%, with the observation group showing a significantly higher total effective rate than the control group (p < 0.05). Before treatment, there was no significant difference in ACA, MCA, and VA blood flow velocity between the two groups (p > 0.05). However, after treatment, the ACA, MCA, and VA blood flow velocity in the observation group were significantly higher than those in the control group (p < 0.05). Before treatment, there was no significant difference in PT, APTT, TT, and FIB levels between the two groups (p > 0.05). CONCLUSION: In patients with acute severe ischemic stroke undergoing intravenous thrombolysis, the addition of Butylphthalide to the treatment regimen yields favorable clinical outcomes. Compared to Alteplase alone, the addition of Butylphthalide further improves cerebral vascular circulation and coagulation function, promoting the recovery and reconstruction of neurological function in patients. Importantly, the addition of Butylphthalide does not increase the risk of adverse reactions, making it a safe and ideal option for clinical application.

Nasopharyngeal carcinoma (NPC) features high mortality and poor prognosis. Additionally, long non-coding RNAs (lncRNAs) play a significant role in developing NPC and other types of cancer. But the functional mechanism of MIR100HG in NPC remains unclear. The long non-coding RNA MIR100HG messenger RNA (mRNA) expression was thoroughly evaluated in NPC tumors and adjacent tissues using quantitative polymerase chain reaction (qPCR). Furthermore, we employed Kaplan-Meier analysis to compare the expression of MIR100HG with survival outcomes. The CCK8 test was utilized to investigate the impact of the lncRNA MIR100HG/miR-136-5p/IL-6 axis on cell proliferation in NPC. The study's findings indicated overexpression of the lncRNA MIR100HG in both NPC tumors and cell lines. This upregulation was associated with a poorer outcome in individuals with NPC. When lncRNA MIR100HG was knocked down in vitro, NPC cell proliferation was inhibited, resulting in tumor suppression. In certain oncogenic capacities, the lncRNA MIR100HG functions as a competitive endogenous RNA for miR-136-5p, hence impeding the inhibitory effect of miR-136-5p on its target gene, IL-6. In summary, the findings of the present investigation suggested that lncRNA MIR100HG exhibits promising characteristics as a potential indicator for the prognosis and diagnosis of NPC.

The relationship between plant diversity and the ecosystem carbon pool is important for understanding the role of biodiversity in regulating ecosystem functions. However, it is not clear how the relationship between plant diversity and soil carbon content changes under different grassland use patterns. In a 3-year study from 2013 to 2015, we investigated plant diversity and soil total carbon (TC) content of grasslands in northern China under different grassland utilization methods (grazing, mowing, and enclosure) and climatic conditions. Shannon-Wiener and Species richness index of grassland were significantly decreased by grazing and mowing. Plant diversity was positively correlated with annual precipitation (AP) and negatively correlated with annual mean temperature (AMT). AP was the primary regulator of plant diversity. Grazing and mowing decreased TC levels in grasslands compared with enclosures, especially in topsoil (0-20 cm). The average TC content was decreased by 58 % and 36 % in the 0-10 cm soil layer, while it was decreased by 68 % and 39 % in 10-20 cm soil layer. TC was positively correlated with AP and negatively correlated with AMT. Principal component analysis (PCA) showed that plant diversity was positively correlated with soil TC, and the correlation decreased with an increase in the soil depth. Overall, this study provides a theoretical basis for predicting soil carbon storage in grasslands under human disturbances and climate change impacts.

OBJECTIVES: To compare the efficacy and safety of nicorandil monotherapy and nicorandil-clopidogrel combination therapy on cardiac function in patients with coronary heart disease (CHD). METHODS: The clinical data of 200 patients with CHD were retrospectively analyzed. The patients were divided into two groups according to different treatment methods. Group A (n = 100) received nicorandil-clopidogrel combination therapy (intravenously injected with 25 mg of nicorandil and orally administered 300 mg of clopidogrel for 3 months), and Group B (n = 100) received nicorandil monotherapy (intravenously injected with 25 mg of nicorandil for 3 months). The primary endpoints included cardiac function indices and ST-segment behavior on electrocardiogram (ECG) before and after treatment. The secondary endpoints included adverse reactions, clinical efficacy, platelet aggregation, activated partial thromboplastin time (APTT), high-sensitivity cardiac troponin T (hs-cTnT), and creatine kinase isoenzyme MB (CK-MB) levels after treatment. Multivariate regression analyses were used to assess the contribution of a single drug to the ultimate outcome. RESULTS: After treatment, both groups exhibited significant decreases in brain natriuretic peptide (BNP) and N-terminal pro-hormone BNP levels compared with before treatment, with the levels significantly lower in Group A than in Group B (P < 0.05). After treatment, left ventricular ejection fraction was significantly increased in both groups compared with before treatment, and that was much higher in Group A than in Group B (P < 0.05). After treatment, the frequency and duration of ST-segment depression were decreased in both groups compared with before treatment, and they were much lower in Group A than in Group B (all P < 0.05). The total incidence of adverse reactions in Group A (4.00%) was slightly lower than that in Group B (7.00%), with no significant difference (P > 0.05). Group A (92.00%) had a higher overall response rate than Group B (81.00%) (P < 0.05). CONCLUSION: Nicorandil-clopidogrel combination therapy exhibited enhanced clinical efficacy in patients with CHD. In addition, the combination therapy regulated hs-cTnT and CK-MB levels, which may suggest a better patient prognosis.

BACKGROUND: Nasopharyngeal carcinoma is a prevalent malignant tumor in clinical practice, with the highest incidence rate among otorhinolaryngological malignant tumors. OBJECTIVES: This study aims to comprehensively evaluate the clinical efficacy and safety of traditional Chinese medicine compound (CMC) combined with concurrent radiotherapy and chemotherapy in the treatment of locally advanced nasopharyngeal carcinoma (LA-NPC). METHODS: Relevant essays published before November 20, 2021, were retrieved from China National Knowledge Internet (CNKI), China Science and Technology Journal Database (CQVIP), Wanfang database, PubMed, and Web of Science databases. Randomized controlled trials regarding the clinical efficacy of CMC combined with concurrent radiotherapy and chemotherapy in the treatment of LA-NPC were included. RESULTS: A total of 15 publications involving 1324 patients were included in this study, including 665 in the experimental group and 659 in the control group. Meta-analyses revealed that compared with radiotherapy or chemotherapy only, CMC combined with concurrent radiotherapy and chemotherapy for LA-NPC significantly improved the efficacy [risk ratio (RR)=1.15, 95% confidence interval (95% CI) (1.09, 1.20), P<0.00001], the quality of life [RR=1.35, 95% CI (1.13, 1.62), P=0.0009], immune function indices CD4+ levels [RR=6.2, 95% CI (3.64, 8.76), P<0.00001], CD4+/CD8+ [RR=0.33, 95% CI (0.14, 0.53), P=0.0009], and alleviated the decrease in white blood cell counts [RR=0.67, 95% CI (0.52, 0.86), P=0.002]. CONCLUSION: CMC combined with concurrent radiotherapy and chemotherapy for the treatment of LA-NPC can significantly improve the efficacy and reduce severe adverse reactions caused by conventional radiotherapy and chemotherapy. However, due to limitations in the quantity and quality of the included studies, more high-quality, multi-center, and large sample-size studies are needed to provide high-level and high-quality medical evidence for systematic evaluation.

BACKGROUND: To investigate the risk factors for pharyngocutaneous fistulas following total laryngectomy. METHODS: PubMed, Web of Science, CNKI, Medline and Wanfang database were utilized to conduct a systematic literature research. Further, sensitivity and publication bias were analyzed to comprehensively estimate the risk factors associated with pharyngocutaneous fistulas following total laryngectomy. RESULTS: Of the 112 studies identified, 25 were included in this analysis. The results showed that age (OR = 0.21, 95% CI 0.11-0.39, P<0.00001), smoking (OR = 3, 95% CI 1.54-5.84, P<0.00001), T-stage (OR = 0.3, 95% CI 0.22-0.4, P<0.00001), previous radiotherapy (OR = 0.31, 95% CI 0.23-0.44, P<0.000001) and preoperative albumin (OR = 0.28, 95% CI 0.16-0.47, P<0.00001) were risk factors associated with pharyngocutaneous fistulas. CONCLUSIONS: This review is a comprehensive analysis of the risk factors associated with pharyngocutaneous fistulas following total laryngectomy. Age, smoking, T-stage, previous radiotherapy and preoperative albumin were found to be the risk factors.

Inter-annual dynamics of grassland yield of the Three Rivers Headwaters Region of Qinghai-Tibet Plateau of China in 1988-2005 was analyzed using the GLO-PEM model, and the herbage supply function was evaluated. The results indicate that while grassland yield in the region showed marked inter-annual fluctuation there was a trend of increased yield over the 18 years of the study. This increase was especially marked for Alpine Desert and Alpine Steppe and in the west of the region. The inter-annual coefficient of variation of productivity increased from the east to the west of the region and from Marsh, Alpine Meadow, Alpine Steppe, Temperate Steppe to Alpine Desert grasslands. Climate change, particularly increased temperatures in the region during the study period, is suggested to be the main cause of increased grassland yield. However, reduced grazing pressure and changes to the seasonal pattern of grazing could also have influenced the grassland yield trend. These findings indicate the importance of understanding the function of the grassland ecosystems in the region and the effect of climate change on them especially in regard to their use to supply forage for animal production. Reduction of grazing pressure, especially during winter, is indicated to be critical for the restoration and sustainable use of grassland ecosystems in the region.

Relationships of foliar carbon isotope composition (δ(13)C) with foliar C, N, P, K, Ca, Mg contents and their ratios of 219 C3 species leaf samples, obtained in August in 2004 to 2007 from 82 high altitude grassland sites on the Qinghai-Tibet Plateau China, were examined. This was done with reference to the proposition that foliar δ(13)C increases with altitude and separately for the life-form groups of graminoids, forbs and shrubs and for the genera Stipa and Kobresia. For all samples, foliar δ(13)C was negatively related to foliar K, P and ∑K+ Ca+ Mg, and positively correlated to foliar C, C/N and C/P. The significance of these correlations differed for the taxonomic and life-form groups. Lack of a relationship of foliar δ(13)C with foliar N was inconsistent with the majority of studies that have shown foliar δ(13)C to be positively related to foliar N due to a decrease of Ci/Ca (the ratio between intercellular and atmospheric concentration of CO2) and explained as a result of greater photosynthetic capacity at higher foliar N concentration. However this inconsistency relates to other high altitude studies that have found that photosynthetic capacity remains constant as foliar N increases. After accounting for the altitudinal relationship with foliar δ(13)C, of the elements only the K effect was significant and was most strongly expressed for Kobresia. It is concluded that factors critical to plant survival and growth at very high altitudes, such as low atmospheric pressure and low temperatures, may preclude expression of relationships between foliar δ(13)C and foliar elements that have been observed at lower altitudes.
