BACKGROUND AND PURPOSE: The association between alcohol intake and cognitive decline has been widely studied. Sex differences and cognitive domains affected by alcohol intake patterns make this topic complex. The objective of this study was to investigate the effect of alcohol intake on cognition in middle-aged participants in the Brazilian Longitudinal Study of Adult Health by sex during 4 years of follow-up. METHODS: A total of 7595 participants (55% women) aged between 50 and 75 years at baseline were assessed. Semantic and phonemic fluency, memory, and executive functions were assessed at baseline (2008-2010) and repeated during Visit 2. Linear mixed models were used to investigate the association between cognition and current abstainers, never drinkers, light drinkers, moderate drinkers, and heavy drinkers. RESULTS: Heavy alcohol intake accentuated the decline in executive functions for men (ß = -0.01, p < 0.05), and in semantic fluency (ß = -0.02, p < 0.05) and memory (ß = -0.02, p < 0.05) for women. Never drinker men also showed an accentuated decline in semantic fluency (ß = -0.02, p < 0.01). Moderate alcohol intake slowed cognitive decline in phonemic fluency for men (ß = 0.02, p < 0.01) and women (ß = 0.01, p < 0.01), and in executive functions (ß = 0.01, p < 0.05) for women. CONCLUSIONS: Having more than 14 drinks per week can impact executive functions in men and memory in women. In addition, alcohol consumption of seven to 14 drinks per week may have a protective effect on gender-specific cognitive functions. These findings should be considered in public health policies and guidelines on alcohol and cognitive aging.

OBJECTIVES: Normative data should consider sociodemographic diversity for the accurate diagnosis of cognitive impairment. This study aims to provide normative data for a brief neuropsychological battery and present diagnostic criteria for cognitive impairment that could be used in primary care settings. METHODS: We selected 9618 Brazilian middle-aged and older adults after detailed exclusion criteria to avoid subtle cognitive impairment. We analyzed age, sex, and education influence on cognitive performance. To verify the evidence of criterion validity, we compared the cognitive performance of subjects with and without a depressive episode. Additionally, we verified the percentage of spurious scores under three different cutoffs. RESULTS: Age and education had the greatest impact on cognition. Normative scores were provided according to age and education groups. Participants with a depressive episode performed poorer than control subjects. The clinical cutoff of at least two scores below the 7th percentile revealed the adequate percentage of spurious and possible clinical performance. CONCLUSIONS: The Longitudinal Study on Adult Health (ELSA-Brasil) provided normative data based on a unique selected set of cognitively normal subjects. Normative groups were selected based on age and education, and the battery was sensitive to the presence of a depressive episode. We suggested clinical cutoffs for the tests in this battery that could be used in primary care settings to improve the accurate diagnosis of cognitive impairment.

OBJECTIVE: The purpose of this study was to compare the effects of castration on cell death rate of the adult rat prostates and to evaluate the benefic action of alpha tocopherol supplementation to avoid apoptosis post-orchiectomy. MATERIAL AND METHODS: Thirty male Wistar rats weighing 250-300g were divided into three groups: group I - they were subjected to bilateral orchiectomy and sacrificed eight weeks after the procedure; group II - subjected to bilateral orchiectomy and alpha-tocopherol supplementation for four weeks preceding the procedure; and group III - subjected to bilateral orchiectomy and alpha-tocopherol supplementation for four weeks preceding the procedure and for eight weeks afterwards. At the end of the experiment, the prostatectomy was performed in all rats. The presence of oxidative stress was determined by assaying the blood level of 8-isoprostane and the occurrence of apoptosis was evaluated by identification of active caspase-3 through immunohistochemical analysis. RESULTS: The statistic analysis of active caspase-3 showed that in the long-term castrated group the detection was higher than in groups were the alpha-tocopherol was supplemented (p=0.007). Analysis of 8-isoprostane levels showed higher concentrations of reactive oxygen species in group I compared to other groups (p < 0.05). Groups II and III presented active caspase-3 lower than in group I (p < 0.05). CONCLUSION: Our exploratory analyses demonstrate a method to study the aging process and its influence on oxidative stress of prostatic tissue and cells death rate. Based on our results we can suggest that alpha tocopherol supplementation can decrease the apoptotic process as well as the oxidative stress levels induced by androgen deprivation of the prostate gland.

BACKGROUND: Validation of cognitive instruments for detection of Alzheimer's disease (AD) based on correlation with diagnostic biomarkers allows more reliable identification of the disease. OBJECTIVES: To investigate the accuracy of the Brief Cognitive Screening Battery (BCSB) in the differential diagnosis between AD, non-AD cognitive impairment (both defined by cerebrospinal fluid [CSF] biomarkers) and healthy cognition, and to correlate CSF biomarker results with cognitive performance. METHODS: Overall, 117 individuals were evaluated: 45 patients with mild cognitive impairment (MCI) or mild dementia within the AD continuum defined by the AT(N) classification [A+T+/-(N)+/]; 27 non-AD patients with MCI or mild dementia [A-T+/-(N)+/-]; and 45 cognitively healthy individuals without CSF biomarker results. All participants underwent evaluation using the BCSB. RESULTS: The total BCSB and delayed recall (DR) scores of the BCSB memory test showed high diagnostic accuracy, as indicated by areas under the ROC curve (AUC): 0.89 and 0.87, respectively, for discrimination between AD and non-AD versus cognitively healthy controls. Similarly, total BCSB and DR displayed high accuracy (AUC-ROC curves of 0.89 and 0.91, respectively) for differentiation between AD and controls. BCSB tests displayed low accuracy for differentiation between AD and non-AD. The CSF levels of biomarkers correlated significantly, though weakly, with DR. CONCLUSIONS: Total BCSB and DR scores presented good accuracy for differentiation between patients with a biological AD diagnosis and cognitively healthy individuals, but low accuracy for differentiating AD from non-AD patients.

The Mini-Mental Examination for Children (MMC) is a widely used tool for assessing global cognitive deficits, however,is still unknown whether MMC is sensitive for investigating cognitive profiles associated with learning difficulties (LD). OBJECTIVE: Here we investigate the feasibility of using the MMC for screening school-aged children with learning difficulties in spelling and math. METHODS: The MMC and other neurophysiological tests were administered to a sample of 168 children, aged 7 to 12 years. The sample was subdivided into a Control group and LD group (Math Difficulties, Spelling Difficulties, Math and Spelling Difficulties). Diagnostic accuracy was assessed with ROC analysis. Convergent and divergent validity was assessed using correlation analysis. RESULTS: Performance on the MMC was associated with nonverbal intelligence, age and school achievement. The LD group had significantly lower performance on the MMC than the Control group. Performance on the MMC discriminated LD children with a global accuracy of around 0.80. Associations between the MMC and the other neuropsychological variables were higher for finger gnosis (r=0.40) and generally higher for early elementary school grades. The MMC proved satisfactory for identifying LD children with good accuracy. Nonverbal intelligence, and perceptual/motor abilities play an important role in MMC performance. CONCLUSION: The MMC could be a useful instrument for screening children with LD.


Mini-exame mental para crianças (MMC) é uma ferramenta amplamente utilizada para avaliar déficits cognitivos globais, no entanto, ainda é desconhecido se a MMC é sensível para investigar perfis cognitivos associados a dificuldades de aprendizagem. OBJETIVO: Aqui nós investigamos a viabilidade de usar MMC para triagem de crianças em idade escolar com dificuldades de aprendizagem em ortografia e matemática. MÉTODOS: MMC e outros testes neuropsicológicos foram administrados em uma amostra de 168 crianças de 7 a 12 anos de idade. A amostra foi subdividida em um Grupo Controle e um grupo LD (dificuldade na matemática, na escrita, ou na escrita e na matemática). A acurácia diagnóstica foi analisada através de uma análise de curva ROC. A validade convergente e divergente foi investigada através de análises de correlações. RESULTADOS: A performance no MMC foi associada com a inteligência não verbal, idade e desempenho escolar. O grupo LD apresentou desempenho significativamente inferior ao Grupo Controle no MMC. A performance no MMC pôde identificar crianças LD com uma acurácia global em torno de 0.80. As associações entre MMC e outras variáveis neuropsicológicas foram maiores para gnosias digitais (r=0.40) e em geral, mais altas nas séries iniciais. O MMC se mostrou satisfatório para identificar crianças LD com uma boa acurácia. A inteligência não verbal, habilidades perceptivas/motoras tem um importante papel na performance no MMC. CONCLUSÃO: O MMC pode ser um instrumento útil para o rastreamento de crianças com LD.

RESUMO Antecedentes: A validação de testes cognitivos para identificação da doença de Alzheimer (DA) definida por biomarcadores aumenta a confiabilidade diagnóstica. Objetivos: Investigar a acurácia da Bateria Breve de Rastreio Cognitivo (BBRC) no diagnóstico diferencial entre DA, comprometimento cognitivo não-DA (ambos diagnósticos definidos por biomarcadores no líquido cefalorraquidiano-LCR) e indivíduos cognitivamente saudáveis, e investigar correlações entre desempenho nos testes e concentrações dos biomarcadores no LCR. Métodos: No total, 117 indivíduos foram avaliados. Quarenta e cinco pacientes com comprometimento cognitivo leve (CCL) ou demência leve com diagnóstico do continuum de DA definido pela classificação AT(N) [A+T+/-(N)+/-], 27 pacientes com CCL ou demência leve não-DA [A-T+/-(N)+/-], e 45 controles cognitivamente saudáveis sem estudo de biomarcadores no LCR. Os participantes foram submetidos à BBRC. Resultados: O escore total da BBRC e a evocação tardia (ET) no teste de memória da BBRC apresentaram elevada acurácia diagnóstica na diferenciação entre DA e não-DA versus controles, indicada pelas áreas sob a curva ROC (AUC) de 0,89 e 0,87, respectivamente. De modo semelhante, o escore total da BBRC e a ET mostraram elevadas acurácias (AUC-ROC de 0,89 e 0,91, respectivamente) para o diagnóstico diferencial entre DA e controles. A acurácia da BBRC foi baixa na diferenciação entre DA e não-DA. Os níveis dos biomarcadores no LCR se correlacionaram de forma significativa, embora fraca, com ET. Conclusões: Os escores totais da BCSB e a ET apresentaram boa acurácia na diferenciação entre pacientes com diagnóstico biológico de DA e controles cognitivamente saudáveis, mas baixa acurácia para diferenciar DA de não-DA.

Cognitive impairment is frequent in cerebral palsy (CP) and there is a lack of multiprofessional screening instruments. OBJECTIVE: The aim of this study was to investigate the utility of the Mini-Mental State Examination for Children (MMC), an adapted version of the Mini-Mental State Examination, in screening for cognitive impairments in children with CP. METHODS: We assessed 397 Brazilian children, 310 with typical development and 87 with CP (hemiplegic and quadriplegic forms), aged 5-16 years. Association between the MMC and general intelligence was assessed by the Colored Progressive Matrices instrument. RESULTS: Psychometric indexes for the MMC were adequate. ROC analyses revealed effective diagnostic accuracy in all ages assessed. Cut-off values are reported. Major difficulties on the MMC were observed in children with CP, particularly individuals with the quadriplegic form. Moreover, the MMC showed moderate correlation with the intelligence test, and was reliable in discriminating, among clinical cases, those with poorer cognitive abilities. CONCLUSION: The MMC could be useful as a multiprofessional screening instrument for cognitive impairment in children with hemiplegic CP. Results of the MMC in quadriplegic CP children should be interpreted with caution. Diagnosis should be confirmed by further psychological testing.


Comprometimentos cognitivos são frequentes na Paralisia Cerebral (PC) e existe uma falta de instrumentos multiprofissionais para uma triagem. OBJETIVO: Investigar a viabilidade do uso do mini-exame do estado mental para crianças (MMC), uma adaptação do mini-exame do estado mental, como uma triagem para comprometimento cognitivo em crianças com PC. MÉTODOS: Nós avaliamos 397 crianças brasileiras, 310 com desenvolvimento típico e 87 com PC (hemiplégica e quadriplégica), com idades entre cinco e 16 anos. A associação entre MMC e a inteligência geral foi avaliada através das Matrizes Coloridas Progressivas de Raven. RESULTADOS: Os índices psicométricos para o MMC foram adequados. As análises ROC revelaram eficácia diagnóstica para todas as idades avaliadas. Os valores de corte são relatados. Dificuldades importantes na MMC foram observadas em crianças com PC, principalmente em crianças tetraplégicas. Além disso, MMC mostrou correlação moderada com o teste de inteligência e boa precisão na identificação das crianças com PC que possuem habilidades cognitivas prejudicadas. CONCLUSÃO: O MMC poderia ser útil como um instrumento de triagem multiprofissional para comprometimento cognitivo em crianças hemiplégicas. Os resultados de MMC em crianças tetraplégicas devem ser interpretados cuidadosamente. O diagnóstico deve ser confirmado por mais testes psicológicos.

A Síndrome de Williams (SW) é uma doença de etiologia genética, causada por microdeleções na região do cromossomo 7q11.23. Pacientes com SW podem apresentar QI verbal significativamente superior ao executivo, o que explicaria o déficit recorrente nas habilidades visoespaciais e a preservação de aspectos das habilidades verbais. O estudo teve como objetivo analisar o perfil cognitivo de uma série de casos de pacientes com SW, utilizando a Escala Wechsler de Inteligência. A amostra foi composta por dez pacientes. Os resultados apontam para um padrão de desempenho semelhante entre os participantes na escala verbal, porém o QI de execução teve maior correlação com o QI total. Foi observada uma discrepância entre habilidades verbais e não verbais em 40% dos participantes.


Williams Syndrome (WS) is a disease of genetic etiology caused by micro-deletions at the 7q11.23 region. WS's patients may present verbal IQ significantly higher than the performance one, which could explain the recurrent deficit in visuospatial abilities and the preservation of some verbal skills. The study aimed at analyzing the cognitive profile of a case series including WS patients, using Wechsler's Intelligence Scale. The sample was composed of ten patients. The results showed a similar pattern of performance among participants in the verbal scale; however, the performance IQ was more associated with total IQ. We observed a discrepancy between verbal and nonverbal abilities in 40% of the participants.

OBJECTIVE: The purpose of this study was to compare the effects of castration on cell death rate of the adult rat prostates and to evaluate the benefic action of alpha tocopherol supplementation to avoid apoptosis post-orchiectomy. MATERIAL AND METHODS: Thirty male Wistar rats weighing 250-300g were divided into three groups: group I - they were subjected to bilateral orchiectomy and sacrificed eight weeks after the procedure; group II - subjected to bilateral orchiectomy and alpha-tocopherol supplementation for four weeks preceding the procedure; and group III - subjected to bilateral orchiectomy and alpha-tocopherol supplementation for four weeks preceding the procedure and for eight weeks afterwards. At the end of the experiment, the prostatectomy was performed in all rats. The presence of oxidative stress was determined by assaying the blood level of 8-isoprostane and the occurrence of apoptosis was evaluated by identification of active caspase-3 through immunohistochemical analysis. RESULTS: The statistic analysis of active caspase-3 showed that in the long-term castrated group the detection was higher than in groups were the alpha-tocopherol was supplemented (p=0.007). Analysis of 8-isoprostane levels showed higher concentrations of reactive oxygen species in group I compared to other groups (p<0.05). Groups II and III presented active caspase-3 lower than in group I (p<0.05). CONCLUSION: Our exploratory analyses demonstrate a method to study the aging process and its influence on oxidative stress of prostatic tissue and cells death rate. Based on our results we can suggest that alpha tocopherol supplementation can decrease the apoptotic process as well as the oxidative stress levels induced by androgen deprivation of the prostate gland.
