RESUMO

Intestinal microbiome contains several times of functional genes compared to the host and mediates the generation of multiple metabolic products, and therefore it is called "second genome" for host. Crustaceans rank second among the largest subphylum of aquaculture animals that are considered potentially satisfy global substantial food and nutrition security, among which the Pacific white shrimp (Litopenaeus vannamei) ranks the first in the production. Currently, increasing evidences show that outbreaks of some most devastating diseases in shrimp, including white feces syndrome (WFS) and acute hepatopancreatic necrosis disease (AHPND), are related to intestinal microbiota dysbiosis. Importantly, the intestine microbial composition can be altered by environmental stress, diet, and age. In this review, we overview the progress of intestinal microbiota dysbiosis and WFS or ANPHD in shrimp, and how the microbial composition is altered by external factors. Hence, developing suitable microbial micro-ecological prevention and control strategy to maintain intestinal balance may be a feasible solution to reduce the risk of disease outbreaks. Moreover, we highlight that defining the "healthy intestine microbiota" and evaluating the causality of intestinal microbiota dysbiosis and diseases following the logic of "Microecological Koch's postulates" should be the key goal in future shrimp intestinal field, which help to guide disease diagnosis and prevent disease outbreaks in shrimp farming. Objective: To explore the present situation and epidemiological characteristics of pesticide poisoning in Chengdu City from 2012 to 2021, and to provide scientific evidence for further prevention and control. Methods: In January 2022, the pesticide poisoning report cards of Chengdu City from 2012 to 2021 were collected from the China Disease Control and Prevention Information System. The data of the report card was reorganized and the distribution characteristics of pesticide poisoning such as time, region, gender, age and pesticide types were analyzed. Results: 14326 cases of pesticide poisoning were reported in Chengdu City from 2012 to 2021, 651 deaths, and the fatality rate was 4.54%. The cases of productive pesticide poisoning and unproductive pesticide poisoning were 504 and 13822, respectively. The fatality rates of productive and unproductive pesticide poisoning were 1.39% and 4.66%, which were significant different (χ(2)=11.99, P=0.001). The highest reported cases of pesticide poisoning was in 2013 (1779) and the lowest in 2021 (1047). The number of reported cases showed a downward trend year by year (t=-12.30, P<0.001), and the fatality rates also showed a downward trend year by year (χ(2)(trend)=25.48, P<0.001). The fluctuation range of unproductive pesticide poisoning cases in each month of the year was small, and the productive pesticide poisoning mainly occurred from May to August. The regions with the largest number of reported poisoning cases were Pengzhou (1620), Jianyang (1393), Jintang (1266) and Qionglai (1158). The high incidence of poisoning was among 25-54 years old (50.21%, 7193/14326). The fatality rate in the age group 75-96 years old was the highest (8.98%, 95/1058), and the fatality rates increased gradually with age (χ(2)(trend)=186.03, P<0.001). The pesticides causing poisoning were mainly insecticide (43.86%, 6284/14326) and herbicides (35.75%, 5121/14326). Herbicides paraquat had the highest fatality rate (9.54%, 286/2998) . Conclusion: Pesticide poisoning in Chengdu City is mainly unproductive poisoning. Health education should be carried out for key areas and people, and the control of highly toxic pesticides such as insecticide and herbicides should be strengthened.

BACKGROUND: We investigated college students' attitude and compliance towards a prevention strategy involving use of non-pharmaceutical interventions (NPIs) against coronavirus disease 2019 (COVID-19). METHODS: We conducted a cross-sectional online survey in four universities in Guangdong Province (China) based on purposive sampling. A self-administered questionnaire was given to College students (CSs) to measure the supportive attitude towards an outbreak control strategy and adoption of NPIs in respondents. RESULTS: A total of 44,446 CSs participated between 31 January and 10 February 2020; 92.7% of respondents supported the outbreak control strategy. The proportion of respondents who avoided public places, wore a facemask, avoid gatherings, and washed hands more frequently than usual was 94.8, 92.8, 91.2 and 86.9%. respectively. A total of 76.5% respondents adopted all four measures. A supportive attitude was associated with NPI adoption. Students who were female, postgraduate, anxious, and not depressed tended to have a higher supportive attitude and higher chance of NPI adoption. CONCLUSIONS: Higher supportiveness towards the disease control strategy for the Chinese public may lead to higher adoption rate of NPIs. Psychosocial factors were related to a supportive attitude and adoption of the NPI. We believe that our findings could aid policymakers to create NPIs to prevent and control emerging infectious diseases such as COVID-19.

Coronavirus-caused pneumonia (COVID-19) broke out in December 2019. The virus soon proved to be extremely contagious and caused an international pandemic. Clinicians treating COVID-19 patients face considerable danger of occupational exposure because of the highly infectious nature of the virus, and precautions must be taken to prevent medical staff infections. This article lists important measures that may save the lives of patients and medical staff during the COVID-19 pandemic and help stop the transmission of COVID-19 on hospital grounds. The suggestions include establishing detailed infection control and prevention protocols in the operating room; expediting testing procedures and patient screening for COVID-19; using case-specific treatment planning for vascular patients with COVID-19, favoring minimally invasive methods; and establishing and reinforcing protective awareness of medical personnel.

As a new variant of COVID-19 with varied mutations, Omicron is more transmissible, more rapidly contagious, and has a greater risk of reinfection. Given those facts, a precise manage strategy needs to be formulated and implemented in designated megacities. Here, the precise COVID-19 prevention and control strategy for a designated hospital in Shenzhen, China is summarized, including implementation of a two-wing "On duty/On standby" approach based on busy and calm periods, an identification, classification, and grading system for the occupational exposure risks of medical staff, classification of patient transmission risks, separate admission, and an innovative treatment (nasal irrigation). The strategy has enabled the efficient and orderly integration of resources, it has resulted in zero infections among medical staff even during the peak hours of the pandemic at the hospital (1,930 patients admitted to both wings in a single day), and it has significantly reduced the initial period of no virus detection when patients infected with Omicron received saline nasal irrigation (P < 0.001). This strategy has provided evidence of precise prevention and control in a hospital, infection control, and efficient patient treatment in an era when Omicron is widespread.

The COVID-19 pandemic now affects the entire world and has many major effects on the global economy, environment, health, and society. Focusing on the harm COVID-19 poses for human health and society, this study used system dynamics to establish a prevention and control model that combines material supply, public opinion dissemination, public awareness, scientific and technological research, staggered work shifts, and the warning effect (of law/policy). Causal loop analysis was used to identify interactions between subsystems and explore the key factors affecting social benefit. Further, different scenarios were dynamically simulated to explore optimal combination modes. The main findings were as follows: (1) The low supervision mode will produce a lag effect and superimposed effect on material supply and impede social benefit. (2) The strong supervision mode has multiple performances; it can reduce online public opinion dissemination and the rate of concealment and false declaration and improve government credibility and social benefit. However, a fading effect will appear in the middle and late periods, and over time, the effect of strong supervision will gradually weaken (but occasionally rebound) and thus require adjustment. These findings can provide a theoretical basis for improving epidemic prevention and control measures.

Background: Since the first case reported in December 2019, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has caused an outbreak of coronavirus disease 2019 (COVID-19) worldwide. The global case count continued to rise and the WHO declared a Public Health Emergency of International Concern (PHEIC), causing a growing risk of imported COVID-19 infection. This study aimed to provide descriptive and quantitative epidemiological characteristics of imported COVID-19 cases in China. Methods: This cross-sectional study examined all imported COVID-19 cases in Mainland China from 22 January to 21 April 2020. Ratios, Median and percentile were used for descriptive analysis. Spearman's correlation analysis was performed between daily new imported cases in Mainland China and the country of origin. The chi-square test was used to evaluate the difference between home quarantine and compulsory centralized quarantine on native transmission. Results: A total of 1,610 cases of COVID-19 were imported from 49 countries to 27 provincial administrative regions in China; 79.8% were from European countries and the United States of America (the USA). Before 29 March 2020, the imported cases were mainly from the USA (27.7%) and United Kingdom (UK; 42.6%). After 29 March 2020, the daily newly imported cases from Russia rapidly grew. After 12 April 2020, the number of daily newly imported cases gradually decreased and remained at a low level (12±7 cases per day). Airport entry was encouraged, and ground border crossing was limited. Among the 1,610 cases, 54.0% were in the asymptomatic incubation period on arrival in Mainland China. Conclusions: The transmissions by imported COVID-19 were gradually and effectively curbed in Mainland China, despite a disproportionally high number of cases worldwide. Entry screening measures must be implemented universally to all inbound travelers at a point of entry or targeted to specific travel routes or to specific travelers. Compulsory centralized quarantine should be recommended in the prevention of the imported COVID-19 epidemic.

OBJECTIVES: Vancomycin-resistant Enterococcus faecium (VREfm) has emerged as a pathogen of major concern for public health. Although definitive evidence is lacking, contact precautions have been a crucial element in infection prevention and control (IPC) strategies designed to limit nosocomial VRE transmission. This study investigated the effect of discontinuing contact precautions while enforcing basic hygiene measures, including hand hygiene, environmental cleaning and antiseptic body washing, for patients with VRE in intensive care units (ICUs) on the prevention of nosocomial VRE transmission causing bacteraemia. METHODS: Contact precautions were discontinued in January 2018. In total, 96 VREfm isolates from 61 patients with VREfm bacteraemia and/or colonization hospitalized in eight ICUs in a tertiary care hospital in 2016 and 2019 in were characterized by whole-genome sequencing. VRE transmission was investigated using patient movement data and admission screening for reliable identification of nosocomial acquisition. RESULTS: Discontinuation of contact precautions did not increase VREfm transmission events (eight in 2016 vs one in 2019). While the rate of endogenous VREfm was similar in both years (38% vs 31%), the number of non-colonized patients prior to VREfm bacteraemia was 16 (16/29, 55%) in 2019, which was significantly higher than in 2016 (8/32, 25%). The mean incidence density for VREfm bacteraemia was similar for both years (0.26 vs 0.31 per 1000 patient-days in 2016 and 2019, respectively). CONCLUSION: Discontinuation of contact precautions while enforcing basic hygiene measures did not lead to an increase in nosocomial bloodstream infection rates due to VREfm transmission in a hyperendemic ICU setting.

OBJECTIVES: Cardiovascular disease (CVD) is leading cause of death in China. We aimed to provide national and subnational estimates and its change of premature mortality burden of CVD during 2005-2020. METHODS: Data from multi-source on the basis of national mortality surveillance system (NMSS) was used to estimate mortality and years of life lost (YLL) of total CVD and its subcategories in Chinese population across 31 provinces during 2005-2020. RESULTS: Estimated CVD deaths increased from 3.09 million in 2005 to 4.58 million in 2020; the age-standardized mortality rate (ASMR) decreased from 286.85 per 100,000 in 2005 to 245.39 per 100,000 in 2020. A substantial reduction of 19.27% of CVD premature mortality burden, as measured by age-standardized YLL rate, was observed. Ischemic heart disease (IHD), hemorrhagic stroke (HS) and ischemic stroke (IS) were leading 3 causes of CVD death. Marked differences were observed in geographical patterns for total CVD and its subcategories, and it appeared to be lower in areas with higher economic development. Population ageing was dominant driver contributed to CVD deaths increase, followed by population growth. And, age-specific mortality shifts contributed largely to CVD deaths decline in most provinces. CONCLUSION: Substantial discrepancies were demonstrated in CVD premature mortality burden across China. Targeted considerations were needed to integrate primary care with clinical care through intensifying further strategies for reducing CVD mortality among specific subcategories, high risk population and regions with inadequate healthcare resources.

Objective: To analyze the epidemic situation and influencing factors of plague in Dehong Dai and Jingpo Autonomous prefectures (Dehong) from 1950 to 2019, for the improvement of strategies on prevention and control of the disease. Methods: The epidemic situation and surveillance data of animal and human plague in Dehong prefecture from 1950 to 2019 were collected and analyzed by descriptive epidemiological methods. Multiple linear regression equations were established by SPSS 20.0 software to analyze the relationship between plague among animals and rat density and flea index, as well as the relationship between human plague and plague among animals. Results: In Dehong prefecture, plague experienced a phenomenon of epidemic-resting-epidemic-resting cycle, between 1950 and 2019. During this period, 614 epidemic spots were identified in 36 townships of five counties/cities,. Human plague had been prevalent for 15 years, with 1 153 human cases and 379 deaths involved. Between 1982 and 2019, 261 319 rodents were captured, of which Rattus flavipectus accounted for 70.95%(185 421/261 319). A total of 70 124 fleas were seized with 76.65%(53 752/70 124), xenopsylla cheopis as fleas. The index of fleas and free fleas were 0.57 and 0.22 respectively. A total of 1 577 strains of Yersinia pestis were isolated from 285 091 animal specimens and 418 strains of Yersinia pestis were isolated from 60 119 groups of fleas, with 255 F1 antibody positive samples were detected from 64 157 sera samples. Neither Yersinia pestis nor the outbreak of plague had been noticed since 2008. Correlations between both animal plagues and rat density, as well as human plague and animals plague were noticed from the regression analysis. Conclusions: The epidemics of plague were with long-term and stable nature, in Dehong prefecture. Affected by many factors, plague seemed in a resting period currently. However, we should be reminded that the fact that outbreak of plague may revive at some uncertaint point. Hence in order to prevent human plague, strategies as careful surveillance programs on animals, need to be strenthened.

The COVID-19 outbreak, designated a "pandemic" by the World Health Organization (WHO) on 11 March 2020, has spread worldwide rapidly. Each country implemented prevention and control strategies, mainly classified as SARS LCS (SARS-like containment strategy) or PAIN LMS (pandemic influenza-like mitigation strategy). The reasons for variation in each strategy's efficacy in controlling COVID-19 epidemics were unclear and are investigated in this paper. On the basis of the daily number of confirmed local (imported) cases and onset-to-confirmation distributions for local cases, we initially estimated the daily number of local (imported) illness onsets by a deconvolution method for mainland China, South Korea, Japan and Spain, and then estimated the effective reproduction numbers Rt by using a Bayesian method for each of the four countries. China and South Korea adopted a strict SARS LCS, to completely block the spread via lockdown, strict travel restrictions and by detection and isolation of patients, which led to persistent declines in effective reproduction numbers. In contrast, Japan and Spain adopted a typical PAIN LMS to mitigate the spread via maintaining social distance, self-quarantine and isolation etc., which reduced the Rt values but with oscillations around 1. The finding suggests that governments may need to consider multiple factors such as quantities of medical resources, the likely extent of the public's compliance to different intensities of intervention measures, and the economic situation to design the most appropriate policies to fight COVID-19 epidemics.

Objective To analyze the overseas imported malaria situation of Shijiazhuang City from 2012 to 2015, so as to provide the evidence for exploring the prevention and control strategy. Methods The data of overseas imported malaria were collected and analyzed with the descriptive method including Plasmodium species, population characteristics, epidemic focus distribution, onset time, diagnosis and treatment in Shijiazhuang City from 2012 to 2015, and the time from the onset to first medical visit, time from first medical visit to being diagnosed, and time from onset to being diagnosed of different Plasmodium species were analyzed respectively with the statistical analysis method. Results A total of 92 overseas imported malaria cases were reported in Shijiazhuang City from 2012 to 2015, including 88 cases from African countries (falciparum malaria taking 53.41%), and 4 cases from Southeast Asian and other countries (vivax malaria taking 50%). Eighty-nine cases were distributed in 28 counties (districts) of 6 cities in Hebei Province, except 2 persons with foreign nationalities and 1 with Anhui Province census register. The male and young adults were dominant. The median time from the onset to seeing a doctor was one day and the median time from seeing a doctor to being diagnosed was five days. Most cases were reported by the Fifth Hospital of Shijiazhuang which was the sentinel hospital. Totally 42.39% of the cases were misdiagnosed when the first visit to a doctor. All of the cases were laboratory confirmed and 100% of them received the standard treatment after diagnosis. Conclusions The overseas imported malaria cases are increasing rapidly with years and the malignant malaria cases were more than other malaria cases in Shijiazhuang City. It is necessary to further strengthen the long-term cooperation mechanism between the medical institutions and the entry-exit inspection and quarantine department. The technician training should be strengthened to avoid the severe cases and death cases.

Clonorchiasis is one of the food-borne parasitic diseases. Adult parasites live in the human liver and gallbladder tube system, causing serious complications, such as gallstones, cholecystitis and cholangitis, and even bile duct cancer. The disease is very popular in our country, and the population infection rate is high. It is an important public health problem. Guangdong Province is the earliest province being found of clonorchiasis and with serious epidemic. In the second national human parasitic diseases distribution survey, the results showed that the average infection rate of Clonorchis sinensis in the epidemic areas in Guangdong was 16.42%. It is estimated that the population of C. sinensis infection is over 6 million. The prevention and control of clonorchiasis in China is still in the initial stage currently and we face many challenges such as unclear epidemic characteristics and transmission mode, and lack of long-term prevention and control mechanism. This article introduces the epidemic situation of clonorchiasis and prevention and control strategies and measures in Guangdong.

OBJECTIVE: To analyze the epidemic characteristics of the imported malaria cases in Guangxi Zhuang Autonomous Region in 2014, so as to assess the transmission risk and explore the prevention and control strategy. METHODS: The data of the malaria epidemic situation in the network direct report system of Guangxi in 2014 and the annual report of malaria epidemic situation in 14 cities were collected. The epidemiological information of the imported malaria cases was analyzed. RESULTS: A total of 184 malaria patients were reported in Guangxi in 2014, with a descent rate of 85.29% when compared to that in 2013 (1 251 cases), and the incidence rate was 0.35/100 000. All the cases were imported from abroad, and four species of Plasmodium were found in their blood samples. The number of falciparum malaria cases was the most (49.46%), followed by the ovale malaria cases (32.07%). All the cases were distributed in 32 counties (districts) of 11 cities, and 65.76% of them were distributed in Shanglin County. Most of the cases were male (98.37%), and those aged in 20-49 years accounted for 87.50%. The imported cases came from 14 countries of Africa (86.41%) and 2 countries of Southeast Asia (13.59%), in which, 48.37% of the cases were imported from Garner. The main occupation of the cases in abroad was gold mining work (86.96%). The cases were reported all the year around, with no obvious seasonality. The interval time of back home to attack of the patients with tertian malaria and ovale malaria was longer. CONCLUSIONS: Africa and Southeast Asia is the main source of imported malaria cases in Guangxi, and the migrant workers returning home may have the risk of malaria recurrence, which should be paid enough attention to.
