BACKGROUND: Work-related musculoskeletal disorders (WRMDs) are often caused by inadequate use of the musculoskeletal system during work. Evidence suggests that multimodal intervention through exercises, massage, education, and ergonomic guidelines reduces pain and symptoms in the neck and upper extremities and help to prevent musculoskeletal disorders. The purpose of this study will be to assess the additive effectiveness of a specific and individualized workplace strengthening exercise program to an ergonomic guidance in reducing fatigue, pain and discomfort in the upper extremities and neck perceived by workers. METHODS: This trial was designed according to the Consolidated Standards of Reporting Trials - CONSORT guidelines. Participants will be employees of a tertiary hospital, with any complaints of pain or discomfort in the upper extremities during the past 12 months, without clinical musculoskeletal diagnosis. 166 participants will be randomized into parallels groups as control and workplace exercises. The primary outcomes will be Numerical Pain Scale, isokinetic muscle strength of abduction and isometric handgrip strength. Secondary outcomes on discomfort, fatigue, work capacity and dysfunction will be assessed by QuickDASH, Patient Specific Functional Scale, Neck Disability Index, Need for recovery, Work Ability Index self-report questionnaires and FIT-HANSA performance test. The Ergonomic Work Analysis will be done by Quick Expose Check, RULA, REBA, RARME, ROSA and HARM risk assessment ergonomic tools. We will analyze the difference between baseline and 12 weeks of intervention by T test of independent samples (95% confidence interval, p < 0.05). Clinical significance will be analyzed by the minimum clinically important difference and effect size by Cohen index. The association between the variables will be analyzed by construct validity with the hypothesis of correlations between pain and muscle strength, strength and functionality and strength and fatigue. DISCUSSION: Although studies have shown promise outcomes for workplace exercises as an available therapeutic resource used to minimize complaints of pain and discomfort related to work, the results of this study aim to bring evidence about the benefit of a specific resistance exercise as an effective modality to facilitate mechanisms of neuromuscular adaptations, with gradual and posterior hypertrophy in the later phases. This study aimed to analyse the relationship between the organisational culture and feelings of pleasure and suffering among working nursing professionals. This was a cross-sectional correlational study conducted in a tertiary hospital with 214 nursing staff over 3 months using three instruments: professional characterisation, the Brazilian Instrument for Assessment of Organisational Culture, and the Scale of Pleasure and Suffering at Work. The analysis included descriptive statistics and the Spearman correlation test. The external integration practice was the domain most frequently found in the organisational culture and the feeling of pleasure-gratification predominated among the workers. Values of cooperative professionalism and wellbeing, and practices of external integration and relationship promotion, were related to increased pleasure and decreased suffering at work. These aspects depend on the organisational culture of the institution. Investigating organisational culture facilitates the understanding of potential collective coping strategies and the organisational changes that favour good mental health in nurses.

AIMS AND OBJECTIVES: To gain an understanding of medication safety culture and other quality issues in a Brazilian intensive care unit using a restorative approach. BACKGROUND: Patient safety should be considered one of the pillars of quality in health care. Thus, patient safety culture is increasingly being explored as a guide for quality improvement efforts. DESIGN: A qualitative approach. METHODS: Participatory photographic research methods from the field of ecological restoration were adapted in this study. This study used focus groups, then subsequent nurse-led photo-narrated walkabouts, and photo elicitation with 23 nurses and one physician in interactive phases of data collection and analysis over an eight-month timeframe. RESULTS: The core themes identified were: the 'medication system shapes patient safety' and the 'feeling of helplessness in the face of the prevailing organization culture'. Participants discussed supports exiting in the intensive care unit that shape medication safety, the barriers that impede safe medication management, the solutions to improve medication safety and the creation of a better medication safety culture. CONCLUSIONS: The methods used allowed participants to visualise sound practices as well as key safety issues, reflect on their day-to-day work, re-think potential improvements, and enact changes to improve medication safety and medication safety culture. However, the patient safety culture is also marked by administrative pressure. The hospital needs to adopt participatory management, where the health professionals can act together with the organisational leaders to promote a just culture. RELEVANCE TO CLINICAL PRACTICE: The participatory photographic research methods from the field of ecological restoration provided participants with a tool to promote patient safety culture and engage policy change dialogue. However, it will be important in future restorative research to track-specific safety outcomes over time to assess the cost-benefit of the adoption of participatory management models.

Objective To assess the values and practices that characterize the organizational culture of a private hospital in the state of São Paulo in the perspective of nursing professionals. Methods Quantitative, descriptive, cross-sectional study. Data collection was conducted between January and March 2013 using the Brazilian Instrument for Assessing Organizational Culture. Twenty-one nurses and sixty-two nursing aides and technicians participated in the study. The responses of the participants were coded into numerical categories, generating an electronic database to be analyzed by means of the software Statistical Package for the Social Sciences. Results Scores of cooperative professionalism values (3.24); hierarchical strictness values (2.83); individual professionalism values (2.69); well-being values (2.71); external integration practices (3.73); reward and training practices (2.56); and relationship promotion practices (2.83). Conclusion In the perception of workers, despite the existence of hierarchical strictness there is cooperation at work and the institution pursues customer satisfaction and good interpersonal relationships.

RESUMO

The objective of this research was to analyze the organizational culture of a Brazilian public hospital. It is a descriptive study with quantitative approach of data, developed in a public hospital of São Paulo State, Brazil. The sample was composed by 52 nurses and 146 nursing technicians and auxiliaries. Data were collected from January to June 2011 using the Brazilian Instrument for Assessing Organizational Culture - IBACO. The analysis of the organizational values showed the existence of hierarchical rigidity and centralization of power within the institution, as well as individualism and competition, which hinders teamwork. The values concerning workers' well-being, satisfaction and motivation were not highly valued. In regard to organizational practices, the promotion of interpersonal relationship, continuous education, and rewarding practices were not valued either. It becomes apparent that traditional models of work organization support work practices and determine the organizational culture of the hospital.

RESUMO

This study aims to characterize the communication process among nursing assistants who work in vehicles of the basic life support of the mobile emergency service, in the coordination of this service, and in the unified medical regulation service in a city of the state of São Paulo, Brazil. This descriptive and qualitative research used the thematic content analysis for data analysis. Semi-structured interviews were used for the data collection, which was held in January, 2010. Results show diffculties in communication with both the medical regulation service and the coordination. Among the most highlighted aspects are failures during the radio transmission, lack of qualified radio operators, difficult access to the coordination and lack of supervision by nurses. However, it was possible to detect solutions that aim to improve the communication ana consequently, the service offered by the mobile emergency service.

OBJECTIVE: to establish the psychometric properties of the Authentic Leadership Questionnaire (ALQ) applied to Brazilian nurses. METHOD: cross-sectional observational study with a non-probabilistic sample. The psychometric properties of the RATER and SELF versions of the ALQ were calculated using confirmatory factor analysis with the WLSMV robust estimation method. The following indices were used to assess the goodness-of-fit of the model: chi-square by degrees of freedom (χ2/df), Tucker-Lewis Index (TLI), Comparative Fit Index (CFI), Root Mean Square Error of Approximation (RMSEA) and Standardized Root Mean Squared Residual (SRMR). Data reliability was analyzed using the ordinal coefficient alpha and composite reliability. RESULTS: 181 nurses participated of the study (female gender: 80.1%; mean age of 34.6 years; working time of less than five years: 76.3%). The complete ALQ RATER and ALQ SELF models did not present an adequate fit. Therefore, the refined models presented a better fit to the sample data (ALQ RATER: χ2/df=2.77; CFI=0.97; TLI=0.97; RMSEA=0.10; SRMR=0.05; ALQ SELF: χ2/df=2.74; CFI=0.94; TLI=0.92; RMSEA=0.10; SRMR=0.08). In the ALQ RATER model, items 1, 7 and 13 were excluded. Due to the high correlation between the factors Relational Transparency and Moral Perspective, a three-factor model based on the combination of the factors mentioned above was proposed. In the ALQ SELF model, items 2, 5, 9 and 10 were excluded. Likewise, a three-factor model based on the combination of two factors, now called Self-Awareness Balance, was proposed. CONCLUSION: the data obtained with the Authentic Leadership Questionnaire with Brazilian nurses were valid and reliable.

To assess the prevalence of depression, anxiety and stress symptoms in health professionals in the COVID-19 pandemic context. METHOD: Cross-sectional study with non-probabilistic (snow-ball) sampling method. The assessment was performed using the Depression, Anxiety, and Stress Scale (DASS-21) and the prevalence of symptoms severity was calculated by point and 95% confidence interval. The analysis of the psychometric properties of DASS-21 was performed using confirmatory factor analysis (CFA) and the following goodness of fit indices: χ2/df (chi-square ratio by degrees of freedom), Tucker-Lewis index (TLI), comparative fit index (CFI) and root mean square error of approximation (RMSEA) with a 90% confidence interval. RESULTS: The study participants were 529 health professionals (82.4% women and 66.7% nursing professionals). CFA of the DASS-21 structural model presented adequate fit for the sample (χ2/df = 3.530; CFI = 0.979; TLI = 0.976; RMSEA = 0.069). Regarding prevalence, moderate to extremely severe symptoms of depression, anxiety and stress were found in 48.6%, 55.0% and 47.9% of the participants, respectively. CONCLUSION: The use of DASS-21 confirmed the validity and reliability of the data. The prevalence of depression, anxiety and stress symptoms in the participants indicated a high risk of mental illness in health professionals in the COVID-19 pandemic.

OBJECTIVE: to analyze factors associated with presenteeism in nursing workers with sociodemographic variables, health and work conditions, productivity and musculoskeletal symptoms. METHODS: this is a cross-sectional, descriptive and analytical study, with 306 nursing workers from a hospital and municipal emergency room in a Brazilian capital. The Stanford Presenteeism Scale, the Work Limitations Questionnaire, the Nordic Musculoskeletal Questionnaire and a demographic questionnaire on nursing professionals' working conditions and health were used. Bivariate and multivariate analyzes were performed, respecting a significance level of 5%. RESULTS: presenteeism was found in 43.8% of professionals and significant associations with CLT work (p=0.002), workplace - Intensive Care Units (p=0.008), physical exercise twice a week (p=0.008), presence of musculoskeletal symptoms, with low back pain being representative (p=0.001). The productivity loss was 8.8. CONCLUSIONS: the study confirms a high rate of presenteeism among nursing workers.

OBJECTIVE: to evaluate the psychometric properties of the Work Limitations Questionnaire and to measure presenteeism in a sample of nursing workers. METHOD: a cross-sectional study, with non-probabilistic sampling. Data was collected between July 2018 and February 2019 in two high-complexity hospitals, and the sample was composed of 304 participants. The validity analysis of the Work Limitations Questionnaire was performed by means of Confirmatory Factor Analysis. RESULTS: most of the participants were women (88.5%), with a mean age of 40.9 years old. The validities of the factorial, convergent and discriminant construct and the reliability of the complete version and of the 16-item version of the instrument were adequate after adjusting the models to the sample. A statistically significant and negative correlation (p<0.001) was verified between the workload, working time and the Time Management and Physical Demands dimensions; as well as a statistically significant (p<0.001) correlation between working time and the Mental-Interpersonal Demands and Production Demands dimensions. Gender and professional category did not influence presenteeism. The rate of loss of productivity at work was 19.51%. CONCLUSION: the Work Limitations Questionnaire showed adequate validity and reliability and can be considered a valid and reliable instrument for assessing presenteeism in the nursing team.

OBJECTIVE: to translate and culturally adapt the Risk assessment and management of exposure of health care workers in the context of covid-19 questionnaire to the Brazilian context and to develop and evaluate a sociodemographic and occupational characterization questionnaire to compose the adapted questionnaire. METHOD: five stages were conducted to adapt the Risk assessment and management of exposure of health care workers in the context of covid-19 questionnaire, namely: translation, synthesis of the translations, evaluation by a committee of judges, back translation and pre-test. As for the complementary questionnaire, it was elaborated and evaluated by a committee of judges and a pre-test was carried out. RESULTS: the questionnaires were validated and the pre-test stage was conducted with health workers and students. CONCLUSION: the final version adapted to the Brazilian context was called Questionário de avaliação de risco e gestão da exposição de trabalhadores e estudantes de saúde no contexto da covid-19 and is available for use, together with the final version of the Sociodemographic and occupational questionnaire: Risk assessment and management of exposure of health care workers and students in the context of covid-19. These questionnaires may assist in mitigating the risks of infection, illness and death of health workers and students due to covid-19.

RESUMO

Cross-sectional exploratory research developed to outline the roles and functions of occupational health nurses (OHNs) in Brazil and compare them with those in the United States. The sample consisted of 154 Brazilian occupational health nurses. First, the instrument Job Analysis Survey of Occupational Health Nursing Practice was translated into Portuguese, followed by data collection and data analysis, comparing the results of the proposed study in Brazil with the findings of a job delineation study conducted in the United States. It was found that most were women, white, between 41 and 50 years of age and working primarily at hospitals or medical centers as clinicians and managers/administrators. Besides, it was found that most Brazilian OHNs spend more time in managerial roles, followed by consultant and educator responsibilities while, in the United States, OHNs spend significantly more time in education/advisory roles.

RESUMO

This is an exploratory research, with a quantitative approach, developed with the objective of analyzing the work and of life situations that can offer risks to the workers' health involved in the manual and automated cut of the sugar cane. The sample was composed by 39 sugar cane cutters and 16 operators of harvesters. The data collection occurred during the months of July and August of 2006, by the technique of direct observation of work situations and workers' homes and through interviews semi-structured. The interviews were recorded and later transcribed. Data were analyzed according to Social Ecological Theory. It was observed that the workers deal with multiple health risk situations, predominantly to the risks of occurrence of respiratory, musculoskeletal and psychological problems and work-related accidents due to the work activities. The interaction of individual, social and environmental factors can determine the workers' tendency to falling ill.

RESUMO

Qualitative study developed in a public university of São Paulo State, Brazil, to identify the conception of quality of hospital nursing care under the perspective of last year undergraduate nursing students. Semi-structured interviews with 24 students were performed to collect the data and the speeches were analyzed according to Bardin's conceptual framework. The central ideas of the speeches were divided into six categories: holism, humanization, technically and scientifically trained team, promotion of self-care, appropriate management of the units and respecting the safety principles in care. It is concluded that students' view regarding the theme is wide; however it is necessary to incorporate important issues to the concept of quality of hospital care, such as the valorization of team work, the systematization of care and the satisfaction of all actors involved in nursing care.

The aims of this study were to evaluate the psychometric properties of the Copenhagen Burnout Inventory-Brazilian version (CBI-Br) in a sample of university professors and academic staff members of Brazilian public universities, to estimate the level of burnout syndrome (BS) among these workers, and to assess the associations of BS with demographic and occupational determinants of the syndrome. A total of 676 workers participated in the study. Confirmatory factor analysis results supported a three-factor model with 18 items and an acceptable overall fit. Adequate convergent and discriminant validity of the CBI-Br's factors were observed, as well as adequate reliability of the instrument for the sample. In conclusion, the results of this study provide evidence of the validity and reliability of the CBI-Br for the measurement of BS in Brazilian university professors and academic staff members. In addition, the CBI-Br may be an important tool for the diagnosis of psychosocial risks related to BS in the academic environment.

OBJECTIVE: To identify what risks and incidents related to the drug therapy process are presented in an Emergency Care Unit (UPA) and to present proposals for management actions and safe practices, in the perception of the nursing team. METHOD: Qualitative research, in the research-action modality, developed in the scenario of UPA, located in the interior of São Paulo. Data collection was done through interviews and focus groups with 33 professionals, between June 2015 and April 2016. For the data treatment, the Content Analysis Technique was used. RESULTS AND DISCUSSION: From the participants' reports, thematic categories were organized, with the risks and incidents related to the drug therapy process being one of the listed categories, as well as proposals for actions. CONCLUSION: The study allowed the implementation of patient safety actions related to the administration of drugs in a PAU, offering a higher quality of care.

BACKGROUND: Work-related musculoskeletal disorders are the most frequently reported group of diseases in Brazil, their occurrence being favored by work in the industrial sector. The present study is justified by the need to investigate more thoroughly the occurrence of musculoskeletal symptoms among poultry farming workers, since this subject is scarcely explored in the Brazilian literature. OBJECTIVE: To analyze the prevalence of musculoskeletal symptoms among production and administrative employees of a poultry farming company, describe the sociodemographic and occupational profile of both groups of workers and test associations between pain and sociodemographic and occupational variables. METHODS: Cross-sectional correlational study which included sociodemographic and occupational characterization and administration of the Nordic Musculoskeletal Questionnaire and a numeric pain rating scale. RESULTS: The sample comprised 154 production employees and 24 administrative workers. Pain was the single musculoskeletal symptom reported, with prevalence of over 85% in both groups. Female production employees exhibited 2.9 times higher odds of pain compared to the males. The body sites most frequently involved were the neck and upper and lower back among the administrative employees and the shoulders and the upper and lower back among the production workers. CONCLUSION: Pain was the most prevalent musculoskeletal symptom in the analyzed sample of poultry farming workers. Statistically significant difference was not found between the groups despite their different work processes. Both groups exhibited extremely high rates of illness, which points to the need for more thorough investigation of the causal relationships of musculoskeletal symptoms.


INTRODUÇÃO: Distúrbios osteomusculares relacionados ao trabalho constituem o grupo de doenças ocupacionais mais registradas no Brasil, sendo as indústrias ambientes favoráveis para seu desenvolvimento. Este estudo justifica-se pela necessidade de aprofundar investigações sobre a manifestação de sintomas osteomusculares no ramo industrial agroavícola, temática pouco explorada na literatura nacional. OBJETIVOS: Analisar a prevalência de sintomas osteomusculares em trabalhadores da linha de produção e do setor administrativo de uma indústria agroavícola, caracterizar o perfil sociodemográfico e ocupacional dos dois grupos e identificar ocorrência de associações entre dor e variáveis sociodemográficas e ocupacionais. MÉTODO: Estudo correlacional e transversal, realizado por meio de caracterização sociodemográfica e ocupacional, Questionário Nórdico de Sintomas Osteomusculares e Escala Numérica de Intensidade da Dor. RESULTADOS: Participaram 154 trabalhadores do setor de produção e 24 do de administração. O único sintoma osteomuscular apresentado foi a dor, com prevalência superior a 85% para ambos os setores. Mulheres do setor de produção apresentaram 2,9 vezes mais chances de terem dor do que homens. As regiões mais afetadas no setor administrativo foram pescoço e partes inferior e superior das costas. Já no setor de produção, prevaleceram a região inferior das costas e dos ombros e a parte superior das costas. CONCLUSÕES: A dor foi descrita como o sintoma osteomuscular mais prevalente entre os trabalhadores da indústria agroavícola. Apesar das diferenças nos processos de trabalho entre os trabalhadores administrativos e produtivos, não houve diferenças estatisticamente significativas entre os setores. Por outro lado, ambos apresentaram um altíssimo nível de adoecimento, o que sugere a necessidade de aprofundamento em estudos da relação causal desses sintomas.

RESUMO

OBJECTIVE: To identify in the literature the efficacy of needlesticks with safety devices to reduce the occurrence of occupational accidents with exposure to biological material among health workers. METHOD: Integrative literature review, structured in the stages: Guiding question, search, categorization of studies, evaluation, discussion and interpretation of results, and synthesis of knowledge. Search for original articles and systematic reviews on the main bases of the Health area, published from 2000 to 2016 in Portuguese, English and Spanish, with descriptors: needlesticks injuries, exposure to biological agents, needles, protective devices, occupational accidents, accident prevention and health personnel. RESULTS: We selected eleven articles, most characterized the passive safety devices as more effective in reducing the occurrence of injuries by needlesticks. CONCLUSION: The use of needlesticks with safety devices reduces the occurrence of accidents, bringing greater solvency when combined with the training of workers.

ABSTRACT Objective: to analyze factors associated with presenteeism in nursing workers with sociodemographic variables, health and work conditions, productivity and musculoskeletal symptoms. Methods: this is a cross-sectional, descriptive and analytical study, with 306 nursing workers from a hospital and municipal emergency room in a Brazilian capital. The Stanford Presenteeism Scale, the Work Limitations Questionnaire, the Nordic Musculoskeletal Questionnaire and a demographic questionnaire on nursing professionals' working conditions and health were used. Bivariate and multivariate analyzes were performed, respecting a significance level of 5%. Results: presenteeism was found in 43.8% of professionals and significant associations with CLT work (p=0.002), workplace - Intensive Care Units (p=0.008), physical exercise twice a week (p=0.008), presence of musculoskeletal symptoms, with low back pain being representative (p=0.001). The productivity loss was 8.8. Conclusions: the study confirms a high rate of presenteeism among nursing workers.


RESUMEN Objetivo: analizar factores asociados al presentismo en trabajadores de enfermería con variables sociodemográficas, condiciones de salud y trabajo, productividad y síntomas musculoesqueléticos. Métodos: estudio transversal, descriptivo y analítico, con 306 trabajadores de enfermería de un hospital y urgencias municipales de una capital brasileña. Se utilizo el Stanford Presenteeism Scale, el Work Limitations Questionnaire, el Cuestionario Nórdico Musculoesquelético y un cuestionario demográfico sobre las condiciones laborales y de salud de los profesionales de enfermería. Realizaron análisis bivariados y multivariados, respetando un nivel de significancia del 5%. Resultados: se encontró presentismo en el 43,8% de los profesionales y asociaciones significativas con el régimen de trabajo celetista (p=0,002), lugar de trabajo - Unidades de Cuidados Intensivos (p=0,008), ejercicio físico dos veces por semana (p=0,008), presencia de musculoesquelético síntomas, siendo la lumbalgia representativa (p=0,001). La pérdida de productividad fue de 8.8. Conclusiones: el estudio confirma una alta tasa de presentismo entre los trabajadores de enfermería.

Resultados: o presenteísmo foi constatado em 43,8% dos profissionais e associações significativas com o regime de trabalho celetista (p=0,002), local de trabalho - Unidades de Terapia Intensiva (p=0,008), prática de exercícios físicos 2 vezes por semana (p=0,008), presença de sintomas osteomusculares, sendo a dor lombar representativa (p=0,001). A perda de produtividade foi de 8,8. Conclus ões : o estudo confirma alto índice de presenteísmo entre trabalhadores de enfermagem.
