BACKGROUND: Retained ureteral stents can result in significant morbidity and can be surgically challenging to urologists. A multimodal approach is often necessary for removal, potentially including retrograde and antegrade procedures performed over multiple anesthetic sessions. We describe the novel "Tri-Glide" technique for treating retained stents, particularly those with stent shaft encrustation prohibiting safe removal. Case Presentation. Two patients with nephrolithiasis and retained, encrusted ureteral stents were managed with the "Tri-Glide" technique. Patient #1 was a 58-year-old man with a severely calcified ureteral stent, retained for 14 years. After undergoing simultaneous cystolitholapaxy and percutaneous nephrolithotomy to treat proximal and distal encrustations, the stent shaft remained trapped in the ureter due to heavy calcifications. Three hydrophilic guidewires were passed alongside the stent, allowing it to easily slide out of the ureter intact. Patient #2 was a 74-year-old man who after only 3-months of stent dwell time developed severe stent shaft encrustation preventing removal. After multiple maneuvers failed, the "Tri-Glide" technique was used to create a smooth track for stent to slide out intact with gentle traction. Both patients did well postoperatively with no complications. CONCLUSION: The "Tri-Glide" technique can aid in the management of complex encrusted stent extractions, especially when there is significant shaft encrustation.

RESUMO

Balloon dilation is an effective method of percutaneous renal access, but it can be impossible in patients with perirenal scarring. We report the use of an atherotomy cutting balloon for dilation of the chronic nephrostomy tract. The ease of use makes this device a useful alternative to standard dilation techniques.

PURPOSE: There is controversy regarding ureteroscope durability. Little is known regarding the subsequent durability of a flexible ureteroscope after major damage has been incurred and the ureteroscope has been repaired. Maintenance and repair are associated with significant cost. We reviewed and assessed the frequency and cause of ureteroscope damage at our medical center. MATERIALS AND METHODS: From December 2001 we prospectively recorded the specific use of all ureteroscopes and any resultant damage at a single tertiary care institution. We then reviewed a total of 601 ureteroscopic cases involving 654 semirigid and flexible ureteroscope uses from December 2001 to November 2004. Cases were performed by multiple residents and fellows under the supervision of 3 attending urologists (CML, RJL and VGB). Retrograde and antegrade cases involving stones, urothelial carcinoma, strictures and diagnostic evaluations were included. Repairs for the respective ureteroscopes were performed by the original manufacturer. RESULTS: A total of 53 reports of damage (8.1% of total uses) were recorded. Major damage when the scope was deemed unusable and required repair was seen in 39 cases (6.0%). Four newly purchased flexible ureteroscopes were entered into the study and they provided 40 to 48 uses before the initial repair was needed. After these new ureteroscopes underwent comprehensive repair for major damage they averaged only 11.1 uses (median 8) before needing repair again. Older model ureteroscopes that underwent repair before being entered into our study averaged between 4.75 and 7.7 uses before being sent for subsequent repair. Of the total of 39 breakages 39 for which ureteroscopes were sent for repair 14 (35.9%) were the result of errant laser firing, 11 (28.2%) were the result of excessive torque, 8 (20.5% 8) were the result of decreased flexion in the distal tip or another loss of function without obvious iatrogenic cause, 3 (7.7%) were the result of multifocal catastrophic damage involving laser firing and excessive torque, and 3 (7.7%) were the result of cleaning and processing outside of the ureteroscopy suite. CONCLUSIONS: The most important risk factors for predicting the number of uses expected from a ureteroscope at our institution is ureteroscope age and whether the ureteroscope has undergone comprehensive repair as the result of prior damage. Our analysis suggests that after damage occurs to a ureteroscope more damage occurs with greater frequency. The cost of maintaining previously used ureteroscopes should be carefully considered in comparison to the cost of purchasing a new ureteroscope.

BACKGROUND: Standard treatment of traumatic thoracic aortic transection (TTAT) is open repair by left thoracotomy with or without the use of partial cardiopulmonary bypass. However, open repair is associated with high rates of morbidity and mortality, particularly in multiply injured trauma patients. We reviewed our experiences of endovascular repair of acute TTAT. METHODS: Between February 2001 and February 2006, 30 patients (male 24, female 6, mean age 43 years) who had sustained severe blunt trauma with multiple injuries (mean injury severity score = 42) underwent endovascular repair for TTAT. Devices used included commercially available proximal abdominal aortic extension cuffs and thoracic stent-grafts. Either low dose or no systemic heparin was used. Arterial access was obtained by femoral-iliac cutdown (n = 19) or completely percutaneous through the femoral artery (n = 11). Mean follow-up was 11.6 months (range, 1 to 48 months). RESULTS: Technically success was achieved in 100% of patients, as determined by angiographic and computed tomographic (CT) scan exclusion of TTAT. Mean operating time was 132 minutes. Mean blood loss was 300 cm3. Three patients had complications: 1 iliac artery rupture, 1 cerebellar stroke, and 1 partial stent collapse. There were 2 perioperative deaths. There were no instances of procedure-related paralysis. Clinical and CT follow-up did not reveal evidence of endoleak, stent migration, or late pseudoaneurysm formation. CONCLUSIONS: The adaptation of commercially available stent-graft devices to treat TTAT is technically feasible, and can be performed with low rates of morbidity and mortality. The long-term durability of endovascular repair of TTAT remains unknown, but early and midterm results appear promising.
