OBJECTIVE: To study patient-reported hearing aid (HA) rehabilitation outcomes, social-communicative functioning, and expectations/experiences during eight months of HA use. DESIGN: Three self-reporting instruments, the International Outcome Inventory for Hearing Aids (IOI-HA), the Quantified Denver Scale of Communicative Function (QDS), and questionnaires tapping pre-rehabilitation expectations (HA-EXP-Q1) and post-rehabilitation experiences (HA-EXP-Q2) were administered. STUDY SAMPLE: 144 patients ages 23-66 with gradually acquired, adult-onset, mild-to-moderate sensorineural hearing loss affecting both ears who acquired their first HAs. RESULTS: According to self-reports, HA rehabilitation outcomes were good, and everyday social-communicative functioning improved after one month and after eight months of HA use. When the effects from demographic and audiological variables were analysed, younger age and positive expectations of HAs were associated with better outcomes and social-communicative functioning. The form or hearing loss severity, and the type or number of HAs did not affect outcomes. CONCLUSION: Working-age HA users reported better HA outcomes than older adults in previous studies. Coping in work life may be a strong motivator for active HA use. Considering that younger age and positive expectations resulted in better outcomes, early rehabilitation that supports positive and realistic expectations of HA performance is essential.

Hearing impairment is a common chronic condition in middle-aged and elderly adults. The number of individuals with hearing impairment is expected to rise because of the longer life expectancies and trends in the population growth. Acquired hearing impairment in adulthood is not just a disorder of the sense of hearing. It is primarily a social disability because its handicapping effect is experienced in interaction with other people. This paper aims to explore how the repair of problems in hearing is initiated by hearing-impaired individuals with acquired mild to severe hearing impairment. By using the method of conversation analysis (CA), this paper examines the occurrence of other-initiations of repair (OIR) and how it is typically resolved in actual mundane interaction. In addition, this paper reveals the challenges and the impact of hearing impairment as the state of hearing deteriorates. This article argues that the frequency of OIR in mild hearing impairment does not differ from normally hearing individuals. However, in a more severe grade of hearing impairment, the OIR sequences are longer, more frequent, multimodal and may require more vigilance from the normally-hearing conversation partner. Implications for counselling are suggested.

This study applies conversation analysis to compare everyday conversation samples between a person with aphasia (PWA) and a familiar communication partner (CP) before and after intensive language-action therapy (ILAT). Our analysis concentrated on collaborative repair sequences with the assumption that impairment-focused therapy would translate into a change in the nature of trouble sources, which engender collaborative repair action typical of aphasic conversation. The most frequent repair initiation technique used by the CP was candidate understandings. The function of candidate understandings changed from addressing specific trouble sources pre-ILAT to concluding longer stretches of the PWA's talk post-ILAT. Alongside with these findings, we documented a clinically significant increase in the Western Aphasia Battery's aphasia quotient post-ILAT. Our results suggest that instead of mere frequency count of conversational behaviours, examining the type and function of repair actions might provide insight into therapy-related changes in conversation following impairment-focused therapy.

BACKGROUND: Even though research has increasingly focused on the qualitative features of natural conversations, which have improved the communication therapy for hearing-impaired individuals (HI) and familiar partners (FP), very little is known about the interactions that occur outside clinical settings. AIMS: This study investigated qualitatively how both HI and FP initiated repair due to misperceptions or to a difficulty in understanding during conversations conducted at home. The HI participant's multimodal production style was adopted in the present analysis, and the frequencies were calculated for the different types of verbal repair initiations. METHODS & PROCEDURES: Participants with acquired hearing loss (43-69 years) and their familiar partners (24-67 years) were video recorded (total time approximately 9 h) in their homes. The data consisted of eight conversational dyads. The transcription and analysis utilized Conversation Analysis. OUTCOMES & RESULTS: A total of 209 (HI 164/FP 45) verbal repair initiations were identified. The five major types of initiations found in the data (used by both HI and FP) were: open repair initiation, targeting question word, question word with repetition, repetition, and candidate understanding. HI participants rarely explicitly verbalized their difficulty to hear, but the production style, which included a fast speech rate and 'trouble posture', indicated a sensitive routine that was visible particularly in clear misperceptions. Furthermore, the alerting action of overlapping turn taking with the FP participant's turn could be seen to reveal the depth of misperception. The individual differences between HI participants were found predominantly in the frequency of their repair initiations, but also in how they used the different types of repair initiation. CONCLUSIONS & IMPLICATIONS: Through a deeper qualitative analysis, conversational research can provide extended knowledge of the occurrence and style of ordinary repair initiations and highlight their relationship in certain conversational environments. A robust starting point in communication therapy is increasing the awareness of HI individuals' existing skills.

RESUMO

Hearing impairment is on the rise with the aging population. It causes conversational problems, whose prevention or solving can be influenced by the physician's own action. The aim in talking with a hearing-impaired patient is clarity of communication. Tranquility of the surroundings, visibility of the face and lucid speech are essential aspects. Other useful means of conversation include division of speech, emphasized gestures and, in the case of inhibited spoken communication, writing. It is always possible to improve conversation by paying attention to one's own style of communication and constructing of the situation.
