RESUMO

Background: Thyroid eye disease (TED; also known as thyroid - associated orbitopathy, Graves ophthalmopathy) is an autoimmune inflammatory disease which presents in typical signs and symptoms such as deep orbital pain, chemosis with or without caruncular edema, unilateral or bilateral proptosis, eyelid retraction, eyelid edema or erythema, restrictive strabismus and compressive optic neuropathy. Objective: The aim of this study was to investigate the role of thermal camera in the assessment of thyroid eye disease (TED) activity compared to the Clinical Activity Score (CAS) scale, exophthalmometry values, and thyroid hormone and antibody levels. Methods: A total of 50 patients participated in this cross-sectional study of whom 29 were in the active phase of TED according to the sum on CAS scale and 21 patients in the inactive phase. The Flir E8® thermal camera was used to measure the temperature of the orbital area and the values were compared with the CAS scale, exophthalmometry values and thyroid hormone and antibody levels. Results: Higher values of temperature (p>0.0001), CAS score (p>0.0001), exophthalmometry (p=0.022), FT4 (p=0.0176) and TRAb (p=0.0091) were found in patients in the active phase of TED. Temperature of orbital area showed statistically significant positive correlation with CAS scale (p=0.0001), exophthalmometry values (p=0.0022) and anti-TPO levels (p=0.019). Conclusion: Thermal camera showed higher values of the temperature of the orbital area in patients in the active phase of the disease and positively correlated with the CAS scale, exophthalmometry findings and anti-TPO levels.

The purpose of this study was to evaluate results of combined phacoemulsification, intraocular lens implantation and pars plana vitrectomy in patients with advanced diabetic retinopathy. We retrospectively evaluated postoperative outcomes and complications in 102 eyes of 102 patients who underwent a combined procedure. All patients had a visually significant cataract. Forty two patients had vitreous hemorrhage and mild proliferative diabetic retinopathy. Sixty patients had a mild tractional retinal detachment. The median follow up was 14 months (range 6-36 months). 80% of patients had an increase of visual acuity of at least 2 Snellen lines. The most frequent early postoperative complication was elevated intraocular pressure, followed by mild fibrinous reaction. The most frequent late postoperative complication was the presence of posterior synechia, followed by glaucoma. Eleven patients required a repeated pars plana vitrectomy. Combined phacoemulsification, posterior chamber intraocular lens implantation and pars plana vitrectomy can be successfully performed in selected diabetic patients with favorable postoperative outcome.

Clinical records of 6 children (7 eyes) with juvenile rheumatoid arthritis (JRA) who underwent cataract surgery with IOL implantation between January 1998 and December 2002 were reviewed. The median age at the time of cataract surgery was 8 years (range 5-14 years). The median follow up was 48 months (range 26 to 60 months). Five of six children (6 eyes) were on systemic immunosuppressive or anti-inflammatory therapy. Glaucoma was present in three eyes before surgery, and all three eyes underwent combined cataract surgery and trabeculectomy with mitomycin C. A final best corrected visual acuity of 0.5 or better was achieved in all eyes Postoperative complications included posterior capsule opacification (n = 5), glaucoma (n = 1), and cystoid macular edema (n = 1). Intraocular lens implantation in children with control of preoperative and postoperative ocular inflammation could lead to favorable visual results.

The purpose of the study was to present operative technique and results of a passive hydrodynamic expression of silicone oil through planned posterior capsulorhexis during cataract surgery in patients after pars plana vitrectomy. The retrospective analysis was done on 57 eyes with cataract after a previous pars plana vitrectomy, operated on between 2001 and 2004 at the Clinical hospital "Sestre milosrdnice" Zagreb. Preoperative and postoperative best corrected visual acuity (BCVA), preoperative and postoperative intraocular pressure (IOP), and postoperative complications were reviewed. Visual acuity improved or stabilized in all patients with an attached retina. Retinal detachment occurred in 11 eyes. Transient vitreous hemorrhage, that resolved within 1 week of surgery without treatment, was observed in 4 eyes. Asymptomatic intraocular lens (IOL) decentration occurred in 2 eyes. Our findings suggest that silicone oil removal and cataract surgery can be performed as a single procedure in selected patients in the absence of macular pucker and retinal reproliferation, and in a presence of a stable retina.

It seems that patient's knowledge about ophthalmologist's work is very insufficient, especially about what type of examination should be undertaken for refraction condition during the "simple" eye check-up and which serious systemic diseases could be discovered thorough eye examinations. The aim of the study was to determine patients' knowledge about ophthalmologist examinations during the check-up for refraction condition, knowledge about differences between ophthalmologists and opticians, main sources of patients' ophthalmologic cognition and the main reasons for coming to special locations. Patients (311) were examined by applying the questionnaire, immediately before the eye check-up at three various refraction units. Statistical analysis used Chi-square test and test of significance between proportions, except for patients' age where Student t-test was used. Differences were statistically significant at p = 0.05. The findings show that the patients' knowledge about eye examination during the check-ups for refraction abnormalities was not satisfactory. Twenty-two percent (22%) of examined patients did not know the differences between ophthalmologists and opticians and 16% believed that after computer testing of refraction further ophthalmologic examinations were not necessary. The main sources of medical cognition were the mass media while twenty percent (20%) of the participating patients named doctor's lectures as the source of their cognition. This study revealed that a lot of work needs to be done to improve patients' education and indirectly for better screening of very serious systemic diseases and blind threatening diseases which could be discovered during the first visit for spectacle prescription.

Limbal relaxing incisions are one of the more commonly performed procedures with phacoemulsification to correct preexisting astigmatism during cataract surgery. The aim of the study was to evaluate the effect and stability of limbal relaxing incisions in reducing preexisting astigmatism at the time of phacoemulsification. The study included 10 eyes in 12 patients who underwent limbal relaxing incisions during cataract surgery, with preexisting astigmatism of 1-2 diopter (D). Assessments were made preoperatively, and 1 and 3 months postoperatively. Outcome measures included uncorrected distance visual acuity, best corrected distance visual acuity and keratometric astigmatism measures. The mean preoperative and postoperative refractive astigmatism was 1.50 D (+/- 0.75D) and 0.25 D (+/- 0.25 D), respectively. There were no serious postoperative complications. Limbal relaxing incisions provide a viable option for correcting preexisting astigmatism at the time of cataract surgery with mild complications.

The aim of this study was to evaluate clinical outcomes after toric intraocular lens implantation for astigmatism correction in patients undergoing cataract surgery. This prospective observational study included eyes undergoing cataract surgery at Sestre milosrdnice University Hospital Center between February and September 2011. The study enrolled eyes that had visually significant cataract and preexisting regular corneal astigmatism of 1.50 D or greater. Phacoemulsification was performed and all eyes received an AT TORBI 709 M toric foldable intraocular lens. At 3 months, the uncorrected distance visual acuity was 0.8 or better in 9 of 10 eyes. At 3 months postoperatively, the median reduction in refractive cylinder was statistically significant (P < 0.05). The median intraocular lens axis rotation was 3.0 degrees interquartile range (2.0-4.0 degrees). Our results showed AT TORBI 709 IOL to be an effective surgical option to correct preexisting corneal astigmatism and spectacle dependence after cataract surgery.
