BACKGROUND: No studies have been conducted to define the lengths of the upper airway's different segments in normal healthy adults. AIMS/OBJECTIVES: This study aimed to determine the length of the subglottis and extrathoracic trachea and the factors affecting it. MATERIAL AND METHODS: This was an observational retrospective review study. Included 102 adult patients who underwent CT scan during the quiet inspiration phase of the upper airway. RESULTS: The results revealed significant positive linear relationships between height and both anterior and posterior subglottic measurements (p < 0.001). Additionally, a statistically significant, moderately strong negative correlation between age and extrathoracic tracheal measurements (p > 0.001) was observed. Men exhibited longer anterior (p < 0.001) and posterior (p > 0.001) subglottic measurements. In both sexes, the average length of the anterior subglottis was 14.16 (standard deviation [SD]: 2.72) mm, posterior subglottis was 14.51 (SD: 2.85) mm and extrathoracic trachea was 66.37 (SD: 13.71) mm. CONCLUSION AND SIGNIFICANCE: We concluded that a normal healthy adult's anterior subglottis length is 6.3-19.3 mm (mean: 14.16 [SD: 2.72] mm), posterior subglottis length is 6.1-20.0 mm (mean: 14.51 [SD: 2.85] mm) and extrathoracic trachea length is 25.2-98.5 mm (mean: 66.37 [SD: 13.71] mm). Age, height and sex affected the upper airway length.

Subglottic masses is very rare. The clinical data of five cases of subglottic mass in our hospital from 2017 to 2022 were summarized, and their clinical manifestations, auxiliary examination findings, treatment plan and pathological features were analyzed. Among the 5 patients, 1 case was subglottic pleomorphic adenoma, 1 case was subglottic granuloma, 1 case was subglottic breast cancer metastasis, 1 case was subglottic primary adenoid cystic carcinoma, and 1 case was immunoglobulin G4-related disease. No recurrence was observed in the patients so far. Subglottic mass is easy to be missed. Therefore, when the lesion is suspected in this area, the examination of ear, nose and throat should be carried out systematically to detect the lesion early and improve the prognosis.

RESUMO

Endoscopic posterior cricoid split and costal rib graft placement (EPCSCG) is an important tool in enlarging the glottic and subglottic airway, both of which can be disproportionally affected in the small airways of neonates and early infants. We present a series of 8 patients under the age of one who successfully underwent EPCSCG, with 7/8 patients avoiding tracheostomy entirely. Of these patients, the indication for EPCSCG was isolated bilateral vocal fold immobility (6/8), bilateral vocal fold immobility with subglottic stenosis (1/8), and isolated subglottic stenosis (1/8). EPCSCG can be safely applied to select patients less than one year of age.

OBJECTIVES: Recent immunologic study of the adaptive immune repertoire in the subglottic airway demonstrated high-frequency T cell clones that do not overlap between individuals. However, the anatomic distribution and antigenic target of the T cell repertoire in the proximal airway mucosa remain unresolved. METHODS: Single-cell RNA sequencing of matched scar and unaffected mucosa from idiopathic subglottic stenosis patients (iSGS, n = 32) was performed and compared with airway mucosa from healthy controls (n = 10). T cell receptor (TCR) sequences were interrogated via similarity network analysis to explore antigenic targets using the published algorithm: Grouping of Lymphocyte Interactions by Paratope Hotspots (GLIPH2). RESULTS: The mucosal T cell repertoire in healthy control airways consisted of highly expressed T cell clones conserved across anatomic subsites (trachea, bronchi, bronchioles, and lung). In iSGS, high-frequency clones were equally represented in both scar and adjacent non-scar tissue. Significant differences in repertoire structure between iSGS scar and unaffected mucosa was observed, driven by unique low-frequency clones. GLIPH2 results suggest low-frequency clones share targets between multiple iSGS patients. CONCLUSION: Healthy airway mucosa has a highly conserved T cell repertoire across multiple anatomic subsites. Similarly, iSGS patients have highly expressed T cell clones present in both scar and unaffected mucosa. iSGS airway scar possesses an abundance of less highly expanded clones with predicted antigen targets shared between patients. Interrogation of these shared motifs suggests abundant adaptive immunity to viral targets in iSGS airway scar. These results provide insight into disease pathogenesis and illuminate new treatment strategies in iSGS. LEVEL OF EVIDENCE: NA Laryngoscope, 134:3245-3252, 2024.

Abstract Objectives: Ectopic thymic tissue in the subglottis is an extremely rare disease that causes airway obstruction. Few cases reported were accurately diagnosed before surgery. Methods: A case of a 2-year-old boy with airway obstruction caused by a left subglottic mass was reported. The presentation of radiological imaging, direct laryngoscopy and bronchoscopy, pathology, and surgical management were reviewed. An extensive search in PubMed, EMBASE, Web of Science, Google Scholar, and EBSCO of English literature was performed without a limit of time. Results: Besides our case, only six cases were reported since 1987. The definitive diagnosis on these patients were made with the findings of pathology, of which, five were ectopic thymus and two were ectopic thymic cysts. Our case was the only one with a correct suspicion preoper-atively. Four cases underwent open surgical resection, and two cases underwent microlaryngeal surgery, while one deceased after emergency tracheostomy. No recurrences were found by six patients during the follow-up after successful treatments. Conclusion: Ectopic thymus is a rare condition, infrequently considered in the differential diagnosis of subglottic masses. Modified laryngofissure may be an effective approach to removing the subglottic ectopic thymus and reconstructing the intact subglottic mucosa.

La enfermedad relacionada con inmunoglobulina (Ig) G4 es una enfermedad de reciente conocimiento que puede comprometer cualquier órgano teniendo preferencias por ciertas regiones del cuerpo, donde la región de cabeza y cuello es uno de sus principales puntos afectados, pudiendo comprometer tanto la órbita, glándulas salivales, glándulas lagrimales, glándula tiroides, cavidades paranasales, hueso temporal, faringe y laringe. Este último órgano es infrecuentemente comprometido, solo existiendo 12 casos registrados en la literatura antes de la publicación de este escrito. Presentamos un caso de una mujer de 49 años con historia de disnea frente a esfuerzo, diagnosticándose una estenosis subglótica la cual fue manejada quirúrgicamente con una reconstrucción laringotraqueal. En el estudio histopatológico se evidenció histología compatible con enfermedad relacionada con IgG4, por lo que se inició tratamiento médico con corticotera- pia oral por un lapso de 2 meses en conjunto con inmunología. Paciente luego de 4 años de seguimiento, no ha presentado recaídas, manteniendo un lumen subglótico adecuado.


Immunoglobulin (Ig) G4-related disease is a medical condition of recent knowledge that can compromise any organ, having preferences for certain regions of the body, where the head and neck region is one of the main affected points, being able to affect orbit, salivary glands, lacrimal glands, thyroid gland, paranasal cavities, temporal bone, pharynx and larynx. The latter is infrequently compromised, with only 12 cases registered in the literature before the publication of this writing. We present a case of a 49-year-old woman with a history of exertional dyspnea, diagnosed with a sub- glottic stenosis which was managed surgically with laryngotracheal reconstruction. The histopathological study revealed histology compatible with IgG4-related disease, so medical treatment with oral corticosteroid therapy was started for a period of 2 months in conjunction with immunology. After 4 years of follow-up, the patient has not presented relapses, maintaining an adequate subglottic lumen.

Introducción: La lesión por ahorcamiento se inscribe dentro del traumatismo cerrado de la tráquea cervical. Objetivo: Presentar seis enfermos tratados por secuelas de un intento fallido de ahorcamiento y revisión la literatura sobre los métodos diagnósticos y terapéuticos. Métodos: Se realizó un estudio descriptivo de seis pacientes tratados entre 1989 y 2011 en el Hospital "Hermanos Ameijeiras". Todos fueron remitidos de diferentes hospitales, con una traqueostomía para el control de la vía respiratoria. Las variables estudiadas fueron: sexo, edad, diagnóstico definitivo, técnicas quirúrgicas empleadas, evolución posoperatoria, complicaciones y mortalidad. Resultados: Todos los pacientes fueron del sexo masculino, con edad entre 16 y 66 años (mediana 28,5). Cuatro tenían menos de 30 años, uno 38 y el último 66. El diagnóstico se hizo mediante traqueoscopia flexible y tomografía axial computarizada. La lesión se localizó en subglotis (2) y tráquea (4). Dos pacientes fueron operados antes de la remisión: resección traqueal (1) y laringotraqueal (1). El tratamiento definitivo, en nuestro centro, consistió en resección traqueal (4) y subglótica (2). No se presentaron complicaciones ni fallecidos. Durante el seguimiento postoperatorio (7 a 26 meses), cinco lesionados tuvieron resultados excelentes y uno satisfactorio, asociado con voz débil. Conclusiones: El traumatismo traqueal por ahorcamiento fallido es muy raro. El tratamiento oportuno y adecuado puede reducir las complicaciones y la mortalidad. Un equipo quirúrgico con experiencia puede lograr resultados satisfactorios en un grupo importante de lesionados. La traqueostomía, con anestesia local, es el método de elección para controlar la VR en pacientes con traumatismo cervical(AU)


Introduction: The hanging injury is considered a closed trauma of the cervical trachea. Objective: To present the cases of six patients treated for sequelae after failed hanging attempt, and to review the literature about diagnostic and therapeutic methods. Methods: A descriptive study was carried out of six consecutive patients treated between 1989 and 2011 at Hermanos Ameijeiras Hospital. All were referred from different hospitals, with a tracheostomy for controlling the airway. The variables studied were sex, age, definitive diagnosis, surgical techniques used, postoperative evolution, complications and mortality. Results: All patients were male, aged between 16 and 66 years (median: 28.5). Four were less than 30 years old; one, 38; and the last, 66. The diagnosis was made by flexible tracheoscopy and computerized axial tomography. The injury was located in the subglottis (2) and the trachea (4). Two patients were operated on before referral, through tracheal (1) and laryngotracheal (1) resection. Definitive treatment in our center consisted of tracheal (4) and subglottic (2) resection. There were no complications or deaths. During the postoperative follow-up (7-26 months), five injured patients had excellent outcomes and one had satisfactory outcomes, associated with a weak voice. Conclusions: Tracheal trauma after failed hanging is very rare. Timely and appropriate managment can reduce complications and mortality. An experienced surgical team can achieve satisfactory outcomes in a large group of injured people. Tracheostomy, under local anesthesia, is the method of choice for controlling the airways in patients with cervical trauma(AU)

Introducción: La estenosis laringotraqueal es una patología de difícil manejo y obtener un resultado que permita, al sujeto que la padece, volver a tener una función adecuada fonorrespiratoria y deglutoria, no siempre es factible de obtener. Objetivo: Presentar una serie clínica de pacientes que tienen una estenosis de la vía aérea superior (VAS) y que fueron intervenidos quirúrgicamente. Como objetivo secundario es evaluar si a técnicas similares hay o no diferencias entre los grupos etarios. Material y método: Presentamos un análisis retrospectivo de los pacientes intervenidos quirúrgicamente por los autores. En él se realiza la descripción demográfica de los casos, sitio de la estenosis, tipo de intervención según edad; porcentaje de éxito en la decanulación después de una o varias intervenciones, necesidad de reoperación, tipo de injerto y tutores utilizados. Se dividió el grupo en pediátrico (hasta los 18 años) y adulto (mayores de 19 años). Los resultados fueron analizados con las pruebas no paramétricas de x² y de Fisher. Resultados: La casuística consta de 88 pacientes intervenidos quirúrgicamente para reparar una estenosis laringotraqueal. Los menores de 18 años corresponden a 45 casos (51 por ciento). El porcentaje de éxito alcanzado en la primera cirugía es de 75,6 por ciento (34/45 casos) en los menores de 18 años y de 76,7 por ciento por ciento (33/43 casos) en los mayores de 19 años. Se reoperan 15/21 casos fracasados en primera instancia; logrando decanular a 13 de ellos aumentando el éxito a 90,9° por ciento. Fracasan a las reoperaciones y pendientes de resolver aún, 8 casos. La técnica más utilizada fue la reconstrucción laringotraqueal con injerto de cartílago costal ya sea anterior y/o posterior, por ser la estenosis subglótica el sitio de la lesión. Introduction: Laryngo-tracheal stenosis is a condition difficult to manage and obtain results which permit the person who suffers it recover phonorespiratory and deglutory function. This is not always possible to achieve. Aim: Present a clinical series of patients with stenosis of the VAS and underwent surgery. A secondary objective is to assess whether or not there are similar technical differences between age groups. Material and method: We present a retrospective analysis of patients surgically treated by the authors. They present demographic description cases, site of stenosis, type of intervention according to age; percentage of successful decannulation after one or more interventions; reoperation, type of graft and stents used. The group was divided into pediatric and adult. Statistical analysis was performed with x2 and Fisher. Results: The case mix consists of 88 patients who underwent surgery to repair laryngo-tracheal stenosis. Children under 18 years correspond to 45 cases (51 percent percent). The percentage of success in the first surgery is 75.6 percent percent (34/45 cases) in children under 18 and 76.7 percent percent (33/43 cases) over 19years. 15/21 failed cases were reoperated in the first instance; 13 of them were decannulated increasing success to 90.9 percent percent. Reoperation failure and still unresolved, 8 cases. Surgical technique used was laryngotracheal reconstruction with costal cartilage graft either anterior or posterior being subglottic the site of stenosis.
