Background: Prostitution is a social phenomenon, and its underlying factors require more scholarly attention. This study aimed to investigate the relationship of childhood traumas and sexual guilt with sexual addiction in Iranian prostitutes. Methods: This was a cross-sectional study conducted on prostitutes who visited a health promotion center in Khorasan Razavi province, Iran in 2020. A total of 100 women agreed to participate in the study. Data collection tools included the Sexual Addiction Screening Test (SAST), Mosher Sex-Guilt Scale, and Childhood Trauma Questionnaire (CTQ). Data were analyzed using Pearson's correlation coefficient and multiple regression analysis. Findings: The age of the participants ranged from 23 to 42, with a mean age of 33.54±8.9 years. About 40% of the participants were divorced, 13% were married, 27% were single, and 10% were widows. The findings indicated that 84% of the prostitutes met the criteria for sexual addiction according to SAST. Emotional abuse (r=0.41, P<0.001), physical abuse (r=0.32, P<0.001), sexual abuse (r=0.33, P<0.001), emotional neglect (r=0.52, P<0.001), and physical neglect (r=0.37, P<0.001) had a positive and significant relationship with sexual addiction in prostitutes. There was no correlation between sexual guilt and sexual addiction (r=0.13, P=0.09). Furthermore, the linear regression results showed that emotional neglect was the only variable positively associated with sexual addiction (ß=0.5, P<0.001). Conclusion: The findings of this study suggested that childhood traumas can predict sexual addiction in female prostitutes.

Violence against women is a form of gender violence, and the lethal aspect of it, defined as femicide, is a global health and human rights problem.This study looked at 330 cases of femicide that occurred in North West Italy, between 1970 and 2020, committed by 303 male perpetrators. The victims included women who were prostitutes and those who were not.Findings show that only a small proportion of femicide occurs within an anonymous setting: Victims were mostly killed by a man they knew. The type and intensity of the relationship was likely to affect how the violence occurred. In those cases in which victims and perpetrators had an intimate relationship, the risk of overkill, that is, an excessive use of violence that goes further than what is necessary to cause death, was four times higher in comparison with the murder of unknown victims. As with non-prostitutes, the risk of overkill was almost fourfold for those prostitutes who knew their perpetrators. Furthermore, when comparing prostitutes with any unknown victims, the risk of being overkilled was almost five times higher for the former, suggesting that prostitutes are more at risk of being murdered with excessive violence. In addition, prostitutes were more likely to be victims of sexual murder, postmortem mutilation, and being killed by men who had previous criminal records.Women who are victims of violence are not a homogeneous group, although some of the psychosocial correlates are the same and relevance should be given to the features behind the type, intensity, and nature of the relationship between prostitutes and non-prostitutes and their perpetrators. These variables are what make violence against women a preventable problem.

A crucial requirement in the rational design of a prophylactic vaccine against the human immunodeficiency virus (HIV) is to establish whether or not protective immunity can occur following natural infection. The immune response to HIV infection is characterized by very vigorous HIV-specific cytotoxic T-lymphocyte (CTL) activity. We have identified four HIV-1 and HIV-2 cross-reactive peptide epitopes, presented to CTL from HIV-infected Gambians by HLA-B35 (the most common Gambian class I HLA molecule). These peptides were used to elicit HIV-specific CTLs from three out of six repeatedly exposed but HIV-seronegative female prostitutes with HLA-B35. These women remain seronegative with no evidence of HIV infection by polymerase chain reaction or viral culture. Their CTL activity may represent protective immunity against HIV infection.

The study population consisted of 20 women: 14 had been prostitutes for more than 5 years and reported little condom usage and 6 were long-term sexual partners of HIV-infected men. Peptide-stimulated cultures were also set up from 8 known seropositive donors with HLA-B35 or B53, and from a control group of volunteers at low-risk of HIV infection with HLA-B35 (12 Gambian and 7 European) and 2 Gambians with HLA-B53. Specific CTL activity against one or more peptides was repeatedly detected after 10-14 days in the peptide-stimulated cultures from 3 of the 6 high-risk seronegative women with HLA-B35, but not in their three counterparts with HLA-B53 nor in any of the low-risk volunteers. The strongest responses were generated toward the HIV-1 pol peptide, which lies close to the active site of reverse transcriptase, and to the nef peptide, which is conserved between HIV-1 and -2. HIV-specific CTL in seronegative subjects could potentially be a response to acute HIV infection, before the development of antibodies, but the women were still seronegative and virus-culture negative 3 months after the CTL were first detected, making recent infection extremely unlikely. These women remain seronegative with no evidence of HIV infection by polymerase chain reaction or viral culture. Their CTL activity may represent protective immunity against HIV infection.

PIP: Health officials in the Indian state of Maharashtra have ordered the compulsory testing of all girls 12 years and older who live in designated "destitute homes." The officials also plan to tattoo a symbol on the thighs of all HIV-positive prostitutes. By April 1998, this December 1997 order had resulted in the compulsory testing of women living in 50 boarding houses and the transfer of several found to be HIV-positive to a separate institution 200 miles from the state capital. Nongovernment organizations (NGOs) have mounted a protest over this statute, but state governments in India are free to enact their own health laws. The Maharashtran government is also seeking to legalize prostitution and to force prostitutes to register with a Board that will be able to order compulsory HIV tests and tattooing. Women with HIV who continue to engage in prostitution will be quarantined, and their clients will be jailed. In response, prostitutes in the capital city of Mumbai have threatened to release a list of their client's names to the press. The only recourse available to NGOs who oppose this action is to generate a large enough public outcry to stop it. A Mumbai-based attorney noted that many private companies are also requiring HIV testing and dismissing those who test positive.^ieng

Introduction: This article provides insight into a rare instance of a collaborative governance approach to sex work that led to the decriminalisation, design and implementation of the sex work policy governance framework in New Zealand with the Prostitution Reform Act 2003. Methods: Drawing on a sample of 17 interviews conducted between the years 2012 and 2019 in addition to associated archival material originating from government and non-government sectors including sex worker representative organisations. Results: It finds that non-sex workers' endorsement for the decriminalisation of sex work was motivated by the New Zealand Prostitutes Collective (NZPC) and occurred primarily within a human rights and harm minimisation framework. But that during the lobbying and parliamentary process, amendments to the Bill emerged that contradicted the NZPC's main goal which was for sex work to be recognised as a legitimate labour activity and for all sex workers to benefit from decriminalisation and policy reform. Conclusions: As such, this article broadens the scope of analysis related to the sex worker rights movement by examining how and why sex workers and their allies came to communicate and act on the impetus for sex work law reform and how it affected policy outcomes. Policy Implications: Those involved in collaborative governance sex work law reform projects could consider adopting Östegren's typology of repressive, restrictive or integrative approaches to sex work law reform in negotiations that concern regulation and policies.

We present a retrospective study of prostitutes' homicide in the Milanese area over a 26-year period (1993-2018), and compare the results with general women's homicides: 294 women were killed of whom 71 were prostitutes. In the general population, the type-victim is an Italian woman aged between 31 and 40 years suffering from no particular pathologies or drug dependence. Prostitutes are 10 years younger, mostly Italian, suffering from pathologies probably related to their activities. Prostitutes remain at high risk of violence, due to gaps in the crime prevention system.

INTRODUCTION: The cornerstone of HIV prevention among female adolescent prostitutes is awareness promotion complemented with advocacy on consistent and correct use of condom. The study aimed at reviewing HIV awareness and condom use among female adolescent prostitutes in Lagos communities, Nigeria. METHODS: It was a mixed-method study realized through a questionnaire-based survey and in-depth interviews of adolescent sex workers in Oyingbo and Yaba communities of Lagos State, Nigeria; conducted between 1st of April, 2014 and 30th of September, 2014. SPSS version 17.0 and content analysis were used in analyzing quantitative and qualitative data respectively. RESULTS: 97.3% had heard about HIV/AIDS; with 86.9% being tested for HIV in the preceding 6 months. While there was consistent use of male condom in 99.7% of the respondents, 90% had experience with the use of female condom, however, 95.5% would allow non- use of condom for higher financial reward. CONCLUSION: Although the level of awareness of HIV/AIDS was high among the respondents, there is need to improve on the level of awareness and preventive strategies for HIV/AIDS, with more emphasis laid on the consistent and correct use of condom in this highly vulnerable class of people.

There have been no studies related to groups at the highest risk for HIV-1 infection in Korea before 1993. In this study, for the first time, we report the distribution of HIV subtypes in overseas sailors (OSs) and prostitutes who worked in brothels near U.S. military bases in Korea. We retrospectively determined the sequences of nef in 131 patients using reverse transcription polymerase chain reaction (RT-PCR). These patients composed of 102 OSs, 14 OS spouses, and 15 prostitutes. Phylogenetic analysis was performed using 128 Korean OSs, OS spouses, and prostitutes. The distribution of non-B subtypes (n = 105) was as follows: 39, CRF02_AG; 15, CRF01_AE; 7, A1; 7, A2; 6, D; 2, CRF06_cpx; 3, C; 6, G; 11, untypable; and 1 each for CRF09_cpx, CRF12_BF, CRF50_A1D, A3, AFG, H, F1, F2, and A. Of the 116 OSs and OS spouses, 101 (87%), 11 (9%), and 4 (3%) subjects had non-B, Western B, and Korean subclade B (KSB) HIV-1s, respectively. Among the 15 prostitutes, 10 had Western B (67%), 4 non-B (27%), and 1 KSB (7%) HIV-1s. All 14 couples, each comprising of an OS and his spouse, had the same subtype. KSB (5%) was detected in OSs and prostitutes in 1990 and 1994, respectively. Of the 131 patients analyzed in this study, 105 (80%), 21 (16%), and 5 (4%) were infected with the non-B, Western B, and KSB subtypes of HIV, respectively. In future, these data may provide an important foundation for analysis of HIV-1 subtypes in Korea.

The severe adverse effects of gonococcal infection on human fertility suggests that Neisseria gonorrhoeae would exert powerful selection for the development of a protective immune response in humans. N. gonorrhoeae is an obligate human pathogen and must persist in humans to survive. Since it is an ecologically successful organism, it must have evolved strategies to evade any human immune response it elicits. In a longitudinal study among 243 women working as prostitutes and experiencing frequent gonococcal infection, younger women, women with HIV infection, and women with antibody to the gonococcal outer membrane protein 3 (Rmp) were at increased risk of infection (adjusted odds ratio 3.4, CI95% 1.1-10.4, P < 0.05). Rmp is highly conserved in N. gonorrhoeae and the blocking of mucosal defences may be one of its functions. As similar proteins occur in many gram negative mucosal pathogens, the enhancing effect of such proteins may be a general strategy whereby bacteria evade human immune responses.


PIP: Between March 1985 and July 1986 researchers enrolled 243 female prostitutes in Pumwani community of Nairobi, Kenya, in a longitudinal study to examine the relationship between the antibody to the gonococcal outer membrane protein 3 (Rmp Ab) and gonococcal mucosal infection. Few women used condoms. 69% were HIV-1 seropositive. Just 9.5% (23) of the women had not had any gonococcal infections, despite probable exposure to them, indicating the possibility of some acquired protective immunity to Neisseria gonorrhoea. 90.5% had had at least 1 gonococcal infection. Women with Rmp Ab faced a greater risk of gonococcal infection than those who were Rmp Ab negative (OR = 3.4;l p .05), denoting that Rmp Ab increases susceptibility to gonococcal mucosal infections. Women older than 29 years were at lower risk of gonococcal infection than those younger than 29 years (odds ratio [OR] = 0.3; p .03). Women who used oral contraceptives (OCs) were also likely to be infected with N. gonorrhoea (OR = 3; p = .062). Further, 31% of OC users had cervical ectopy compared to just 14% of nonusers (OR = 2.8; p .005), suggesting that the effect of OCs on the cervix make it more susceptible to gonococcal infection. Rmp Ab also exists in many other gram-negative mucosal pathogens, often playing the same role as it does in N. gonorrhoea infection. Thus, Rmp Ab may be a common scheme bacteria used to elude human immune responses. These findings provide more understanding as to why N. gonorrhoea is an ecologically successful human pathogen.

Este artigo analisa inquéritos policiais referentes a feminicídios de trabalhadoras sexuais ocorridos no município de Porto Alegre entre 2006 e 2010. Trata-se de um estudo de caso que investigou 12 relatos de feminicídios ocorridos com prostitutas e identificados em 94 inquéritos policiais referentes a assassinatos de mulheres, disponibilizados pela Delegacia de Homicídios do município. Os feminicídios foram classificados segundo três categorias: crimes da miséria, pois todas as mulheres mortas eram muito pobres; crimes de ódio, nos quais houve estupro, mutilações, tortura e uso extremado de instrumentos letais; e crimes sexuais, nos quais houve violência sexual. No período estudado, os feminicídios de trabalhadoras sexuais apresentaram frequência elevada em relação a outras mulheres. Essas mortes expressam o descaso e a misoginia da sociedade patriarcal e a pouca valia dessas vidas humanas. (AU)


Este artículo analiza investigaciones de la policía relativas a feminicidios de trabajadoras sexuales habidos en el municipio de Porto Alegre entre 2006 y 2010. Se trata de un estudio de caso que investigó 12 relatos de feminicidios ocurrido con prostitutas e identificados en 94 investigaciones de la policía referentes a asesinatos de mujeres, puestos a disposición por la Comisaría de Homicidios del municipio. Los feminicidios se clasificaron en tres categorías: crímenes de la miseria, puesto que todas las mujeres muertas eran muy pobres; crímenes de odio, en los cuales hubo violación, mutilaciones, tortura y uso extremado de instrumentos letales; y crímenes sexuales, en los que hubo violencia sexual. En el período estudiado, los feminicidios de trabajadoras sexuales presentaron una frecuencia elevada con relación a otras mujeres. Esas muertes expresan el poco caso y la misoginia de la sociedad patriarcal y el poco valor dado a esas vidas humanas. (AU)


This article presents the feminicides among sex workers in the city of Porto Alegre, in the years 2006-2010. A case study analyses 12 feminicides that occurred in prostitutes and were identified in 94 police inquiries, referring to the women murders available by the Police State Department of the city. Feminicides were characterized into three categories: crimes of misery, because all the women were very poor; hate crimes, in which were rape, mutilations, torture and extreme use of lethal instruments; and sexual crimes in which were sexual violence. During the period studied, feminicides by sexual workers were highly frequent in relation to other women. These deaths express the neglect and the misogyny of patriarchal society and the little value of these human lives. (AU)

India is the first country outside Africa where an HIV-2 epidemic is running in parallel to an HIV-1 epidemic, resulting in a significant proportion of double infections. HIV is spreading rapidly, mainly by heterosexual contact, but also among intravenous drug users. Genetic analyses of the HIV variants circulating in India point towards HIV-1 and HIV-2 having been introduced into the country recently.


PIP: The genetic strains of HIV which have been uncovered in India have a different origin from those present in Thailand. Also, both HIV-1 and HIV-2 have been detected in India. In a 1991-93 study of clients of a sexually transmitted disease clinic in Bombay, it was found that seroprevalence of HIV in 241 clients was 39%, the 78.5% positive for HIV-1, 6.5% for HIV-2, and 15% for both. Distribution was even for males and females, reflecting heterosexual transmission. HIV-2 has also been detected in other parts of India, and HIV-1 is spreading throughout the country. HIV in children is a sequelae to blood transfusion. The prevalence of HIV in areas where IV drug use is rampant is increasing rapidly and is spreading to the general population. Genetic analysis revealed that the most closely related genetic sequence to the Indian HIV-1 from Bombay occurs in a strain of the virus from South Africa. One subtype of HIV-2 and several subtypes of HIV-1 have been found, with HIV-1 subtype C and HIV-2 subtype A the most frequently encountered. An examination of phylogenetic trees shows the relationships among these different strains. The close genetic relationship between isolates of HIV-1 and HIV-2 from patients in different parts of the country reflects the recent introduction of these strains. Thus, India is an ideal site for studying the efficacy of a vaccine designed specifically for a population of highly similar strains (the high similarity among HIV-2 isolates is unique in India). Since HIV-2 strains are diverging at a rate of 1% per year, any such research would have to occur soon.

PIP: To understand the spread of AIDS among prostitutes, it is important to understand the distinctions among those who "sell" themselves for sex. In studying the anthropology of prostitutes, 3 general geographical boundaries and the characteristics of each should be considered: the West, Asia and sub-Saharan Africa. Distinctions also exist among societies where there is almost no visible separation in the activities of marriage or prostitution and in societies where the nature of work and filiation differ. The effects of HIV transmission, condom use, legal control, drug use, the client and economic and non-economic aspects are considered with each broad geographical area. Important measures in the control of HIV infection will be the distribution of condoms and clean needles. Preventive programs must make it of utmost importance to 1st strive to encourage prevention among the prostitution community. Programs should also not be limited to areas where there are larger communities of prostitution and will trace migrational routes of prostitution back to even smaller communities.^ieng

The relationship between HIV seroprevalence and the proportion of uncircumcised males in African countries is examined to determine whether circumcision practices play a role in explaining the large existing variation in the sizes of African HIV epidemics. A review of the anthropological literature yielded estimates of circumcision practices for 409 African ethnic groups from which corresponding national estimates were derived. HIV seroprevalence rates in the capital cities were used as indicators of the relative level of HIV infection of countries. The correlation between these two variables in 37 African countries was high (R = 0.9; P less than 0.001). This finding is consistent with existing clinic-based studies that indicate a lower risk of HIV infection among circumcised males.


PIP: Wide variations in the HIV epidemic exist among the different countries in Africa. The relationship between HIV seroprevalence and the proportion of uncircumcised males is examined to determine whether circumcision practices play a role in explaining the variation in the sizes of the African HIV epidemics. Circumcision practices for 409 African ethnic groups were corresponded with national estimates of HIV infection levels. Although age at circumcision and other ethnic group identity characteristics were known, this information was not considered in this study. HIV seroprevalence rates in the capital cities were used to indicate the relative level of HIV infection in the countries. In 5 countries where more than 3 quarters of males were estimated to be uncircumcised, the average HIV prevalence was 16.4%. In none of the capital cities of these 5 countries was the seroprevalence less than 9.5%. Among the 20 countries where more than 90% of males were circumcised, the average seroprevalence was 0.9% and in no case did it exceed 4%. The correlation between these 2 variables was high (R=0.9%;P 0.001). Although strong population level correlation between circumcision and HIV seroprevalence exists, it does not prove a cause and effect relationship. However, the results are consistent with existing clinic-based studies that indicate a lower risk of HIV infection among circumcised males and suggest that male circumcision is a cofactor in HIV infection.

RESUMO

PIP: A review of the literature indicates that the association between human immunodeficiency virus (HIV) and prostitution varies by geographic region and can be altered substantially by well-planned public health interventions. In most African countries and in Asian countries such as Thailand, the rate of HIV infection among female prostitutes is substantially higher than the rate in the general population. Relatively few commercial sex workers in South and Central America are HIV-positive; however, their extremely high rates of infection with sexually transmitted diseases indicates the potential for future epidemic spread of HIV. In Europe and North America, HIV infection is most prevalent among drug-injecting or crack-using prostitutes. Neglected has been research on the high incidence of HIV among male transvestite and transsexual prostitutes. The lowest levels of condom use in commercial sex encounters have been recorded in regions in developing countries with the highest HIV prevalence. Also of concern are high condom breakage rates (20-50%) among female prostitutes who use petroleum-based lubricants and male prostitutes who practice anal sex. Valuable would be quantification of the additional HIV risk resulting from sex with a prostitute. Other recommended research areas include estimates of the number of male and female prostitutes working in certain geographic areas, mechanisms for monitoring condom use and substance abuse among prostitutes, the impact of HIV infection on movement into and out of prostitution, the dynamics of prostitute-client condom negotiation, and profiles of the clients of male prostitutes.^ieng

OBJECTIVES: To determine the age- and sex-specific prevalence of HIV-1 infection in a general ambulatory population in northern Thailand in 1990 and 1992, and the incidence of HIV-1 infections between 1990 and 1992. DESIGN: Health fairs were held in five villages in Chiang Mai province in northern Thailand between December 1989 and January 1990 and again in January 1992. Participation of all village residents was encouraged. Villagers were offered testing for serological markers of hepatitis B virus (HBV) infection and free HBV vaccine was made available to susceptible individuals. METHODS: Sera from the two surveys were linked and coded by demographic characteristics (age group and sex). Individual identifiers were removed and the sera tested for HIV-1 antibodies by enzyme-linked immunosorbent assay with Western blot confirmation. RESULTS: In 1990, 21 out of 1161 (1.8%) individuals were HIV-1-seropositive; the infection rates were 3.3% in men and 0.5% in women. In 1992, 44 out of 956 (4.6%) individuals were HIV-1-seropositive. Among people included in both surveys the 2-year incidence was 2.0% in women and 4.1% in men (annual incidence, 1.0 and 2.05%, respectively); however, among men over the age of 14 years the 2-year incidence was 6.3%, whereas among women of this age the 2-year incidence was 1.8% (annual incidence, 3.15 and 0.9%, respectively). Among men, incident HIV-1 infections were common, even among those aged 50 years or more. CONCLUSIONS: Infections with HIV-1 are disturbingly frequent and increasing among adult populations in semi-rural areas of northern Thailand. In order to contain further spread of the epidemic public-health strategies targeted to the general public, including those in rural areas, will be needed.

PIP: The development and testing of safe, effective, and affordable microbicidal products for women at risk of human immunodeficiency virus (HIV) raise complex scientific, ethical, and political issues. Ideal would by range of products (gels, foams, suppositories) that protect against HIV and other sexually transmitted diseases without impairing conception. This, in turn, requires the development of vaginal compounds that prevent the attachment of HIV-infected lymphocytes to epithelial surfaces, the secretion of virus from these cells, or the uptake of the virus by the mucosal epithelium with minimal cell membrane disruption. Problematic would be the design of a Phase III effectiveness trail that distributes the burdens and benefits of research equitably and provides controls with an alternate means of protection against HIV while maintaining scientific rigor. Since clinical trials call for a study population with a high seroincidence rate attributable primarily to the sexual transmission of HIV, prostitutes are the logical choice of subjects; sex workers are, however, the women most vulnerable to physical and economic exploitation. Women's input will be essential at each stage of development and testing, and such consultation can provide valuable feedback on method of application and timing of insertion. Finally, public-sector organizations must assume a leadership role in the coordination of the development of HIV prevention technologies.^ieng

OBJECTIVES: To assess the extent to which condoms are used effectively in commercial heterosexual intercourse. Data on the number of condoms that had broken or slipped off, the sexual technique during which this had occurred and the perceived cause of failure were collected. The use of non-water-soluble lubricants and non-fortified condoms during anal intercourse, and the demand for a greater variety of condom sizes were also examined. SUBJECTS AND METHODS: One hundred and twenty-seven female prostitutes and 91 male clients from different parts of The Netherlands were interviewed face-to-face between July 1990 and March 1991. RESULTS: Of those who used condoms during vaginal intercourse, 49% of the prostitutes had experienced condom breakage in the previous 6 months, and 16% of the clients in the previous 12 months. The breakage rate was 0.8% for prostitutes and 1.5% for clients. Condom quality was seldom reported as the cause; breakage was generally attributed to human factors, such as rough or prolonged intercourse, incorrect handling of the condom or the use of insufficient lubricant. Prostitutes also identified penis size as a cause. Condoms slipping off before or after ejaculation was reported less frequently than breakage. Thirteen per cent of clients and 36% of prostitutes expressed a need for either smaller or larger condoms. Of the prostitutes, 9% used oil or vaseline as a lubricant. CONCLUSIONS: In view of the low rate of condom failure in heterosexual prostitution in The Netherlands, the potential spread of HIV by this means is small. The use of a greater variety of condom sizes may further reduce the failure rate. Few prostitutes remain ignorant about the adverse effects of oil-based lubricants on condoms.

PIP: In Africa, HIV transmission occurs mainly through heterosexual intercourse. High-frequency transmitter core groups are key to the epidemiology of HIV-1 and STD on the continent. The rapid growth of the HIV-1 epidemic in Africa appears to have resulted, in part, from social and economic factors which result in individuals' frequent engagement in sexual intercourse with members of HIV-infected core groups. Understanding the importance of core groups in HIV-1 transmission is therefore key to developing more effective programs for the control of HIV-1. Sections explore the core groups concept and the sexual transmission of infection, social and economic forces creating core groups in Africa, the interaction of STD and HIV-1 in core groups, the effect of STD on HIV-1 disease progression in core groups in accelerating the HIV-1 epidemic, the role of core group interventions in control programs, balancing disease control with the potential for victimization, and research needs.^ieng

OBJECTIVE: To develop and evaluate a simple V3 peptide-based enzyme immunoassay (EIA) for large-scale serotyping of HIV-1 specimens from Thailand. DESIGN: Serologic reactivities with synthetic peptides derived from the V3 loop of gp120 were used for typing HIV-1 specimens. METHODS: Synthetic peptides PND-A and PND-B, derived from the consensus amino-acid sequences of the V3 loop of gp120 from two major genomic variants of HIV-1 in Thailand (A and B), were evaluated in an EIA on 61 Thai HIV-1 sera for which genotypes had been determined by polymerase chain reaction. The peptide EIA was then applied to sera from 188 HIV-1-infected patients, selected in non-random, convenience samples of known risk groups from four geographic regions of Thailand. RESULTS: The sensitivities and specificities of PND-A and PND-B were 86% (30 out of 35) and 96% (25 out of 26) and 92% (24 out of 26) and 94% (33 out of 35), respectively, with 100% predictive values of a monoreactive positive test for both peptides. The assay classified 101 specimens as serotype A, 39 as serotype B, eight as serotype AB (dually reactive), and 40 as untypable (non-reactive). Excluding dual reactors and non-reactors, 92% (77 out of 84) of specimens from patients probably infected by sexual contact were serotype A; conversely, 76% (28 out of 37) of injecting drug users were serotype B. CONCLUSION: The serologic results corroborated previous findings, in a smaller subset of samples, of an apparent segregation of viral subtypes by mode of transmission, suggesting two separate HIV-1 epidemics in Thailand. This peptide EIA could be a valuable epidemiologic tool in determining the dynamics of the rapid spread of HIV-1 in Thailand.


PIP: A simple synthetic enzyme immunoassay (EIA) for serotyping HIV-1 specimens from Thailand, based on gp120 V3 loop peptide, was developed and tested on 188 sera from 4 regions of the country. There are 2 major known gene variants of HIV-1 in Thailand designated genotype A and B. The peptide EIA was tested on 61 sera that had been characterized by polymerase chain reaction and DNA sequencing. The EIA was then tested on 188 sera from high risk groups collected in the northern, northeastern, central and southern regions in mid-1991. The PND-A assay was 86% sensitive and 96% specific; the PND-B assay was 96% sensitive and 92% specific. The EIAs showed 100% predictive values when sera known to be reactive to only HIV A or B were tested. In the series there were also 8 sera reactive to both A and B and 40 not reactive to either variant. Excluding dual and non-reactors, 92% of patients with sexual high risk factors had HIV-1 type A and 76% of those with IV drug use history had type B. The results suggest that 2 HIV-1 epidemics have occurred in Thailand, an initial wave in 1988 among IV drug users and a later wave centered among prostitutes and their clients.

OBJECTIVE: To investigate the relationship between HIV seropositivity and bacterial vaginosis (BV) in a population at high risk for sexual acquisition of HIV. DESIGN: A cross-sectional study was conducted among 144 female commercial sex workers in Chiang Mai, Thailand. METHODS: The participants were tested for cervical gonorrhea and Chlamydia infection, syphilis, Trichomonas vaginitis, Candida vaginitis, BV, and HIV infection. BV was diagnosed by clinical criteria (pH > 4.5, positive amine test, and presence of clue cells) and using Gram stains. RESULTS: Thirty-three per cent of participants had BV, and 43% were HIV-positive. Using clinical criteria, the association of BV and HIV seropositivity was significant [odds ratio (OR), 2.7; 95% confidence interval (CI), 1.3-5.0]. Although the association between BV and HIV prevalence was not significant using Gram stains alone for diagnosis of BV, an association was found between abnormal vaginal flora and HIV (OR, 2.1; 95% CI, 1.0-4.8). In multiple logistic regression analysis, adjusting for age, number of sexual encounters per week, current condom use, and currently having a sexually transmitted disease (STD), both BV and a history of an STD were independently associated with HIV seropositivity (adjusted OR for BV, 4.0 and 95% CI, 1.7-9.4; adjusted OR for history of an STD, 6.9 and 95% CI, 2.1-22.9). CONCLUSIONS: When diagnosed clinically, BV is independently associated with HIV seroprevalence. HIV infection may promote abnormal vaginal flora, or BV may increase susceptibility to sexual transmission of HIV. Alternatively, the association seen here may result from intervening variables; in this case BV may be a marker or a cofactor of HIV transmission.


PIP: To explore a possible association between bacterial vaginosis and human immunodeficiency virus (HIV) infection, 144 consecutively enrolled commercial sex workers from a sexually transmitted disease clinic (STD) in Chiang Mai, Thailand, were interviewed and underwent serologic testing and genital examination. 62 (43%) of sex workers were HIV-positive. A self-reported history of syphilis, chancroid, herpes, gonorrhea, or Chlamydia was significantly associated with HIV infection. Bacterial vaginosis, detected in 49 (34%), was also associated with HIV infection. Sex workers reporting 10-19 and 20 or more sexual encounters per week were 2.2 and 3.5 times, respectively, more likely to be infected with HIV than those reporting under 10 encounters. A clinically established diagnosis of bacterial vaginosis was independently associated with HIV seropositivity even when age, number of sexual encounters per week, current condom use, and past and current STD infection were controlled (odds ratio, 4.0; 95% confidence interval, 1.7-9.4). When the bacterial vaginosis diagnosis was based on Gram stain (score 7-10), however, the association with HIV seropositivity disappeared, but having abnormal vaginal flora (gram stain score 4-10) was related to HIV status. Further epidemiologic studies are recommended to investigate the possibility that bacterial vaginosis--the most prevalent genital infection in Thailand--acts as a cofactor for the heterosexual transmission of HIV.
