RESUMO

OBJECTIVE: Individuals with psychosis are at elevated risk of adverse sexual and reproductive health (SRH) outcomes, and not receiving adequate SRH care. SRH is important for youth, yet little is known about SRH care access and experiences among those with early psychosis. This study explored SRH care experiences among women and nonbinary individuals with early psychosis. METHOD: We conducted semistructured qualitative interviews with 19 service users (cisgender and transgender women, nonbinary individuals) receiving care in 2 early psychosis programs in Ontario, Canada. We also conducted semistructured interviews and focus groups with 36 clinicians providing SRH or mental health care to this population. Participants were asked about SRH care access/provision experiences and the interplay with psychosis. Using a social interactionist orientation, a thematic analysis described and explained service user and clinician perspectives regarding SRH care. RESULTS: Amongst both service users and clinician groups, common themes developed: (a) diversity of settings: SRH services are accessed in a large range of spaces across the health care system, (b) barriers in nonpsychiatric SRH care settings: psychosis impacts the ability to engage with existing SRH services, (c) invisibility of SRH in psychiatric settings: SRH is rarely addressed in psychiatric care, (d) variability of informal SRH-related conversations and supports, and cutting across all of the above themes, (e) intersecting social and cultural factors impacted SRH services access. CONCLUSIONS: SRH is important for health and wellbeing; improvements are urgently needed across the healthcare system and within early psychosis programs to meet this population's multifaceted SRH needs.

Sit-to-walk (STW) is a crucial daily task that impacts mobility, independence, and thus quality of life. Existing repositories have limited STW data with small sample sizes (n = 10). Hence, this study presents a STW dataset obtained via the time-up-and-go test, for 65 healthy adults across three age groups - young (19-35 years), middle (36-55 years) and older (above 56 years). The dataset contains lower body motion capture, ground reaction force, surface electromyography, inertial measurement unit data, and responses for the knee injury and osteoarthritis outcome score survey. For validation, the within subjects intraclass correlation coefficients for the maximum and minimum lower body joint angles were calculated with values greater than 0.74, indicating good test-retest reliability. The joint angle trajectories and maximum voluntary contractions are comparable with existing literature, matching in overall trends and range. Accordingly, this dataset allows STW biomechanics, executions, and characteristics to be studied across age groups. Biomechanical trajectories of healthy adults serve as a benchmark in assessing neuromusculoskeletal impairments and when designing assistive technology for treatment or rehabilitation.

Background Cancer patients are at increased risk of multi-organ failure due to either the primary disease burden or certain non-cancer-related risk factors. Among the most common complications is acute kidney injury (AKI), which is frequently seen in cancer settings. Among patients with cancer, the incidence of renal injury reaches up to 12.5%. However, critical care units have a much higher incidence, up to 50%. This study aimed to describe the characteristics of Asian populations with AKI with a background of malignancy, along with risk factors and outcomes. Materials and methods A retrospective tertiary-care single-center study was conducted in the intensive care unit (ICU). It included 182 cancer patients with AKI who were followed over a 36-month period. Results Our results revealed a mortality rate of 50.5% among cancer patients with AKI, with the highest mortality rate being among those with solid and hematologic malignancies. Common predisposing factors were sepsis (28%), dehydration (18.1%), and hypotension (9.9%). Several drugs were found to be among the most toxic agents, including vancomycin, colistin, nonsteroidal anti-inflammatory drugs, angiotensin receptor blockers, amphotericin, and certain chemotherapeutic drugs (especially cisplatin). A strong association was found between the length of ICU stay and mortality (p=<0.05), indicating a reduced survival rate with prolonged hospital stay even in critical care settings. Conclusion In summary, AKI in cancer patients increases their mortality due to a variety of risk factors. However, the early involvement of onconephrology and a multidisciplinary approach will result in better outcomes.

BACKGROUND: Women and gender-diverse people with early psychosis are at risk for suboptimal sexual health outcomes, yet little research has explored their sexual health experiences. AIMS: This study explored sexual health experiences and related priorities among women and gender-diverse people with early psychosis, to identify opportunities for improvements in sexual health and well-being. METHOD: Semi-structured individual qualitative interviews explored how patient participants (n = 19, aged 18-31 years, cisgender and transgender women and non-binary individuals) receiving clinical care from early psychosis programmes in Ontario, Canada, experienced their sexual health, including sexual function and behaviour. Thematic analysis was conducted, with triangulation from interviews/focus groups with clinicians (n = 36) who provide sexual and mental healthcare for this population. RESULTS: Three key themes were identified based on patient interviews: theme 1 was the impact of psychotic illness and its treatments on sexual function and activity, including variable changes in sex drive, attitudes and behaviours during acute psychosis, vulnerability to trauma and medications; theme 2 related to intimacy and sexual relationships in the context of psychosis, with bidirectional effects between relationships and mental health; and theme 3 comprised autonomy, identity and intersectional considerations, including gender, sexuality, culture and religion, which interplay with psychosis and sexual health. Clinicians raised each of these priority areas, but emphasised risk prevention relative to patients' more holistic view of their sexual health and well-being. CONCLUSIONS: Women and non-binary people with early psychosis have wide-ranging sexual health priorities, affecting many facets of their lives. Clinical care should incorporate this knowledge to optimise sexual health and well-being in this population.

The focus of this research is to analyse both human hand motion and force, during eating, with respect to differing food characteristics and cutlery (including a fork and a spoon). A glove consisting of bend and force sensors has been used to capture the motion and contact force exerted by fingers during different eating activities. The Pearson correlation coefficient has been used to show that a significant linear relationship exists between the bending motion of the fingers and the forces exerted during eating. Analysis of variance (ANOVA) and independent samples t-tests are performed to establish whether the motion and force exerted by the fingers while eating is influenced by the different food characteristics and cutlery. The middle finger motion showed the least positive correlation with index fingertip and thumb-tip force, irrespective of the food characteristics and cutlery used. The ANOVA and t-test results revealed that bending motion of the index finger and thumb varies with respect to differing food characteristics and the type of cutlery used (fork/spoon), whereas the bending motion of the middle finger remains unaffected. Additionally, the contact forces exerted by the thumb tip and index fingertip remain unaffected with respect to differing food types and cutlery used.
