RESUMO

OBJECTIVE: To investigate the relationship between the work environment and leadership in nursing. METHOD: An integrative review of literature which was based on data from LILACS, PubMed, CINAHL and the SciELO portal for journals covering the period from January to April 2013. The inclusion criteria were: the indexing of research covering leadership exercised by nurses over a team and whether the research was available in English, Spanish or Portuguese. RESULTS: The sample consisted of 12 articles that met the criteria. CONCLUSION: The results showed that leadership had an impact on the work environment. However, no studies were found that showed the influence of the working environment on leadership in nursing.

OBJECTIVE: To position the Brazilian Transplant System within the context of the 4 Strategic Lines of Action proposed by the Pan-American Health Organization. METHOD: A specialist analysis was conducted through comparative analyses of the 4 Strategic Lines of Action, objectives, and indicators outlined in the Action Plan, along with the donation and transplantation system in accordance with Brazilian transplantation law. Subsequent to an in-depth review of the document issued by the health authority, a series of meetings involving 8 specialists in organ donation and transplantation were conducted. During these meetings, discussions were carried out with the objective of numerically interpreting each strategy presented in the document, and recommendations constructed. RESULTS: Four strategies were evaluated and only 2 of them the third (81,3%) and the fourth (90%) do not achieve the indicators to complete the objective related to equitable access to organ, tissue, and cell transplants in Brazil. CONCLUSION: The recommendations developed carry importance, as they are innovative and contribute to the establishment of priorities when shaping public policies. The report not only highlights indicators that were not satisfactorily met but also provides insights into the recommendations formulated to improve those indicators that have already been achieved and to work toward achieving those that have not yet been realized. Additionally, these recommendations can justify actions and establish priorities for research efforts in the field.

RESUMO

OBJECTIVES: to raise reflections on the need for health services and professionals to implement sustainable actions, aiming at their own survival and that of the planet. METHODS: reflective essay based on international reports regarding the impact of climate change on people's health and the role of institutions in this context. RESULTS: the article focused on three fundamentals: climate change continues to be a threat to the health and well-being of all beings on Earth; the institutions that should contribute to health are great agents of contamination of the environment and emission of gases that aggravate the greenhouse effect; and there are several benefits for health institutions to act sustainably. FINAL CONSIDERATIONS: we cannot wait any longer; we must develop policies and management models aimed at environmentally responsible, economically viable, and socially more collaborative healthcare.

This article aims to analyze the effects of the COVID-19 infodemic from the perspective of Brazilian health workers regarding the patients, measure the impacts of fake news on health professionals, and verify the perception of health multidisciplinary teams regarding the authorities' stance. This cross-sectional study is nested in the research "Health Professionals' Working Conditions in the Brazilian Context of COVID-19". The nationwide non-probability sampling included 15,132 professionals who worked in the COVID-19 frontline in health institutions of 2,200 Brazilian municipalities. Approximately 91% of respondents believed that fake news is an obstacle in fighting the SARS-CoV-2 virus; 76.1% declared they had seen patients who expressed faith in fake news about COVID-19; 29.3% agreed that the health authorities' stance about COVID-19 was consistent and enlightening, and 62.6% disagreed about this. The respondents believe that the COVID-19 infodemic confused patients, impaired adherence to PHC measures, and stirred people's negative behavior vis-à-vis the pandemic. The lack of clarity of the authorities' stance influenced the COVID-19 infodemic process.


O objetivo deste artigo é analisar os efeitos da infodemia relacionada à COVID-19 na perspectiva dos trabalhadores da saúde no Brasil em relação aos pacientes, mensurar os impactos das notícias falsas para profissionais de saúde e verificar a percepção das equipes multiprofissionais de saúde quanto ao posicionamento das autoridades. Estudo transversal vinculado à pesquisa Condições de Trabalho dos Profissionais da Saúde no Contexto da COVID-19 no Brasil. A amostra não probabilística, de abrangência nacional, incluiu 15.132 profissionais que atuaram na linha de frente do enfrentamento à COVID-19 em instituições de saúde de 2.200 municípios de todo o Brasil. Para 91% dos entrevistados, as fake news são um obstáculo no combate ao vírus Sars-CoV-2; 76,1% declararam ter atendido pacientes que expressaram fé em fake news sobre a COVID-19; 29,3% concordam que os posicionamentos das autoridades sanitárias sobre a COVID-19 foram consistentes e esclarecedores, e 62,6% discordam. Para os entrevistados, a infodemia sobre a COVID-19 confundiu os pacientes, prejudicou a adesão a medidas sanitárias fundamentais e estimulou comportamentos negativos da população em relação à pandemia. A falta de clareza dos posicionamentos das autoridades influiu no processo infodêmico sobre a COVID-19.

OBJECTIVE: To analyze the relationship between leadership coaching and nurses' resilience in hospital environments. METHOD: This is a cross-sectional study, carried out with nurses, nursing assistants and technicians. Nurses answered the Nurse Self-Perception Questionnaire in Leadership Exercise Questionnaire (QUAPEEL) and the Connor-Davidson Scale for Brazil-25 (CD-RISC-25BRASIL). Nursing technicians/assistants answered the Nursing Technician and Assistant Perception in Leadership Exercise Questionnaire (QUEPTAEEL). Student's t test and Pearson's correlation were used (p ≤ 0.05). RESULTS: There was a statistically significant difference between nurses' self-perception and nursing technicians/assistants' hetero-perception in coaching leadership exercise for the total score (p = 0.002) and in the "Give and receive feedback" (p < 0.001), "Delegate power and exert influence" (p < 0.001) and "Support the team so that the organizational results are reached" (p = 0.020) domains. There was a statistically significant correlation between all the Nurse Self-Perception Questionnaire in Leadership Exercise Questionnaire Coaching and Resilience domains. CONCLUSION: Resilience was positively related to nurses' self-perception of coaching leadership.

RESUMO

The present study aims to describe the sociodemographic and health profile of nursing professionals in the context of the COVID-19 pandemic and propose a reflective analysis on the essentiality of the category facing the intrinsic demands of patients and the Brazilian health system, especially in the context of the public health emergency triggered by the exponential advance of the SARS-CoV-2 virus. This study reveals the relationship between historical injustices and the different types of inequality that impacted and caused the vulnerability of the profession, with an emphasis on the presentation of potential perspectives arising from this historical process and recent events.


A pesquisa tem por objetivo descrever o perfil sociodemográfico e de saúde dos profissionais de enfermagem no contexto da pandemia da COVID-19 e propõe uma análise reflexiva sobre a essencialidade da categoria frente às demandas intrínsecas dos pacientes e do sistema de saúde brasileiro, especialmente, no contexto da emergência de saúde pública deflagrada pelo avanço exponencial do vírus SARS-CoV-2. O estudo revela a relação entre as injustiças históricas e os diferentes tipos de desigualdade que afetam e causam a vulnerabilidade da profissão, com fulcro na apresentação de potenciais perspectivas decorrentes desse processo histórico e dos acontecimentos recentes.

OBJECTIVES: to define and validate 37 concepts emanating from the epistemology of knowledge about Nursing Administration. METHODS: theoretical-methodological study using the Delphi technique in 37 concepts and definitions built on the knowledge of more than half a century of research on Nursing Administration. The concepts were submitted to the judgment of a panel of 21 judges and the validation was measured by the content validity index (> 0.78) and Kappa coefficient (> 0.61). RESULTS: enunciation of 37 concepts and definitions capable of reflecting the knowledge about Nursing Administration. The 37 concepts were validated by the judges, resulting in content validity indices that ranged from 0.81 to 1.00, with reliability higher than 0.79. CONCLUSIONS: the epistemological solution presented was validated by the judges with indices above 0.80 and high reliability of universal agreement, constituting a new object of ontological understanding for the scientific nursing community.

RESUMO

This paper presents the evolution of fake news disseminated about vaccines and the SARS-CoV-2 virus and its adverse impacts on the current Brazilian health crisis. This quantitative, empirical study is based on the notifications received by the Eu Fiscalizo app, through which the Instagram, Facebook, Twitter, and WhatsApp platforms were identified as the principal means for disseminating and sharing rumors and misinformation about COVID-19. We observed large-scale circulation of fake news about vaccines directly related to the Brazilian political polarization, which became prevalent four months after the first COVID-19 case was recorded in the country. We can conclude that this phenomenon was crucial in discouraging the adherence of segments of the Brazilian population to social distancing and vaccination campaigns.


Este artigo apresenta a evolução das notícias falsas disseminadas a respeito das vacinas e do vírus Sars-CoV-2 e os impactos negativos desse fenômeno sobre a crise sanitária que o Brasil atravessa. Trata-se de um estudo empírico quantitativo, realizado a partir das notificações recebidas pelo aplicativo Eu Fiscalizo, por meio do qual foi identificado o predomínio das plataformas Instagram, Facebook, Twitter e WhatsApp como os principais meios de difusão e compartilhamento de boatos e desinformações acerca da COVID-19. Foi observada a circulação em escala de fake news sobre vacinas, diretamente relacionadas à polarização política brasileira, tornando-se prevalente quatro meses depois de ser registrado o primeiro caso de COVID-19 no Brasil. Conclui-se que o fenômeno colaborou para desestimular a adesão de parcelas da população brasileira às campanhas de isolamento social e de vacinação.

OBJECTIVES: to present the visual identity of a soft-hard technology; to describe its technical characteristics, standardization and norms of criteria for the interpretation of results, in order to measure clinical competencies in emergencies of nurses. METHODS: article developed as a methodological study for the creation of the graphic image of the scale and standardization based on previous studies of validity and reliability evidence. RESULTS: the designed technology was named "Lilalva Scale for Measuring Clinical Competencies in Emergencies of Nurses". The graphic image text is organized by title, instructions on how to fill it out, description of the quality of delivery, and behavioral actions with respective degrees of response. The Scale has 22 items with characterization data. The standardization of the measurement instrument included: instructions for use, calculation protocol, and hypothetical example to calculate scores, the sum of the resulting scores metric for each clinical competence and diagnostic classification. CONCLUSIONS: the visual identity and the standardization instrumentalize the use of the Scale for those interested in the theme.

OBJECTIVE: To evaluate the hospital indicators and their repercussions on the number of monthly admissions to a public university hospital, before and after implementing the Internal Regulation Center. METHOD: An evaluative research study, of the Case Study type, developed in a public university hospital. A total of 28 indicators related to structure, production, productivity and quality were measured, which are part of internal Benchmarking. The data were analyzed by means of descriptive statistics and multiple regression to identify the independent factors and those associated with the number of monthly hospitalizations with 95% confidence intervals. RESULTS: Implementation of the Center significantly increased (p<0.001) the number of discharges, the bed utilization factor and the bed renewal rate, emergency hospitalization, bed occupancy percentage, surgical procedures performed and the patient-day mean value (p=0.027). There was a reduction (p<0.001) in the number of visits to the medical, obstetric and orthopedic emergency room, in the rates of in-hospital infection and infant mortality, as well as a mean reduction of 0.81/day, approximately one day less of hospitalization per patient, or a gain of 40 available beds per month. CONCLUSION: Although the number of available beds was lower in the post-implementation period, the bed replacement interval was reduced, representing an increase of 40 more beds per month due to the reduction in the patients' length of stay in the institution.

Objective: To analyze the coping strategies used by primary healthcare (PHC) professionals. Methods: A cross-sectional, descriptive-analytical study realized with professionals working in primary healthcare units in São José do Rio Preto, a large city in the interior of São Paulo, Brazil. For data collection, we used an instrument developed by the researchers containing sociodemographic and professional variables, as well as the Problem Coping Modes Scale (EMEP). Results: We evaluated 333 PHC professionals. A difference was observed between the scores of the four coping strategies (p < 0.001), with the highest score for the problem-focused strategy (3.8) and the lowest score for the emotion-focused strategy (2.4). Physicians had the lowest scores in coping strategies focused on religious practices/fantastical thinking (p < 0.001) and pursuit of social support (p = 0.045), while community health agents had the highest scores in these coping strategies. Conclusions: Professionals working in PHC have different coping strategies for the problems and stressful situations experienced in the work environment. These strategies can involve more positive attitudes focused on confrontation and problem solving, and on emotional responses that involve attitudes of avoidance and denial of the problem.

RESUMO

This paper highlights the advance of science in interpreting pandemics, in contrast to the failure of governments that politicized the approach to the global public health emergency resulting from the COVID-19 pandemic. This study reflects on cognitive dissonance caused by the infodemic. It addresses the need to apply infodemiology to mitigate the deleterious effects of fake news intentionally fabricated to confuse, mislead, manipulate, and deny the reality without losing sight of the fact that the roots of the problem are historical, circumstantial, profound, and challenging. This work reveals the impacts of this situation for health professionals and exposes the fine line between freedom of expression and the fundamental right to life, leading to the conclusion that wrong choices in public health can cause preventable deaths.


O artigo evidencia o avanço da ciência na interpretação de pandemias, em contraste com o fracasso de governos que politizaram a abordagem da emergência de saúde pública global decorrente da COVID-19. Trata-se de um estudo que apresenta uma reflexão sobre o processo de dissonância cognitiva causada pela infodemia e aborda a necessidade de aplicar a infodemiologia para mitigar os efeitos deletérios de notícias falsas que são fabricadas intencionalmente, com o objetivo de confundir, enganar, manipular e negar a realidade, sem, contudo, perder de vista que as raízes do problema são históricas, conjunturais, profundas e de difícil solução. O trabalho revela os impactos dessa situação para profissionais de saúde e expõe a linha tênue que existe entre a liberdade de expressão e o direito essencial à vida, levando à conclusão de que escolhas erradas, no que tange à saúde pública, podem causar mortes evitáveis.

RESUMO

This article aimed at reflecting and discussing about some difficulties in the submission and evaluation of scientific projects to the Research Ethics Committee, from an experience in post-graduation. Among these difficulties, there was a need for submission of a single project to several CEP as demand for part of the health facilities involved, which showed discrepancies with regard to the matter. Another issue, involving the reports, which are still based on biomedical models, which hinder the evaluation of research projects in nursing and other sciences. However, one of the great challenges of CEP is to ensure that researches with human beings are carried out within an ethics framework, without be an obstacle to their development.

Reflection about a doctorate's thesis. It was carried through an abstraction of the described theory by specialist authors in thematic, a transposition for the practical of competency management in nursing. The essential competency shows attributes of knowledge, ability and attitude - called CHA, to add value to the organization of health with focus in the customer. It is essential to awake in all the team, the responsibility and real concern with the patient, family and visitors. The professional constitutes oneself as the inspired leader of its group. The management competency of the nurse can give change to the development of the human resources and promote the enchantment in the attendance of the customers, making the difference in the health organizations.

OBJECTIVES: To reflect on the scope of the working process in nurse participation political activities as a possibility for expanding its performance. METHODS: this is a reflective study, supported by the discursive elaboration on the nursing participation political activities and work process, structured through concepts, theoretical perspectives and practices based on the experiences described in references. RESULTS: studies on this process demonstrate that nurses need sensitivity to understand the complexity of their social practice. FINAL CONSIDERATIONS: in their professional actions, nurses must mobilize political resources, making their knowledge, skills and attitudes available to their practices, aiming at power and improvement of conditions for the work of Nursing.

OBJECTIVE: Reflecting critically on the epistemological aspects of the of knowledge construction on Nursing Administration in Brazil. METHODS: This is an article of theoretical reflection that aims to assess the epistemological aspects of the construction of knowledge about Nursing Administration. RESULTS: The knowledge about Nursing Administration has a long way to go, as its epistemological repertoire is still timid. In addition, the lack of distinction of this knowledge in the communication of research products underestimates the production on Nursing Administration, as these products appear diluted in other areas of Nursing (care, research, and teaching). FINAL CONSIDERATIONS: Due to the reflections, the need for new scientific evidence is highlighted, whose formulation of research questions can contribute to the delineation of phenomena that distinguish Nursing Administration as an area of research knowledge.

OBJECTIVE: to analyze how nursing courses in the State of São Paulo, Brazil have operationalized the supervised curricular internship and to identify those that approach the recommendations proposed by the National Curriculum Guidelines. METHOD: a quantitative, descriptive-exploratory study. The sample consisted of 38 course coordinators. The data collection instrument was developed based on the Curricular Guidelines. Data collection took place electronically and, for data analysis, descriptive and inferential statistics were used. RESULTS: the undergraduate schools have developed internships for a mean of 860.4 hours in primary and tertiary care settings, with learning based on professional practice being the main teaching method. Formative assessment is the predominant mode of assessment, and nurses from health institutions participate in 44.7% of courses. The mean score obtained was 3.1 points (scale from 1 to 5), with the evaluation processes used being the most influential factor (p<0.001). CONCLUSION: the courses have partially met the educational legislation regarding the hours and participation of professionals from health institutions granting internship field, which can compromise the quality of training and the safety of care.

The paper reflected on the Brazilian Nursing education from the regulation of Unified Health System in a comparative historical perspective of the evolutionary processes of the Anglo-American and French schools, influencing Brazilian nursing education, as well as the guiding aspect of nursing education for the Unified Health System. Thus, nursing training initiatives guided by the National Curriculum Guidelines are developed to provide meaningful experiences in the Unified Health System's daily routine, as well as the transforming movement of Permanent Health Education in the context of the world of work. Therefore, overcoming training challenges must consider the social, political, and cultural path of the profession in order to allow changes that affect pedagogical projects, course offerings, teaching-learning methodologies, and daily work.


O artigo refletiu sobre a formação em Enfermagem no Brasil a partir da regulamentação do Sistema Único de Saúde numa perspectiva histórica comparada dos processos evolutivos das escolas anglo-americana e francesa, influenciadoras do ensino de Enfermagem no Brasil, bem como o aspecto orientador da formação em Enfermagem para o Sistema Único de Saúde. Neste sentido, são desenvolvidas inciativas de formação em Enfermagem, orientadas pelas Diretrizes Curriculares Nacionais, com vistas a propiciar vivências significativas no cotidiano do Sistema Único de Saúde, assim como o movimento transformador da Educação Permanente em Saúde no contexto do mundo do trabalho. Dessa maneira, a superação dos desafios da formação precisa considerar a trajetória social, política e cultural da profissão, de modo a possibilitar mudanças que impactam nos projetos pedagógicos, na oferta de cursos, nas metodologias de ensino-aprendizagem e no cotidiano do trabalho.

OBJECTIVES: to identify the knowledge of nursing professionals about leadership models and evaluate the authentic leadership profile among them. METHODS: analytical study, conducted between August and December 2015, involving 84 nursing professionals working in a public and tertiary hospital. We used two instruments: Sociodemographic Questionnaire with questions about leadership and the Authentic Leadership Questionnaire. RESULTS: both nurses and nursing technicians were unaware of authentic leadership. Both pointed to communication, planning, and organization as competencies of the leader (n = 58, 95%). Regarding the authentic leadership profile, we observed that the score was "high" among nurses and "low" among technicians. Holding a leadership position and professionally upgrading has positively influenced the highest-profile of authentic leadership. CONCLUSIONS: nurses demonstrated to know behavioral leadership, while nursing technicians showed knowledge about situational leadership. Nurses had a high score of authentic leadership behaviors, while nursing technicians had a low score, but we found no significant difference between them. Holding a leadership position and professionally upgrading has positively influenced the highest profile of authentic leadership.

RESUMO

This study aims to explore the association between nurses' leadership styles and personal and professional nursing profile and workload. The sample consisted of seven nurses and seven nursing technicians who were grouped into pairs. At the end of three months, nurses were queried regarding what leadership style would be adopted when the nursing technician under their evaluation delivered care to patients admitted to the ICU. Relevant data was analyzed by applying descriptive statistics, Tukey's multiple comparison test and Student's t-test (p< 0.05). Nursing workload reached 80.1% on average. The personal and professional profile variables did not show any relation with the leadership styles chosen by nurses (p>0.05). The determine, persuade, and share leadership styles prevailed. However, whenever the nursing workload peaked, the determine and persuade styles were used (p<0.05).
