RESUMO

Allergic diseases have risen sharply in recent decades. After some epidemiological data, we take a look at the various hypotheses explaining this allergy "epidemic". Changes in our environment, such as pollution, are a source of climate change and an increase in allergic diseases through inflammation of epithelial barriers. Allergy prevention, a public health emergency, relies on environmental actions at both individual and collective levels.

RESUMO

Pediatricians and allergists have noted a recent increase in cases of food allergy and anaphylaxis to peanuts and nuts, affecting very young children with worrying consequences in terms of quality of life. Children suffering from persistent cow's milk protein allergies that do not heal spontaneously are at very high risk of a fatal accident. Based on the findings of these studies, recommendations for primary prevention are made.

RESUMO

The Angele project (for Allergies complexes: prise en charge globale, diététique et environnementale) is an article 51 experiment in shared care pathways in allergology. These care paths, dedicated to patients with allergies to house dust mites and/or food, involve collaboration between doctors and paramedics. The aim of this initiative is to optimize patient care by structuring their care pathways. This article presents these pathways and the preliminary results of the experiment.

Purpose: To assess knowledge of Canadian dietitians on the topics of food allergy and food allergy prevention guidelines, including introduction of allergenic solids to infants at risk of food allergy.Methods: An online survey was distributed via email listservs targeting Canadian dietitians.Results: In total, 144 of 261 dietitians completed the survey (60.5%). Respondents recommend introduction of peanut (89.5%) and allergenic solids (91.2%) within the recommended age of 4-6 months for infants at high risk of food allergy, but only 26.2% recommend offering peanut three times per week once it has been introduced. In identifying what constitutes an infant at high risk of developing peanut allergy, dietitians expressed lower comfort levels and lower number of correct responses.Conclusions: Dietitians demonstrated they are up to date regarding the timing of introduction of allergenic solids, but not the frequency of consumption once introduced, for infants at high risk of food allergy. They also expressed low comfort level identifying risk factors for peanut allergy. There are opportunities for further education of dietitians, as well as potential to further utilize dietitian services for the benefit of patients with food allergy or who are at risk for food allergy.

In Europe, the prevalence of food allergy is estimated at 6-8% of children. Ten to 20% of pediatric food-induced anaphylaxis reactions occur at school. Individual healthcare plans (IHP) for food allergy aim at: identifying children at risk of allergic reactions; reducing the risk of allergen exposure; providing emergency kits containing adrenaline auto-injectors (AAI) if needed with emergency action plans and instructions about when and how to use AAI. In France, IHP were introduced into law in 2003 and was updated in 2021. The number of IHP for allergy is increasing since 10 years (50,000 IHP for allergy/year). While the recommendations of the learned societies have resulted in the national harmonization of criteria for the implementation of IHP for allergy and for the prescription of emergency kits with AAI, adrenaline remains underused. In 2019, a national policy stated that all high schools must have a provision of spare AAI in case of anaphylaxis and the promotion of school staff training about food allergy and anaphylaxis was encouraged. These recommendations should be assessed widely and allergy training should be widespread. Pharmacists play an important role to take care of food-allergic children: provision of AAI prescribed for the most at-risk food allergic patients, advice and information on AAI. The pharmacist is therefore a key player in the therapeutic education of the patient to reinforce the key messages on the efficacy and safety of adrenaline used for anaphylaxis.

Allergy is a frequent and often disabling condition for the patients. In the context of allergic rhinitis and asthma or anaphylactic shock to hymenoptera venom, allergenic desensitization can be proposed to limit the symptoms or recurrence of major reactions. Access to this treatment is made difficult by a lack of allergists, a lack of products or, more recently, by the Covid-19 crisis.

Allergy, anaphylaxis and adrenaline are three closely related terms in medicine. Upon identification or suspicion of anaphylaxis, adrenaline should be administered as soon as possible. An allergy can be characterised by anything from a simple skin rash to severe vital parameter abnormalities. The role of the reception nurse will be first to identify clinically relevant features, to assign the right level of triage to the patient and adjust the care pathway.

Isolated mucocutaneous involvement and anaphylaxis are two frequent pathologies in the emergency room. Recognition of the symptoms and the rapidity of treatment are essential for the clinical evolution. Monitoring and standardisation of patient discharge from the emergency department is a guarantee of quality and reduces the risk of morbidity and mortality.

Un nourrisson est à haut risque d'allergie alimentaire si lui ou un membre de sa famille immédiate présente une affection atopique (comme l'eczéma). Il faut promouvoir et soutenir l'allaitement, quels que soient les enjeux reliés à la prévention des allergies alimentaires, mais chez les nourrissons dont la mère ne peut pas allaiter ou choisit de ne pas le faire, il n'est pas recommandé d'utiliser une préparation en particulier (p. ex., les préparations hydrolysées) pour prévenir les allergies alimentaires. Lorsque les préparations à base de lait de vache sont introduites dans l'alimentation d'un nourrisson, il faut s'assurer de maintenir une ingestion régulière (pas nécessairement plus de 10 mL par jour) pour éviter la perte de tolérance. Chez les nourrissons à haut risque, des données concluantes indiquent que l'introduction précoce d'aliments allergènes (vers l'âge de six mois, mais pas avant l'âge de quatre mois) peut prévenir les allergies alimentaires courantes, notamment les allergies aux arachides et aux œufs. Lorsqu'un aliment allergène a été introduit, il est important d'en maintenir une ingestion régulière (p. ex., quelques fois par semaine) pour maintenir la tolérance. Il est possible d'introduire les aliments allergènes courants sans faire de pause de quelques jours entre chaque nouvel aliment. Par ailleurs, le risque d'une grave réaction lors de la première exposition est très faible chez le nourrisson. Il n'est pas recommandé de procéder au dépistage préventif en cabinet avant d'introduire des aliments allergènes. Aucune recommandation ne peut être formulée pour l'instant sur le rôle des modifications à l'alimentation de la mère pendant la grossesse ou l'allaitement, ou sur les suppléments de vitamine D, d'oméga 3, de prébiotiques ou de probiotiques pour prévenir les allergies alimentaires.

Sexual activity may on occasion be associated with local trauma, infections or allergies. During visits, patients may be reluctant to report on the circumstances leading to such lesions. The dermatologist may be unfamiliar with these manifestations, especially where they occur outside the genital area. Our aim is to present the spectrum of genital and extra-genital signs associated with sexuality. The dermatologist must feel sufficiently confident to broach the patient's sexual history in order to avoid any diagnostic errors and unnecessary explorations.

RESUMO

Cutaneous drug reactions are a common reason for calls and visits. This term chiefly refers to hypersensitivity reactions ranging from benign rash without contraindication of treatment to severe life-threatening clinical pictures, such as anaphylactic shock and epidermal necrolysis. They should be carefully managed from the outset. Indeed, history taking and precise semiological description of the lesions are crucial to the formulation of recommendations for the patient. Allergological investigation of such reactions has developed greatly in recent decades and must now be carried out much more extensively. The arrival of new drug families such as biotherapies and the development of drug habituation protocols constitute the challenges of tomorrow for cutaneous drug reactions.

Introduction: La sensibilisation aux trophallergènes, bien que mal connue, est décrite comme responsable d'une sensibilité relativement fréquente. L'objectif de cette étude était de décrire le profil de la sensibilisation aux trophallergènes chez les enfants. Méthodes: Il s'est agi d'une étude prospective, descriptive et analytique qui s'est déroulée de 1er juillet 2015 à 30 juin 2022 dans le service de pneumo-allergologie du CHU-Campus de Lomé et ayant porté sur les patients qui après acceptation et réalisation des tests cutanés étaient sensibilisés aux trophallergènes. Résultats: Nous avons recensé au total 468 patients âgés de 7 mois à 18 ans ; la fréquence de réalisation des tests était de 25,9% et celle de la sensibilisation aux trophallergènes de 66,9%. L'âge moyen des patients était de 8,12 ± 0,77 ans. Le sexe ratio était de 1,3. La rhinite allergique était le motif de consultation dans 39,9 % des cas. L'arachide était l'aliment incriminé dans 31,5 % des cas comme trophallergène dans les antécédents personnels. Les patients étaient sensibilisés aux oléagineuses 70,4% (arachide, sésame) ; aux légumineuses 24,6% (soja, haricot) ; aux légumes 4,7% (gombo, corète potagère) ; aux fruits 33,6% (banane, avocat) ; aux céréales 11,8% (blé, maïs) ; aux tubercules 1,9% (manioc, igname) ; à l'œuf 54,9% ; à la crevette 53,6% ; au lait de vache 51,1% ; au crabe 16,5%. Conclusion: La sensibilisation aux trophallergènes était élevée et concernait aussi bien les protéines animales que végétales.

Whether they are professional or amateur, musicians are exposed to a various number of complications in relation to their practice, including cutaneous ones. The latter are often a simple musical stigma and a reason for pride for the musician. If they are symptomatic and disturb the musician, they have to be treated and prevented. We reviewed here those occupational and peculiar lesions that are often not that well known to the dermatologist.

Cow's milk protein allergy (CMPA) is a public health issue in children whose quality of life is strongly affected. The objective of this article is to review the present state of knowledge on the CMPA, and highlight some emerging alternatives in its management and in its prevention. Good knowledge in the appropriate infant formula, exclusion diets, the handling of the emergency treatment thanks to the use of epinephrine auto-injector pens, the use of personalized care project and accessibility to allergic patients' association are factors that secure the management. Breastfeeding and appropriate dietary diversification are for their part major preventive measures. The use of probiotics and desensitization immunotherapy are interesting emerging tracks. The role of community pharmacists in all these steps is discussed. It is indeed a nearby health professional involved both in improving prevention and in the optimization of the management. Its educational posture is crucial in assisting patients to help them better understand the CMPA and its treatment; as part of its new tasks, he can be integrated into therapeutic education programs to help allergic children and their families in a multidisciplinary context to better manage their daily life.

INTRODUCTION: Allergen-specific immunotherapy (ASIT) is the only etiologic treatment of atopic dermatitis in dogs. In humans it has been shown that intralymphatic immunotherapy (ILIT) enhanced efficacy and patient compliance and reduced treatment time from 3 years to 8 weeks. As only safety data have been published yet, the aim of this study was to evaluate the clinical efficacy of ILIT in dogs. 20 atopic dogs underwent ILIT with alum-precipitated allergens administered every 4 weeks for 3 to 7 times in the popliteal lymph node. Pruritus (Hill score), CADESI (canine atopic dermatitis severety index), concurrent medications and adverse reactions were recorded initially and every 4 weeks for a total period of 24 weeks. The observed clinical response was good in 12/20 (60%) patients and improvement could be seen in some dogs already after 4 weeks. The median number of injections was 5.6. All dogs tolerated the procedure well and no adverse effects were recognized during or after ILIT. Therefore ILIT should be regarded as a safe alternative to subucaneous ASIT, enabling a faster clinical improvement with the same response rate.


INTRODUCTION: L'immunothérapie spécifique de l'allergène est le seul traitement étiologique de la dermite atopique du chien. On a pu montrer que, chez l'homme, l'immunothérapie intralymphatique (ITIL) augmente la fiabilité du traitement et permet de réduire sa durée de 3 ans à 8 semaines. Comme jusqu'à ce jour seules des données relatives à la tolérance avaient été publiées, la présente étude a pour but d'examiner l'efficacité clinique de l'ITIL chez les chiens. Vingt chiens atopiques ont été désensibilisés au moyen d'allergènes précipités à l'aluminium par ITIL dans les ganglions poplités toutes les 4 semaines. Le prurit (Hill score), le CADESI (canine atopic dermatitis severity index), les médicaments appliqués et les effets secondaires observés ont été enregistrés au début du traitement puis toutes les 4 semaines durant au total 24 semaines. 12/20 (60%) des patients ont bien répondu au traitement. L'amélioration clinique a pu être partiellement constatée après 4 semaines déjà. En moyenne, 5.6 injections ont été nécessaires. Tous les chiens ont bien toléré l'ITIL et il n'a pas été observé d'effet secondaire pendant ou après le traitement. L'immunothérapie intralymphatique semble donc une alternative sure à l'immunothérapie spécifique de l'antigène sous-cutanée et permet d'obtenir un effet plus rapide avec le même taux de réponse.

RESUMO

So-called "allergic" reactions to ink or colouring agents constitute the main current complication associated with tattoos that lead individuals to consult. However, general practitioners are frequently at a loss about how to manage such complications. In order to assist clinicians in their daily practice, we propose an update of the modes of managing allergic reactions to tattoos, and we offer a therapeutic scale and a decision-making algorithm.

Urticaria is a common skin disease that may affect 20 % of the general population. Most of the time, urticaria is an acute disorder that rarely can be chronic. The difficulty in urticaria is not the clinical diagnosis because the rash is characteristic, but the underlying causes and treatment that result. Urticaria is a benign disease when chronic and potentially dangerous when acute and associated with allergy. This allergy risk, needs an allergy exploration, based on skin tests and / or specific IgE assays. Because allergy is unusual in chronic urticaria, no allergy tests should be performed. By contrast, these tests must be undertaken in case of acute urticaria with a strong suspicion of IgE-mediated reaction because of the risk of severe anaphylaxis in case of allergenic re-exposure.

BACKGROUND: Allergic hypersensitivity to unfractioned or low-molecular-weight heparins is uncommon but is known, and in particular the most common form is localized dermatitis, although such cases have seldom turned into maculopapular exanthema. Since cross-reactions with other heparins are frequent, identification of therapeutic alternatives is essential. PATIENTS AND METHODS: This retrospective study included patients referred to the Department of Dermatology and Allergology at Tenon Hospital between 2000 and 2012 with suspicion of allergy to unfractionated heparin (UFH) or low-molecular-weight heparin (LMWH) and sensitized to at least one heparin (i.e. positive skin tests to at least one heparin). The heparins and hirudins used were tested in the forearm by means of intradermal skin tests. All patients were contacted in 2012 to establish whether they had used some form of heparin since the cutaneous allergy tests. RESULTS: Nineteen patients had at least one positive skin test for heparin; 1 patient had presented anaphylactic shock, while 18 others had presented localized eczema (12) or generalized dermatitis (6). The heparin most often responsible for these adverse reactions was enoxaparin (13/19). An LMWH was responsible in most cases (18 vs. 1 with UFH). Of these 18 patients, 16 also presented positive skin tests for UFH, 9 for synthetic heparinoid and 1 for hirudin. 11/19 patients were tested for fondaparinux (a synthetic pentasaccharid) and all had negative skin tests. 5/7 patients with negative skin tests had taken fondaparinux without any visible reaction, whereas 2 who also tested negative experienced localized eruption at the injection site. DISCUSSION: Our results underline the greater frequency of delayed hypersensitivity reactions compared with immediate reactions to heparins. Skin tests can help to identify substitution molecules. Fondaparinux might be an alternative but certain diagnosis relies on rechallenge.
