RESUMO

Systolic heart failure is a problem of substantial magnitude worldwide. Over the last 25 years great progress has been made in the medical management of heart failure with the recognition of the benefits of beta-adrenergic blockade, modulation of the renin-angiotensin and mineralocorticoid axes and judicious diuretic therapy. In addition, cardiac resynchronization therapy and prophylactic implantation of cardiac defibrillators have been responsible for measurable benefits in terms of functional status and dysrhythmia-related mortality, respectively. Unfortunately, progressive cardiac dysfunction often results in activity limitation, symptoms at rest, hospital admission, end-organ dysfunction and death despite maximal implementation of standard therapies. Heart transplantation has been a dramatic and effective therapy for end-stage heart failure, but it remains limited by a shortage of donor organs, strict criteria defining acceptable recipients and often unsatisfactory long-term success. Mechanical alternatives to support the failing circulation have been sought for the last 50 years. The history of device development has been marked in general by the slow progress achieved by a few dedicated and persevering pioneers. In the past decade, however, evolving technology has dramatically changed the field and broadened the options for the treatment of advanced heart failure. This review will detail the important milestones and the current state of the art, with an emphasis on implantable devices for intermediate to long term support.

RESUMO

Innovation is defined as the introduction of something new, whether an idea, method or device. In this article, we describe the most important and innovative concepts and techniques that have advanced patient care within modern surgical subspecialties. We performed a systematic literature review and consulted academic subspecialty experts to evaluate recent changes in practice. The identified innovations included reduced blood loss and improved training in hepatobiliary surgery, total mesorectal excision and neoadjuvant therapies in colorectal surgery, prosthetic mesh in outpatient surgery, sentinel lymph node theory in surgical oncology, endovascular and wire-based skills in vascular and cardiovascular surgery, and the acceptance of abnormal anatomy through damage-control procedures in trauma and critical care. The common denominator among all subspecialties is an improvement in patient care manifested as a decrease in morbidity and mortality. Surgeons must continue to pursue innovative thinking, technological advances, improved training and systematic research.

Type B dissection has traditionally been managed medically if uncomplicated and surgically if associated with complications. This practice has resulted in most centers reporting significant morbidity and mortality if open repair is required. In the setting of malperfusion, operative repair has been conjoined with fenestration or visceral stenting to improve outcomes. Endovascular stent grafts seem to offer an attractive alternative in the acute complicated type B dissection, with reduced mortality and morbidity, particularly paralysis, compared with open repair. It is reasonable to consider endovascular stent grafts as another tool in managing dissection, but to recognize that open surgical repair still plays an important role, and that the data that define indications and outcomes are still emerging.

Blunt aortic injury (BAI) is a devastating consequence of high-energy trauma. The majority of its victims do not survive; those who do generally have significant associated injury. The standard treatment of BAI has been emergent replacement or repair of the damaged aorta via a posterolateral thoracotomy, with or without perfusion adjuncts. In addition to the substantial morbidity and mortality secondary to multisystem traumatic injuries, patients surviving to reach the operating room have been exposed to the risks related to their surgical treatment, namely death, paraplegia, hemorrhage, transfusion, organ dysfunction, prolonged intensive care unit stays, and extensive rehabilitation requirements. Contributions to the literature over the past several years have provided support for changing practice patterns in the management of BAI. Aggressive control of blood pressure has made it safe to delay high-risk interventions in patients with complex injuries. Advanced perfusion strategies using little or no anticoagulation appear to have positively affected bleeding complications and neurologic risk. Finally, endovascular stent grafting, though not yet rigorously evaluated in BAI, has been shown to be feasible and effective in the short term. This case presentation and literature review will examine treatment options and propose a management algorithm.

OBJECTIVE: Patients referred for implantable continuous-flow left ventricular assist devices (cfLVAD) frequently have preoperative right heart failure and tricuspid regurgitation (TR). The objective of this report is to examine early clinical benefits of concomitant tricuspid surgery for these patients. METHODS: Sixty-one of 200 consecutive cfLVAD patients at our institution displayed preimplant right heart dysfunction and significant TR. Thirty-three underwent cfLVAD plus a tricuspid valve procedure (TVP), and 28 had cfLVAD alone. Preimplant characteristics and clinical outcomes were retrospectively studied. As previously described, post-LVAD right ventricular failure was defined as need for right ventricular assist device (RVAD) support or greater than 14 days of intravenous inotropic support. RESULTS: Preimplant characteristics were similar between the 2 groups. Cardiopulmonary bypass time was increased for the group that received concomitant TVPs. The most common TVP consisted of an undersizing ring annuloplasty. The cfLVAD-alone group had greater TR after implant relative to the cfLVAD+TVP group. The cfLVAD-alone group experienced greater postprocedure right ventricular failure relative to cfLVAD+TVP (46.4% vs 18.2%; P < .05). Furthermore, prolonged hospitalization was increased for the cfLVAD-alone group versus the cfLVAD+TVP. Survival was similar between the 2 groups. CONCLUSIONS: Concomitant TVP appears to reduce postprocedure right ventricular failure for patients with significant TR undergoing cfLVAD implantation.

RESUMO

An unusual problem associated with the use of left ventricular assist devices (LVADs) relates to malposition of the apical inflow cannula. From 2005 to 2010, we implanted 154 continuous-flow HeartMate II (Thoratec, Pleasanton, CA) LVADs at our institution. In 4 separate instances, patients appeared to have malposition of the inflow cannula that resulted in serious symptoms. All 4 patients underwent surgery to reposition the cannula. Successful outcomes were achieved for 3 of the 4 patients.

RESUMO

Traumatic rupture of the descending thoracic aorta remains a leading cause of death following major blunt trauma. Management has evolved from uniformly performing emergent open repair with clamp and sew technique to include open repair with mechanical circulatory support, medical management and most recently, endovascular repair. This latter approach appears, in the short term, to be associated with perhaps better outcome, but long term data is still accruing. While an attractive option, there are specific anatomic and physiologic factors to be considered in each individual case.
