To investigate the reliability of the ballistic hip thrust (BHT) test performed on force plates as a diagnostic tool to monitor posterior chain neuromuscular status and compare its usefulness with the counter movement jump (CMJ). Twenty-two male football players from an elite Under-19 French Ligue 1 football club (16.4 ± 0.6 years) performed two assessments; Assessment 1: Two testing sessions separated by one week were performed to assess the reliability of the test. Participants performed a 3-set workout of 4 repetitions of the BHTs each session. Intra-day (between set) and inter-day (between testing day) reliability of concentric mean force, takeoff peak force and peak power were assessed. Assessment 2: Participants performed a 1-set workout of 3 repetitions of the CMJ and 4 repetitions of the BHT tests pre- and post-training to compare the usefulness of both tests. Concentric mean force and takeoff peak force showed small-to-moderate standardised typical errors (TE: 0.2-0.7) for inter- and intra-day reliability. Inter-day concentric mean force showed a coefficient of variation (CV) of 7.2%. Intra-day concentric mean force and take-off peak force showed a CV of 4.2% and 5.5%. BHT and CMJ showed similar moderate levels of usefulness. The BHT test showed moderate levels of reliability and usefulness. This test could be a useful addition to a testing battery to monitor posterior chain neuromuscular status.

Sunlight disinfection is important for treatment of wastewater within maturation ponds. This study analyses the movement of Escherichi coli within a slice of a maturation pond, being affected by stratification, sunlight attenuation and mixing driven by wind shear and natural convection using computational fluid dynamics (CFD). Since the exposure to ultraviolet light is most effective in the near-surface region of the pond, natural convective mixing mechanisms to transport the pathogens from the lower parts of the pond are critical for disinfection efficacy. Different turbulence models are considered for closure of the momentum conservation equations and compared with a laminar flow simulation and a completely stirred tank reactor (CSTR) model. The effect of turbulence and stratification is shown to be significant for thermal and velocity distributions, and predictions of E. coli die-off. Greater volume-averaged E. coli die-off was predicted by the computationally convenient CSTR model than the CFD turbulence and laminar models. The simulation results are compared with experimental data and show that complete vertical mixing occurs in a diurnal pattern aiding die-off in sunlight-attenuating water. Practical applications of the model can assist in management strategies for maturation ponds such as off-take locations/times and evaluating seasonal variations in sunlight disinfection.

To compare relative match intensities of sea-level versus high-altitude native soccer players during a 2-week camp at 3600 m, data from 7 sea-level (Australian U17 National team, AUS) and 6 high-altitude (a Bolivian U18 team, BOL) native soccer players were analysed. Two matches were played at sea-level and three at 3600 m on Days 1, 6 and 13. The Yo-Yo Intermittent recovery test (vYo-YoIR1) was performed at sea-level, and on Days 3 and 10. Match activity profiles were measured via 10-Hz GPS. Distance covered >14.4 km.h(-1) (D>14.4 km·h(-1)) and >80% of vYo-YoIR1 (D>80%vYo-YoIR1) were examined. Upon arrival at altitude, there was a greater decrement in vYo-YoIR1 (Cohen's d +1.0, 90%CL ± 0.8) and D>14.4 km·h(-1) (+0.5 ± 0.8) in AUS. D>14.4 km.h(-1) was similarly reduced relative to vYo-YoIR1 in both groups, so that D>80%vYo-YoIR1 remained similarly unchanged (-0.1 ± 0.8). Throughout the altitude sojourn, vYo-YoIR1 and D>14.4 km·h(-1) increased in parallel in AUS, so that D>80%vYo-YoIR1 remained stable in AUS (+6.0%/match, 90%CL ± 6.7); conversely D>80%vYo-YoIR1 decreased largely in BOL (-12.2%/match ± 6.2). In sea-level natives competing at high-altitude, changes in match running performance likely follow those in high-intensity running performance. Bolivian data confirm that increases in 'fitness' do not necessarily translate into greater match running performance, but rather in reduced relative exercise intensity. Key pointsWhen playing at high-altitude, players may alter their activities during matches in relation to their transient maximal physical capacities, possibly to maintain a 'tolerable' relative exercise intensity.While there is no doubt that running performance per se in not the main determinant of match outcomes (Carling, 2013), fitness levels influence relative match intensity (Buchheit et al., 2012, Mendez-Villanueva et al., 2013), which in-turn may impact on decision making and skill performance (Rampinini et al., 2008).In the context of high-altitude competitions, it is therefore recommended to arrive early enough (i.e., ~2 weeks) to allow (at least partial) acclimatisation, and in turn, allow sea-level native players to regulate their running activities in relation to both actual game demands and relative match intensity.

RESUMO

A role for vitamin D in immune modulation and in cancer has been suggested. In this work, we report that mice with increased availability of vitamin D display greater immune-dependent resistance to transplantable cancers and augmented responses to checkpoint blockade immunotherapies. Similarly, in humans, vitamin D-induced genes correlate with improved responses to immune checkpoint inhibitor treatment as well as with immunity to cancer and increased overall survival. In mice, resistance is attributable to the activity of vitamin D on intestinal epithelial cells, which alters microbiome composition in favor of Bacteroides fragilis, which positively regulates cancer immunity. Our findings indicate a previously unappreciated connection between vitamin D, microbial commensal communities, and immune responses to cancer. Collectively, they highlight vitamin D levels as a potential determinant of cancer immunity and immunotherapy success.

OBJECTIVES: To examine the time course of changes in wellness and health status markers before and after episodes of sickness in young soccer players during a high-altitude training camp (La Paz, 3600 m). METHODS: Wellness and fatigue were assessed daily on awakening using specifically-designed questionnaires and resting measures of heart rate and heart rate variability. The rating of perceived exertion and heart rate responses to a submaximal run (9 km/h) were also collected during each training session. Players who missed the morning screening for at least two consecutive days were considered as sick. RESULTS: Four players met the inclusion criteria. With the exception of submaximal exercise heart rate, which showed an almost certain and large increase before the day of sickness (4%; 90% confidence interval 3 to 6), there was no clear change in any of the other psychometric or physiological variables. There was a very likely moderate increase (79%, 22 to 64) in self-reported training load the day before the heart rate increase in sick players (4 of the 4 players, 100%). In contrast, training load was likely and slightly decreased (-24%, -78 to -11) in players who also showed an increased heart rate but remained healthy. CONCLUSIONS: A >4% increased heart rate during submaximal exercise in response to a moderate increase in perceived training load the previous day may be an indicator of sickness the next day. All other variables, that is, resting heart rate, heart rate variability and psychometric questionnaires may be less powerful at predicting sickness.

OBJECTIVES: To examine the time course of wellness, fatigue and performance during an altitude training camp (La Paz, 3600 m) in two groups of either sea-level (Australian) or altitude (Bolivian) native young soccer players. METHODS: Wellness and fatigue were assessed using questionnaires and resting heart rate (HR) and HR variability. Physical performance was assessed using HR responses to a submaximal run, a Yo-Yo Intermittent recovery test level 1 (Yo-YoIR1) and a 20 m sprint. Most measures were performed daily, with the exception of Yo-YoIR1 and 20 m sprints, which were performed near sea level and on days 3 and 10 at altitude. RESULTS: Compared with near sea level, Australians had moderate-to-large impairments in wellness and Yo-YoIR1 relative to the Bolivians on arrival at altitude. The acclimatisation of most measures to altitude was substantially slower in Australians than Bolivians, with only Bolivians reaching near sea-level baseline high-intensity running by the end of the camp. Both teams had moderately impaired 20 m sprinting at the end of the camp. Exercise HR had large associations (r>0.5-0.7) with changes in Yo-YoIR1 in both groups. CONCLUSIONS: Despite partial physiological and perceptual acclimatisation, 2 weeks is insufficient for restoration of physical performance in young sea-level native soccer players. Because of the possible decrement in 20 m sprint time, a greater emphasis on speed training may be required during and after altitude training. The specific time course of restoration for each variable suggests that they measure different aspects of acclimatisation to 3600 m; they should therefore be used in combination to assess adaptation to altitude.

OBJECTIVES: We investigated the effect of high altitude on the match activity profile of elite youth high altitude and sea level residents. METHODS: Twenty Sea Level (Australian) and 19 Altitude-resident (Bolivian) soccer players played five games, two near sea level (430 m) and three in La Paz (3600 m). Match activity profile was quantified via global positioning system with the peak 5 min period for distance ((D5(peak)) and high velocity running (>4.17 m/s, HIVR5(peak)); as well as the 5 min period immediately subsequent to the peak for both distance (D5(sub)) and high-velocity running (HIVR5(sub)) identified using a rolling 5 min epoch. The games at 3600 m were compared with the average of the two near sea-level games. RESULTS: The total distance per minute was reduced by a small magnitude in the first match at altitude in both teams, without any change in low-velocity running. There were variable changes in HiVR, D5(peak) and HiVR5(peak) from match to match for each team. There were within-team reductions in D5(peak) in each game at altitude compared with those at near sea level, and this reduction was greater by a small magnitude in Australians than Bolivians in game 4. The effect of altitude on HiVR5(peak) was moderately lower in Australians compared with Bolivians in game 3. There was no clear difference in the effect of altitude on maximal accelerations between teams. CONCLUSIONS: High altitude reduces the distance covered by elite youth soccer players during matches. Neither 13 days of acclimatisation nor lifelong residence at high altitude protects against detrimental effects of altitude on match activity profile.

BACKGROUND: Altitude exposure causes acute sleep disruption in non-athletes, but little is known about its effects in elite athletes. The aim of this study was to examine the effects of altitude on two groups of elite athletes, that is, sea-level natives and high-altitude natives. METHODS: Sea-level natives were members of the Australian under-17 soccer team (n=14). High-altitude natives were members of a Bolivian under-20 club team (n=12). Teams participated in an 18-day (19 nights) training camp in Bolivia, with 6 nights at near sea level in Santa Cruz (430 m) and 13 nights at high altitude in La Paz (3600 m). Sleep was assessed on every day/night using activity monitors. RESULTS: The Australians' sleep was shorter, and of poorer quality, on the first night at altitude compared with sea level. Sleep quality returned to normal by the end of the first week at altitude, but sleep quantity had still not stabilised at its normal level after 2 weeks. The quantity and quality of sleep obtained by the Bolivians was similar, or greater, on all nights at altitude compared with sea level. The Australians tended to obtain more sleep than the Bolivians at sea level and altitude, but the quality of the Bolivians' sleep tended to be better than that of the Australians at altitude. CONCLUSIONS: Exposure to high altitude causes acute and chronic disruption to the sleep of elite athletes who are sea-level natives, but it does not affect the sleep of elite athletes who are high-altitude natives.

BACKGROUND: We describe here the 3-year process underpinning a multinational collaboration to investigate soccer played at high altitude--La Paz, Bolivia (3600 m). There were two main aims: first, to quantify the extent to which running performance would be altered at 3600 m compared with near sea level; and second, to characterise the time course of acclimatisation of running performance and underlying physiology to training and playing at 3600 m. In addition, this project was able to measure the physiological changes and the effect on running performance of altitude-adapted soccer players from 3600 m playing at low altitude. METHODS: A U20 Bolivian team ('The Strongest' from La Paz, n=19) played a series of five games against a U17 team from sea level in Australia (The Joeys, n=20). 2 games were played near sea level (Santa Cruz 430 m) over 5 days and then three games were played in La Paz over the next 12 days. Measures were (1) game and training running performance--including global positioning system (GPS) data on distance travelled and velocity of movement; (2) blood--including haemoglobin mass, blood volume, blood gases and acid-base status; (3) acclimatisation--including resting heart rate variability, perceived altitude sickness, as well as heart rate and perceived exertion responses to a submaximal running test; and (4) sleep patterns. CONCLUSIONS: Pivotal to the success of the project were the strong professional networks of the collaborators, with most exceeding 10 years, the links of several of the researchers to soccer federations, as well as the interest and support of the two head coaches.

BACKGROUND: Altitude training is used by elite athletes to improve sports performance, but it may also disrupt sleep. The aim of this study was to examine the effects of 2 weeks at high altitude on the sleep of young elite athletes. METHODS: Participants (n=10) were members of the Australian under-17 soccer team on an 18-day (19-night) training camp in Bolivia, with six nights at near sea level in Santa Cruz (430 m) and 13 nights at high altitude in La Paz (3600 m). Sleep was monitored using polysomnography during a baseline night at 430 m and three nights at 3600 m (immediately after ascent, 1 week after ascent and 2 weeks after ascent). Data were analysed using effect size statistics. RESULTS: All results are reported as comparisons with baseline. Rapid eye movement (REM) sleep was likely lower immediately upon ascent to altitude, possibly lower after 1 week and similar after 2 weeks. On all three nights at altitude, hypopneas and desaturations were almost certainly higher; oxygen saturation was almost certainly lower; and central apnoeas, respiratory arousals and periodic breathing were very likely higher. The effects on REM sleep were common to all but one participant, but the effects on breathing were specific to only half the participants. CONCLUSIONS: The immediate effects of terrestrial altitude of 3600 m are to reduce the amount of REM sleep obtained by young elite athletes, and to cause 50% of them to have impaired breathing during sleep. REM sleep returns to normal after 2 weeks at altitude, but impaired breathing does not improve.

BACKGROUND: There is emerging evidence of improved outcomes with induction of labour for pregnancies in which the baby is thought to be large. This trial identifies scan accuracy and the effect of intervention for pregnancies complicated by suspected large for gestational age (LGA) on customized chart outside an academic center. METHODS: This is a retrospective cohort study of 3 groups of induced pregnancies; women with a suspected LGA fetus, women with diabetes (DM) and a control group (C) of women that underwent induction of labour on or after 280 days gestation. Data collection and analysis were prespecified. Scan accuracy and outcomes between the cohorts were compared. RESULTS: Over 1 year there were 845 cases: LGA (128), DM (116) and control cases (601). Mean birthweights differed significantly. PPV of EFW for birthweight >90th centile on GROW chart, WHO chart, and >4 kg was 0.35-0.40. Projected birthweight of >4 kg significantly better predicted itself (AUROC 0.70, 0.74 and 0.80). Mean scan error was -5.2% and +15.6% for DM and LGA. Shoulder dystocia and neonatal morbidity were not increased in LGA despite the significant increase in AVD 28/128, 21.9% vs. 99/601, 16.5%, aOR 2.20 (1.07-4.5). SVD was significantly less likely LGA vs. C at 69/128, 53.9% vs. 413/601, 68.7% aOR 0.38 (95% CI: 0.21-0.70). CONCLUSIONS: Third trimester EFW for bigger babies was poorly predictive of macrosomia. Fetal outcomes were good but women selected and induced as LGA had higher rates of hemorrhage and intervention.

OBJECTIVE: Current practice to diagnose pancreatic cancer is accomplished by endoscopic ultrasound guided fine needle aspiration (EUS-FNA) using a cytological approach. This method is time consuming and often fails to provide suitable specimens for modern molecular analyses. Here, we compare the cytological approach with direct formalin fixation of pancreatic EUS-FNA micro-cores and evaluate the potential to perform molecular biomarker analysis on these specimen. METHODS: 130 specimens obtained by EUS-FNA with a 22G needle were processed by the standard cytological approach and compared to a separate cohort of 130 specimens that were immediately formalin fixed to preserve micro-cores of tissue prior to routine histological processing. RESULTS: We found that direct formalin fixation significantly shortened the time required for diagnosis from 3.6 days to 2.9 days (p<0.05) by reducing the average time (140 vs 33 min/case) and number of slides (9.65 vs 4.67 slides/case) for histopathological processing. Specificity and sensitivity yielded comparable results between the two approaches (82.3% vs 77% and 90.9% vs 100%). Importantly, EUS-FNA histology preserved the tumour tissue architecture with neoplastic glands embedded in stroma in 67.89% of diagnostic cases compared to 27.55% with the standard cytological approach (p < 0.001). Furthermore, micro-core samples were suitable for molecular studies including the immunohistochemical detection of intranuclear Hes1 in malignant cells, and the laser-capture microdissection-mediated measurement of Gli-1 mRNA in tumour stromal myofibroblasts. CONCLUSIONS: Direct formalin fixation of pancreatic EUS-FNA micro-cores demonstrates superiority regarding diagnostic delay, costs, and specimen suitability for molecular studies. We advocate this approach for future investigational trials in pancreatic cancer patients.

A low digit ratio (2D:4D) and low 2D:4D in the right compared with the left hand (right-left 2D:4D) are thought to be determined by high in utero concentrations of testosterone, and are related to "masculine" traits such as aggression and performance in sports like running and rugby. Low right-left 2D:4D is also related to sensitivity to testosterone as measured by the number of cytosine-adenine-guanine triplet repeats in exon 1 of the androgen receptor gene. Here we show that low right-left 2D:4D is associated with high maximal oxygen uptake (VO2(max)), high velocity at VO2(max), and high maximum lactate concentration in a sample of teenage boys. We suggest that low right-left 2D:4D is linked to performance in some sports because it is a proxy of high sensitivity to prenatal and maybe also circulating testosterone and high VO2(max).

The aim of this study was to examine the effect of age and spa treatment (i.e. combined sauna, cold water immersion, and jacuzzi) on match running performance over two consecutive matches in highly trained young soccer players. Fifteen pre- (age 12.8 ± 0.6 years) and 13 post- (15.9 ± 1 y) peak height velocity (PHV) players played two matches (Matches 1 and 2) within 48 h against the same opposition, with no specific between-match recovery intervention (control). Five post-PHV players also completed another set of two consecutive matches, with spa treatment implemented after the first match. Match running performance was assessed using a global positioning system with very-high-intensity running (> 16.1-19.0 km · h(-1)), sprinting distance (>19 km · h(-1)), and peak match speed determined. Match 2 very-high-intensity running was "possibly" impaired in post-PHV players (-9 ± 33%; ± 90% confidence limits), whereas it was "very likely" improved for the pre-PHV players (+27 ± 22%). The spa treatment had a beneficial impact on Match 2 running performance, with a "likely" rating for sprinting distance (+30 ± 67%) and "almost certain" for peak match speed (+6.4 ± 3%). The results suggest that spa treatment is an effective recovery intervention for post-PHV players, while its value in pre-PHV players is questionable.

The aim of this study was to examine the impact of maximal sprinting speed (MSS) on the peak speed attained during soccer matches. Time-motion analysis of running activity was collected from 14 highly trained young male outfield footballers (8 wide midfielders [WM] and 6 central defenders [CD], 173.2 ± 0.06 m, 60.8 ± 8.1 kg, 16.7 ± 0.7 years) during 14 different friendly international club level matches. The 2 fastest players (a WM and a CD) were compared with the slower players who played in the same position. Each player's MSS was determined using the fastest 10-m split time during an electronically timed 40-m sprint. Game speed was recorded via portable global positioning systems. Faster players reached higher absolute peak running speeds in games than did their slower counterparts regardless of the playing position, with large to very large effect sizes and qualitative indications of "almost certain" and "very likely" positive effects associated with being fast. None of the players reached their MSS during the matches; however, the fastest CD attained a lower percentage of his MSS compared to both, his slower CD counterparts and the fastest WM. Given the higher peak speeds reached in games by the fastest players, and the fact that all players (irrespective of their MSS) used a high percentage of their MSS, these preliminary results provide direct support to the hypothesis that MSS can impact on what a player can do in actual playing conditions. Our results also indicate that playing position has an important role in influencing the expression of MSS.

Aim: To quantify the occurrence of near-to-maximal speed-running bouts in elite soccer players.Methods: Tracking match data from 35 professional players (23 ± 3 yrs) were analysed over four seasons (2015-2019). The number of runs reaching >80, >85 and >90% of players' individual maximal sprinting speed (MSS) was computed. Differences in peak speed occurrences between halves and positions were analysed.Results: Occurrences were not different between halves, but slightly-to-moderately position-dependent, with wingers (3.5 ± 2.0 runs >80% per half) and central midfielders (1.6 ± 1.5 runs >80% per half) performing the most and the least number of near-to-maximal speed-running bouts. Players did not reach >90% of MSS at all in 35% (Attackers) to 65% (Midfielders) of their matches; they reached ≥3 times >90% MSS per match in 2% (Midfielders) to 11% (Attackers) of their matches only. The maximal number of cumulated match occurrences during congested periods was observed in acentral defender (2 runs >90% of MSS every 4 days for 31 days) and a winger (1 run every 4 days for 52 days).Conclusions: The occurrence of near-to-maximal speed-running bouts is low in elite soccer. Match context, playing position and individual player profile are the main determinants of those occurrences.

PURPOSE: To compare between-tests changes in submaximal exercise heart rate (HRex, 3 min, 12 km/h) and the speed associated with 4 mmol/L of blood lactate (V4mmol) in soccer players to get insight into their level of agreement and respective sensitivity to changes in players' fitness. METHODS: A total of 19 elite professional players (23 [3] y) performed 2 to 3 graded incremental treadmill tests (3-min stages interspersed with 1 min of passive recovery, starting speed 8 km/h, increment 2 km/h until exhaustion or 18 km/h if exhaustion was not reached before) over 1.5 seasons. The correlation between the changes in HRex and V4mmol was examined. Individual changes in the 2 variables were compared (>2 × typical error considered "clear"). RESULTS: The changes in HRex and V4mmol were largely correlated (r = .82; 90% confidence interval, .65-.91). In more than 90% of the cases, when a clear individual change in HRex was observed, it was associated with a similar clear change in V4mmol (the same direction, improvement, or impairment of fitness) and conversely. CONCLUSIONS: When it comes to testing players submaximally, the present results suggest that practitioners can use HRex or V4mmol interchangeably with confidence. However, in comparison with a field-based standardized warm-up run (3-4 min, all players together), the value of a multistage incremental test with repeated blood lactate samplings is questionable for a monitoring purpose given its time, labor, cost, and poorer player buy-in.

AIM: To compare the effect of low versus high volume of eccentric-biased hamstring training programs on knee-flexor strength and fascicle length changes in elite soccer players. METHODS: A total of 19 elite youth soccer players took part in this study and were randomly assigned into 2 subgroups. For 6 weeks in-season, the groups performed either a low-volume (1 set per exercise; 10 repetitions in total) or a high-volume (4 sets; 40 repetitions) eccentric training of their knee flexors. After 6-weeks midtraining (MID), players performed the alternate training regimen. Each training set consisted of 4 repetitions of the Nordic hamstring exercise and 6 repetitions of the bilateral stiff-leg deadlift. Eccentric knee-flexor strength (NordBord) as well as biceps femoris long head and semimembranosus fascicle length (scanned with ultrasound scanner) were assessed during pretraining (PRE), MID, and posttraining (POST) tests. RESULTS: Knee-flexor eccentric strength very likely increased from PRE to MID (low volume: +11.3% [7.8%] and high volume: 11.4% [5.3%]), with a possibly-to-likely increase in biceps femoris long head (+4.5% [5.0%] and 4.8% [2.5%]) and semimembranosus (+4.3% [4.7%] and 6.3% [6.3%]) fascicle length in both groups. There was no substantial changes between MID and POST. Overall, there was no clear between-group difference in the changes from PRE to MID and MID to POST for neither knee-flexor eccentric strength, biceps femoris long head, nor semimembranosus fascicle length. CONCLUSIONS: Low-volume knee-flexor eccentric training is as effective as a greater training dose to substantially improve knee-flexor strength and fascicle length in-season in young elite soccer players. Low volume is, however, likely more appropriate to be used in an elite team facing congested schedules.
