BACKGROUND: Pharmacoinvasive strategy is an effective myocardial reperfusion therapy when primary percutaneous coronary intervention (p-PCI) cannot be performed in a timely manner. METHODS: Authors sought to evaluate metrics of care and cardiovascular outcomes in a decade-long registry of a pharmacoinvasive strategy network for the treatment of ST-elevation myocardial infarction (STEMI). Data from a local network including patients undergoing fibrinolysis in county hospitals and systematically transferred to the tertiary center were accessed from March 2010 to September 2020. Numerical variables were described as median and interquartile range. Area under the curve (AUC-ROC) was used to analyze the predictive value of TIMI and GRACE scores for in-hospital mortality. RESULTS: A total of 2,710 consecutive STEMI patients aged 59 [51-66] years, 815 women (30.1%) and 837 individuals with diabetes (30.9%) were analyzed. The time from symptom onset to first-medical-contact was 120 [60-210] minutes and the door-to-needle time was 70 [43-115] minutes. Rescue-PCI was required in 929 patients (34.3%), in whom the fibrinolytic-catheterization time was 7.2 [4.9-11.8] hours, compared to 15.7 [6.8-22,7] hours in those who had successful lytic reperfusion. All cause in-hospital mortality occurred in 151 (5.6%) patients, reinfarction in 47 (1.7%) and ischemic stroke in 33 (1.2%). Major bleeding occurred in 73 (2.7%) patients, including 19 (0.7%) cases of intracranial bleeding. C-statistic confirmed that both scores had high predictive values for in-hospital mortality, demonstrated by TIMI AUC-ROC of 0.80 [0,77-0.84] and GRACE AUC-ROC of 0.86 [0.83-0.89]. CONCLUSION: In a real world registry of a decade-long network for the treatment of ST-elevation myocardial infarction based on the pharmacoinvasive strategy, low rates of in-hospital mortality and cardiovascular outcomes were observed, despite prolonged time metrics for both fibrinolytic therapy and rescue-PCI. Pharmacoinvasive treatment is an acceptable alternative for patients with ST-segment elevation myocardial infarction (STEMI) in developing countries. The present study evaluated the influence of gender on the risks of death and major adverse cardiovascular events (MACE) in this population. Seven municipal emergency rooms and the Emergency Mobile Healthcare Service in São Paulo treated STEMI patients with tenecteplase. The patients were subsequently transferred to a tertiary teaching hospital for early (<24 h) coronary angiography. A total of 469 patients were evaluated [329 men (70.1%)]. Compared to men, women had more advanced age (60.2 ± 12.3 vs. 56.5 ± 11 years; p = 0.002); lower body mass index (BMI; 25.85 ± 5.07 vs. 27.04 ± 4.26 kg/m2; p = 0.009); higher rates of hypertension (70.7 vs. 59.3%, p = 0.02); higher incidence of hypothyroidism (20.0 vs. 5.5%; p < 0.001), chronic renal failure (10.0 vs. 8.8%; p = 0.68), peripheral vascular disease (PVD; 19.3 vs. 4.3%; p = 0.03), and previous history of stroke (6.4 vs. 1.3%; p = 0.13); and higher thrombolysis in myocardial infarction risk scores (40.0 vs. 23.7%; p < 0.001). The overall in-hospital mortality and MACE rates for women versus men were 9.3 versus 4.9% (p = 0.07) and 12.9 versus 7.9% (p = 0.09), respectively. By multivariate analysis, diabetes (OR 4.15; 95% CI 1.86-9.25; p = 0.001), previous stroke (OR 4.81; 95% CI 1.49-15.52; p = 0.009), and hypothyroidism (OR 3.75; 95% CI 1.44-9.81; p = 0.007), were independent predictors of mortality, whereas diabetes (OR 2.05; 95% CI 1.03-4.06; p = 0.04), PVD (OR 2.38; 95% CI 0.88-6.43; p = 0.08), were predictors of MACE. In STEMI patients undergoing pharmacoinvasive strategy, mortality and MACE rates were twice as high in women; however, this was due to a higher prevalence of risk factors and not gender itself.

However, the characterization of ischemic symptoms is unclear, especially in subgroups such as women and the elderly. OBJECTIVES: To analyze the typification of ischemic symptoms, temporal metrics and observe the occurrence of in-hospital outcomes, in the analysis of predictive scores, in patients with STEMI, in a drug-invasive strategy. METHODS: Study involving 2,290 patients. Types of predefined clinical presentations: typical pain, atypical pain, dyspnea, syncope. We measured the time between the onset of symptoms and demand for care and the interval between arrival at the medical unit and thrombolysis. Odds-ratios (OR; CI-95%) were estimated in a regression model. ROC curves were constructed for mortality predictors. The adopted significance level (alpha) was 5%. RESULTS: Women had a high prevalence of atypical symptoms; longer time between the onset of symptoms and seeking care; delay between arrival at the emergency room and fibrinolysis. Hospital mortality was 5.6%. Risk prediction by Killip-Kimball classification: AUC: [0.77 (0.73-0.81)] in class ≥II. Subgroups studied [OR (CI-95%)]: women [2.06 (1.42-2.99); p=0.01]; chronic renal failure [3.39 (2.13-5.42); p<0.001]; elderly [2.09 (1.37-3.19) p<0.001]; diabetics [1.55 (1.04-2.29); p=0.02]; obese 1.56 [(1.01-2.40); p=0.04]: previous stroke [2.01 (1.02-3.96); p=0.04] correlated with higher mortality rates. CONCLUSION: Despite higher mortality rates in some subgroups, significant disparity persists in women, with delays in symptom recognition and prompt thrombolysis. We highlight the applicability of the Killip-Kimball score in prediction, regardless of the clinical presentation.

BACKGROUND: Primary percutaneous coronary intervention is considered the "gold standard" for coronary reperfusion. However, when not available, the drug-invasive strategy is an alternative method and the electrocardiogram (ECG) has been used to identify reperfusion success. OBJECTIVES: Our study aimed to assess ST-Segment changes in post-thrombolysis and their power to predict recanalization and using the angiographic scores TIMI-flow and Myocardial Blush Grade (MBG) as an ideal reperfusion criterion. METHODS: 2,215 patients with ST-Segment Elevation Myocardial Infarction (STEMI) undergoing fibrinolysis [(Tenecteplase)-TNK] and referred to coronary angiography within 24 h post-fibrinolysis or immediately referred to rescue therapy were studied. The ECG was performed pre- and 60 min-post-TNK. The patients were categorized into 2 groups: those with ideal reperfusion (TIMI-3 and MBG-3) and those with inadequate reperfusion (TIMI and MBG <3). The ECG reperfusion criterion was defined by the reduction of the ST-Segment >50%. A p-value <0.05 was considered for the analyses, with bicaudal tests. RESULTS: The ECG reperfusion criterion showed a positive predictive value of 56%; negative predictive value of 66%; sensitivity of 79%; and specificity of 40%. There was a weak positive correlation between ST-Segment reduction and ideal reperfusion angiographic data (r = 0.21; p <0.001) and low diagnostic accuracy, with an AUC of 0.60 (95%CI: 0.57-0.62). CONCLUSION: The ST-Segment reduction was not able to accurately identify patients with adequate angiographic reperfusion. Therefore, even patients with apparently successful reperfusion should be referred to angiography soon, to ensure adequate macro and microvascular coronary flow.

OBJECTIVE: To determine the prevalence of hyperhomocystinemia in patients with acute ischemic syndrome of the unstable angina type. METHODS: We prospectively studied 46 patients (24 females) with unstable angina and 46 control patients (19 males), paired by sex and age, blinded to the laboratory data. Details of diets, smoking habits, medication used, body mass index, and the presence of hypertension and diabetes were recorded, as were plasma lipid and glucose levels, C-reactive protein, and lipoperoxidation in all participants. Patients with renal disease were excluded. Plasma homocysteine was estimated using high-pressure liquid chromatography. RESULTS: Plasma homocysteine levels were significantly higher in the group of patients with unstable angina (12.7+/-6.7 micromol/L) than in the control group (8.7+/-4.4 micromol/L) (p<0.05). Among males, homocystinemia was higher in the group with unstable angina than in the control group, but this difference was not statistically significant (14.1+/-5.9 micromol/L versus 11.9+/-4.2 micromol/L). Among females, however, a statistically significant difference was observed between the 2 groups: 11.0+/-7.4 micromol/L versus 6.4+/-2.9 micromol/L (p<0.05) in the unstable angina and control groups, respectively. Approximately 24% of the patients had unstable angina at homocysteine levels above 15 micromol/L. CONCLUSION: High homocysteine levels seem to be a relevant prevalent factor in the population with unstable angina, particularly among females.

Os fibrinolíticos pertencem a uma classe de medicamentos especializada em promover a lise da fibrina e a consequente dissolução do trombo. Esse efeito baseia-se na transformação do plasminogênio em plasmina, potente enzima proteolítica. A sua aplicação nas diferentes síndromes cardiovasculares agudas alterou o curso natural do infarto agudo do miocárdio, da embolia pulmonar e do acidente vascular cerebral isquêmico agudo. Na prática, temos três gerações de fibrinolíticos disponíveis: estreptoquinase, alteplase e tenecteplase, essa última com alta afinidade à fibrina. As complicações hemorrágicas, embora raras, devem ser identificadas e tratadas de forma precoce. Isso se deve aos distúrbios da coagulação, especialmente, queda do fibrinogênio e outros fatores de coagulação, mais predominantes com a estreptoquinase. A correção baseia-se na administração de crioprecipitado e plasma fresco, ricos, respectivamente, em fibrinogênio e fatores de coagulação. A inexperiência ao administrar tais medicamentos e o receio hemorrágico tem sido fator agravante na sobrevida dessas doenças. Campanhas educacionais poderão amenizar tal cenário, especialmente, em centros mais carentes de recursos tecnológicos
Fibrinolytics belong to a class of drugs specialized in promoting lysis of fibrin and consequent dissolution of the thrombus. This effect is based on the conversion of plasminogen into plasmin, a potent proteolytic enzyme. Its application in different acute cardiovascular syndromes has changed the natural course of acute myocardial infarction, pulmonary embolism, and acute ischemic stroke. In practice, three generations of fibrinolytics are available: streptokinase, alteplase, and tenecteplase (the latter having high affinity to fibrin). Hemorrhagic complications, although rare, must be identified and treated early. This is due to coagulation disorders, especially the decrease in fibrinogen and other coagulation factors, more prevalent with streptokinase. Correction is based on the administration of cryoprecipitate and fresh plasma, which are rich in fibrinogen and coagulation factors, respectively. Inexperience in administering these drugs and hemorrhagic fear are aggravating factors of disease progression. Educational programs can mitigate such scenarios, especially in centers that lack technological resources.Inexperience administering such medicines and hemorrhagic fear has been an aggravating factor in the lives of these diseases. Educational campaigns will be able to mitigate such a scenario especially in the most lacking centers of technological resources

As doenças cardiovasculares, especialmente, o infarto agudo do miocárdio, são responsáveis pela primeira causa de óbito em nosso país. Baseando-se em sua fisiopatogenia e nos fatores de risco envolvidos, a taxa de mortalidade, em 30 dias, varia de 3-5% em centros avançados e quase 30% naqueles cujo atendimento não aplicam as diretrizes recomendadas. Tal mudança dependerá de um aprimoramento organizacional com ações educativas para o paciente, profissionais do setor de emergência e plena sintonia com os gestores do sistema de saúde público ou privado. A angioplastia primária é o tratamento padrão-ouro da reperfusão miocárdica, porém, disponível apenas em 15% dos hospitais. Em geral, o setor público carece dessa disponibilidade, sendo a estratégia fármaco-invasivo-terapia fibrinolítica seguida da transferência precoce para angioplastia a recomendada. O diagnóstico baseia-se em critérios eletrocardiográficos, porém, nem sempre há médicos habilitados para tal confirmação. Isso retarda o tratamento e compromete bastante os resultados. A disponibilidade da telemedicina ou teleECG permite que profissionais mesmo à distância possam corroborar com o diagnóstico, orientar o tratamento e obter métricas de qualidade e mortalidade. A rápida identificação do paciente com sinais e sintomas de infarto agudo, diagnóstico imediato no ECG e administração do fibrinolítico não devem ultrapassar 20 minutos. São minutos dependentes da prévia organização que salvam vidas! O tratamento fibrinolítico é a realidade para um país com dimensões continentais e sérias limitações logísticas de transporte. Cardiovascular events, especially acute myocardial infarction, are the main cause of death in our country. In addition to its physiopathogenesis and the involved risk factors, the 30-day mortality rates vary from 3% to 5% in advanced centers and 30% in those where care does not apply the recommended guidelines. Such change will depend on organizational improvement, as well as patient education, professionals in the emergency department, and harmony with agents in the public or private health system. Primary angioplasty is the gold standard treatment for myocardial reperfusion, but is only available in 15% of hospitals. If it is not available in the public sector, a pharmacoinvasive strategy fibrinolytic therapy followed by patient transfer for angioplasty has been recommended. Diagnosis is based on ECG criteria, but there is a shortage of physicians qualified for such confirmation. This delays the treatment and compromises the results. Telemedicine or teleECG allows distant professionals to corroborate with the diagnosis, guide the treatment, and obtain quality and mortality metrics. The rapid identification of patients with signs and symptoms of acute myocardial infarction, immediate diagnosis on ECG, and fibrinolytic administration should not exceed 20 min. They are life-saving minutes dependent on prior organization. Fibrinoly-tic treatment is the reality for a country with continental dimensions and transport logistic limitations. It is the responsibility of health managers to make them available to the care of patients with infarction

BACKGROUND: The relationship between renal dysfunction and mortality after myocardial infarction (MI) has been demonstrated in patients with reduced ejection fraction. The importance of diastolic dysfunction in this scenario is unknown. METHODS: We studied 749 patients with acute MI who were evaluated within 24 hours of symptom onset. The Modification of Diet in Renal Disease equation was used to calculate the estimated glomerular filtration rate (eGFR). Preserved and depressed renal functions were defined as eGFR >60 and <60 mL/min, respectively. Diastolic function was determined by echocardiography and classified as normal or mildly, moderately or severely reduced. The left ventricular systolic function (cutoff of 0.55) was assessed by echocardiography. RESULTS: The mean age of the cohort was 62 years (±13 years); 61.3% were male, 70.2% were hypertensive, 32% were diabetic and 34.8% had hyperlipidemia. Seventy-eight patients died in hospital (10.4%), and 319 (42.6%) had an eGFR <60 mL/min. Diastolic dysfunction was present in 520 (69.4%) patients. Renal function was independently associated with worse in-hospital mortality (adjusted odds ratio 3.12, 95% confidence interval 1.71-5.69, per 10 mL/min decrease in eGFR <60 mL/min). For patients with normal-to-moderate diastolic dysfunction, normal renal function was not associated with increased in-hospital mortality (p-interaction = 0.01). CONCLUSIONS: Impaired renal function and both systolic and diastolic dysfunctions were associated with worse in-hospital mortality. However, normal-to-moderate diastolic dysfunction in the presence of normal renal function was not associated with worse outcome. Efforts to preserve renal function in patients with acute myocardial infarction should be made, particularly in those with diastolic dysfunction.

OBJECTIVES: To correlate the importance of the ankle-brachial index in terms of cardiovascular morbimortality and the extent of coronary arterial disease amongst elderly patients without clinical manifestations of lower limb peripheral arterial disease. METHODS: We analyzed prospective data from 100 patients over 65 years of age with coronary arterial disease, as confirmed by coronary angiography, and with over 70% stenosis of at least one sub-epicardial coronary artery. We measured the ankle-brachial index immediately after coronary angiography, and a value of <0.9 was used to diagnose peripheral arterial disease. RESULTS: The patients' average age was 77.4 years. The most prevalent risk factor was hypertension (96%), and the median late follow-up appointment was 28.9 months. The ankle-brachial index was <0.9 in 47% of the patients, and a low index was more prevalent in patients with multiarterial coronary disease compared to patients with uniarterial disease in the same group. Using a bivariate analysis, only an ankle-brachial index of <0.9 was a strong predictive factor for cardiovascular events, thereby increasing all-cause deaths and fatal and non-fatal acute myocardial infarctions two- to three-fold. CONCLUSION: In elderly patients with documented coronary disease, a low ankle-brachial index (<0.9) was associated with the severity and extent of coronary arterial disease, and in late follow-up appointments, a low index was correlated with an increase in the occurrence of major cardiovascular events.

OBJECTIVES: To identify predictors of in-hospital mortality in patients with acute myocardial infarction undergoing pharmacoinvasive treatment. METHODS: This was an observational, prospective study that included 398 patients admitted to a tertiary center for percutaneous coronary intervention within 3 to 24 hours after thrombolysis with tenecteplase. ClinicalTrials.gov: NCT01791764 RESULTS: The overall in-hospital mortality rate was 5.8%. Compared with patients who survived, patients who died were more likely to be older, have higher rates of diabetes and chronic renal failure, have a lower left ventricular ejection fraction, and demonstrate more evidence of heart failure (Killip class III or IV). Patients who died had significantly lower rates of successful thrombolysis (39% vs. 68%; pâ=â0.005) and final myocardial blush grade 3 (13.0% vs. 61.9%; p<0.0001). Based on the multivariate analysis, the Global Registry of Acute Coronary Events score (odds ratio 1.05, 95% confidence interval (CI) 1.02-1.09; pâ=â0.001), left ventricular ejection fraction (odds ratio 0.9, 95% CI 0.89-0.97; pâ=â0.001), and final myocardial blush grade of 0-2 (odds ratio 8.85, 95% CI 1.34-58.57; pâ=â0.02) were independent predictors of mortality. CONCLUSIONS: In this prospective study that evaluated patients with ST-segment elevation myocardial infarction treated by a pharmacoinvasive strategy, the in-hospital mortality rate was 5.8%. The Global Registry of Acute Coronary Events score, left ventricular ejection fraction, and myocardial blush were independent predictors of mortality in this high-risk group of acute coronary syndrome patients.

BACKGROUND: Little is known in our country about regional differences in the treatment of acute coronary disease. OBJECTIVE: To analyze the behavior regarding the use of demonstrably effective regional therapies in acute coronary disease. METHODS: A total of 71 hospitals were randomly selected, respecting the proportionality of the country in relation to geographic location, among other criteria. In the overall population was regionally analyzed the use of aspirin, clopidogrel, ACE inhibitors / AT1 blocker, beta-blockers and statins, separately and grouped by individual score ranging from 0 (no drug used) to 100 (all drugs used). In myocardial infarction with ST elevation (STEMI) regional differences were analyzed regarding the use of therapeutic recanalization (fibrinolytics and primary angioplasty). RESULTS: In the overall population, within the first 24 hours of hospitalization, the mean score in the North-Northeast (70.5 ± 22.1) was lower (p <0.05) than in the Southeast (77.7 ± 29.5), Midwest (82 ± 22.1) and South (82.4 ± 21) regions. At hospital discharge, the score of the North-Northeast region (61.4 ± 32.9) was lower (p <0.05) than in the Southeast (69.2 ± 31.6), Midwest (65.3 ± 33.6) and South (73.7 ± 28.1) regions; additionally, the score of the Midwest was lower (p <0.05) than the South region. In STEMI, the use of recanalization therapies was highest in the Southeast (75.4%, p = 0.001 compared to the rest of the country), and lowest in the North-Northeast (52.5%, p <0.001 compared to the rest of the country). CONCLUSION: The use of demonstrably effective therapies in the treatment of acute coronary disease is much to be desired in the country, with important regional differences.

In the past two years we observed several changes in the diagnostic and therapeutic approach of patients with acute heart failure (acute HF), which led us to the need of performing a summary update of the II Brazilian Guidelines on Acute Heart Failure 2009. In the diagnostic evaluation, the diagnostic flowchart was simplified and the role of clinical assessment and echocardiography was enhanced. In the clinical-hemodynamic evaluation on admission, the hemodynamic echocardiography gained prominence as an aid to define this condition in patients with acute HF in the emergency room. In the prognostic evaluation, the role of biomarkers was better established and the criteria and prognostic value of the cardiorenal syndrome was better defined. The therapeutic approach flowcharts were revised, and are now simpler and more objective. Among the advances in drug therapy, the safety and importance of the maintenance or introduction of beta-blockers in the admission treatment are highlighted. Anticoagulation, according to new evidence, gained a wider range of indications. The presentation hemodynamic models of acute pulmonary edema were well established, with their different therapeutic approaches, as well as new levels of indication and evidence. In the surgical treatment of acute HF, CABG, the approach to mechanical lesions and heart transplantation were reviewed and updated. This update strengthens the II Brazilian Guidelines on Acute Heart Failure to keep it updated and refreshed. All clinical cardiologists who deal with patients with acute HF will find, in the guidelines and its summary, important tools to help them with the clinical practice for better diagnosis and treatment of their patients.

Acute pulmonary congestion in patients with cardiac disease is a clinical manifestation of extreme severity, occurring in approximately 25% of cases of acute heart failure. Diagnosis is essentially clinical, based on history and physical examination. Complementarytests should not delay the start of treatment in the emergency room. A dor torácica é o sintoma que leva o paciente ao serviço de emergência e, nesse contexto,é fundamental que se faça o diagnóstico de uma síndrome coronariana aguda (SCA). Aavaliação adequada das características dos sintomas e a realização imediata de eletrocardiogramano atendimento inicial permitem o diagnóstico e, na maioria das vezes, apontamo melhor tratamento. Pelo eletrocardiograma podemos prontamente estabelecer o diagnóstico de SCA com supra de ST, que caracteriza infarto agudo do miocárdio e orienta parao tratamento de reperfusão o quanto antes. A ausência desse achado eletrocardiográfico caracteriza SCA sem supra de ST, que requer melhor definição prognóstica. Os marcadores de necrose miocárdica contribuem para melhor definição diagnóstica e estratificação de risco desses pacientes. Para aqueles cujos aspectos clínicos fazem suspeitar de quadrode isquemia aguda do miocárdio, mas que não apresentam um traçado eletrocardiográfico e marcadores de necrose com alterações suficientes para um diagnóstico mais definitivo, os testes isquêmicos não invasivos como o teste ergométrico, o ecocardiograma ou acintilografia de perfusão miocárdica, de repouso ou de esforço ou ainda com estresse farmacológico, podem contribuir tanto para o diagnóstico, quanto para a estratificaçãode risco. Mais recentemente, com a angiotomografia das coronárias ou com protocolos de atendimento de curta duração que utilizam troponinas de alta sensibilidade, podemos confirmar a presença de SCA ou afastá-la com segurança, reduzindo significativamente a permanência hospitalar desses pacientes...
Chest pain is the symptom that brings the patient to the emergency department and in this context, it is essential to make the diagnosis of an acute coronary syndrome (ACS). Proper evaluation of the characteristics of the symptoms and the immediate realization of an electrocardiogram in the initial care allow the diagnosis and, in most cases, indicate the best treatment. The electrocardiogram can readily establish the diagnosis of ACS with STelevation, featuring acute myocardial infarction, and guides for the treatment of reperfusionas soon as possible. The absence of this electrocardiographic finding is compatible with an ACS without ST elevation, which requires better prognostic definition. Myocardial necrosis markers contribute to better diagnostic definition and risk stratification of these patients. For those whose clinical aspects lead to suspicion of acute myocardial ischemia, but do not have an electrocardiographic tracing and necrosis markers with enough change for a more definitive diagnosis, non-invasive ischemic tests such as exercise test, echocardiography or stress-rest myocardial perfusion scintigraphy, or pharmacologic stress can contribute to both the diagnosis and the risk stratification. More recently, with CT angiography of the coronaryor with short-term treatment protocols using high-sensitivity troponins, we can confirm the presence of ACS or rule out it safely, significantly reducing the hospital stay of these patients...

Há uma forte relação da hipertensão arterial com manifestações clínicas da coronariopatia. A emergência hipertensiva associada a um quadro isquêmico miocárdico agudo, angina instável ou infarto agudo do miocárdio requer tratamento rápido e seguro, para controle dos níveis pressóricos e redução do fenômeno isquêmico. Dentre os recursos terapêuticos disponíveis por via endovenosa, destacam-se a nitroglicerina e os betabloqueadores, ambos contribuindo para melhora do fluxo sanguíneo coronário e, ao mesmo tempo, reduzindo o consumo de oxigênio miocárdico. Os altos níveis de pressão arterial na vigência do infarto agudo do miocárdio tornam-se uma séria contraindicação para o emprego imediato de trombolítico no tratamento de reperfusão e o risco de acidente vascular cerebral é elevado. Portanto, devemos retardar esse tipo de recurso ou utilizar a via percutânea para angioplastia como tratamento ideal.
Arterial hypertension has a strong relationship with clinical manifestation of coronary heart disease. The hypertensive emergency associated with an acute myocardial ischemia, instable angina or an acute myocardial infarct needs a fast and safe procedure, to control the arterial pressure and reduce ischemic phenomenon. Among the available therapeutic resources by intravenous way, we can highlight nitroglycerine and betablockers, in that both reduce myocardial oxygen consumption and improve coronary flow. The high levels of arterial pressure in acute myocardial infarct become a serious contraindication for the immediate use of thrombolytic agents in reperfusion treatment and the risk for stroke is high. Thus, we should delay this action or use percutaneous way for angioplasty as a better treatment.

A doença aterosclerótica coronária é a principal causa de morte nos países desenvolvidos e, nos países em desenvolvimento, vem ocorrendo uma mudança progressiva, sendo hoje também as doenças cardiovasculares a maior causa de morte na população. Assim, é fundamental, uma atitude proativa do médico no consultório ou nos ambulatórios para o melhor controle do paciente já portador de coronariopatia, prevenindo a progressão da aterosclerose e dos eventos isquêmicos agudos. No seguimento o coronariopata crônico, uma avaliação clínico e eletrocardiográfica periódica, além de exames laboratoriais, é, geralmente, o suficiente para orientar de forma adequada o tratamento clínico e o controle dos fatores de risco deste paciente. Testes que avaliam a função ventricular e a presença e extensão da isquemia miocárdica são úteis para a estratificação inicial do risco de morte nos portadores de coronariopatia e, também, serão realizados quando houver mudança significativa no quadro clínico, procurando-se melhor adequação do tratamento medicamentoso e, quando apropriada, a indicação de intervenção percutânea ou de cirurgia de revascularização miocárdica. Atenção especial deverá ser dada a grupos específicos de coronariopatas, principalmente os idosos, às mulheres e aqueles portadores de doença renal crônica.
Coronay heart disease is the most common cause of death on the develop world, and in developing countries progressive changes are occurring with coronary heart disease being the first cause of death. The medical approach to the patient with chronic coronary heart disease to prevent the progression and the occurrence of acute events is a fundamental proactive attention. The follow up of the patients with chronic heart disease mens a clinic evaluation with electrocardiogram and laboratory tests, therefore it is not anymore a need to control patients and their risk factors. Tests to evaluate the ventricular function, the ischemic extension are very importante to the stratification risk of patients with chronic coronary heart disease, these tests are also helpful when alterations occur in the severity of cardiac symptons, so in this situation we will able to choose from the optimal medical therapy, coronary revascularization with percutaneous coronary intervention or coronary artery bypass grafting. In the follow up of chronic coronary heart disease specific groups need attention, the elderly, women and those with chronic kidney disease.
