RESUMO

OBJECTIVES: The use of technology in healthcare to manage patient records, guide diagnosis, and make referrals is termed electronic healthcare. An electronic health record system called Lightwave Health Information Management System (LHIMS) was implemented in 2018 at Cape Coast Teaching Hospital (CCTH). This study evaluated the impact of LHIMS on the quality of healthcare data at CCTH, focusing on the extent to which its use has enhanced the main dimensions of data quality. METHODS: Structured questionnaires were administered to doctors at CCTH to enquire about their opinions about the present state of LHIMS as measured against the parameters of interest in this study, mainly the dimensions of quality healthcare data and the specific issues plaguing the system as reported by respondents. RESULTS: Most doctors found LHIMS convenient to use, mainly because it made access to patient records easier and had to some extent improved the dimensions of quality healthcare data, except for comprehensiveness, at CCTH. Major challenges that impeded the smooth running of the system were erratic power supply, inadequate logistics and technological drive, and poor internet connectivity. CONCLUSIONS: LHIMS must be upgraded to include more decision support systems and additional add-ons such as patients' radiological reports, and laboratory results must be readily available on LHIMS to make patient health data more comprehensive.

BACKGROUND: In achieving the WHO's Universal Health Coverage and the Global Developmental Agenda: Sustainable Development Goal 3 and 9, the Ministry of Health launched a nationwide deployment of the lightwave health information management system (LHIMS) in the Central Region to facilitate health service delivery. This paper assessed the efficient use of the LHIMS among health professionals in the Central Region. METHODS: A non-interventional descriptive cross-sectional study design was employed for this research. The study used stratified and simple random sampling for selecting 1126 study respondents from 10 health facilities that use the LHIMS. The respondents included prescribers, nurses, midwives and auxiliary staff. Descriptive statistics (weighted mean) was computed to determine the average weighted score for all the indicators under efficiency. Also, bivariate (χ2) and multivariate (ordinal logistic regression) analyses were conducted to test the study's hypotheses. RESULTS: Findings revealed that the LHIMS enhanced efficient health service delivery. From the bivariate analysis, external factors; sex, educational qualification, work experience, profession type and computer literacy were associated with the efficient use of the LHIMS. However, training offered prior to the use of the LHIMS, and the duration of training had no association. At the multivariate level, only work experience and computer literacy significantly influenced the efficient use of the LHIMS. CONCLUSION: The implementation of LHIMS has the potential to significantly improve health service delivery. General computing skills should be offered to system users by the Ministry of Health to improve literacy in the use of computers. Active participation in the use of LHIMS by all relevant healthcare professionals should be encouraged.

BACKGROUND: Healthcare providers play a major role in the implementation of family planning policies. In Ghana, there has been a conscious effort to improve the knowledge of preservice and practicing health professionals on family planning. However, there have been concerns about the appropriateness of the attitudes and practices of these health cadres and, hence, their propensity to become barriers to the uptake of contraception in the general population. This study is aimed at assessing the attitudes and practices of healthcare workers and clinical-year medical students in contraceptives use, advocacy, and service provision. METHODS: A cross-sectional survey was conducted among health workers and clinical-year medical students from January 1 to June 30, 2018. Variables assessed included sexual activity status, previous and current contraceptive use, and satisfaction with contraceptive use among others. Data from 400 self-administered, structured questionnaires comprising close- and open-ended questions was entered in SPSS version 22 and analysed using same. The variables assessed were presented as means, frequencies, and percentages. RESULTS: About 58% of the respondents were sexually active. Half of the participants (50.2%) had used a form of contraception before, with condoms and other barrier methods being the most preferred (67.7%). However, only 18% of respondents were on a form of contraceptive at the time of the survey. Four out of five (82.6%) of the users of these contraceptives were satisfied with their past use. A little over half of the participants had discussed contraception with their partners. Over four-fifths of participants thought family planning was beneficial and were willing to encourage others to use a method of family planning. Majority (63.7%) of the participants had had formal training in family planning, but only 72 (18%) were actively involved in the provision of family planning services. CONCLUSIONS: Although the attitudes of the health workers and trainees toward family planning were excellent generally, only a few were using a method of contraception at the time of the survey despite the fact that most of them were sexually active. There is a need to intensify communication on behaviour change towards contraception among health professionals and clinical-year medical students in order to strengthen their role as change agents in an effort to improve community uptake.

OBJECTIVES: Unintended pregnancy presents a crisis situation with limited options for resolution. Abortion appears to be a commonly chosen option but is stigmatized in many societies including Ghana. Keeping a child from an unintended pregnancy is also unsuitable for many people. Carrying through with the pregnancy and placing the child up for adoption is a potential management option but there is scanty literature on how viable this option is to women globally including Ghana. The study sought to assess acceptability of this option and its barriers and facilitators in Ghana. METHODS: This study was a part of a bigger analytical cross-sectional study on unintended pregnancy in Kumasi conducted in three centres from January to April 2014. Exit interviews were conducted for 461 consenting women to capture data on demography, reproductive profile and acceptability of giving up a child from an unintended pregnancy for adoption. Frequencies, proportions and means were computed and presented in tables. RESULTS: Over 85% of respondents would not give up their children for adoption as a way to manage their unintended pregnancy, whereas about 6% were undecided. A need for the child to grow up in a two-parent home was considered more important than the financial security of the adoptive parents while disappointment from family and friends came up as marked barrier to adoption. CONCLUSIONS: Keeping a pregnancy and placing the child up for adoption is presently not ideal for managing an unintended pregnancy crisis. More education is needed to increase awareness of adoption as an option in resolving this crisis while continued efforts are made at primary prevention through using contraceptives. The complex adoption process must be made friendly for women with unintended pregnancies who neither desire parenting nor abortion.

BACKGROUND: Information technology has become an inevitable, constitutive element of the healthcare institution as well as health education. This study investigates the effects of mobile phone use in clinical practice at the Cape Coast Teaching Hospital. The use of mobile phones to access health information by health professionals has the potential to improve the provision of health service to the population. In addition, primary care physicians can use mobile phones to communicate with their patients before and after they are discharged, thereby improving the health of individual patients and the population at large. METHOD: The study adopted the cross-section survey design and obtained data using questionnaire from 100 medical students (medical, nursing and midwifery students) through purposive sampling procedure. Descriptive statistics and Pearson Chi-square were used for the analysis. RESULTS: The results show that 98% of the respondents owned smartphones, thus, they are receptive to and can confidently use their phones to access medical information (65%). It also emerged that, respondents can render effective and continuous service to clients (90%) with assistance from mobile medical apps. Respondents dispelled fears that it was unethical to always depend on mobile apps for medical information. However, there was no significant relationship between using mobile apps to access medical information and ensuring effective and continuous service to clients (p≤ 0.937). CONCLUSION: In spite of high patronage of mobile phone, respondents maintained that accessing mobile phones during medical practice does not interfere with the service delivery, rather it facilitates effective and continuous service, speed up access to healthcare information and helps to increase knowledge as well as improve care giving skills. Mobile phone use can ensure quick communication between health facilities and health professionals which can help control diseases of public health concern thereby improving the health of the population.

The use of computers in the delivery of health care has significantly improved the way health service is delivered to clients and patients in the world. Despite the importance of computing to the delivery of health service, developing countries have not greatly benefited from it. Nursing informatics has been in existence and part of academic curriculum for the past 2 decades in some advanced countries. The Ghana Nursing and Midwifery Council introduced the nursing and midwifery informatics course during the 2015/2016 academic year. This seeks to train student nurses on the relevance of computers to health care. Two separate workshops were organised to ascertain the preparedness of tutors (teachers at the nursing and midwifery training institutions) for teaching the new nursing and midwifery informatics course as well as to compare the curriculum with other international recommendations. The nursing and midwifery informatics course is taught at the first year where students have not been introduced to the nursing processes for them to appreciate the use of nursing informatics skills. It would be better if the nursing and midwifery informatics course is rather introduced during the second year second semester when students are about going for the hands-on training at the various health care institutions. Examining the course content reveals that the practical aspect within the course is very small. It is expected that more practical contents will be introduced. Tutors are not adequately prepared to teach this new course. More training is therefore needed to make tutors fully prepared to teach both the theory and practical aspects of the nursing and midwifery informatics course. It is expected that the nursing and midwifery informatics course would prepare student nurses on all nursing informatics competencies. It is essential that nurse educators incorporate the entire concept of informatics into the education of nurses.

RESUMO

Information and Communication Technology (ICT) has become a major tool in delivery of health services and has had an innovative impact on quality of life. ICT is affecting the way healthcare is delivered to clients. In this paper, we discuss the state of ICT and health informatics in Ghana. We also discuss the state of various relevant infrastructures for the successful implementation of ehealth projects. We analyse the past and present state of health informatics in Ghana, in comparison to other African countries. We also review the challenges facing successful implementation of health informatics projects in Ghana and suggest possible solutions.
