OBJECTIVE: The transverse relaxation time (T2) in MR imaging has been identified as a potential biomarker of hyaline cartilage pathology. This study investigates whether MR assessments of T2 are comparable between 3-T scanners from three different vendors. DESIGN: Twelve subjects with symptoms of knee osteoarthritis and one or more risk factors had their knee scanned on each of the three vendors' scanners located in three sites in the U.K. MR data acquisition was based on the United States National Institutes of Health Osteoarthritis Initiative protocol. Measures of cartilage T2 and R2 (inverse of T2) were computed for precision error assessment. Intrascanner reproducibility was also assessed with a phantom (all three scanners) and a cohort of 5 subjects (one scanner only). RESULTS: Whole-organ magnetic resonance (WORM) semiquantitative cartilage scores ranged from minimal to advanced degradation. Intrascanner R2 root-mean-square coefficients of variation (RMSCOV) were low, within the range 2.6 to 6.3% for femoral and tibial regions. For one scanner pair, mean T2 differences ranged from -1.2 to 2.8 ms, with no significant difference observed for the medial tibia and patella regions (p < 0.05). T2 values from the third scanner were systematically lower, producing interscanner mean T2 differences within the range 5.4 to 10.0 ms. CONCLUSION: Significant interscanner cartilage T2 differences were found and should be accounted for before data from scanners of different vendors are compared.

OBJECTIVE: Anti-inflammatory therapies are effective analgesics for OA. This study determined whether low-dose oral prednisolone (PNL) was an effective analgesic for hand OA. METHODS: This was a randomized, double-blind, placebo-controlled trial of people with ACR criteria hand OA and baseline hand pain visual analogue scale (VAS) of >40/100 mm. Participants received 5 mg PNL or placebo daily for 4 weeks. Pain VAS, disease activity VAS, Australian/Canadian Hand Osteoarthritis Index and joint counts were performed at baseline, 4 and 12 weeks. Primary outcome was the change in hand pain VAS at 4 weeks. Analysis of covariance was used for analysis, controlling for baseline values. To explore potential mechanism of action of PNL, non-contrast 0.2 Tesla MRI was performed on the most painful hand at baseline and 4 weeks. RESULTS: A total of 70 participants were recruited (57 women, mean age 61 years, mean baseline pain VAS 61.5 mm); 75% had more than one joint with definite MRI synovitis/effusion. At 4 weeks the adjusted mean reduction in pain VAS was 19.9 mm (PNL group) and 16.8 mm (placebo group) (P = 0.54). There were no statistically significant differences in VAS, Australian/Canadian Hand Osteoarthritis Index or joint counts between placebo and PNL groups at 4 or 12 weeks. A total of 20 participants in each group achieved an Outcome Measures in Rheumatology-Osteoarthritis Research Society International response. Baseline synovitis/effusion did not predict response to treatment. CONCLUSION: This is the first randomized controlled trial of low-dose corticosteroid alone for painful hand OA, which demonstrated that short-term low-dose oral PNL is not an effective analgesic treatment for hand OA. OBJECTIVE: This study was designed to assess the accuracy of unenhanced 3T MRI supplemented with dedicated true plane reformats of 3D T2 Dual Echo Steady State (DESS), in assessing tears of scapholunate, lunotriquetral ligaments and triangular fibrocartilage complex, using arthroscopy as gold-standard. METHODS: In a retrospective cohort review, patients who underwent wrist arthroscopy and MRI over 2 years (n=46) were identified. Dedicated axial and coronal reformats were obtained from 3D T2 DESS sequence for assessing intrinsic ligaments and triangular fibrocartilage (TFCC). At arthroscopy, tears were classified using Geissler's classification and compared to MRI findings. RESULTS: The sensitivity of unenhanced 3T MRI in detecting scapholunate ligament tears was 87%, specificity was 90% and negative predictive value of 93%. The lunate triquetral ligament was assessed with a high specificity (97%) and negative predictive value (93%), sensitivity was 63%. TFCC tears were assessed with a sensitivity of 100%. The overall diagnostic accuracy of unenhanced 3T MRI of wrist in detecting ligament tears was 91%. CONCLUSION: 3T wrist MRI with dedicated reformats from 3D T2 DESS has a high diagnostic accuracy in assessing acute intrinsic ligament and TFCC injuries. High strength 3T MRI with good technical quality isotropic 3D T2 DESS is critical for accurate wrist ligament assessment. Negative predictive values of 3T MRI of 95% can lead to reduced need for diagnostic arthroscopy of the wrist. ADVANCES IN KNOWLEDGE: This study assesses the diagnostic performance of unenhanced 3T MRI with 3D T2 DESS in assessing traumatic wrist ligament tears. The accuracy of T2 DESS in wrist imaging has not been previously reported.

BACKGROUND: External fixation is an important tool in the management of variety of tibial fractures. Appropriate half pin insertion is important, to provide stable fixation without compromising the surgical field for definitive surgical procedures, and avoiding further damage to the important structures of the traumatized limb. There is paucity of literature about the optimal trajectories and safe corridors for half pins insertion based on in vivo studies. The available studies are based on anatomic atlases, cadaveric studies or half pin related complications.The aim of the current study is to present the findings of CT angiograms, in patients with external fixation of tibia, to enhance our understanding of optimal trajectories in safe corridors for half pins insertion. MATERIAL AND METHODS: We performed a retrospective study of patients with external fixators on the tibia, who had undergone CT angiogram as part of pre-operative planning for orthoplastic reconstructive procedures. The relationship between the tips of the fixator half pins and named vessels of the leg were analyzed, pins within 5 mm of a named vessel were considered to be a risk of causing iatrogenic injury. RESULTS: A total 51 patients, with in situ temporizing external fixators, with 134 half pins in different segments of the tibia were analyzed. More than 5 mm of penetration beyond the far cortex was noted in 47%, while in another 16% of pins penetration was more than 10 mm beyond the cortex. A tip to vessel distance (TVD) of 5 mm or less was noted in 28/134 (21%) of the pins, which highlights potential risk to the neurovascular bundles of the leg. CONCLUSION: Risk of iatrogenic injury to neurovascular structures from half pin insertion can be reduced by meticulous use of fluoroscopy, by avoiding penetration beyond the far cortex, and avoiding exiting with half pins on the lateral surface in the distal 1/3rd of segment II of tibia. Moreover observing optimal trajectories and safe corridors for pin insertion, and selection of appropriate type of half pin can mitigate the risk to these structures.

INTRODUCTION: Cartilage thickness from MR images has been identified as a possible biomarker in knee osteoarthritis (OA) research. The ability to acquire MR data at multiple centers by using different vendors' scanners would facilitate patient recruitment and shorten the duration of OA trials. Several vendors manufacture 3T MR scanners, including Siemens, Philips Medical Systems, and GE Healthcare. This study investigates whether quantitative MR assessments of cartilage morphology are comparable between scanners of three different vendors. METHODS: Twelve subjects with symptoms of knee OA and one or more risk factors had their symptomatic knee scanned on each of the three vendor's scanners located in three sites in the UK: Manchester (Philips), York (GE), and Liverpool (Siemens). The NIH OAI study protocol was used for the Siemens scanner, and equivalent protocols were developed for the Philips and GE scanners with vendors' advice. Cartilage was segmented manually from sagittal 3D images. By using recently described techniques for Anatomically Corresponded Regional Analysis of Cartilage (ACRAC), a statistical model was used anatomically to align all the images and to produce detailed maps of mean differences in cartilage-thickness measures between scanners. Measures of cartilage mean thickness were computed in anatomically equivalent regions for each subject and scanner image. RESULTS: The ranges of mean cartilage-thickness measures for this cohort were similar for all regions and across all scanners. Philips intrascanner root-mean-square coefficients of variation were low in the range from 2.6% to 4.6%. No significant differences were found for thickness measures of the weight-bearing femorotibial regions from the Philips and Siemens images except for the central medial femur compartment (P = 0.04). Compared with the other two scanners, the GE scanner provided consistently lower mean thickness measures in the central femoral regions (mean difference, -0.16 mm) and higher measures in the tibial compartments (mean difference, +0.19 mm). CONCLUSIONS: The OAI knee-imaging protocol, developed on the Siemens platform, can be applied to research and trials by using other vendors' 3T scanners giving comparable morphologic results. Accurate sequence optimization, differences in image postprocessing, and extremity coil type are critical factors for interscanner precision of quantitative analysis of cartilage morphology. It is still recommended that longitudinal observations on individuals should be performed on the same scanner and that assessment of intra- and interscanner precision errors is undertaken before commencement of the main study.
